Abstract

Cerebral cortex has a motoric area which function in controlling muscle movement. When this area does not develop properly or damaged, cerebral palsy can occur. Cerebral palsy is a disorder of the motoric area of the brain which causes a dysfunction on muscle activities. Eventually, this will lead to limitation of daily activities. Disorder can occur in prenatal, perinatal, or postnatal period. There are four types of cerebral palsy which are spastic, dyskinetic, ataxic, and mixed cerebral palsy. Patient with cerebral palsy experience changes in production of force, balance, and gait as well as a decreased in production of volunteer force. The most dominant sign is balance disorder, but sensoric deficiency could also be seen. In diagnosing cerebral palsy, Levine criteria called POSTER can be used. A child is said to have cerebral palsy when four out of six categories are fulfilled. The aim of treatment of cerebral palsy is to increase functionality, improve capability, and sustain health. Treatment should be done as early as possible and intensive to achieve maximum output. Aside from pharmacological and surgical treatment, mechanic aids, and management of associated medical condition, treatment of cerebral palsy includes physiotherapy. Physiotherapy helps to improve gross motor quality in children with cerebral palsy. Although there are no medicines that can cure cerebral palsy, physiotherapy approach can be done includes re-patterning movement, neurodevelopmental technique (NDT), body weight support treadmill training, atlanto-occipitalisi mobilisation, and standing frame exercise.

Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia due to problems with insulin performance, with prevalence continuing to increase throughout the world. According to Bodke et al (2023), in the last three decades, the number of diabetes sufferers is estimated to reach 642 million in 2040, with the majority being in low and middle income countries. Type 2 diabetes mellitus is the most common form, caused by insulin resistance and insufficient insulin secretion so that the sugar that enters the body is difficult to process. Management of diabetes mellitus requires a comprehensive approach that includes diabetes patient education, lifestyle changes, a balanced diet, and physical activity. Patient education is essential to increase their understanding of the disease and how to manage their health condition. Lifestyle changes and a balanced diet can help regulate blood glucose levels, while physical activity plays a role in improving insulin sensitivity. Antidiabetic drugs consist of oral and injectable preparations. The combination of pharmacological therapy with oral antidiabetic drugs such as metformin and SGLT2 inhibitors has also proven effective in lowering blood glucose levels and reducing the risk of complications. This approach is expected to improve the quality of life of diabetes mellitus patients and reduce the burden of disease in society. This literature review aims to explore the importance of rationality in administering antidiabetic drugs to type 2 diabetes mellitus patients in Indonesia.

Abstract

Trigeminal Neuralgia (TN) is a sudden pain in the facial area, short duration for about one second to two minutes, being-stabbed feels like, and occurs repeatedly in one or more branches of the trigeminal nerve. Type 1 (one) as intermittent pain and type 2 (two) as constant pain with different clinical, pathological, and prognostic complaints. The causes of Trigeminal Neuralgia involve peripheral pathology in the roots (compression or traction), brainstem dysfunction, basal ganglia, and cortical pain modulation mechanisms, but the most common is due to neurovascular factors. The diagnosis of Trigeminal Neuralgia is basically clinical, to rule out secondary causes, detect pathological changes in the affected roots and neurovascular compression (NPC), Magnetic Resonance Imaging (MRI) may be used. The choice of first-line drug therapy is carbamazepine, and for the second line patient may use oxcarbazepine, baclofen, lamotrigine, phenytoin, and topiramate. Microvascular decompression (MVD) is the surgical treatment of choice for TN that is resistant to medical management. Patients with significant medical comorbidities, without neurovascular compression and multiple sclerosis are generally advised to undergo gamma knife radiosurgery, percutaneous balloon compression, glycerol rhizotomy, and radiofrequency thermocoagulation procedures. The most effective medical therapy is with carbamazepine with a reported reduction in facial pain after taking it as much as 47 percent. For patients who do not respond to medical treatment, microvascular decompression surgery is performed with an effectiveness rate in patients as much as 90 percent.

Abstract

Cataract ranks second as the most common visual impairment and first as a cause of blindness, as the lens hardens and causes a decrease in sharp vision. Based on age, cataracts are divided into several types: congenital cataract, juvenile cataract and senile cataract. Senile cataract is a cataract that often occurs in older people. The classification of senile cataracts is based on the progression of the cataract's opacity, namely, incipient, immature, mature, and hypermature. Some of the risk factors for senile cataracts include a history of diabetes mellitus, family history, smoking, and exposure to ultraviolet light. Smokers have a higher risk of developing senile cataracts than non-smokers. Ingredients in cigarettes such as cyanate, nicotine and tar can cause changes and denaturation of proteins in the lens of the eye. Meanwhile, sunlight absorbed by the eye produces free radicals and affects the tissues in the lens of the eye, triggering oxidative reactions, causing changes in amino acid reactions and leading to senile cataracts.

Abstract

Diarrhea is a condition of defecating more than 3 times a day accompanied by a change in the consistency of the stool to liquid with or without mucus and blood that lasts less than one week. Diarrhea results in a loss of fluids and electrolytes, especially sodium and potassium, in large amounts. Fluid loss that continues and is not balanced with adequate replacement will result in dehydration. The risk of dehydration in toddlers is greater due to the inability to meet their fluid needs while their body fluid composition is large. Dehydration that is allowed to continue will result in several complications and even death. Therefore, diarrhea is one of the diseases that is the main cause of morbidity and death in children under five. One prevention of diarrhea is rotavirus immunization. Rotavirus immunization is an effort to actively create/increase a person's immunity against a disease so that if one day they are exposed to the disease they will not get sick or will only experience mild illness. This research was conducted to determine the relationship between rotavirus immunization and the incidence of diarrhea in toddlers. This research uses a literature review method with data sources from the National Center for Biotechnology Information, Google Scholar, and Science. The research year range of articles is 2018 to 2021 and articles conducted in English and Indonesian literature. The results of this literature review from the 6 articles analyzed stated that there was a relationship between rotavirus immunization and the incidence of diarrhea in toddlers.

Abstract

Obesity is a complicated and multifaceted medical disorder defined by excess fat buildup, which can have a severe impact on an individual's physical and mental health. In 2022, the World Health Organization (WHO) reported that around 2.5 billion persons globally are overweight, with obesity incidence in Indonesia reaching 23.1%. This number indicates a notable rise and raises major public health concerns. Obesity has a significant effect on a person's mental health, including sadness, anxiety, and low self-esteem, in addition to raising the risk of several chronic diseases like type 2 diabetes, hypertension, and cardiovascular disease. According to recent studies, psychological variables are crucial in the emergence and maintenance of obesity. Unhealthy eating habits, such as consuming foods high in calories and low in nutrients, can be brought on by stress, social stigma, and environmental pressure. As a result, obesity management requires a multidisciplinary strategy that not only focuses on physical therapies such as food and exercise, but also takes into account the psychological factors that influence eating behavior. Acceptance and Commitment Therapy (ACT), which aims to help people manage psychological stress and build mental resilience, is one promising strategy. Accepting challenging emotional situations and resolving to act in line with one's own ideals are the main goals of ACT. It is believed that removing the stigma attached to weight and offering sufficient psychological assistance can enhance the lives of those who suffer from obesity and stop the rise in obesity rates in Indonesia. Better outcomes in controlling this condition can be accomplished by improving our understanding of the link between mental health and obesity and implementing comprehensive intervention techniques. This comprehensive strategy is anticipated to offer a more successful way to deal with the issue of obesity in society by incorporating a number of academic fields, such as psychology, nutrition, and sports.

Abstract

Ultrasonography (USG) is a diagnostic tool in medical imaging that operates on the principle of ultrasound waves emitted by a transducer. Recent advancements in USG technology have significantly enhanced its diagnostic capabilities. Renal elastography, a specialized application, enables the assessment of tissue stiffness or elasticity. The technique involves applying pressure to the tissue and measuring the resulting strain, providing valuable insights into the extent of fibrosis in the renal parenchyma. This article presents a comprehensive review of renal elastography ultrasonography and its clinical applications. USG is a non-invasive imaging modality that requires no special preparation, typically performed with the patient in a supine position. The kidneys are evaluated in longitudinal and transverse planes using a transducer positioned at the flank. Various elastography techniques exist, categorized based on the intensity of external pressure applied.

Abstract

Congenital Heart Disease (CHD) is a heart condition present at birth, with a global incidence of 9.4 cases per 1,000 live births, and in Indonesia, around 8 cases per 1,000 births. Children with CHD, especially those experiencing malnutrition, are at high risk of growth disorders due to energy imbalance and nutrient deficiencies. Studies on infants with cyanotic and acyanotic CHD show a significant relationship between the type of CHD and the child’s nutritional status. Cyanotic CHD has a greater impact on growth disorders compared to acyanotic CHD, with infants with cyanotic CHD having lower weight and height. The research also indicates that infants with cyanotic CHD have a higher risk of malnutrition due to difficulty meeting their energy needs through food intake. Malnutrition in infants with CHD can be caused by insufficient food intake, inefficient nutrient absorption, and increased energy demands due to the heart working harder. Hypoxemia in cyanotic CHD leads to decreased appetite, increased workload on the heart and lungs, and inefficient thermoregulation, ultimately increasing calorie needs. Additionally, hypoxemia causes changes in body tissue, such as a reduction in fat cells, leading to chronic malnutrition. Increased blood viscosity due to erythrocytosis impedes blood flow and oxygen distribution, worsening the energy imbalance that significantly affects the growth of infants.

Abstract

Stroke is a condition characterized by the rapid development of clinical signs of focal and global neurological deficits, which can worsen and persist for 24 hours or more, potentially leading to death, with no clear cause other than vascular. Transient Ischemic Attack (TIA) is a temporary neurological dysfunction caused by a blockade or reduction in blood perfusion to brain tissue, lasting from a few seconds to minutes, and in some cases, up to 24 hours. Diagnosing TIA is challenging because patients typically present after the TIA has resolved, making it crucial to diagnose TIA clinically before confirming the diagnosis with additional examinations such as CT-Scan or MRI. There are meta-analysis studies that describe the role of certain biomarkers in supporting the diagnosis of TIA. Due to the closely related pathomechanisms between TIA and ischemic stroke, almost all the same biomarkers can be used to diagnose both conditions. There are scoring systems used to identify the risk of stroke in patients with TIA, with the ABCD² score being the most commonly used. Rapid assessment and the implementation of adequate therapy reduce the risk of TIA progressing to ischemic stroke.

Abstract

Small ball game sports are a category of sports that involve small-sized balls and bats. Examples of these sports include badminton, rounders, baseball, softball, tennis, and others. Indonesia has not yet achieved notable achievements in small ball game sports. To improve performance and achieve better results, athletes must maintain optimal performance levels. It is crucial to identify and optimize all factors affecting the performance of small ball game athletes to help them reach their full potential. Optimal physical condition is the most essential factor for athletes. Physical fitness can be improved through the right combination of frequency, intensity, duration, and types of exercise. Another factor that affects athletic performance is nutrition, including macronutrients such as carbohydrates, proteins, and fats, as well as micronutrients like vitamins and minerals. Athletes have different nutritional needs compared to non-athletes. Furthermore, nutritional requirements vary across different sports disciplines and are highly individualized. Psychological factors also significantly affect an athlete's performance. An athlete's psychological state greatly influences their concentration during competitions and plays a crucial role in their ability to perform at their best. Key psychological aspects to consider include discipline, motivation, self-confidence, and personality. To achieve optimal performance, athletes require physical training, nutritional monitoring, and psychological support.
