Abstract

Adolescence is a transitional period that is vulnerable to the emergence of risky behaviors, such as smoking, promiscuity, and lack of health concern. These behaviors can negatively impact adolescent growth and development physically, mentally, and socially. This community service activity aims to increase knowledge and awareness regarding preventing risky behaviors through a health education program for students of SMPN 23, Padang City. The activity methods include preparation and problem identification stages, coordination with partners, and implementation stages, including interactive counseling, group discussions, and distribution of leaflets and promotional banners. The first day of education targeted all students in grades VII and VIII of SMPN 23 Padang. On the second day, education focused on members of the Adolescent Health Cadre (KKR) and the Student Council (OSIS), totaling 27 people. Evaluation was carried out with pre-tests and post-tests to measure the increase in knowledge. The activity results showed a significant increase in the average knowledge score of the cadre students (77.78 to 94.07). In addition, the students showed enthusiasm and active involvement throughout the activity. This educational activity was successfully implemented smoothly and brought about an increase in knowledge among students, KKR cadres, and OSIS. Cadre students understand their role and are expected to be able to serve as peer educators when their peers encounter health issues or risky behavior. Similar activities, involving teachers and parents, need to be implemented continuously to optimize risky behavior prevention.

Abstract

Background: Papillary thyroid carcinoma (PTC) is generally indolent, yet specific histological subtypes defined by the World Health Organization (WHO) are linked to aggressive behavior and poor prognosis. The loss of the cell-adhesion protein E-cadherin is a hallmark of the epithelial-to-mesenchymal transition (EMT), a process implicated in tumor aggression. However, its role in stratifying PTC subtypes versus its correlation with tumor stage remains a significant controversy in the literature. This study aimed to disentangle these two parameters by clarifying the relationship between E-cadherin expression and both histological phenotype and tumor stage. Methods: This was an observational, cross-sectional pilot study on 40 randomly selected, formalin-fixed, paraffin-embedded (FFPE) PTC cases from a 2024 cohort (N=74) at a tertiary hospital in Indonesia. All cases were re-evaluated and classified according to the WHO 5th Edition (2022) criteria as non-aggressive (n=34) or aggressive (n=6). E-cadherin expression was assessed by immunohistochemistry (IHC) using a standardized semi-quantitative scoring system (product of intensity and proportion) adapted from previous studies, with inter-rater reliability assessed (Cohen’s Kappa = 0.88). Scores were dichotomized as 'High' (n=25) or 'Low' (n=15). The association between E-cadherin expression and both histological subtype and AJCC 8th Edition tumor stage (Early: I/II [n=32] vs. Advanced: III/IV [n=8]) was analyzed using Fisher's Exact Test, with Odds Ratios (OR) and 95% Confidence Intervals (CI) calculated. Results: High E-cadherin expression was observed in 62.5% of cases. A statistically significant and strong association was found between E-cadherin expression and histological subtype (p=0.021; OR 12.0; 95% CI 1.2–118.9). Low E-cadherin expression was present in 83.3% (5 of 6) of aggressive-subtype tumors, versus only 29.4% (10 of 34) of non-aggressive subtypes. In contrast, no significant correlation was found between E-cadherin expression and advanced tumor stage (p=0.126; OR 3.67; 95% CI 0.7–18.6). Conclusion: Loss of E-cadherin expression is a significant biomarker associated with high-risk, aggressive histological phenotypes in PTC. Its lack of correlation with tumor stage, confirmed by an uncertain OR, suggests E-cadherin's role is indicative of an inherent tumor biological phenotype (aggressiveness) rather than a linear marker of tumor progression (stage). This dichotomy, likely reflecting EMT/MET plasticity, positions E-cadherin IHC as a powerful ancillary tool for pathological risk stratification.

Abstract

Elementary schools are strategic environments for developing healthy lifestyles from an early age through a promotive and preventive approach. The little doctor training program as school health cadres is one effective strategy for strengthening School Health Program (UKS). The training was initiated as a post-COVID-19 pandemic recovery program to revitalize UKS in response to the low implementation of UKS and the high percentage of students who have not implemented Clean and Healthy Living Behaviours (PHBS). To increase the capacity of students at SDN 05 Surau Gadang as school health cadres to achieve school health empowerment. The training was conducted in collaboration with the Nanggalo Community Health Centre with group-based interventions for 63 students in grades 3–5 at SDN 05 Surau Gadang, which included the delivery of material on UKS, PHBS, Primary Preventive Care (P3P), and First Aid for Accidents (P3K), as well as hands-on practice in six-step hand washing and first aid simulations. Evaluation was conducted using pre-tests and post-tests. There was an increase in the average knowledge score from 65.56 to 71.59 (p = 0.019), and the proportion of students in the “good” category increased from 36.5% to 46%. Little doctor training is effective in improving understanding of PHBS and UKS and forming independent elementary school health cadres. This program positively impacts the capacity of community health centres in optimizing ongoing little doctor training programs post-pandemic.

Abstract

Background Urothelial carcinoma is the most common malignancy in the bladder with an incidence up to 95% and 70-85% of them are bladder infiltrating urothelial carcinoma. To improve the management of bladder urothelial carcinoma, prognostic factors are required. One such factor is angiogenesis, which affects growth, development and metastasis. Vascular endothelial growth factor (VEGF) is the main pro-angiogenic factor to control angiogenesis. Expression of VEGF is correlated with progression of bladder infiltrating urothelial carcinoma, such as grade and lymphovascular invasion. The aim of this study was to determine the correlation of VEGF expression with grade of differentiation and lymphovascular invasion in bladder infiltrating urothelial carcinoma. Methods This research was a retrospective observational cross sectional study with 48 cases of bladder infiltrating urothelial carcinoma in four Anatomical Pathology Laboratory in West Sumatera 2018 and 2019. Samples were reevaluated of HE slide for grade of differentiation and lymphovascular invasion. VEGF expressions in tumor cell s were analyzed using immunohistochemistry staining. Bivariate statistical analysis used Fisher's Exact test and value p<0.05 was considered significant. Results Bladder infiltrating urothelial carcinoma high grade had more positive VEGF expression (91.7%), while low grade had more negative VEGF expression (58.3%). Lymphovascular invasion positive was mostly found with positive VEGF expression (75%). Statistical analysis showed significant correlation between VEGF expression with grade of differentiation (p=0.001) and lymphovascular invasion (p=0.004). Conclusion The conclusion was VEGF expression had significant correlation with grade of differentiation and lymphovascular invasion of bladder infiltrating urothelial carcinoma.
