Abstract

Obesity continues to be a major health problem worldwide and has become an epidemic that has worsened over the past 50 years. Obesity has a prevalence that continues to increase every year due to unhealthy lifestyles and energy intake that is greater than energy expenditure. Obesity is closely related to the habit of consuming high-fat foods, which can increase the risk of increased blood pressure. Several studies have found that a 15% increase in body weight can cause an 18% increase in blood pressure. The results of the 2018 Indonesian Basic Health Research showed that the prevalence of obesity in the Indonesian population aged 18 years and over reached 21.8%. The purpose of this study was to determine the difference in obesity status on blood pressure at the Panjang Health Center, Bandar Lampung City. This study used an observational analytical method with a cross-sectional approach. The population in this study were obese patients registered at the Panjang Health Center, Bandar Lampung City in the period August 2023 to August 2024. The total number of research subjects who met the inclusion and exclusion criteria was 113 research subjects obtained from medical records of obese patients at the Panjang Health Center, Bandar Lampung City in the period August 2023-August 2024. Data are presented in a distribution table and analyzed using the Mann-Whitney test. The results showed a P value of 0.001 which indicated that there was a difference in the effect of obesity status on blood pressure status at the Panjang Health Center, Bandar Lampung City.

Abstract

Cerebral cortex has a motoric area which function in controlling muscle movement. When this area does not develop properly or damaged, cerebral palsy can occur. Cerebral palsy is a disorder of the motoric area of the brain which causes a dysfunction on muscle activities. Eventually, this will lead to limitation of daily activities. Disorder can occur in prenatal, perinatal, or postnatal period. There are four types of cerebral palsy which are spastic, dyskinetic, ataxic, and mixed cerebral palsy. Patient with cerebral palsy experience changes in production of force, balance, and gait as well as a decreased in production of volunteer force. The most dominant sign is balance disorder, but sensoric deficiency could also be seen. In diagnosing cerebral palsy, Levine criteria called POSTER can be used. A child is said to have cerebral palsy when four out of six categories are fulfilled. The aim of treatment of cerebral palsy is to increase functionality, improve capability, and sustain health. Treatment should be done as early as possible and intensive to achieve maximum output. Aside from pharmacological and surgical treatment, mechanic aids, and management of associated medical condition, treatment of cerebral palsy includes physiotherapy. Physiotherapy helps to improve gross motor quality in children with cerebral palsy. Although there are no medicines that can cure cerebral palsy, physiotherapy approach can be done includes re-patterning movement, neurodevelopmental technique (NDT), body weight support treadmill training, atlanto-occipitalisi mobilisation, and standing frame exercise.

Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia due to problems with insulin performance, with prevalence continuing to increase throughout the world. According to Bodke et al (2023), in the last three decades, the number of diabetes sufferers is estimated to reach 642 million in 2040, with the majority being in low and middle income countries. Type 2 diabetes mellitus is the most common form, caused by insulin resistance and insufficient insulin secretion so that the sugar that enters the body is difficult to process. Management of diabetes mellitus requires a comprehensive approach that includes diabetes patient education, lifestyle changes, a balanced diet, and physical activity. Patient education is essential to increase their understanding of the disease and how to manage their health condition. Lifestyle changes and a balanced diet can help regulate blood glucose levels, while physical activity plays a role in improving insulin sensitivity. Antidiabetic drugs consist of oral and injectable preparations. The combination of pharmacological therapy with oral antidiabetic drugs such as metformin and SGLT2 inhibitors has also proven effective in lowering blood glucose levels and reducing the risk of complications. This approach is expected to improve the quality of life of diabetes mellitus patients and reduce the burden of disease in society. This literature review aims to explore the importance of rationality in administering antidiabetic drugs to type 2 diabetes mellitus patients in Indonesia.

Abstract

Infectious and communicable diseases remain one of the leading causes of mortality in Indonesia and worldwide. Several pathogenic bacteria can cause infections, one of which is Staphylococcus aureus. This bacterium can lead to a variety of conditions, including mastitis, dermatitis (skin inflammation), respiratory tract infections, impetigo, abscesses, toxic shock syndrome, and food poisoning. With the advent of antibiotics, an increasing number of bacterial strains have developed antibiotic resistance. The primary cause of this resistance is the widespread and irrational use of antibiotics, which allows some bacteria to survive and become resistant. The snake plant (Sansevieria trifasciata laurentii) contains compounds with antibacterial properties, such as tannins, saponins, and flavonoids. This study aimed to investigate the antibacterial activity of snake plant extract against Staphylococcus aureus. The research design used was a laboratory-based observational study. The antibacterial activity of the snake plant leaf extract was tested at concentrations of 20%, 40%, 60%, 80%, and 100% against Staphylococcus aureus using the dilution method to determine the Minimum Inhibitory Concentration (MIC) and Minimum Bactericidal Concentration (MBC). The results showed that the snake plant extract exhibited antibacterial activity against Staphylococcus aureus at all tested concentrations (20% to 100%). The MIC was observed at a concentration of 20%, while the MBC was found at 40%. These findings confirm that the extract of Sansevieria trifasciata laurentii has inhibitory effects on the growth of Staphylococcus aureus.

Abstract

Wound causes mechanical bleeding, inducing a response of the body to fix itself which undergoes a wound healing process through a few phases; hemostasis, inflammation, proliferation, and maturation or remodelling. Wound healing is a complex process due to biocellular and biochemistry activities occuring continously. One of the natural treatments is the administration of randu (Ceiba pentandra) leaf, a Spermatophyte which has been identified and used for its healing properties. This study was an experimental study with a post-test-only control group design, and datas were analyzed using a non-parametric Kruskal Wallis test. Treatments were given randu (Ceiba pentandra) leaf extract, grouped into 4: (K1): a negative control group, (P1): 40 ml of randu (Ceiba pentandra) leaf extract, (P2): 60 ml of randu (Ceiba pentandra) leaf extract, and (P3): 80 ml of randu (Ceiba pentandra) leaf extract. Through this study, randu (Ceiba pentandra) leaf extracts were rated based on the its wound healing rate and showed results of 80 ml, 60 ml, and 40 ml respectively. There is an effect of administration of randu (Ceiba pentandra) leaf extract on cut wound healing in mice (Mus musculus L) (p=0,037)

Abstract

Patient satisfaction is a key indicator in assessing healthcare quality, especially in hospital pharmacy services which play a vital role in treatment effectiveness. Hypertension is a major chronic illness requiring strict medication adherence to prevent complications such as heart disease, stroke, and kidney failure. This review examines the relationship between outpatient hypertension patients' satisfaction with pharmacy services and their medication adherence. Findings highlight that satisfaction dimensions such as reliability, empathy, responsiveness, tangibles, and assurance significantly shape patient perceptions. Higher satisfaction correlates with increased adherence. Continuous education, empathetic interaction, efficient service systems, and clear pharmacist communication are essential. Quality pharmacy services not only improve patient outcomes but also support long-term hypertension control.

Abstract

During the COVID-19 pandemic, dexamethasone and methylprednisolone are used to treat COVID-19 patients, particularly those who have acute respiratory distress syndrome (ARDS). Numerous studies have demonstrated that dexamethasone significantly improves respiratory function and lowers mortality in COVID-19 patients, particularly those who need mechanical ventilation. Patients with ARDS with COVID-19 who received dexamethasone had fewer days alive and ventilator-free days, according to a randomized study like the CoDEX trial. But methylprednisolone has also demonstrated encouraging outcomes in treating severe COVID-19 pneumonia, particularly when used as a short-term treatment. The dosage and mode of administration of the two medications differ; dexamethasone is more frequently used to treat the systemic inflammation brought on by this viral infection.

Abstract

Coronary heart disease is one of the main causes of death in the world, with more than 17 million deaths every year due to heart and blood vessel diseases, with around 8.7 million of them caused by coronary heart disease and around 15 out of 1000 residents in Indonesia which is equivalent to around 2,78,064 people suffer from coronary heart disease. The high frequency of use of cooking oil means that cooking oil is often used repeatedly. Repeated use of cooking oil can cause damage to various body organs such as the liver, heart, kidneys and arteries. Exposure to free radicals and free fatty acids obtained from used cooking oil if it enters the body will result in oxidative stress. The atherosclerosis process is characterized by the change of k-LDL to Ox-LDL which triggers endothelial dysfunction and the formation of atherosclerotic plaque on the walls of blood vessels. The high fatty acid content in used cooking oil can easily stick to the walls of blood vessels, so that it will accumulate and form atherosclerotic plaque. One of the alternative antioxidants that comes from herbs is bitter melon (Momordica Charantia L). Bitter melon contains flavonoid compounds. Flavonoids are secondary metabolites which are antioxidants that can inhibit free radical activity and can prevent atherosclerosis. Antioxidant compounds can reduce atherosclerosis by inhibiting LDL metabolism in secondary atherosclerotic lesions to prevent LDL oxidation in these lesions.

Abstract

Migraine is a neurovascular disorder characterized by recurrent headaches with symptoms such as nausea, phonophobia and photophobia. Factors related to migraine include food, psychosocial and environmental factors. The purpose of this article is to identify migraine trigger factors. The type of research is a literature review. Migraines can occur due to trigger factors including foods with certain ingredients (such as tyramine, chocolate), menstrual cycle, weather, sleep patterns, bright light, and screen time. The prevalence of migraine in the world reaches 10-14% with the highest incidence in North America, Central America and South America. In Indonesia alone, the prevalence of migraine has reached 3.5 million people with the highest coming from individuals aged 15-24 years. Until now, the exact cause of migraines is not known. However, there are many risk factors that can influence the occurrence of migraines. The diagnosis of migraine can be made through the results of the history and physical examination, and if necessary, supporting examinations can also be carried out to rule out other secondary causes that may influence migraine. Efforts to prevent migraines from occurring by controlling and avoiding trigger factors and living a healthy lifestyle. Migraine prevention can be done by avoiding trigger factors and adopting a healthy lifestyle, such as maintaining a regular sleep pattern, managing stress, and consuming a balanced nutritious diet.

Abstract

Ectopic pregnancy is a condition in which the gestational sac is located outside the uterine cavity, representing the most common life-threatening emergency during the first trimester of pregnancy. Implantation in ectopic pregnancy frequently occurs in the fallopian tube. The clinical manifestations are typically characterized by amenorrhea, followed by vaginal bleeding and sudden onset abdominal pain. In this case, a 29-year-old woman, G3P1A1, presented at 6–7 weeks of gestation with severe lower abdominal pain accompanied by vaginal bleeding, along with a risk factor of a previous history of ectopic pregnancy. Data were obtained through anamnesis, general physical examination, obstetric assessment, ultrasonography, and both medical and non-medical management. Physical examination revealed conjunctival pallor, abdominal distension with muscular defense, tenderness in the lower abdomen, and a non-palpable uterine fundus. Pelvic examination demonstrated cervical motion tenderness and bulging in the pouch of Douglas. Ultrasonography findings were highly suggestive of ectopic pregnancy. The diagnosis was established as gravida 3 para 1 abortus 1, 6–7 weeks of gestation, with a complicated ectopic pregnancy. The patient underwent exploratory laparotomy with left salpingectomy due to rupture of the left ampullary segment of the fallopian tube. Methotrexate therapy was not administered, as the patient did not meet the eligibility criteria for medical management, given that tubal rupture had already occurred.
