Abstract

Latar belakang. Laringomalasia merupakan kondisi kelemahan struktur supraglotis yang menimbulkan gejala stridor. Umumnya, penyakit penyerta laringomalasia adalah refluks laringofaring (RLF). Pemberian omeperazol dapat memperbaiki gejala regurgitasi dan stridor serta memperpendek durasi perjalanan alamiah laringomalasia.Tujuan. Mengetahui efektifitas omeperazol pada bayi dan anak dengan laringomalasia.Metode. Uji pre-post experimental pada 42 subyek yang mendapat omeperazol 2x2 mg/kg/bb selama 3 bulanHasil. Pemberian omeperazol pada laringomalasia dengan gejala berat, 58,8% mengalami perbaikan. Pemberian omeperazol pada laringomalasia dengan RLF positif, 58,3% mengalami perbaikan. Tampak perbaikan bermakna secara statistik pada perbandingan nilai awal dan akhir reflux finding score (RFS) dan laringomalacia score symptom (LSS)Kesimpulan. Prevalensi RLF positif berdasar RFS menurut Belafsky adalah 24,6% dan berdasar LSS dengan gejala berat adalah 44,6%. Efektifitas pemberian omeperazol selama 3 bulan terbukti efektif pada perbaikan nilai LSS, RFS dan status gizi.

Abstract

Background: The BODE index is generally used for predicting mortality risk of COPD patients. The BODE index included the body mass index, degree of airflow obstruction (FEV1), dyspnea (MMRC questionnaire), and exercise capacity (6-minute walk test). Exacerbation of COPD associated with decreased health related quality of life (HRQoL). HRQoL has become an important outcome in respiratory patients as proved by St.George’s Respiratory Questionnaire (SGRQ). This study aim to find the correlation between BODE index with exacerbation and quality of life of COPD patients. We hypothesized that the higher BODE index score, the more frequent exacerbation occurrence and HRQoL decreased. Methods: Prospective cohort study of COPD patients was conducted in Persahabatan Hospital in November 2010 till June 2011. This study assessed the BODE index (at baseline) and followed at 3, 6, 9 and 12 months. Patient were also examined with SGRQ at baseline and followed at 6 and 12 months. We monitored the occurrence of exacerbation every month by telephone, observed their medical record, or visited the COPD’s clinic and emergency departement. Results: Eighty-five patients were examined at baseline with mean of BODE index 4.29 and SGRQ total score 41.42%. After one year follow up 52 patients have completed examination, 29 patients have not complete examination and 4 patients died. Abstract

Background: The study aims to determine the difference in the proportion of nicotine dependence among high school students using Fagerstrom Test for Nicotine Dependece set in urban and rural environment. Method: Sample size is 757 high school students from six different high school consists of class 1, 2 and 3 were selected based on stratified cluster random sampling was asked to fill out the question of smoking status and filling fagerstorm test for nicotine dependence if the respondent is smokers. Result: Amount of 167 students with smoking status and nicotine dependence measured results obtained by 28 (16.8%) persons with nicotine dependence with 8 (11.1%) people in urban areas and 20 (21.1%) people in the rural area. Factors were statistically significant to nicotine dependence is gender, the originator, type of inhale, age first smoked and number of cigarettes smoked per day. CO levels of relationship with the level of nicotine dependence shows a strong and positive patterned. Conclusion: There is a significant relationship between gender, the originator, type of inhale, age first smoked, number of cigarettes smoked per day to nicotine dependence and and level of CO exhalation to nicotine dependence. Abstract

Background: The admission of residency program in the Ophthalmology Department, FKUI–RSCM is based on the accumulative score from some of the selection's assessment items. The coordinator expects the candidates who received high marks on selection process would also give excellent performance during the residency and therefore the resident would graduate as a qualified ophthalmologist. The aim of this study is to know the association between selection's assessment items and the performance during the residency program.Methods: This was a retrospective cross-sectional study using secondary data. The inclusion criteria were all ophthalmology residents with complete data information of selection, during, and at the end of the residency program from 1999 to 2006, in which results in 101 subjects. The selection data collection includes gender, age, the medical faculty origin, interview result, psychological test result, the entrance examination result, and the selection result. Data during the residency program involve the result from each end of the program's step, result of national examination, and difficulties encountered during the residency program. And lastly, data at the end of the residency program are the GPA and the duration of the residency.Results: There was a significant correlation between the residents' medical faculty origin and their results at the end of each step (p=0.004). There were also significant correlations between age (p=0.004), medical faculty origin (p=0.008), selection result (p=0.002) and their final GPA. Gender also have a significant correlation with difficulties encountered during the residency program (p=0.001). No significant correlation found between selection’s assessment items and duration of the residency.Conclusion: There were some significant correlations between some selection's criteria and the performance of ophthalmology residency program's participants. The medical faculty origin was found to be the strongest predictor to predict the performance of the residents.

Abstract

Background Malnutrition in infants remains a challenge in Indonesia. Malnutrition often arises as a result of errors in complementary feeding practices. An education module for Indonesian mothers may help them correctly implement complementary feeding practices. Objective To develop a module on correct complementary feeding practices for infants aged 6-9 months that can be read and understood by Indonesian mothers. Methods We performed qualitative research using the two-round Delphi method combined with group discussions. The first round listed the most important sub-themes of the module, while the second round sought approval for the module from experts. The Delphi team members were included using purposive and snowball sampling methods. Expert opinions were summarized and rearranged using the Steps for Coding and Theorization (SCAT) method. Results Ten experts were included in the Delphi team, with a mean duration of work experience of 28.5 (SD 12.37) years. The correct practices for giving complementary foods according to scientific evidence was named the ABC-Makanan PendampingASI/MPASI-Press (“Printed ABC of Complementary Feeding”) module and was summarized into four main topics: timely, adequate, safe, and responsive feeding. This module was made using language, photos, and images that were easily understood by mothers and included the suitable nutritional compositions to meet the needs of infants aged 6-9 months. Conclusion We developed the ABC-MPASI-Press module based on scientific evidence and experts’ reviews using the combined Delphi method. This educational guide is expected to inform mothers about correct complementary feeding practices for infants aged 6-9 months in order to prevent malnutrition.

Abstract

Vitamin D juga memengaruhi dan meregulasi fungsi sistem imun tubuh. Tujuan: Mengetahui proporsi defisiensi vitamin D-25(OH) pasien poliklinik Alergi dan Imunologi, serta untuk meninjau tatalaksana defisiensi vitamin D-25(OH) di poliklinik Alergi dan imunologi. Metode: Penelitian retrospektif data rekam medis pasien Poliklinik Alergi dan Imunologi yang mendapat terapi vitamin D serta kadar vitamin D-25 (OH). Hasil: Sejumlah 97,1% pasien memiliki kadar vitamin D kurang dari normal (insufisiensi vitamin D dan defisiensi vitamin D). Setelah pemberian vitamin D dosis tinggi (1000 IU/hari atau lebih) selama 3 bulan, terjadi peningkatan bermakna status vitamin D. Simpulan: Proporsi defisiensi vitamin D tinggi pada pasien penyakit imunologi dan pemberian vitamin D dosis tinggi (1000 IU/hari atau lebih) dapat meningkatkan status vitamin D pasien dengan penyakit imunologi.Vitamin D also plays a role in influencing and regulating the function of the immune system in the body. Objective: To determine the proportion of vitamin D-25 (OH) deficiency in patients in the Allergy and Immunology Polyclinic, and to evaluate the management of vitamin D-25 (OH) deficiency in patients in the Allergy and Immunology Polyclinic. Methods: Retrospective study on medical records data from Allergy and Immunology Polyclinic patients with vitamin D-25 (OH) level data and with vitamin D therapy. Results: Vitamin D level less than normal in 97,1% patients. After administration of high dose vitamin D (1000 IU/day or more) for 3 months, a significant increase in the patient’s vitamin D status was observed. Conclusion: The proportion of vitamin D deficiency in patients with immunological diseases is high and administration of high doses vitamin D (1000 IU/day or more) can improve the vitamin D status.

Abstract

AbstractDouble burdens of malnutrition among women have occurred across most developing countries including Indonesia. This study compared the associated factors among overweight and underweight of women in reproductive age (WRA) in rural Ciampea Sub-district, Bogor, West Java. This cross-sectional study surveyed the nutritional status of 575 mothers (16-49 years old) who have under two-years-old children. Nutritional status was assessed by body-mass-index (BMI) and mid-upper arm circumference (MUAC), food security status by US-Food Security Survey Module (US-FSSM), dietary intake by a single 24-H dietary recall. The prevalence of underweight, overweight, and obese among this group were 10%, 15.8%, and 34.2%. Food security status was the single factor associated with overweight (p=0.026). However, after adjustment with other factors, food insecurity with hunger was found to be the highest risk of being underweight (AdjOR=3.95; 95%CI: 1.46-10.64). Contrarily, it contributed to lower chances of being overweight among WRA (AdjOR=0.40, 95%CI: 0.21-0.77). In conclusion, food security status in this population associated with both under- and over-nutrition, in addition to other factors such as age and education level of WRA. Ensuring the availability and affordability of nutritious food together with proper nutrition education to rural communities might be worthwhile to improve this condition.AbstrakBeban ganda malnutrisi pada wanita terjadi di sebagian besar negara berkembang, termasuk Indonesia. Penelitian ini membandingkan faktor-faktor yang berhubungan pada wanita usia subur (WUS) dengan status gizi kurang dan lebih di pedesaan Ciampea, Bogor, Jawa Barat. Studi potong lintang ini mensurvei status gizi pada 575 Ibu (usia 16-49 tahun) yang memiliki baduta. Penilaian status gizi menggunakan indeks massa tubuh (IMT) dan lingkar lengan atas (LILA), status ketahanan pangan oleh US-Food Security Survey Module (US-FSSM), asupan makanan dengan food recall 24-jam. Prevalensi gizi kurang, lebih, dan obesitas pada kelompok ini adalah 10%, 15,8%, dan 34,2%. Status ketahanan pangan merupakan faktor tunggal yang terkait dengan kelebihan berat badan (p = 0,026). Namun, setelah disesuaikan dengan faktor lain, kerawanan pangan dengan kelaparan ditemukan sebagai risiko tertinggi terjadinya gizi kurang (AdjOR = 3.95; CI 95%: 1.46-10.64). Sebaliknya, kondisi tersebut berisiko lebih rendah terhadap kejadian gizi lebih pada WUS (AdjOR = 0,40, 95% CI: 0,21-0,77). Kesimpulannya, status ketahanan pangan pada populasi ini berhubungan dengan kejadian ganda malnutrisi, selain faktor lain seperti usia dan tingkat pendidikan. Memastikan ketersediaan dan keterjangkauan pangan bergizi bersama dengan edukasi gizi yang tepat untuk masyarakat pedesaan mungkin bermanfaat untuk memperbaiki kondisi ini.

Abstract

Background: Shoe manufacturing factory workers in developing countries are daily exposed to complex mixtures of organic solvents. Chemical exposure occurs through inhalation / respiration and skin which can affect many physiological systems. The main chemical exposure to organic solvents in shoe manufacturing comes from the process of gluing and cleaning process by using organic solvents on footwear in the assembling section. Methyl ethyl ketone is one type of organic solvent which, when entering into the body through inhalation routes can cause irritation of the nose, throat, lungs and chest becomes congested. Acute neurotoxicity is a collection of symptoms in the central nervous system (central nervous system depression, psychomotor disorders, narcosis, drowsiness, headache, dizziness, dyspepsia, and nausea). Method: The research design was a prospective cohort research consisting of 45 people for each high MEK exposed group and low exposed to MEK. Data collection was done by interview, physical examination, MEK level measurement of pre-shift urine and end-shift work, and filling out questionnaires Form Acute symptoms rating questionnaire at the beginning of work shift (07.00), 2 hours after work (09.00), break (12.00), 2 hours after break (15.00), and end of work shift (16.00). Result: The incidence of acute neurotoxicity symptoms in the MEK-high exposed group based on the Acute Symptoms questionnaire was 51.1%. Age factor with symptoms of acute neurotoxicity in workers at PT. X with p value = 0.432, gender factor with p value = 0.162, education factor with p value = 0.897, nutritional status factor with p value = 0.865, alcohol habit factor with p value = 0.181, coffee drinking habit factor with p value = 0.265, and smoking habit factor with p value = 0.968, work duration factor with symptoms of acute neurotoxicity at worker in PT. X with p value = 0.533, and the use factor of personal protective equipment (PPE) with p value = 0.470. In this research, symptoms of acute neurotoxicity were dominant are fatigue (80%), headache (77.8%), nausea (71.1%), dizziness (66.7%), and feeling of intoxication (53.3%). Conclusion: There are no subject and occupational factors that have a significant effect to the symptoms of acute neurotoxicity. Conducting isolation of other work areas for workers exposed to chemicals that because of high potential to harm other workers in the vicinity. Reassessing the hazard risk of using MEK substitutes with other chemicals that are of lower risk or replace with water-based solvents. Provide PPE eligible to all workers exposed to chemicals and workers around them. A PPE fit test is used for workers to feel comfortable while wearing it, giving strict sanctions to workers who do not consistently use personal protective equipment.

Abstract

Background: Natural killer (NK) cells play a role in pathogenesis of endometriosis. Lower expressions of NK cells receptor group 2D (NKG2D) ligands inhibits cytotoxic activity of NK cells; a common immunity avoidance mechanism in neoplasms. Literatures have proven miRNAs regulatory effect on NKG2D expression. There has been no specific biomarker for diagnosing endometriosis. Non-invasive means of diagnosing endometriosis may reduce well-known risks of invasive method of diagnosis and yield better results. Purpose: To investigate the correlation between miRNA-519a-3p expression with NKG2D ligands (MICA, MICB, ULBP 1-6) on endometriosis and non-endometriosis patients. Methods: This was a cross-sectional study held in five centers: dr. Cipto Mangunkusumo General Hospital, Pelni Hospital, Bunda Hospital, YPK Mandiri Hospital, and Primaya Evasari Hospital from October 2020 to July 2021. miRNA and NKG2DL analysis were done in Human Reproduction, Infertility and Family Planning (HRIFP) cluster at IMERI FKUI. Results: We obtained 19 patients in each study groups. NKG2D ligands and miRNA519a-3p relative expressions were not significantly different (p > 0.05). Increased miRNA519a-3p expression negatively affected NKG2D ligands expression. A decrease in ULBP1 and an increase in ULBP2 increased the probability for endometriosis. NKG2D ligands expression may be influenced by infection, pro-inflammatory cytokine production, dan polymorphism. NKG2D ligands expression level can be different depending on the origin of the sample. Lower expression of miRNA519a-3p indirectly inhibits tumor apoptosis by lowering NKG2D ligands, caspase, or mRNA. Conclussion: We did not manage to establish a correlation between NKG2D ligands with miRNA519a-3p in endometriosis and non-endometriosis patients.

Abstract

Background: Various pathogenic bacteria are reported as the cause of infectious colitis in children. Infectious colitis does not have a specific sign, therefore an accurate examination is required. The implementation of fecal cultures accompanied with drug resistance tests often have constraints, beside the relatively expensive costs, longer times are needed, and not all health care facilities have required instruments. On the other hand, this condition requires an immediate antibiotic therapy, so that the infection should not be continued. In daily practice, it is not uncommon to find diarrhea with the amount of fecal leukocyte 10/hpf with pathogenic bacteria on the examination of the fecal culture.Method: Cross-sectional study was conducted to observe the pattern of bacterial distribution in children’s fecal who have acute diarrhea and  the correlation between the existence of pathogenic bacteria and the number of leukocytes in the fecal, as well as antibiotic resistance patterns. The population of this study is children with age of 6 months old - 18 years old who were suffering from acute diarrhea with the  amount of  fecal leucocyte  ≥ 5/hpf, who recruited from polyclinic or patient admitted at Cipto Mangunkusumo Hospital and Fatmawati  General Hospital, Jakarta.Results: Based on examinations of fecal cultures and PCR, Salmonella sp and C. dificille were found subsequently in 2 children (33.3%), Enterophatogenic E. Coli(EPEC) and Shigella were found subsequently in 1 child (16.7%). Based on the ROC curve, it was found that there was no intersection of maximum and  minimal leukocyte value with the midline, whereas the best sensitivity and specificity value was found at the cut-off point of 8.5, hence the cut-off  point  of leukocytes was determined at 8 and 8. The sensitivity value was 83.3% and the specificity value was 45.1%.Conclusion: The antibiotic sensitivity test showed that one child infected  by EPEC was sensitive to ciprofloxacin. Two children infected  by Salmonella, were still sensitive to chloramphenicol, cotrimoxazole, cefixime, and ceftriaxone. Two children infected by C. Difficile were sensitive to ceftriaxone, and 1 child infected by Shigella was sensitive to cefixime, ceftriaxone and ciprofloksazine.
