Abstract

HIGHLIGHTS 1. The presence of Human Papillomavirus (HPV) can be detected using urine samples, as compared to cervical samples, by means of Cobas® 4800 HPV test.2. The concordance between cervical and urine samples for HPV DNA was found as much as 84.72%.3. Urine samples subjected to Cobas® 4800 HPV test is helpful to treat HPV infection.   ABSTRACT Objective: To find the accuracy, sensitivity, specificity, as well as predicted values, both positive and negative, of urine samples using Cobas® 4800 in detecting high risk Human Papillomavirus. Materials and Methods: This study was a cross-sectional study with a total of 72 samples taken from medical records of hrHPV DNA examination with Cobas® 4800 in 2017-2020. Study subjects were called for re-examination of urine samples and cervical samples using Cobas® 4800. Samples with positive hrHPV DNA in the cervix, urine, or both were examined for cervical fluid-based cytology (LBC). Data were analyzed using Chi-square. Results: Overall, 84.72% agreement was detected through specimens of urine and cervical mucus tested of hrHPV DNA with Cobas® 4800. In all samples, a significant rate of concordance detection of hrHPV DNA with Cobas® 4800 was reported (ka = 0.62; 95% IC: 39-84). In this population, in determining the presence of hrHPV DNA in cervical and urine specimens, it was found that the sensitivity, specificity, positive predictive value and negative predictive value were respectively 87.5% (95% IC: 64–97%), 84% (95% IC: 72–91%), 60.9% (95% IC: 40.8–77.8%), and 96 % (95% IC: 86.3–98.9%). Conclusion: The presence of hrHPV infection in the cervix can be determined by detecting hrHPV DNA in the urine. According to these findings, urine samples subjected to the Cobas® 4800 HPV test may be helpful for the clinical treatment of HPV infection.

Abstract

The high-intensity interval training has been seen to be beneficial for various populations. This study aimed to determine the effect of high-intensity interval training (HIIT) on blood lactate levels and rate of perceived exertion (RPE) in sedentary healthy adults. A pre-post study was conducted on adults that were stated healthy and sedentary on physical activity. All participants were given HIIT with a total duration of 20 minutes for 12 sessions using a treadmill. Blood lactate level and RPE was recorded on the 1st and 12th session. A significant decrease in blood lactate level and RPE after giving HIIT was found with a reduction of 1,1 mmol/ L and 2 on the Borg Scale respectively (p<0.0001). Further research is needed using field-based exercise testing to determine exercise prescription and to obtain the benefit of HIIT in populations with cardiovascular risk factors before utilizing it in patients.

Abstract

Background: The Swedish Occupational Fatigue Inventory (SOFI) is an instrument to assess a person’s fatigue, which is not limited to workers of a company, but can also be used for the general public. SOFI can determine fatigue from physical, cognitive and psychosocial conditions.1 This study aims to translate the instrument so that it can be used in Indonesia, then assess its validity and reliability.Methods: This study was conducted using a 10-step transcultural adaptation method from ISPOR followed by item validity and reliability tests on 224 workers at a geothermal company in Indonesia. Item-total correlation and Confirmatory Factor Analysis was performed using online version 0.14.3 of JASP statistic program. And for reliability test using SPSS tool.Results: Swedish Occupational Fatigue Inventory (SOFI) has been adapted into an Indonesian version through the ISPOR transcultural adaptation method. All items in the Indonesian version of SOFI which amounted to 20 items proved the validity and reliability of this questionnaire was very good (α-Cronbach 0.969) and the Confirmatory Factor Analysis test showed: Factor model (224, p < 0,001) = 337.649, CFI = 0,958, NFI = 0,924, RFI = 0,909, IFI = 0,958, TLI = 0,950, dan RMSEA = 0,070, 90% CI: 0,060 – 0,081. Fit indices results had acceptable model fits corresponding to the structure of the original instrument. Item-rest correlation range from 0.321 to 0.847, these results reveal that each item-rest correlation is extremely strong.Conclusion: The Indonesian version of SOFI is proven to have good validity and reliability as an instrument for assessing fatigue/work fatigue for workers in Indonesia.

Abstract

Background: Despite the COVID-19 pandemic, the workers in food manufacturing continue to work. The Government of Indonesia issued a regulation of 10-day isolation for those asymptomatic patients. This regulation implies long sick leave for workers with asymptomatic COVID-19 infection, leading to possible disruption in fulfilling target production. This study aims to understand the factors associated with the length of sick leave due to COVID-19 among food manufacturing workers. Methods: The design of this study was cross-sectional, used 205 workers’ data of confirmed COVID-19 infection, and returned to work from March 2020 – to August 2021. The data were obtained by the company medical team from medical resumes at the companies’ clinics and then sent to Ikatan Dokter Kesehatan Kerja Indonesia (IDKI) Banten. The data was screened as inclusion and exclusion criteria and then analyzed by descriptive test. unpaired T-test and linear regression. Results: The mean length of sick leave of 205 data was 19.3 (± 4,8) days. Treatment facilities, work shifts, and smoking were associated with the length of sick leave (p<0.05). Home Self-isolating, shift, and non-smoking workers had longer sick leave than workers treated at COVID center, non-shift, and smoking workers Conclusion: The mean length of sick leave among food workers in Cilegon is 19.3 (±4,8) days. There is a significant relationship between the length of sick leave with treatment facilities, shift work, and smoking. Treatment facilities were the most dominant factor.

Abstract

ABSTRACTBackground: In Indonesia, no research has been carried out or reported on pharyngeal transit time and a view of the swallowing process in a sample without complaints of dysphagia using the Flexible Endoscopic Evaluation of Swallowing (FEES) method and using five food consistencies. Purpose: To obtain the value of pharyngeal transit time and a view of the swallowing process in subjects without dysphagia problems. Method: Twenty-eight subjects without dysphagia (based on Ohkuma’s Dysphagia Screening) underwent FEES to determine pharyngeal transit time and dysphagia profile based on leakage, standing secretion, residue, penetration, and aspiration. Result: The median value of pharyngeal transit time on puree consistency was 0.799(0.499-5.666), gastric rice 0.966(0.433-5.733), oatmeal 0.849(0.533- 4.399), thick liquid 0.982(0.399-4.633), thin liquid 0.566(0.366-1.366). The pharyngeal delay time on the puree consistency was 0.566(0.199-5.333), gastric rice 0.799(0.233-2.799), oatmeal 0.899(0.099-3.633), thick liquid 0.833(0.033-3.733), and thin liquid mean was 0.294 (± 0.232). The pharyngeal response time on the puree consistency was 0.566(0.167-1.300), gastric rice 0.583(0.300-2.934), oatmeal 0.583(0.367- 1.233), thick liquid 0.549(0.333-1.533), thin liquid 0.549(0.366-1.399). There was no standing secretion, pre-swallowing leakage, penetration, and aspiration found in FEES. A grade 1 residue was found in 3(10.7%) subjects of puree, in 2(7.2%) subjects of gastric rice, in 3(10.7%) subjects of oatmeal, and in 9(32.2%) subjects of thick liquid. Conclusion: There was no prolonged pharyngeal transit time, pre-swallowing leakage, standing secretion, penetration, and aspiration in all subjects without dysphagia complaints at all food consistencies. There was minimal residue within normal limits in some subjects. ABSTRAKLatar belakang: Di Indonesia, belum pernah dilaporkan penelitian mengenai waktu transit faring dan gambaran proses menelan pada sampel tanpa keluhan disfagia dengan menggunakan metode Flexible Endoscopic Evaluation of Swallowing (FEES) dan menggunakan lima konsistensi makanan. Tujuan: Penelitian ini bertujuan untuk memperoleh nilai waktu transit faring dan gambaran proses menelan pada subjek tanpa masalah disfagia. Metode: Dua puluh delapan subjek tanpa disfagia (berdasarkan Skrining Disfagia Ohkuma) menjalani FEES untuk menentukan waktu transit faring dan profil disfagia berdasarkan leakage, standing secretion, residue, penetration, dan aspiration. Hasil: Nilai median waktu transit faring pada konsistensi puree adalah 0,799(0,499-5,633), gastric rice 0,966(0,433-5,733), oatmeal 0,849(0,533-4,399), thick liquid 0.982(0.399-4.633), thin liquid 0.566(0.366- 1.366). Waktu tunda faring pada konsistensi puree adalah 0,566 (0,199-5.333), gastric rice 0,799 (0,233-2,799), oatmeal 0,899 (0,099-3,633), thick liquid 0,833 (0,033-3,733), dan rerata thin liquid 0,294 (± 0,232). Waktu respon faring terhadap konsistensi puree adalah 0,566 (0,167-1,300), gastric rice 0,583 (0,300-2,934), oatmeal 0,583 (0,367-1,233), thick liquid 0,549 (0,333-1,533), thin liquid 0,549 (0,366-1,399). Tidak didapati adanya leakage, standing secretion, penetration, dan aspiration pada pemeriksaan FEES. Residu grade 1 ditemukan pada 3 (10,7%) subjek puree, pada 2 (7,2%) subjek gastric rice, pada 3 (10,7%) subjek oatmeal, dan pada 9 (32,2%) subjek thick liquid. Kesimpulan: Tidak terdapat perpanjangan waktu transit faring, leakage, standing secretion, penetration, dan aspiration pada semua subjek tanpa keluhan disfagia dengan semua konsistensi makanan. Terdapat residu minimal dalam batas normal pada beberapa subjek.

Abstract

Introduction: Epiretinal membrane is one post-operative complication of vitrectomy that can lead to decrease visual acuity. Epiretinal membrane is the early step of proliferative vitreoretinopathy caused by inflammation. Steroid has been used in the treatment of intraocular inflammation. The aim of this study is to compare oral methylprednisolone and placebo toward incidence and severity of epiretinal membrane post pars plana vitrectomy in rhegmatogenous retinal detachment. Methods: This was prospective, double blind, randomized clinical trial. Fourty six eyes who met inclusion criteria were randomized into 2 groups: 26 patients received oral methylprednisolone 0.8 mg /kgBW/day for 6 days, 0.4 mg/kg BW/day for 4 days, and 0.2 mg/kg BW/days for 4 days. The control group of 26 patients received placebo in a comparable manner. Result: Four weeks after vitrectomy incidences of epiretinal membrane were 47.6 % and 58.8 % in methylprednisolone group and placebo group, respectively. Eight weeks post vitrectomy incidences of epiretinal membrane was 47.6 % and 56.2 % in methylprednisolone group and placebo group, respectively. At 4 weeks the severity of epiretinal membrane in methylprednisolone group was 60%; 0%; 40% in grade 0, 1, and 2, respectively. Meanwhile, in placebo group were 60%; 10%; 30% in grade 0, 1, and 2,respectively. Eight weeks post vitrectomy the severity of epiretinal membrane in methylprednisolone group were 40%; 0 %; 60% in grade 0, 1, and 2, respectively. Meanwhile, at placebo group were 55.6 %; 11.1 %; and 33.3 % in grade 0, 1, and 2, respectively. Conclusion: There were no significant differences in incidence and severity of epiretinal membrane at 4 and 8 weeks among 2 groups. Oral methylprednisolone had a tendency to lower incidence of epiretinal membrane compared to placebo.

Abstract

Introduction: Multifocal electroretinography (ERG) is an examination which measures retinal electrical activity as a response to lighting stimulus and allows simultaneous recording in many topographic locations. Various electrodes have been developed to balance examination accuracy and also patients’ comfort. The objective of this study is to establish normal values of multifocal ERG and to compare the values and the comfort level using Jet, Dencott and DTL electrode in Indonesian Adult. Methods: Through convenient sampling 49 normal Indonesian subjects between 19 and 49 years old were selected. Multifocal ERG amplitudes and implicit time values were measured according to recommendation by the ISCEV. Evaluation consisted of N1 and P1 wave in ring 1 to 5. after the examination, all subjects filled in a questionnaire about comfort level, adopted from the visual analog scale. Result: We observed a statistically significant difference in multifocal ERG normal values between electrode with higher wave amplitudes and longer implicit time in Jet and Dencott electrodes compared to DTL electrodes. Jet and DTL electrodes are more comfortable than Dencott electrodes for Indonesian adults in multifocal ERG. Conclusion: DTL electrodes give the lowest wave amplitude and the shortest implicit time and are the most comfortable electrode compared to Jet and Dencott electrodes, in multifocal ERG in Indonesian adults.

Abstract

BACKGROUND: Currently, laboratory diagnosis of intestinal tuberculosis (ITB) is limited based on clinical manifestations, providing opportunities for alternative laboratory tests to diagnose ITB. At the present time, the role of serum adenosine deaminase (ADA) and hematological tests in ITB patients are not widely known. The objective of this study was to determine the role of ADA and hematological tests in patients suspected with ITB.METHODS: Subjects that were suspected of ITB were classified as ITB group, while subjects with inflammatory bowel disease, hemorrhoid, and intestinal malignancy were classified as non-ITB group. Colonoscopy, histopathological examinations, and hematological test were performed. ADA measurement was also performed with clinical chemistry analyzer based on enzymatic colorimetry principle.RESULTS: Out of 143 subjects, 16 (11.2%) subjects were diagnosed with ITB and 127 (88.8%) subjects were classified as non-ITB group. ADA level and erythrocyte sedimentation rate (ESR) of ITB group were significantly higher than the ones of non-ITB group (p<0.05). Cut-off, sensitivity, and specificity of ADA level were 12.56 IU/L, 75%, and 57%, respectively. Cut-off, sensitivity, and specificity of ESR were 32.5 mm/hour, 81%, and 62%, respectively. Colonoscopy of ITB subjects displayed multiple ulcerations, edema, and hyperemic mucosa. Histopathological examination of ITB subjects exhibited granulomatous inflammation, epitheloid cells, giant cells, and lymphocyte aggregates.CONCLUSION: ADA level and ESR were significantly higher among ITB patients compared with non-ITB patients. Since the sensitivities of ADA and ESR tests were high, the ADA and ESR tests could be considered as a screening test for ITB.KEYWORDS: intestinal tuberculosis, adenosine deaminase, hematological tests

Abstract

Background:  Understanding the barriers and support that family caregivers face when caring for older adults with dementia is critical for improving the quality of  dementia care. However, research in this area remains limited in Indonesia. Purpose: This study aimed to explore the barriers and support for family caregivers in caring for older adults with dementia in Indonesia.Methods: A descriptive phenomenological study was conducted among 15 family caregivers who cared for older adults with dementia. Purposive sampling was used to recruit the participants. Inclusion criteria included primary family caregivers who provided care for four hours per day for at least six months, were proficient in Indonesian, assisted older adults in daily activities, and were capable of expressing their experiences. Data were collected using in-depth semi-structured interviews and analyzed using the Colaizzi method.  Results: This study found that family caregivers faced barriers during the care of older adults with dementia, which have decreased the quality of care. These barriers included inadequate knowledge and skills of family caregivers regarding dementia, stigma, lack of knowledge and awareness in the community, limited availability of healthcare services, and healthcare professionals’ lack of knowledge about dementia. On the other hand, support received by family caregivers within the family, support from other family caregivers, and education provided by NGOs about caring for someone with dementia have shown a positive impact in reducing the burden on caregivers and improving the quality of care. Therefore, appropriate support is essential for family caregivers caring for older adults with dementia.Conclusion: This study showed that barriers originating from family caregivers, community awareness, access to health services, and barriers from health workers have impacted poor quality of care, while the support of families, fellow family caregivers, and NGOs have contributed to good quality of care. Further studies are needed to explore the perspectives of family caregivers representing different cultures in Indonesia.

Abstract

Introduction. Sickness absence, which can be caused by an unhealthy lifestyle, is an important productivity parameter for the company. Therefore, there are many wellness programs developed in the workplace in an effort to improve workers’ health, fitness, and productivity. The aim of this study was to assess the effect of a physical training program for fitness improvement (VO2 max value) on sickness absence. Methods. This study was conducted by observing the results of the workers' physical training program in the chemical company during 2018, using a cohort retrospective design. SPSS Statistics version 22.0 was used for analysis, with the chi-square test for bivariate analysis and logistic regression for multivariate analysis. Results. The analysis showed that there was a significant difference between the active group (AG) and inactive group (IG) in improving fitness (adjusted RR 4.82, CI 95% 1.54-15.05) and decreasing sickness absence (adjusted RR 3.46, CI 95% 1.03- 11.57). In addition, it was found that participants with normal weight were more likely to increase fitness compared to participants with overweight or obesity (adjusted RR 3.57, 95% CI 1.09-11.64). Conclusion. Participating in a physical training program has been shown to improve fitness levels and reduce sickness absence. Normal nutritional status is more likely to have improved fitness than overweight or obese.
