Abstract

Introduction: Health education program to recognize and prevent degenerating disease is important for elderly. therefore, a national pension bank in Indonesia set initiative to provide health empowerment program in their waiting room, to optimize momentum when elderly community come and gather regularly in pension bank at every first week of the month. The bank organized health education and medical consultation program in the waiting room, cooperated with trained health educator and physician. This study investigated how elderly satisfy with the programs, and its effect to program and institution image. Method: We investigated 438 elderlies in 10 cities of 14 pension bank branches in Indonesia who participated in health education and consultation program. We assessed their satisfaction level related to health education and its cause using SERVQUAL approaches that consist of tangible, reliability, responsiveness, assurance and empathy. We also investigated how it would trigger individual willingness to spread word of mouth in promoting the health education program and the institution to other elderly, using standardized equation modeling (SEM) analysis. Result: Most of elderly satisfy and understood the both health education content (90%) and considered this program as meaningful program (84%). In health education, the highest factor contributes to elderly satisfaction were tangibles (educator and doctor performance) and reliability factors (regular schedule and delivery method of health education content). Meanwhile in medical consultation, doctor’s performance (tangible factor), responsiveness and empathy to elderly health problem contributed equally important to elderly satisfaction. Medical consultation contributed more in triggering word of mouth to promote the program and institution (SEM r=0.81) rather than of health education (SEM r=0.58)Conclusion: Not only doctor’s performance and health content delivery method, but responsiveness and empathy were important key factors to provide successful health education and services for elderly. Elderlies highly appreciated pension bank’s initiative in providing health education and consultation while optimizing time in waiting room, therefore this model can be applied in further health education program.

Abstract

Introduction: Emotional mental disorder (EMD) is a state of psychological distress that, if not handled properly, can lead to severe mental disorders. One potential psychosocial hazard that is thought to be related to mental health disorders is work-family conflict (WFC). The issue of family-work balance has become an important issue among Indonesian female workers, especially with the rapidly growing female worker society and the strong family culture. This study aimed to identify the association between work-family conflict and emotional mental disorder in female nurses. Methods: This cross-sectional study included 264 female nurses who work at national referral general hospital in Jakarta. Their emotional mental disorder was measured using Self-Reported Questionnaire 20 (SRQ-20) and the work-family conflict was measured using the Work-Family Conflict Scale (WCFS). Results: The prevalence of emotional mental disorder in female nurses was 23.5%. The most dominant factor associated with emotional mental disorder is work-family conflict (OR 2,40, CI 95% 1,32-4,35, p=0,004). Conclusion: There is a significant association between work-family conflicts and emotional mental disorders in female nurses in Indonesia. Nurses with work-family conflicts are more likely to have emotional mental disorders. Regular mental health checks and counseling should be performed along with periodic health checks.

Abstract

Introduction: Osteoarthritis (OA) of the knee causes disability due to pain and will affect the functional ability to walk. Low-Level Laser Therapy (LLLT) has been shown to reduce pain in knee OA, while High Intensity Laser Therapy (HILT) is able to reach deeper joint areas. Method: This study aimed to compare the effect differences between LLLT and HILT on pain and functional capacity in knee OA. Methods: This is a double-blind randomized controlled trial with 61 subjects randomized into LLLT (n=31) and HILT (n=30) groups. All the subjects were knee OA patient with Visual Analog Scale (VAS) ≥ 4. The laser therapy and exercise were given 3 times per week for 6 sessions. The pain scale evaluated using VAS and functional ability evaluated using 50-feet walk test. Result: After 6 sessions, both LLLT and HILT group showed reduced VAS score [∆VAS Score LLLT = 3 (2 – 4), HILT = 3 (2 – 5)] and increased walking speed (∆ walking speed for LLLT = 0.23 (0.02 – 1.24) m/s, HILT = 0.22 (0.08 – 0.7) m/s) which were statistically (p less than 0.001) and clinically significant. HILT group had faster walking speed and greater VAS reduction compared to LLLT group (p less than 0.001), but there was no significant difference in walking speed between two groups (p=0.655). Conclusion: HILT and LLLT combined with exercise were effective in reducing pain and increasing functional ability in knee OA after 6 sessions of intervention. Pain and functional ability improvement was faster and greater in HILT group than LLLT group.

Abstract

Background: Osteonecrosis, or avascular necrosis (AVN) of the femoral head, is a progressive and irreversible bone pathological condition with an increasing amount of morbidity each year in Indonesia. Testing therapeutic strategies requires an ethical and reliable protocol for inducing femoral head AVN in animal models. This study evaluates a rabbit protocol's feasibility to support a larger investigation into secretome-based treatments.Methods: In this in vivo study, New Zealand white rabbits were divided into three test groups and one control group (n = 3 per group). Methylprednisolone was injected three times at 24-hour intervals, with lipopolysaccharide administered to the third test group on day four. Following a four-week observation period, effects were evaluated using histology, immunohistochemistry, lipid profiles, and radiology.Results: The results showed that both a dose of 20 mg/kgBW methylprednisolone plus 10 μg/kgBW lipopolysaccharide and 20 mg/kgBW methylprednisolone alone resulted in statistically significant femoral head AVN induction, characterized by an increase number of osteoclasts on caspase-3 staining and empty lacunae on hematoxylin–eosin staining. Conversely, the test group with a lower concentration did not show significant differences.Conclusion: These findings support the recommendation of a credible and promising animal model of femoral head osteonecrosis as a basis for the study of osteocytes, osteoblasts, and osteoclast regeneration, along with secretome therapy, which acts as a potential non-surgical treatment option. The absence of radiological confirmation limits a more established analysis, and further studies on this matter are needed.

Abstract

Objectives: This study examined the association between sexual debut and HPV infection in Indonesian women with a clinically normal cervix, contextualized within the Sustainable Development Goals (SDGs). Methods: This prospective cohort study utilized primary data from structured interviews and gynecological exams conducted by the Female Cancer Program (FCP) team across public and private health providers in Jakarta (January 2012–July 2018). Cervical cancer risk factors, including early sexual debut, were analyzed using odds ratios (ORs). Results: A total of 1,390 women with a clinically normal cervix were enrolled in this study, with 52 (3.7%) testing positive for HPV-DNA. Statistical analysis indicated an odds ratio (OR) of 1.176 (95% CI: 0.598–2.316, p = 0.601) for early sexual debut. Conclusion: This study showed that there was an insignificant association between sexual debut and HPV infection in clinically normal cervix. According to the SDGs, early sexual debut and cervical cancer are highly associated. In order to meet the SDGs by 2030, more research is required to validate this finding and assess additional co-factors connected to cervical cancer.

Abstract

Introduction: Among the many risk factors for stress in the workplace, bullying is a factor that is rarely discussed. As many as 19% of people in America experience bullying at work, followed by other European countries. Bullying is a strong link to worker stress, especially in blue-collar workers. Gender differences, especially in women, also have an important prevalence in workplace bullying. In Indonesia itself, there is no data regarding bullying in the workplace, especially among women and blue-collar workers. Objective: This study aims to find the relationship between bullying and stress in the workplace among female blue-collar workers. Method: This research is a cross-sectional analytical study using a negative act questionnaire-revised (NAQ-R) questionnaire on work stress in female blue collar workers in garment factories, in Bekasi city, Indonesia. Job stress assessment uses the Perceived Stress Scale-10 (PSS-10) questionnaire. Other variables assessed are age, salary, marital status, and subjective questions related to workload and work communication. Results: A total of 379 female blue collar workers in garment factories, in the city of Bekasi, Indonesia took part in this research. Workplace bullying is associated with job stress in female blue-collar workers. Workers who are often bullied tend to have medium-high levels of work stress. Bullying is the type of bullying with the highest scale. Conclusion: Workplace bullying is the factor that has the most influence on work stress in female blue-collar workers. Then, high workloads, poor communication, and lower salaries are related to bullying and stress in the workplace.

Abstract

Introduction. In Indonesia, burn injuries cause about 195,000 deaths annually. Data from the Ministry of the Health Republic of Indonesia showed the incidence of burns predominated at 1-4 years old. The mortality of pediatric burn patients in a tertiary hospital was 37.26%. This study aimed to find an association between known and unknown prognostic factors of mortality in Indonesian-specific characteristics. Method. A retrospective analytical study included all pediatric burns admitted to Dr. Cipto Mangunkusumo General Hospital (CMGH) from 1998 to 2010. Variables within a period of the first 72 hours of admission were the focus of interest and were extracted from the medical record. Results. Of 609 pediatric burns, the mortality rate is 37.8%. Some contributing variables significantly associated with the mortality were TBSA, inhalation injury, length of hospitalization, hemoglobin 0-h level, hematocrit 24-h, and 48-h level, INR 0-h, and 48-h, fluid balance 24-h, base deficit, serum lactate, pulmonary edema, systemic inflammatory response syndrome (SIRS) + multiorgan failure (MOF), and acute coronary syndrome (ACS) (p <0.05). On multivariate analysis, the significant variable was length of hospitalization <14 days, SIRS+MOF, abnormal hematocrit 0-h level, and abnormal serum lactate level. Conclusion. The more identified prognostic factors a patient finds, the more the mortality risk. In addition, excessive fluid resuscitation leads to a high likelihood of pulmonary edema, SIRS+MOF, and ACS complications, followed by increased mortality risk

Abstract

Introduction. The strategy for treating limb soft tissue sarcoma (STS) is challenging due to delayed diagnosis and the non-specific clinical picture, leading to the mortality of nearly 50% of newly diagnosed patients. Various therapeutic modalities have been applied to increase the survival of patients with extremity soft tissue sarcoma. However, clinicopathological factors can influence the survival rate and thus affect the effectiveness of therapy. This study aims to determine the numbers and factors influencing overall survival five years after treatment for patients with soft tissue sarcoma of the extremities at Dr. Cipto Mangunkusumo General Hospital (CMGH) in 2011-2015. Method. A prognostic study was run on patients with complete histopathology who underwent treatment during 2011-2015. Data analysis using the Kaplan-Meier method, cox regression test, and Cox regression with time-dependent variable. Results. A total of 42 were enrolled in the study. The median survival after therapy for patients was six years (3 months – 8.25 years), with a five-year survival of 52.4%. Factors affecting five-year survival were surgical procedures in limb-saving surgery (HR 0.852 95% CI 0.68 – 1.07, p = 0.163). Conclusion. The five-year survival rate of soft tissue sarcomas of the extremities was 52.4%. Overall survival is affected by higher-grade sarcoma, incomplete therapy, and worse clinical stage.

Abstract

Parents play an important role in their children's sexual health education. The purpose of this study was to examine the materials or topics that parentsconvey when providing sexual health education to their children. The study design was an integrative review using the PRISMA flow diagram withthe CoCoPop framework. The databases used were Cochrane, Scopus, and EBSCO Medline. The inclusion criteria for articles were: abstract andfull text, qualitative, quantitative, and mixed- method research journal articles, English language, published between 2020 and 2024, and a population of parents with children aged 0 to 18 years. The JBI tool was used to evaluate the articles. The results of the study found nine articles, including three quantitative articles, four qualitative articles, and two mixed-method articles. The conclusion of this integrative review is that eachparent conveys different topics or materials, depending on the knowledge, abilities, and stigma of the parents and children regarding sex education. The materials most frequently discussed by parents were puberty, safe touching, unsafe pregnancy and abortion, premarital sex, contraception, sexually transmitted infections, reproductive anatomy, and protection from sexual violence. The recommendation from this study is that there is a need for clear guidelines on age- appropriate sexual health education that can be used by parents from various backgrounds.
