Abstract

Low back pain is one of the musculoskeletal disorders often associated to work. Based on previous research, 70.3% low back pain was found in manual handling workers and 22.98% in female nurses in hospital wards. The effects of low back pain are disability, decreased productivity and attendance rate, and loss of working hours. Returning to work as soon as possible after illness in a certain capacity is a highly recommended action. Until now in Indonesia there is no data on the length of time return to work on workers with low back pain and related factors. This study used cross-sectional design to determine the length of time return to work   and its relation to individual factors, work, and work environment. It used the Oswestry questionnaire and the population is workers who have low back pain and treated in a hospital, given sick leave, by the doctor as outpatient or inpatient. Workers with low back pain treated at the hospital (November 2016-April 2017), obtained a doctor's rest and or hospitalization letter amounting to 40 people (total sampling). The average value of the duration of return to work is 3 days (1-15 days) although clinical complaints have not fully disappeared. Factors relating to the length of the return to work were age (OR 9,71, 95% CI 1.08-87.31) and disability level before treatment (OR 2.50, 95% CI1.25-4.99). Length of time return to work due to low back pain is 3 days and has association  to age and disability level.

Abstract

Background: The COVID-19 pandemic has altered the pattern of human life toward a new life order,particularly in the office setting. Appeals, prohibitions, even large-scale social restrictions were implemented in order to decrease the cases. To cope with the rapid spread of the outbreak, citizens have to increase health consciousness and reduce their social interactions. The health consciousness require health protocols implementation. Ensuring high levels of compliance and several modifiable occupational risk factors in the office setting has been a challenge to manage the pandemic effectively. Methods: The research method used in this study is empirical research, by taking a problem-based approach to a behavioral approach namely health consciousness supported by primary data sourced from data sources. The data were obtained in October to November 2021. Results: When taken as a whole, the implementation of health protocols as part of the effort to prevent the spread of COVID-19 was relatively good; it's just that certain obstacles must be evaluated. The highest health consciousness scores were personal responsibility and health motivation. While the lowest of it were the psychological/inner state aspect and the health information and seeking usage. There was a positive correlation between the dimensions of health consciousness and several items of COVID-19 PHL (r > 0.3). The ease of applying COVID-19 PHL was significantly better for shift workers compared to non-shift workers, namely to severalitems of PHL. Based on the work pattern groups (WFH < 20 hours, WFH 20-40 hours and WFH > 40 hours, alternate to Work From Office (WFO)), there were no significance difference in the ease of implementation COVID-19 Preventive Healthy Lifestyle. Conclusion: Our results suggest that to effectively manage the pandemic, we should ensure that health consciousness is well-maintained, especially aspects of health information and seeking usage and psychological/inner state. Abstract

Background: The incidence of chronic kidney disease is increasing with a fairly high annual death rate which is a serious health problem in the world because it has increased incidence, has an impact on morbidity and mortality as well as a serious socio-economic world including in Indonesia with the majority of the working age population in the manufacturing industry. Therefor an early detection of decreased kidney function and factors that influence it become necessity. Methods: This research is analytical quantitative research with a cross-sectional design using secondary data of MCU in Occupational Health and Safety Service Company in 2022 with total 2,304 data. The data includes of eGFR as dependent variable and individual risk factors, history of illness risk factors, clinical risk factors and occupational risk factors as independent variable.Result: The results of the study suggest there was a decrease in kidney function (Glomerular Filtration Rate / GFR <90 ml/minute/1.73m2) of 33,8% of the total manufacturing industry workers in West Java in 2022. The decrease of eGFR is influenced by individual, clinical and occupational risk factors.Conclusion: The conclusion, risk factors that influence the incidence of decreased renal function in manufacture industry workers in West Java in 2022 are male, age over 40 years old, conditions of grade I and II hypertension, obesity and periodes of work of more than 19 years, with the most dominant influence factor is male gender.

Abstract

Background: Exposure to noise in workplace is the leading cause of increased hearing threshold and hearing loss among factory workers. There are other factors other than noise than can cause increased hearing threshold such as age, length of work, history of hypertension, diabetes, dyslipidemia, Body Mass Index and smoking habit. PT K is a company that produce heavy machinery parts for construction where workers are exposed to noise above Threshold Limit Value (TLV).Methods: A cross-sectional research of 965 employees was conducted after passing the ethical evaluation based on the 2022 medical check-up results. Samples were taken from total secondary data from the company’s medical records and data verification was done to eliminate duplication and confirm data validity. Univariate, chi-square for bivariate analyses were performed.Result: The prevalence of increased hearing threshold among workers is 39.6%. The factors that associated with the prevalence of increased hearing threshold were determined to be older age group (p<0.001), history hypertension (p=0.003), dyslipidemia (p=0.008) and overweight – obese Body Mass Index (p=0.002).Conclusion: Older age group, length of work, works departments, history of high blood pressure, dyslipidemia and overweight – obese Body Mass Index were significant risk factors associated with increased hearing threshold. The risk of increased hearing threshold is 3.2 times higher in workers above 36 years old. This indicate the importance of preparing better mitigation strategy of noise control and hearing conservation program, particularly in workers older than 36 years old.

Abstract

Introduction: Among the many risk factors for stress in the workplace, bullying is a factor that is rarely discussed. As many as 19% of people in America experience bullying at work, followed by other European countries. Bullying is a strong link to worker stress, especially in blue-collar workers. Gender differences, especially in women, also have an important prevalence in workplace bullying. In Indonesia itself, there is no data regarding bullying in the workplace, especially among women and blue-collar workers. Objective: This study aims to find the relationship between bullying and stress in the workplace among female blue-collar workers. Method: This research is a cross-sectional analytical study using a negative act questionnaire-revised (NAQ-R) questionnaire on work stress in female blue collar workers in garment factories, in Bekasi city, Indonesia. Job stress assessment uses the Perceived Stress Scale-10 (PSS-10) questionnaire. Other variables assessed are age, salary, marital status, and subjective questions related to workload and work communication. Results: A total of 379 female blue collar workers in garment factories, in the city of Bekasi, Indonesia took part in this research. Workplace bullying is associated with job stress in female blue-collar workers. Workers who are often bullied tend to have medium-high levels of work stress. Bullying is the type of bullying with the highest scale. Conclusion: Workplace bullying is the factor that has the most influence on work stress in female blue-collar workers. Then, high workloads, poor communication, and lower salaries are related to bullying and stress in the workplace.

Abstract

ABSTRACTBackground: In Indonesia, no research has been carried out or reported on pharyngeal transit time and a view of the swallowing process in a sample without complaints of dysphagia using the Flexible Endoscopic Evaluation of Swallowing (FEES) method and using five food consistencies. Purpose: To obtain the value of pharyngeal transit time and a view of the swallowing process in subjects without dysphagia problems. Method: Twenty-eight subjects without dysphagia (based on Ohkuma’s Dysphagia Screening) underwent FEES to determine pharyngeal transit time and dysphagia profile based on leakage, standing secretion, residue, penetration, and aspiration. Result: The median value of pharyngeal transit time on puree consistency was 0.799(0.499-5.666), gastric rice 0.966(0.433-5.733), oatmeal 0.849(0.533- 4.399), thick liquid 0.982(0.399-4.633), thin liquid 0.566(0.366-1.366). The pharyngeal delay time on the puree consistency was 0.566(0.199-5.333), gastric rice 0.799(0.233-2.799), oatmeal 0.899(0.099-3.633), thick liquid 0.833(0.033-3.733), and thin liquid mean was 0.294 (± 0.232). The pharyngeal response time on the puree consistency was 0.566(0.167-1.300), gastric rice 0.583(0.300-2.934), oatmeal 0.583(0.367- 1.233), thick liquid 0.549(0.333-1.533), thin liquid 0.549(0.366-1.399). There was no standing secretion, pre-swallowing leakage, penetration, and aspiration found in FEES. A grade 1 residue was found in 3(10.7%) subjects of puree, in 2(7.2%) subjects of gastric rice, in 3(10.7%) subjects of oatmeal, and in 9(32.2%) subjects of thick liquid. Conclusion: There was no prolonged pharyngeal transit time, pre-swallowing leakage, standing secretion, penetration, and aspiration in all subjects without dysphagia complaints at all food consistencies. There was minimal residue within normal limits in some subjects. ABSTRAKLatar belakang: Di Indonesia, belum pernah dilaporkan penelitian mengenai waktu transit faring dan gambaran proses menelan pada sampel tanpa keluhan disfagia dengan menggunakan metode Flexible Endoscopic Evaluation of Swallowing (FEES) dan menggunakan lima konsistensi makanan. Tujuan: Penelitian ini bertujuan untuk memperoleh nilai waktu transit faring dan gambaran proses menelan pada subjek tanpa masalah disfagia. Metode: Dua puluh delapan subjek tanpa disfagia (berdasarkan Skrining Disfagia Ohkuma) menjalani FEES untuk menentukan waktu transit faring dan profil disfagia berdasarkan leakage, standing secretion, residue, penetration, dan aspiration. Hasil: Nilai median waktu transit faring pada konsistensi puree adalah 0,799(0,499-5,633), gastric rice 0,966(0,433-5,733), oatmeal 0,849(0,533-4,399), thick liquid 0.982(0.399-4.633), thin liquid 0.566(0.366- 1.366). Waktu tunda faring pada konsistensi puree adalah 0,566 (0,199-5.333), gastric rice 0,799 (0,233-2,799), oatmeal 0,899 (0,099-3,633), thick liquid 0,833 (0,033-3,733), dan rerata thin liquid 0,294 (± 0,232). Waktu respon faring terhadap konsistensi puree adalah 0,566 (0,167-1,300), gastric rice 0,583 (0,300-2,934), oatmeal 0,583 (0,367-1,233), thick liquid 0,549 (0,333-1,533), thin liquid 0,549 (0,366-1,399). Tidak didapati adanya leakage, standing secretion, penetration, dan aspiration pada pemeriksaan FEES. Residu grade 1 ditemukan pada 3 (10,7%) subjek puree, pada 2 (7,2%) subjek gastric rice, pada 3 (10,7%) subjek oatmeal, dan pada 9 (32,2%) subjek thick liquid. Kesimpulan: Tidak terdapat perpanjangan waktu transit faring, leakage, standing secretion, penetration, dan aspiration pada semua subjek tanpa keluhan disfagia dengan semua konsistensi makanan. Terdapat residu minimal dalam batas normal pada beberapa subjek.

Abstract

Background: E-learning readiness became a new workload for teachers during the Covid-19 pandemic because of the obligatory online teaching. E-learning readiness comprises four factors: the readiness of technology, innovation, people, and self-development; and it might cause job stressors and affect teachers' performance. The research objective was to explore job stressors that influence teachers the most and how they might be associated with E-learning readiness. Method: The cross-sectional method assessed 186 teachers from public and private elementary schools in the urban city area in Jakarta. The data obtained from questionnaires: Teacher Stress Inventory (TSI) Boyle and E-learning Readiness (ELR) Aydin and Tasci. TSI assessed teachers' job stressors in five main components: workload, student behavior, professional recognition, time and resources, and interpersonal relationships. Results: Most teachers perceived readiness at e-learning (84.9%). E-learning readiness was a profound problem for female and non-classroom teachers. The dominant job stressors for teachers were workload (27%), professional recognition (22%), time and resources (21%), and student behavior (21%). The number of students influenced workload and professional recognition. Self-development was the prominent e-learning readiness factor associated with teacher job stressors, especially professional recognition. Conclusions: In urban areas like Jakarta, fifteen percent of teachers remain unprepared for e-learning teaching. The unpreparedness was related to the female gender and non-classroom teachers, who need e-learning skills training to improve their readiness and control job stressors for better teaching performance in a prolonged pandemic situation.

Abstract

Objective: To describe the prevalence of HPV infection in women with negative Visual Inspection with Acetic Acid (VIA) and introduce Teleconsultation of Documented VIA (TeleDoVIA) as an objective test and provide a rationalization for recommending TeleDoVIA as a “high-performance” test for cervical cancer screening in lower resource settings, from SDGs perspective, to accelerate the achievement of second pillar elimination and the third SDGs target in 2030. Methods: This is a 7-year cross-sectional study. Subjects were recruited consecutively from several public and private health providers in Jakarta. VIA test was documented and consulted to the experts panel (TeleDoVIA). Negative VIA women underwent HPV-DNA testing using SPF10-DEIA-LiPA25 for PCR and electrophoresis. Results: A total of 1,397 negative VIA subjects were collected, consist of 52 HPV-DNA positive. False-negative of VIA was 3.7% (95% CI 0.027–0.047). Conclusion: VIA is a reliable screening method with a low false-negative rate. TeleDoVIA could be recommended as a reliable cervical cancer screening method in low resource settings such as Indonesia, which is in line with the third SDG: good health and well-being. Abstract

Understanding the significance of handgrip strength is essential for identifying frailty in heart failure patients. The aim of this study was to identify the association between handgrip strength and cardiorespiratory endurance while highlighting the importance of the musculoskeletal system in cardiac rehabilitation for patients with heart failure. An observational cross-sectional study was conducted at Harapan Kita Hospital, Jakarta, Indonesia, from April 2022 to April 2023, among patients with heart failure with reduced ejection fraction (HFrEF) attributed to cardiomyopathy or coronary artery disease. Patients were classified by a 6-minute walk test (6MWT) distance into <400 meters (low endurance) or ≥400 meters (high endurance). The short physical performance battery (SPPB), handgrip strength, ultrasonographic forearm muscle thickness, left ventricle ejection fraction, tricuspid annular plane systolic excursion, and N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels were measured. Results indicated significant differences in non-dominant handgrip strength, gait speed, and sit-to-stand SPPB scores between patients achieving a 6MWT distance of ≥400 meters and those below this threshold, with values of 31.11±6.88 kg vs 27.66±6.66 kg (p=0.049), 0.52±0.08 m/s vs 0.61±0.13 m/s (p=0.001), and 10.71±2.47 seconds vs 12.85±4.11 seconds (p=0.014), respectively. Stronger non-dominant handgrip strength (>30 kg) was associated with higher endurance (odds ratio (OR): 3.80; 95%CI: 1.35–10.67; p=0.010) and thicker forearm muscles (>1.9 cm) as measured by ultrasonography (AUC: 0.713; 95%CI: 0.585–0.840, p=0.001). In conclusion, a cut-off of ≤30 kg for non-dominant handgrip strength could effectively stratify the male patients into a lower endurance group (6MWT ≤400 meters), which is associated with elevated NT-proBNP levels and reduced forearm muscle thickness.

Abstract

Introduction Heart failure with a reduced ejection fraction (HFrEF) significantly contributes to global morbidity and mortality, necessitating effective rehabilitation programs. Exercise-based rehabilitation improves functional capacity and quality of life in HFrEF patients, though responses vary. The tailored BEST (Breathing, Endurance, and Strengthening) exercise protocol addresses both cardiac and extracardiac rehabilitation, benefiting all patients regardless of response status. This study evaluated the protocol's effects on HFrEF patients and classified responses based on VO2max changes. Methods In this etiologic study with prospective cohort design, all participants underwent a three-month cardiac rehabilitation program using the BEST Exercise Protocol. Assessments included the 6-minute walk test (6MWT), short physical performance battery (SPPB), handgrip strength, chest expansion, ultrasonographic measurements, and NT-proBNP levels before and after the intervention, with statistical comparisons made within and between groups. Groupings of responder level will be reliant on 6MWT distance achievement at the end of the program, with ≥6% improvement classified as good responders. Results Out of 107 HFrEF patients (median age 55 years, ejection fraction 29.50±7.34%), 63.56% were good responders and 36.44% were poor responders (<6% improvement). Good responders showed significant improvements in most extracardiac parameters, including a 20% increase in 6MWT distance (470.96±69.21 meters post-rehabilitation), chest expansion, handgrip strength, and SPPB scores (p<0.001 for all). Poor responders also improved in chest expansion, sit-to-stand time, and postural balance, with minor 6MWT gains (407.33±72.50 meters). NT-proBNP levels decreased in both groups but were not statistically significant (p=0.288 and 0.368 for good and poor responders, respectively). Conclusion The tailored BEST Exercise Protocol offers substantial cardiac and extracardiac benefits for HFrEF patients by enhancing functional capacity and muscle strength. Both good and poor responders exhibited significant improvements, indicating the protocol's broad applicability. However, the lack of statistically significant NT-proBNP reduction suggests further studies on cardiac biomarkers are needed.
