Abstract

Background: The cause of chordee in hypospadias is not fully understood. Dartos fascia resection has been shown to correct penile curvature, suggesting its potential involvement. This study aimed to evaluate the histopathological properties of dartos tissue and their association with hypospadias severity and chordee.Methods and Materials: Dartos tissue samples were collected from 37 children with hypospadias and ten controls undergoing circumcision between March and October 2024. Histopathological analysis using hematoxylin-eosin and Masson’s trichrome staining assessed for collagen, smooth muscle, and vascular density. Assessment and quantification of all histopathological components were blinded. Results: The hypospadias group showed lower collagen density (60.85±6.75% vs. 67.31±4.61%; p=0.003) and higher smooth muscle density (7.34±4.92% vs. 2.51±2.24%; p<0.001) compared to controls, with no significant difference in vessel density. Sub-analysis showed severe chordee was linked to lower collagen density compared to mild (Mean Difference [MD] -6.24±2.41%; p=0.014) and moderate chordee (MD -5.73±2.54%; p=0.031). Conclusions: The dartos tissue in hypospadias patients exhibited lower collagen density and higher smooth muscle density, with chordee severity specifically linked to decreased collagen density.

Abstract

Introduction: Heart failure with reduced ejection fraction (HFrEF) is well-known as a systemic disease that involves cardiac and extracardiac issues, with respiratory function playing on of the key role in rehabilitation prognosis. Biomarkers such as soluble suppression of tumorigenicity 2 (sST2), myostatin, miRNA-133, and NT-proBNP indicate disease progression. Notably, sST2, which is also produced by the lungs, predicts heart failure outcomes. This study examines the relationship between comprehensive respiratory assessments (e.g., diaphragmatic ultrasonography, spirometry) and intra-extracardiac biomarkers to improve rehabilitation strategies. Methods: Sixty-nine HFrEF patients underwent respiratory evaluations, including diaphragmatic ultrasonography, spirometry, chest expansion measurements, and a six-minute walking test (6MWT). Biomarkers assessed were sST2, myostatin, miRNA-133, and NT-proBNP. Associations between respiratory parameters and biomarkers were analyzed using t-tests and correlation analyses. Results: The median age was 56 years, and 33 (47.82%) of the subjects had diaphragmatic dysfunction, resulting in poorer 6MWT performance (378.03±58.15 m vs 409.75±63.65 m, p=0.017) and other parameters. Superior chest expansion negatively correlated with sST2 (r=−0.387, p=0.001) and positively with miRNA-133 (r=0.442, p<0.001). Similar results were found for inferior chest expansion. No significant correlations were observed for other biomarkers. Conclusion: This study highlights strong associations between chest expansion and sST2/miRNA-133, suggesting that incorporating respiratory assessments and training into HFrEF rehabilitation could enhance outcomes by addressing cardiorespiratory insufficiencies. Given sST2's predictive value for heart failure prognosis, these findings support a multi-component rehabilitation strategy incorporating respiratory training, such as aerobic and inspiratory muscle exercises, to enhance cardiopulmonary outcomes. This integrated approach offers promise for future HFrEF rehabilitation protocols.

Abstract

BACKGROUND Open reduction and internal fixation are considered the gold standard for managing mandibular fractures, as they effectively restore anatomical structure and functionality. Three-dimensional (3D) miniplates were developed to obtain a geometrically stable structure. However, the rigid structure of 3D miniplate may cause screw holes to be located within the fracture lines or directly above important anatomical structures. Hence, we have developed customizable interlocking 3D miniplates that allow for modifications in the configuration of the horizontal miniplates and vertical cross-struts to meet specific requirements. METHODS This study included 24 goats divided into 2 groups: interlocking 3D miniplate and standard plate. Each group was further divided into subgroups sacrificed on Days 5, 28, and 56. Radiological assessments were performed to evaluate bone, muscle, peri-implant bone, and peri-implant muscle density. Histological assessments were performed using Masson's trichome (MT) for bone healing evaluation and hematoxylin and eosin (H&E) staining for biocompatibility. RESULTS Radiological assessment indicated an increase in density across all measured parameters from Day 5 to 56. Histological evaluation from MT staining showed a significantly higher fibrocartilage in the interlocking 3D miniplate group (6.109%, IQR 1.453–10.828) compared to standard plate (1.311%, IQR 0.636–2.553; p = 0.001). Furthermore, H&E staining showed significantly higher inflammatory cell counts in the standard group on Days 28 and 56 compared to the interlocking 3D miniplate group (p = 0.027). CONCLUSIONS This study concluded that the interlocking 3D miniplate showed better bone healing properties and biocompatibility than the standard plate.

Abstract

BACKGROUND A notable rise in sexually transmitted infection (STI) cases among youth correlates with a lack of reproductive health awareness, affecting nearly 80% of Indonesian adolescents. Additionally, sociocultural taboo acts as a challenge to reproductive health implementation in Indonesia. Therefore, this study aimed to develop an online reproductive health module focused on STIs for Indonesian adolescents. METHODS This research was a mixed methods approach, integrating quantitative study using univariate analysis (survey and questionnaire) and qualitative study using the Delphi method (open discussion and focus group discussion). The study was conducted between March 2 and July 28, 2022. We began by conceptualizing a literature-based module, an online survey, and virtual discussions involving students, teachers, and parents. Then, we consulted with clinical psychologists and dermatovenereologists. Additionally, students completed an assessment to establish a baseline of their knowledge, attitudes, and behaviors (KAB) regarding reproductive health and STIs. RESULTS Of 327 high school students across Jakarta, analysis of the 129 completed KAB surveys showed that only 56.6% and 65.1% attained acceptable scores (>65) for knowledge and behavior, respectively, with 24.8% being at risk for STIs. Key recommendations included incorporating multimedia elements, ensuring clarity and simplicity in language, adopting non-judgmental tone, providing comprehensive explanations of STIs and their long-term impact, and supporting learning with face-to-face sessions and ongoing access to concise, cost-effective educational resources. CONCLUSIONS Adolescents need a comprehensive, interactive, instructive, accessible, non-discriminatory reproductive module that strengthens the self-concept that upholds Indonesian values and norms.

Abstract

Cardiopulmonary bypass in tetralogy of Fallot (TOF) corrective surgery induces hyperinflammation by activating NLRP3, caspase-1, and interleukin-1β (IL-1β), subsequently triggering an interleukin-10 (IL-10) response. Despite its known metabolic and anti-inflammatory effects, the impact of the modified Atkins diet (MAD) in pediatric cardiac surgery remains unexplored, with no studies on its use in TOF patients undergoing open-heart surgery. The aim of this study was to assess the effect of MAD on the expression of NLRP3, caspase-1, IL-1β, and IL-10, in TOF patients undergoing open-heart surgery. A double-arm, randomized-controlled trial was conducted with 44 TOF patients. The treatment group (n=22) received the MAD, a low-carbohydrate, high-fat regimen with unrestricted fat and protein intake for at least 14 days preoperatively, while the control group (n=22) followed a standard diet without carbohydrate restriction. Blood plasma and infundibulum heart tissues were collected for analysis. Whole blood samples were collected using a winged infusion needle before the intervention, an Abbocath infusion needle after 14 days of intervention, and a syringe without a needle connected to an arterial line in patients undergoing open-heart surgery at 6, 24, and 48 hours post-surgical correction. Infundibulum heart tissues were collected during the open-heart surgery. This study demonstrated significant differences in NLRP3 protein expression (p=0.015), caspase-1 protein expression (p=0.001), and IL-10 levels between before intervention and 6-, 24-, and 48-hours post-surgery in the MAD group compared to the control group. In contrast, no significant differences in IL-10 levels were observed in the control group between before intervention and 48 hours post-surgery (p=0.654). In conclusion, MAD may modulate perioperative inflammation in TOF patients undergoing open-heart surgery by downregulating NLRP3 and caspase-1 expression while sustaining IL-10 levels. Despite reduced NLRP3 and caspase-1 expression, unchanged IL-1β levels indicate alternative regulatory mechanisms.

Abstract

N-acetylcysteine (NAC) is known to enhance neuroplasticity and help reduce smoking addiction by modulating brain metabolites. The use of magnetic resonance spectroscopy (MRS) in smokers receiving NAC as an adjuvant to motivational enhancement therapy (MET) represents a novel approach to understanding how this combination therapy influences brain chemistry. By utilizing MRS, the effectiveness of NAC can be quantitatively assessed by analyzing changes in smoking-affected brain metabolites. The aim of this study was to evaluate the efficacy of NAC combined with MET for nicotine addiction, using MRS to assess neurochemical alterations associated with treatment response. A stratified, randomized, parallel-group clinical trial was conducted, comparing NAC and MET combination to MET only among smokers. The study analyzed the effectiveness of NAC by evaluating glutamate-glutamine (Glx) to creatine ratio (Glx/creatine ratio) and N-acetylaspartate (NAA) to creatine ratio (NAA/creatine ratio) in the nucleus accumbens, bilateral cerebellum, medial prefrontal cortex, ventromedial prefrontal cortex, and bilateral precuneus. Our data indicated that the Glx/creatine ratios for the intervention versus control groups were as follows: nucleus accumbens (0.68 vs 0.43), bilateral cerebellum (0.68 vs 0.43), left medial prefrontal cortex (1.11 vs 0.82), ventromedial prefrontal cortex (0.32 vs 0.86), and bilateral precuneus (0.75 vs 0.58). The NAA/creatine ratios for the intervention versus control groups were as follows: nucleus accumbens (3.55 vs 8.35), bilateral cerebellum (7.82 vs 4.02), left medial prefrontal cortex (5.47 vs 5.20), ventromedial prefrontal cortex (3.55 vs 7.46), and bilateral precuneus (4.73 vs 4.00). Our analysis indicated that the Glx/creatine ratio was higher in the intervention group than in the control group in the medial prefrontal cortex (p=0.02), while the NAA/creatine ratio was higher in the intervention group than in the control group in the bilateral cerebellum (p<0.001). The reported side effects were mild to moderate discomfort and well-tolerated across both groups. These findings highlight the potential of NAC and MET combination in promoting neuroplasticity and supporting nicotine addiction treatment.

Abstract

Introduction: Military pilots require good cardiorespiratory fitness due to the need to perform tasks in difficult and varied environmental conditions. Cardiorespiratory fitness is usually expressed in terms of maximal oxygen consumption (V̇O2 max). It is important to know the factors associated with V̇O2 max in order to maintain the fitness of Indonesian Air Force pilots. This study was conducted to assess the association between body fat percentage with V̇O2 max of Indonesian Air Force pilots.Methods: A cross-sectional study was conducted on active Indonesian Air Force pilots who carried out periodic medical examinations from October to November 2022 at Lakespra dr. Saryanto, Jakarta. The research subjects underwent body composition examinations using Bioelectrical Impedance Analysis (BIA), and V̇O2 max assessment was conducted using the Bruce protocol. The association between body fat percentage and V̇O2 max value was assessed by Pearson’s correlation test.Results: Of the 64 subjects analysed, the average value of V̇O2 max was 44.6±6.2 ml/kg/min and the average body fat percentage was 23.4±5.0 %. There is a significant correlation between body fat percentage and V̇O2 max. (r=-0.334; p=0.007)Conclusion: There is a significant negative correlation with moderate strength between the body fat percentage and the V̇O2 max of Indonesian Air Force pilots who carried out medical examination at Lakespra dr. Saryanto.

Abstract

Background Correct complementary feeding practice can be a challenge for mothers. Our complementary feeding practice module provides practical information regarding evidence-based complementary feeding practices. Objective To assess the effect of educational intervention using the complementary feeding practice module on maternal knowledge and behavior and to compare the effect of video vs. printed delivery of the module. Methods This open label randomized controlled trial included mothers with babies aged 4 to 8 months in Pontianak, West Kalimantan, Indonesia, who engaged in complementary feeding. Subjects were randomized into an intervention group that received a printed booklet of the module, an intervention group that received a video of the module, or a control group that received the standard Indonesian mother and child health (MCH) handbook. Maternal knowledge and behavior regarding complementary feeding practice were assessed before and after the educational intervention using the PI-MPASI questionnaire. Results Of 360 mothers screened, 241 subjects were randomized into the video education group (n=76), the printed education group (n=84), and the control group (n=81). A total of 212 subjects completed the study by submitting both the pre- and post-test: 66 in the video group, 74 in the printed booklet group, and 72 in the control group. Subjects who received the module (video or printed) had significantly higher knowledge and behavior scores than those in the control group. Video education was associated with a greater increase in behavior score than printed education and control. The outcome of video education was not influenced by maternal educational level. Conclusion  Education using a simple and practical video or printed module improves maternal knowledge and behavior in complementary feeding practices. Video education is associated with a greater improvement in maternal complementary feeding behavior than printed education.

Abstract

Pilot errors accounts for 80% of accidents and 50% of serious incidents, thus identifying the risk factors is crucial to mitigation strategies. The research aimed to develop a model of pilot errors’ risk factors identification and mitigation by conducting a Focus Group Discussion (FGD) with stakeholders in Indonesia. This was a qualitative analysis by conducting hybrid FGD, divided into four groups of experts consisted of officials from the Directorate General Civil Aviation, human factors experts, active pilots and investigators from the National Transport Safety Committee, from 28 to 29 August 2024. The data was collected by transcribing verbatim transcript, then analysed using the Steps for Coding and Theorization and Content Validity Index. model was developed, consisted of pilots’ risk factors, divided into three themes, 16 sub-themes, grouped into internal and external factors, which can be identified by four activities and can be mitigated by three activities. The factors contributed to aviation safety are multifactorial and interrelated. The complexity of pilots’ risk factors involves a continuous assessment of identification and mitigation, emphasizes the adaptive collaboration among stakeholders. A significant contribution in aviation human factors data for the Safety Management System implementation in Indonesia.

Abstract

Background: Obesity is a complex public health issue influenced by multiple determinants, including environmental factors that promote excessive calorie intake and physical inactivity, known as obesogenic environments. Objective: This literature review is to broaden understanding the environmental determinants of obesity in Indonesia. Method: A narrative literature review was conducted focusing on studies published between 1999 and 2024. Sources were retrieved from databases including PubMed, WHO Global Health Observatory, and national health surveys such as Riskesdas. The ecological framework by Swinburn et al. (1999) was used to analyse environmental influences on obesity. Results: Findings indicate that built and food environments play significant roles in shaping obesity patterns in many places. Studies show increasing obesity prevalence in both urban and rural Indonesia, with environment-related factors such as food availability, urbanization, and reduced physical activity opportunities contributing to this trend. Conclusion: Food environment, land use and physical infrastructure were identified as potential proxies for obesogenic environments in Indonesia. Further studies are needed to evaluate and validate these proxies to support the development of environments conducive to healthy lifestyles and to reduce obesity prevalence.
