Abstract

Puasa Ramadhan dapat meningkatkan kebahayaan komplikasi di pasien Diabetes Melitus (DM) tipe 1, yang dapat dicegah dengankendali glikemik yang baik. Pemeriksaan fruktosamin dan albumin glikat digunakan untuk menggambarkan rerata kadar glukosa darahselama 2-3 minggu sebelumnya, sehingga lebih sesuai digunakan untuk menggambarkan kendali glikemik selama puasa Ramadhan(1 bulan). Tujuan penelitian ini adalah membandingkan dan membuktikan adanya kenasaban antara kadar fruktosamin dengan nilaialbumin glikat sebelum, pertengahan dan akhir bulan puasa Ramadhan di pasien DM tipe 1 di RSUD dr.Soetomo Surabaya. Penelitianini menggunakan desain analitik observasional di 13 pasien DM tipe 1 berusia 9–18 tahun yang menjalankan puasa Ramadhan.Pemeriksaan fruktosamin menggunakan metode nitroblutetrazolium (NBT) dengan alat Cobas Integra. Nilai albumin glikat dihitungsebagai persentase kadar albumin glikat (menggunakan metode enzimatik) terhadap kadar jumlah keseluruhan albumin serum(menggunakan metode bromcresol purple), dengan alat Proline R-910. Hasil dianalisis menggunakan uji statistik t-berpasangan dankenasaban Pearson. Tidak didapatkan perbedaan bermakna antara rerata kadar fruktosamin pertengahan dibandingkan sebelum(p=0,307), akhir dibandingkan sebelum (p=0,249) dan akhir dibandingkan pertengahan bulan puasa Ramadhan (p=0,362). Tidakdidapatkan perbedaan bermakna antara rerata nilai albumin glikat pertengahan dibandingkan sebelum (p=0,478), akhir dibandingkansebelum (p=0,285) dan akhir dibandingkan pertengahan bulan puasa Ramadhan (p=0,247). Kenasaban positif bermakna didapatkanantara kadar fruktosamin dan nilai albumin glikat sebelum (p=0,0001, r=0,952), pertengahan (p=0,0001, r=0,948) dan akhir bulanpuasa Ramadhan (p=0,0001 dan r=0,963). Kadar fruktosamin dan nilai albumin glikat dapat digunakan sebagai tolok ukur kendaliglikemik di pasien DM tipe 1 yang menjalankan puasa Ramadhan.

Abstract

The determination of thyroxin (T4) is known as a good indicator to know the condition of thyroid function. In the hyperthyroid state, are shown clearly the increased levels of T4, while in the hypothyroid state their levels always decrease. The T4 levels change due to the physiological and pathological conditions on the ability of thyroxin binding globulin (TBG). The T4 measurement can be performed using an enzyme-linked fluorescent immunoassay (ELFA) or enzyme-linked immunosorbant assay (ELISA). Both ELFA and ELISA can detect T4 antigen. In these research fifty one randomized samples sera from the Clinical Pathology laboratory, at Dr. Soetomo Hospital, consisting of 11 males and 40 females, were studied. They were comparing using ELFA and ELISA to know the levels of T4. In this study the method used is a cross sectional observational, and the statistical analysis with nonparametric Spearman Correlation Rank’s test and Wilcoxon Signed Rank’s Test. In the results by using ELFA were found the mean and SD values 8.78 μg/dL/5.39 μg/dL, whereas by ELISA 11.06 μg/dL/5.38 μg/dL. These results showed a correlation between ELFA and ELISA with rs 0.576. The T4 levels results showed as follows, hypothyroid ELFA 8 (15.7%), ELISA 6 (11.8%); normal levels ELFA 32 (62.7%), ELISA 29 (56.9%); hyperthyroid ELFA 11 (21.6%), ELISA 16 (31.4%) with p 0.090 which was not considered significant. It can be concluded from the showen results that in the determination of T4 concentration using ELFA gave a lower yield compared with ELISA. Further examination is required by special treatment of the sample or other way to find out which one is the best way for determination of T4 levels.

Abstract

Tujuan: Tujuan penelitian ini adalah untuk mengetahui eliminasi DNA HPV risiko tinggi pada karsinoma sel skuamous serviks (KSS) dan adenokarsinoma serviks yang mendapat pengobatan kemoradiasi dan kaitannya dengan respons pengobatan. Bahan dan cara kerja: 22 penderita kanker serviks dengan jenis histopatologi adenokarsinoma dan 26 penderita kanker serviks dengan jenis histopatoloi KSS yang terdiri dari stadium IB-IIIB (FIGO) diambil sebagai sampel secara konsekutif dari penderita yang berkunjung dan dirawat di poliklinik dan ruangan onkologi ginekologi Rumah Sakit Umum Pusat Nasional Dr. Cipto Mangunkusumo Jakarta antara Juli 2005 dan Juli 2006. Pemeriksaan DNA HPV risiko tinggi dengan metode Hybrid Capture 2 (HC2) pada semua sampel tersebut menunjukkan hasil yang positif. Semua penderita mendapat pengobatan kemoradiasi. Pascakemoradiasi dilakukan penilaian respons pengobatan secara klinik dan pemeriksaan HC2 untuk melihat eliminasi DNA HPV risiko tinggi pada penderita tersebut. Hasil: Eliminasi DNA HPV risiko tinggi pada kanker serviks jenis adenokarsinoma lebih kecil dibanding dengan kanker serviks jenis KSS. Pascapemberian kemoradiasi, pada adenokarsinoma terjadi eliminasi DNA HPV 59,1% penderita dibanding dengan kelompok KSS di mana terjadi eliminasi DNA HPV risiko tinggi pada 76,9% penderita. Jenis Adenokarsinoma juga mempunyai angka persistensi DNA HPV risiko tinggi yang lebih besar (40,9%) dibanding dengan jenis KSS (23,1%). Penelitian ini menunjukkan respons pengobatan terhadap kemoradiasi antara kanker serviks dengan jenis adenokarsinoma dan KSS tidak berbeda dan hasil analisis menunjukkan bahwa antara respons terapi dan eliminasi DNA HPV risiko tinggi tidak terdapat hubungan. Pemeriksaan HC2 pascakemoradiasi masih bisa positif pada tumor dengan respons komplit. Kesimpulan: Persistensi DNA HPV risiko tinggi pascakemoradiasi lebih banyak terjadi pada kanker serviks jenis adenokarsinoma dibanding dengan jenis KSS. Untuk melihat apakah hal ini berkaitan dengan terjadinya rekurensi maka diperlukan penelitian dengan waktu pengamatan yang lebih lama. Abstract

The purpose of this study is to examine the implementation of SPMI, examine the results of the implementation of SPMI and examine the factors inhibiting the implementation of SPMI in Junior High School 21 Malang as an effort to improve the quality of education. Primary data were obtained from the SPMI implementers, namely the school principal, supervisors and the quality assurance team at Junior High School 21 Malang. Secondary data were obtained from references, books and documents related to the implementation of SPMI. Data analysis techniques used in this study consisted of three stages, namely data reduction, data presentation and conclusion drawing. The results of this study indicate that the implementation of SPMI in Junior High School 21 Malang is documented from 2016 to 2019. The implementation of SPMI involves all components of the school from top to bottom. The implementation of SPMI begins with quality mapping, quality fulfillment planning, implementation of quality fulfillment, evaluation and standard setting is proceeding well. The successful implementation of SPMI has increased as evidenced by graduates in 2019 getting 4th place on the results of the Computer-Based National Examination (UNBK). Implementation of quality has become a culture and character that influences the quality of graduates of Junior High School 21 Malang.

Abstract

Introduction: The duration of diagnosis delay in tuberculosis (TB) control strategy is one of the problems in TB control program. There are a lot of adverse effects, such as increasing the risk of people to become more prone to TB transmission, infectivity period, and worsening the patient’s condition that can furthermore increase the mortality and morbidity numbers. This study aimed to analyze the delay behavior in the society, specifically in Porong, East Java.  Methods: The respondents filled a questionnaire, the answer was crosschecked and complemented with the medical record on the clinic. The data collected then were analyzed with binary logistic regression using SPSS.Results: From the total of 22 samples in this study, 11 of them were classified to be delayed (patients checked their symptoms 2 weeks after the symptoms started to appear). Some factors that might have an effect on the patient’s delay were education level, the patient’s knowledge about TB, and occupation that lead to the patient’s business. Meanwhile, factors like age, gender, and the distance between the patient’s house and the health facility might have an insignificant effect.Conclusion: Duration of patient delay contributed significantly to the duration of diagnosis delay, with the median duration of 29 days. Meanwhile, the duration of system delay that was found only ranging at 0-3 days. In this study, however, evidence of the independent factors might have an effect on the patient’s tendencies to delay treatment that had not yet proven by statistical analysis.

Abstract

Abstrak Latar Belakang: Kecemasan merupakan salah satu gangguan jiwa yang umum terjadi pada masa kehamilan. Prevalensi kecemasan ibu hamil di negara berkembang rata-rata mencapai 20% atau lebih. Kecemasan ibu hamil berbeda-beda tergantung pada faktor yang mempengaruhi dan kemampuan ibu beradaptasi dalam menciptakan kondisi psikologis. Penelitian ini bertujuan untuk mengetahui perbedaan kejadian kecemasan ibu hamil primigravida dan multigravida. Metode: Jenis penelitian adalah analitik observasional dengan desain studi cross sectional comparative untuk membandingkan perbedaan kecemasan primigravida dan multigravida dengan kuesioner. Populasi adalah semua ibu hamil yang tercatat dan berkunjung di Puskesmas Jenggot Kota Pekalongan. Sampel dengan teknik consecutive sampling yang dibedakan menjadi dua kelompok yaitu kelompok primigravida dan multigravida. Masing-masing kelompok 43 orang dengan kriteria inklusi kehamilan tunggal, usia ibu saat hamil 20-35 tahun, kehamilan fisiologis. Penelitian ini dilakukan di Puskesmas Jenggot Kota Pekalongan pada Bulan Februari-April 2019. Analisis data dilakukan dengan teknik komparasi Mann-Whitney, Chi Square dan Independent Samples T-Test. Hasil: Karakteristik responden antara primigravida dan multigravida diketahui bahwa karakteristik yang berbeda adalah usia ibu hamil (p=0,000), pendidikan (p=0,009) dan tipe keluarga (p=0,000), disimpulkan bahwa ada perbedaan usia, pendidikan dan tipe keluarga. Rata-rata skor kecemasan pada ibu hamil primigravida yaitu 70,74 sedangkan pada multigravida yaitu 65,70. Hasil uji Independet Samples T-Test didapatkan nilai p sebesar 0,035 sehingga disimpulkan terdapat perbedaan kecemasan pada ibu hamil primigravida dan multigravida. Kesimpulan: Kecemasan pada primigravida lebih tinggi sebesar 7,67% dibandingkan dengan multigravida berdasarkan rata-rata skor kecemasan.Abstract Background: Anxiety is one of the common mental disorders in pregnancy. The prevalence of maternal anxiety in developing countries reaches an average of 20% and more. The anxiety in pregnancy depends on the influencing factors and the ability of the mothers to adapt and create a psychological condition. Therefore, this research aims to find out the differences in the prevalence of anxiety in primigravida and multigravida pregnancy. Method: The type of this research is analytic observational study with comparative cross-sectional design and questionnaires to compare the incidence of anxiety in primigravida and multigravida. The research population was all pregnant women who visited and recorded at Jenggot Public Health Center, Pekalongan. The sample was taken with consecutive sampling technique and divided into two groups, primigravida group, and multigravida group. Abstract

ABSTRACTBackground: The maternal mortality rate is 305 per 100,000 live births in Indonesia. The most common cause of maternal death is preeclampsia / eclampsia. The maternal mortality rate is 247 per 100,000 live births in 2017 in Kotabaru Regency. Most preeclamptic referral patients are in critical condition when they arrive at the hospital. Method: This type of research is an observational descriptive study. The population is all patients with severe preeclampsia and eclampsia in the delivery room of RS Pangeran Jaya Sumitra Kabupaten Kotabaru from January - December 2018. The sample is total sampling.Results: PEB referral patients was 57 cases. Most of those are in the group age of 20 - 35 years which are 35 people (61.4%), not nullipara is 31 people (54,4%), 34-40 weeks of gestation is 43 people (75,4%), as many as 44 people (77,2%) with previous severe preeclampsia/ hypertension, 41 people (71,9%) brought referral letters, referred by Puskesmas are 47 people (82.5%), most of the locations of referral is from Pulau Laut as many as 35 referrals (61,4%). The initial management of PEB and eclampsia: IV Pathway in 11 patients (19.3%) of severe preeclamptic and eclamptic patients, 50 patients (87.7%) were not given MgSO4, 51 people (89.5% ) did not get anti-hypertension, 52 patients (91.2%) were not installed catheters. Conclusion: Early management of severe preeclamptic and eclamptic patients is not optimal and does not fulfil the standard yet. Proper and appropriate early management can reduce morbidity and maternal mortality from complications of severe preeclampsia and eclampsia that can be prevented by providing right initial management.

Abstract

AbstractBackground: Weight gain is one of the predominantly problem for women who’s in childbearing age. Weight gain that occurs at women in childbearing age is often associated with contraceptive use. One type of contraception that affects weight gain is the hormonal contraception. Lerning the weight gain from each use of contraception is important as a consideration for the family planning program. Methods: This research method was observational analytic with cross-sectional design. Sample size was 136 respondents and the sampling technique used total sampling. The independent variable is the type of contraception includes 1 month injection contraception, 3 month injection contraception, implant contraception, combined pill contraception, and copper T. IUD contraception. The dependent variable is the weight gain. Data was analyzed with Kruskal Wallis test. Results: The results showed that 120 respondents from 5 contraceptive groups experienced weight gain after using contraception. The results of the Kruskal Wallis test were obtained at value of P <0.0001 (p <0.05). that there are differencea in weight gain in 5 groups of contraception. Conclusion: there are difference in weight gain from each contraceptive groups and the contraception that most influences weight gain is 3 months injection contraception.

Abstract

Background: Ectopic pregnancy is a pregnancy outside the uterus, embryo could attached to thefallopian tube, ovary cervix or cervix. Around 85-90% incidence of ectopic pregnancy is found inmultigravid women. According to data from the Health Office, maternal mortality in 2017 tendsto increase in the last two years. Not only it is a major predictor of mortality and morbidity inpregnant women, but individuals with a history of ectopic pregnancy have a greater risk. Thepurpose of this research was to determine the risk factors that cause ectopic pregnancy by usingliterature studies review. Method:This study used a systematic review of articles that publishedbetween 2013-2020 in the Science Direct, Scopus and Google Scholar. 10 articles that meet theinclusion and exclusion criteria, included as a sample for further review. The results regarding torisk factors that caused ectopic pregnancy in each article were systematically synthesized. Results:Risk factors found to be significantly associated with the incidence of ectopic pregnancy in thearticle were age (2), infertility (2), IUD (3), PID (3), Ectopic Pregnancy History (5), Progestine Pills (2), History of Abortion (1), Smoking (3). Conclusions: age, infertility, IUD, PID, history of ectopic pregnancy, progestine pill, History of abortion, and smoking were risk factors that hadsignificant relationship with the incidence of ectopic pregnancy.

Abstract

AbstractBackground: Adolescence is a transition from children to adulthood. In adolescence menarche will occur, menarche is the first menstruation in young women which occurs between 1.5 to 3 years after thelarche. The average menarche occurs at the age of 12 and 13 years. When Menarche happens, many changes occur, one of which is psychological change. Changes in the level of knowledge of adolescents about the level of knowledge of adolescents about menarche. Knowledge about menarche can be obtained through families, print media, electronics, or health workers (midwives, doctors, nurses). Examining the relationship of knowledge level with prepubertal adolescent girls psychological responses in menarche conversation. Method: This study was an observational analytic study with a cross sectional study design. The number of samples was 41 students with total sampling techniques. To find out the relationship between variables, the data test with the chi-square test. Results: The results showed that most teenagers' knowledge in the knowledge category was 70.73%. While the psychological response is mostly in the negative category (68.3%). Contingency coefficient results showed a p value of 0.270 (p <0.05). Conclusion: These data indicate that there is no correlation between knowledge and psychological responses of pre-puberty teenage girls in the face menarche.
