Abstract

The enactment of legislation governing decision-making for incapacitated individuals serves a crucial purpose, primarily enabling individuals to proactively plan for future incapacity by appointing trusted agents to make decisions on their behalf. In the context of England and Wales, adults can achieve this through the utilization of a lasting power of attorney, as stipulated in the Mental Capacity Act 2005 (sections 9-14). Specifically, a health and care lasting power of attorney grants authority to an appointed agent to make day-to-day care decisions in situations where the individual is unable to do so. It is imperative for district nurses to obtain the consent of the appointed attorney before administering treatment to the donor. Furthermore, a district nurse should conduct a comprehensive review of the actions taken by the donor's attorney, involving consultation with the donor, the general practitioner (GP), and the attorney. Should conflicts regarding care persist, legal intervention by the courts may be necessary. Notably, life-sustaining or necessary therapies would continue in such instances. It is essential to emphasize that lasting health and care powers of attorney are not subject to illegality, encompassing situations where the donor expresses wishes for euthanasia or assisted dying. District nurses are required to obtain a copy of the health and care enduring power of attorney, respecting the authority of the appointed attorney to make decisions in cases where the donor lacks capacity. In the Eastern context, considerations of human rights, religion, ethics, and law collectively categorize euthanasia as a criminal act. Conversely, while Indonesia lacks specific regulations addressing euthanasia, various legal sources indicate that both seeking and performing euthanasia are deemed criminal activities.

Abstract

Frambusia, also known as yaws, patek, pian, buba, or ambalo, is a tropical disease categorized as a Neglected Tropical Disease (NTD). The diagnosis of frambusia is established through clinical examination and serological testing. Occasionally, there are cases of frambusia that lack clear clinical lesions, leading to undiagnosed instances. The disease is primarily attributed to unhygienic living conditions, and if left untreated, it can result in various complications in the future. TPHA-RDT testing can be employed for early frambusia detection. Educational activities and disease screening involve 12 pediatric respondents. Frambusia is commonly observed among children aged two to fifteen years, with transmission occurring during play and interaction with peers of the same age group. Therefore, effective preventive measures, such as frambusia screening, need to be implemented. Societal awareness campaigns contribute to enhancing public knowledge of frambusia and educating individuals about effective preventive actions. With an improved understanding of this disease, it is expected that individuals will take necessary preventive steps, such as seeking healthcare when frambusia symptoms or signs arise. Thus, accurate knowledge of frambusia serves as a foundation for adopting effective preventive behaviors and safeguarding the overall health of both individuals and communities.

Abstract

Temporary blood sugar is the level of glucose in the blood that measured without considering the time of the last meal, and often used to detect diabetes mellitus or impaired glucose tolerance. This community service activity aims to describe the distribution of blood sugar levels in 68 men and women of productive age at  Kalam Kudus II High School, Jakarta. This community service activity is carried out using the Plan-Do-Check-Action method. The results showed that the majority of respondents had normal fasting blood sugar levels (97.1%), with an average of 113.26 mg/dL and a median of 104 mg/dL. Lifestyle factors, nutrition, genetics, and chronic medical conditions can affect blood sugar levels at any time. Regular monitoring of blood sugar levels is essential for early detection and management of diabetes mellitus, which can prevent serious complications such as cardiovascular disease and nephropathy. Prevention strategies include a healthy diet, physical activity, patient education, and appropriate medical interventions. Comprehensive public health programs are needed to increase awareness and management of diabetes, as well as improve the quality of life of individuals affected by this disease.

Abstract

Obesitas adalah kondisi di mana seseorang memiliki berat badan melebihi batas normal akibat penumpukan lemak berlebih dalam tubuh. Obesitas berhubungan erat dengan terjadinya resistensi insulin, yaitu suatu kondisi dimana tubuh tidak dapat memberikan respon kepada insulin secara efektif. Oleh sebab itu, edukasi dan skrining rutin pada kelompok lanjut usia dalam rangka mendeteksi dan mengelola resistensi insulin pada tahap awal merupakan hal yang sangat penting. Kegiatan pengabdian ini dilakukan dengan metode PDCA (Plan-Do-Check-Act), yang mencakup edukasi tentang pola makan sehat, aktivitas fisik, dan manajemen berat badan, serta skrining kadar insulin. Berdasarkan kegiatan ini, edukasi dan skrining terbukti penting dalam mencegah komplikasi lebih lanjut dan meningkatkan kualitas hidup lanjut usia.

Abstract

Body composition refers to the distribution of fat, muscle, bone, and other tissues in the human body, which is often expressed as a percentage of total body weight consisting of fat mass and lean body mass. Obesity, characterized by excess fat in adipose tissue, poses a significant health risk and is increasing in countries with low and medium mortality rates, especially in metropolitan areas. Factors contributing to this include genetics, a lack of physical activity, socioeconomic status, unhealthy eating patterns, psychosocial factors, smoking, and alcohol consumption. Excess body fat significantly contributes to morbidity and mortality. Although optimal body fat levels vary, excess fat increases the risk of health issues such as joint problems and cardiovascular disease. People widely use bioimpedance analysis (BIA), an efficient, non-invasive method, to assess nutritional status and body composition. This activity uses the PDCA (Plan-Do-Check-Action) method, which is carried out at PT. Narindo, North Jakarta, in the productive age population. The examination results showed that the average levels of total body fat, visceral fat, total subcutaneous fat, total muscle mass were 28.6%; 11%; 22.2%; 28.3%. Early detection of body composition is critical for assessing nutritional and health status as well as diagnosing disease, potentially reducing obesity-related morbidity and mortality.

Abstract

Muscle strength decline is a factor that affects functional performance and work capacity in productive age. Handgrip strength is a practical method to detect this decline quantitatively. This activity aims to identify muscle strength status through handgrip strength examination and provide education on preventing muscle function decline. The examination was conducted on productive age individuals using a digital handgrip measuring tool. Data were collected from 49 participants, consisting of 25 men and 24 women. Handgrip strength values ​​were recorded and categorized into normal or weak status. Furthermore, participants received education through visual posters explaining the relationship between lifestyle, muscle strength, and productivity. The average handgrip strength in men was 36 kg (SD 8.75) and in women it was 22.94 kg (SD 9). The proportion of participants with weak muscle strength reached 10.20% in men and 12.24% in women. This examination successfully identified risk groups that had not shown any clinical symptoms previously. Handgrip strength examination is effective as an early screening tool for the risk of muscle strength decline in productive age. Education based on examination results can increase awareness and encourage lifestyle changes towards long-term muscle function maintenance.

Abstract

The balance of water and oil levels in the skin is an important factor in maintaining healthy skin, especially in preventing damage due to external factors. In the productive age group, the risk of imbalance in water and oil levels in the skin increases along with environmental exposure and lifestyle. Community service activities are carried out through the Plan-Do-Check-Act (PDCA) approach which includes education about the risk of imbalance in water and oil levels in the skin, as well as routine screening using a skin analyzer. Education focuses on risk factors, prevention, and monitoring of skin conditions, accompanied by counseling on a healthy lifestyle to maintain water and oil levels in the skin. This activity was attended by 40 participants in the productive age group. The results of the examination showed an average water and oil content in the skin on both arms of the participants, with an average value of 56.78% and 25.5% on the right arm, and 53.41% and 23.97% on the left arm. Education succeeded in increasing participants' understanding of the importance of maintaining the balance of water and oil levels in the skin. Maintaining the balance of water and oil levels in the skin has been shown to be important in preventing skin damage. Optimal hydration and proper use of skincare products can minimize the risk of skin imbalance, especially in the productive age group that is vulnerable to environmental exposure.

Abstract

Hypertension is one of the leading causes of cardiovascular morbidity and mortality. Excessive sodium intake is known to contribute to increased blood pressure through certain physiopathological mechanisms. The purpose of this activity is to evaluate the distribution of blood pressure and the role of education on sodium intake restriction in community screening activities. The activity was carried out at the Cengkareng Baptist Foundation School with the Plan-Do-Check-Act (PDCA) approach. Blood pressure examinations were carried out using an automatic digital tensiometer on 57 adult participants. Data were classified according to blood pressure category, and all participants received education on sodium intake restriction. The results showed that the average systolic and diastolic pressures were 132.4 mmHg and 87.64 mmHg, respectively. Most participants were categorized as pre-hypertension and hypertension. Education on sodium intake restriction is important in hypertension prevention strategies at the community level and needs to be integrated into early detection programs.

Abstract

UV rays can affect various skin physiology, including changes in skin thickness and skin color. Vitamin C acts as an antioxidant to reduce melanin and skin damage caused by the sun, but it requires a combination with other therapies to get optimal results. This research focuses on the effectiveness of microneedling with vitamin C against pigmentation and skin damage due to sun exposure. The design of this research is quasi experimental. Patients who have atrophic acne scars who meet the inclusion criteria and are treated at Sukma Cliniq Tangerang are the population in this study. The intervention in this study was microneedling with a mixture of 10% vitamin C serum to improve the skin (given when the intervention was carried out and used at home for 21 days). The intervention will be carried out on day 0 according to the procedure previously explained and measured again on day 42. The variable measurements in this study aim at the value of UV damage and skin pigmentation before and after the combination therapy. Results: There were 36 respondents who met the inclusion criteria for this study with an average age of 33.03 years and 69.4% of respondents were women. There were changes and improvements in roughness, porphyrin, and pigmentation parameters (p value <0.001), but there was no change in UV damage parameters (p value 0.500) both before and after the intervention. There were changes and improvements in roughness, porphyrin, and pigmentation parameters after microneedling and vitamin C therapy before and after 42 days of therapy. However, there are no changes and improvements to the UV damage parameters.

Abstract

ABSTRACT Physiological changes occur to every organ systems along with age. Fecal incontinence (FI) is one of them. FI should not be underestimated. There are a lot of risk factors for FI in elderly, including age, obesity, gender, and comorbidities like dementia and diabetes melitus. High prevalence of FI and diabetes mellitus will affect clinical and managements in elderly care. To find out the correlation between blood sugar level and fecal incontinence severity index (FISI) score in elderly patients. This is an analytic study with cross sectional design. Samples are elderly patients in Santa Anna Nursing Home in July 2023 that met the criteria. Data obtained through questionnaire interview and blood examination. Statistical analysis used in the study are Pearson or Spearman correlation test. Data distribution is tested with Kolmogorov-Smirnov test. Level of significance in the study is 5%. There are 60 respondents with the mean age of 76,30 (±7,88) years, dominated by female (66,7%). High blood sugar level obtained in 11,7% of the respondents. There is a significant correlation between blood sugar level and FISI score (p-value = 0,041; r-correlation : 0,264). From the R square evaluation we found a value of 0.091, which indicates that 9.1% of FISI score is influenced by blood sugar level. There is a correlation between elevated blood sugar and fecal incontinence. Fecal incontinence in older adults requires attention due to its adverse impact on their quality of life. Keywords: Blood Sugar Level, Elderly, Fecal Incontinence  ABSTRAK Perubahan fisiologis terjadi pada seluruh sistem organ seiring dengan bertambahnya usia. Salah satu hal yang dapat terjadi pada lansia adalah fecal incontinence (FI). Hal ini tidak dapat dipandang sebelah mata. Terdapat berbagai faktor risiko untuk FI pada lansia, termasuk bertambahnya usia, obesitas, jenis kelamin, dan berbagai jenis penyakit penyerta, seperti demensia dan diabetes melitus. Tingginya prevalensi FI dan diabetes melitus akan berdampak pada klinis dan manajemen perawatan lansia. Mengetahui korelasi kadar gula darah sewaktu dengan nilai fecal incontinence severity index (FISI) pada kelompok lanjut usia. Penelitian ini merupakan penelitian analitik dengan desain penelitian cross sectional. Sampel merupakan pasien lanjut usia di Panti Lansia Santa Anna pada Juli 2023 serta memenuhi kriteria. Data diperoleh melalui wawancara kuesioner dan pemeriksaan darah. Analisa statistik pada penelitian ini menggunakan korelasi Pearson atau korelasi Spearman. Distribusi data diperiksa menggunakan uji Kolmogorov-Smirnov. Nilai kemaknaan dalam penelitian ini adalah sebesar 5%. Didapatkan 60 responden dengan rerata usia adalah 76,30 (±7,88) tahun, yang didominasi jenis kelamin perempuan (66,7%). Kadar gula darah sewaktu yang tergolong tinggi didapatkan pada 11,7% responden. Terdapat korelasi yang bermakna antara kadar Gula Darah Sewaktu dengan nilai FISI (p-value = 0,041; r-correlation : 0,264). Penelusuran dari nilai R square didapatkan nilai sebesar 0,091, yang menunjukkan bahwa 9,1% nilai FISI dipengaruhi oleh kadar gula darah sewaktu. Terdapat korelasi antara peningkatan gula darah sewaktu dengan inkontinensia fekal.
