Abstract

Myasthenic crisis is a severe acute complication of myasthenia gravis (MG) characterized by respiratory and bulbar muscle insufficiency due to impaired neuromuscular transmission, with pathogenesis involving autoantibodies targeting acetylcholine receptors (AChR), muscle-specific kinase (MuSK), and low-density lipoprotein receptor-related protein 4 (LPR4) that disrupt the integrity of the neuromuscular junction. This review aims to examine the immunobiological mechanisms of myasthenic crisis and explore the potential of modern biotechnological therapies and tropical bioactive compounds as adaptive treatment strategies for tropical populations. A comprehensive literature review was conducted focusing on genetic factors, tropical environmental influences, and advancements in molecular therapies. Genetic factors, including HLA class II and non-HLA genes, interact with tropical environmental conditions such as endemic infections, high temperature, extreme humidity, air pollution, and vitamin D deficiency, potentially exacerbating immune dysregulation and increasing crisis risk. Modern molecular therapies, including rituximab (anti-CD20), eculizumab (anti-C5), and efgartigimod (FcRn inhibitor), have demonstrated efficacy in reducing pathogenic autoantibodies and improving neuromuscular function, while tropical bioactive compounds such as curcumin and andrographolide show potential as adjuvant immunomodulators. Integrating biotechnological approaches with tropical bioprospecting provides a more adaptive and sustainable therapeutic strategy aligned with the biological characteristics of tropical populations. It is recommended that further clinical studies evaluate the safety, optimal dosing, and long-term efficacy of combined biotechnological and bioactive therapies in these populations.

Abstract

Abstract

Choledochal cysts are congenital anomalies characterized by abnormal dilatation of the bile ducts, either intrahepatic or extrahepatic. This condition can lead to serious complications, including cholangitis, pancreatitis, and malignancy. Choledochal cysts are more prevalent in Asian populations compared to Western populations and are more commonly observed in females. Timely diagnosis is crucial to prevent further complications. This study aims to conduct a literature review on the anatomical variations and available therapeutic methods for managing choledochal cysts, as well as to evaluate the effectiveness of various treatment approaches. A systematic literature review methodology was employed, involving database identification, keyword selection, article screening, and the use of recent publications from reputable scientific databases such as PubMed, Google Scholar, and Scopus. This review identifies five types of choledochal cysts based on the Todani classification, with Type I being the most common. The primary treatment for choledochal cysts involves complete cyst excision, usually followed by biliary reconstruction using Roux-en-Y hepaticojejunostomy. In more advanced cases, complex surgical procedures such as pancreaticoduodenectomy may be required. Biliary reconstruction following cyst excision, particularly through Roux-en-Y jejunostomy, has been proven effective and is considered the gold standard in choledochal cyst management. Surgical approaches should be tailored to the cyst type to achieve optimal clinical outcomes.

Abstract

Gastropods play an important role in maintaining the balance of coastal ecosystems, particularly as detritivores that support nutrient cycling. Pantai Meras, located in North Sulawesi, has a complex coastal ecosystem that includes mangrove forests, seagrass beds, and coral reefs, which support high biodiversity. However, information regarding gastropod diversity in this area remains limited. This study aims to identify gastropod species and analyze their diversity levels in the waters of Pantai Meras. A purposive sampling method was used for data collection, employing 0.5 m × 0.5 m plots placed randomly at 45 sampling points across three research stations with different substrate types, namely sandy substrate (Station 1), muddy sand substrate (Station 2), and sand mixed with coral fragments (Station 3). Data were analyzed using PAST software version 4.17 to calculate the Shannon diversity index (H’). The results showed a total of 243 gastropod individuals belonging to 89 species from six orders, with Neogastropoda being the dominant order, comprising 59 species. The most abundant species was Nassarius globosus, with a total of 26 individuals recorded. The overall Shannon diversity index (H') was 4.14, which falls into the high diversity category. The highest H' value was recorded at Station 1 (4.08), followed by Station 3 (4.02), and the lowest at Station 2 (3.43). The high gastropod diversity at Pantai Meras indicates that the coastal ecosystem in this area is still in good condition and capable of supporting sustainable gastropod populations. Environmental factors such as temperature, salinity, as well as the presence of mangroves, seagrass beds, and coral reefs contribute to the high species diversity of gastropods in the study area.

Abstract

This study aims to compare the political economy of the Roman-Persian period and the contemporary era through an Islamic perspective, especially in terms of resource management, wealth distribution, and the influence of policies on society, such as social inequality, resource exploitation, and moral crisis. The research method used is a literature study with a qualitative approach using literature studies on secondary sources. The results showed that the Roman-Persian economic and political system, although effective in maintaining imperial stability, tended to create social inequality due to high taxes and territorial exploitation. Meanwhile, the contemporary economic system presents technological innovation and global economic integration, but it is vulnerable to economic crises and injustices. The conclusion of this study is that Islamic principles such as zakat, the prohibition of usury, and the fair distribution of wealth can be an alternative solution to build a just and sustainable economic system, both in the past and in the present.
