Abstract

CABG is a surgical operation performed by creating new blood vessels or bypassing blocked blood vessels so that the blood flow carrying oxygen to the heart muscle supplied by the vessels is smoothed again. The study aimed to determine the management of physiotherapy in CABG cases. The research method used a case report method conducted on a 64-year-old male patient with a medical diagnosis of post-CABG surgery 4 Grafft A. I CAD3VD With Lm Disease, CTO at Prox RCA, complained of pain in the chest (surgical incision), ineffective breathing patterns, difficulty coughing, and the patient felt that his upper and lower limb movements were still limited. Physiotherapy interventions were conducted in 5 meetings from November 8, November 11, and November 12, November 13, to November 15, 2024. Starting from observation to evaluation. The physiotherapy Intervention given is a Breathing exercise (deep breathing exercise), effective coughing exercise with ACBT huffing), stretching of upper and lower limbs, and gradual mobilization. Measurements are made with a Sphygmomanometer, pulse oximeter, thermometer to measure vital signs during pre-post action, NRS (numbering rating scale) for pain, Meter line for thoracic expansion, and ICUMS for functional ability activities. The results showed a significant decrease in motion pain and still pain, where motion pain decreased from NRS 7 to NRS 0, as well as an increase in thoracic expansion, where the difference in breathing when inhaling and exhaling increased the thoracic cage from a difference of 0.8 cm to 2.8 cm, and an increase in functional ability. Using ICUMS, an increase in functional ability on the first day got a score of 10 (All activities are done above the bed) until the final evaluation got an ICUMS score of 10 (walking independently).

Abstract

This study aims to determine the results of physiotherapy management performed on post-anterior cruciate ligament reconstruction (ACLR) patients. This is a case study method with one sample of post-ACLR dextra patient in phase I. this research evaluates pain using the numeric rating scale (NRS), range of motion (ROM) with goniometer, muscle strength with manual muscle test (MMT), and Lysholm Knee Scoring for functional ability. The physiotherapy management provided to the patient includes Transcutaneous Electrical Nerve Stimulation (TENS), ice compression (cryotherapy), patellar mobilization, heel slide exercise, gait training, and strengthening exercise. From three physiotherapy sessions conducted, the results showed a decrease in resting and pressure pain, as well as an increase of knee ROM. However, no changes were found in movement pain and functional ability evaluations from the first to the third session.

Abstract

Anterior cruciate ligament (ACL) injury is one of the most common knee ligament injuries among physically active adolescents, particularly in sports requiring frequent pivoting and landing such as basketball. Functional outcomes following ACL reconstruction are strongly influenced by the quality of postoperative physiotherapy, especially during Phase II rehabilitation, which serves as a transition toward higher functional activities. This study aimed to evaluate the outcomes of Phase II physiotherapy management incorporating progressive weight-bearing exercises on muscle strength, knee range of motion, pain, and functional performance in an adolescent patient after ACL reconstruction. A case report design was applied to a 17-year-old female patient following anterior cruciate ligament reconstruction (ACLR). The intervention program consisted of progressive strengthening exercises (mini squats and lunges), balance and stability training, range of motion exercises, and functional task-oriented training. Outcome measures included muscle strength assessed using a sphygmomanometer, knee range of motion measured with a goniometer, pain intensity evaluated using the Visual Analogue Scale (VAS), and functional performance assessed using the Lysholm Knee Scoring Scale (LKSS). The results demonstrated improvements in quadriceps strength (145–210 mmHg) and hamstring strength (130–155 mmHg), restoration of full knee extension (−5° to 0°), increased knee flexion to 145°, decreased pain intensity (VAS 4 to VAS 1), and improved functional performance (LKSS 79 to 92). Phase II physiotherapy incorporating progressive weight-bearing exercises resulted in clinically meaningful improvements in knee function and prepared the patient for advanced rehabilitation phases.
