Abstract

The implementation of the Enhanced Recovery After Cesarean Surgery (ERACS) method as an innovation in the Sectio Cesarea (SC) surgical procedure aims to accelerate recovery, minimize pain, and improve patient comfort. Although it has been clinically proven, there is still limited research exploring patients' subjective experiences with this method, particularly in eastern Indonesia. This study aims to explore in depth the physical, emotional, psychological experiences, as well as perceptions of service and patient expectations after undergoing SC with the ERACS method at Banggai Laut General Hospital. The study employs a qualitative approach with a phenomenological study design. Data were collected through in-depth interviews with purposively selected informants and analyzed using Braun & Clarke's thematic analysis technique. The results showed that patients experienced rapid physical recovery, minimal pain, and were able to mobilize early. Emotionally, patients felt calm and stable, supported by good communication with medical staff and family support. However, information about the ERACS procedure before surgery was still perceived as lacking detail. Patients also hope that this method continues to be implemented with improvements in educational aspects and financial support from BPJS. Patients' experiences with the ERACS method are generally very positive and reflect the success of this approach not only clinically but also in creating a more comfortable, efficient, and humane childbirth experience. This study recommends enhancing preoperative education, postoperative monitoring, and more inclusive financial support systems.

Abstract

Transformasi digital dalam sistem pelayanan kesehatan mendorong penerapan Rekam Medis Elektronik (RME) di seluruh fasilitas kesehatan, termasuk puskesmas, sebagai bagian dari kebijakan nasional untuk meningkatkan efisiensi dan mutu layanan. Namun, keberhasilan implementasi RME tidak hanya ditentukan oleh teknologi, melainkan juga oleh kesiapan dan persepsi tenaga kesehatan sebagai pengguna utama. Penelitian ini bertujuan untuk menganalisis persepsi tenaga kesehatan terhadap implementasi RME di Puskesmas Posumaen Kabupaten Minahasa Tenggara dengan menggunakan pendekatan kuantitatif dan desain deskriptif analitik. Instrumen pengumpulan data berupa kuesioner tertutup disusun berdasarkan empat konstruk utama dalam model Unified Theory of Acceptance and Use of Technology (UTAUT): performance expectancy, effort expectancy, social influence, dan facilitating conditions. Seluruh populasi sebanyak 23 tenaga kesehatan dijadikan sampel melalui teknik total sampling. Hasil penelitian menunjukkan bahwa secara umum persepsi tenaga kesehatan terhadap RME berada pada kategori tinggi, dengan 65,2% responden memiliki persepsi positif. Dimensi performance expectancy menunjukkan skor tertinggi (69,6%), diikuti oleh social influence dan facilitating conditions (masing-masing 78,3%), serta effort expectancy (65,2%). Temuan ini mengindikasikan bahwa penerimaan terhadap RME cukup baik, namun masih terdapat sebagian responden yang menunjukkan persepsi “cukup”, sehingga dibutuhkan penguatan dalam hal pelatihan, pendampingan teknis, dan infrastruktur pendukung. Penelitian ini menegaskan pentingnya faktor persepsi pengguna dalam keberhasilan implementasi sistem digital di layanan kesehatan primer, khususnya di wilayah terpencil.

Abstract

Pelayanan kesehatan tidak hanya dipahami sebagai tindakan medis, tetapi juga sebagai bentuk interaksi sosial yang dipengaruhi oleh nilai dan norma budaya masyarakat. Penelitian ini bertujuan untuk menggali makna sosial budaya dalam persepsi pasien rawat inap terhadap kualitas pelayanan kesehatan di RSUD Mitra Sehat Kabupaten Minahasa Tenggara. Menggunakan pendekatan kualitatif dengan metode fenomenologi, penelitian ini melibatkan pasien rawat inap sebagai informan utama yang telah menjalani perawatan minimal tiga hari. Hasil penelitian menunjukkan bahwa persepsi pasien terhadap kualitas pelayanan sangat dipengaruhi oleh nilai-nilai budaya lokal, seperti sopan santun, penghormatan terhadap orang tua, keramahan, dan kesetaraan perlakuan. Pengalaman pelayanan yang dianggap berkualitas tidak hanya mencakup kompetensi teknis tenaga kesehatan, tetapi juga aspek relasional seperti komunikasi yang empatik, sikap hormat, serta kepekaan terhadap bahasa dan kebiasaan lokal. Interaksi sosial yang positif dan penghormatan terhadap identitas budaya pasien memperkuat rasa dihargai dan meningkatkan kepercayaan terhadap institusi rumah sakit. Temuan ini menegaskan pentingnya pelayanan kesehatan yang sensitif budaya untuk menciptakan sistem layanan yang inklusif, manusiawi, dan bermakna bagi masyarakat setempat.

Abstract

Background: Management of puskesmas is critical to providing quality health services through effective planning, organising, implementing, and controlling. Improving the quality of health services builds patient trust. The Healthy Indonesia Programme aims to improve services at first level health facilities, including puskesmas, but many still do not meet quality standards. This study evaluates the management cycle of puskesmas at the South Sorong District health office.Objectives: This study aims to analyse the evaluation of the implementation of the puskesmas management cycle at the South Sorong District Health Office. Methods: A qualitative approach was used with informants from four puskesmas: Teminabuan, Konda, Kokoda, and Kais Darat, representing 16 puskesmas in the region.Results: Some puskesmas follow the planning stages involving mini-workshops and Introspection Survey data, but not consistently. There were challenges maintaining the monthly mini-workshop schedule and using the monthly work programme plan results. Monitoring processes were not always optimal, with most responsibility falling on the head of the puskesmas and administrative staff, while the role of the programme focal point was not fully maximised.Conclusion: puskesmas should improve the consistency, quality, and effectiveness of management through increased managerial competence, efficient budget management, and optimisation of planning, implementation, and monitoring processes.

Abstract

Background: Hypertension is a major risk factor for cardiovascular diseases, driven by lifestyle modernization and its high prevalence. Addressing treatment adherence is crucial for effective management. Objective: This study aimed to analyze the determinants influencing treatment adherence among hypertension patients in South Sorong Regency, with a specific focus on family support. Methods: A quantitative research approach was conducted in South Sorong Regency from May to June 2024. Data were collected through structured questionnaires and analyzed to assess the influence of various factors on treatment adherence. Results: The findings revealed that family support significantly influenced treatment adherence in hypertension patients. In contrast, knowledge, perception, service delivery quality, and patient satisfaction did not show a significant effect on adherence. Conclusion: Family support plays a critical role in enhancing treatment adherence among hypertension patients in South Sorong Regency. To improve adherence, it is recommended to implement family education through counseling at community health centers (puskesmas) and social media, conduct behavior change campaigns, provide family counseling, train health workers to improve service delivery, and strengthen monitoring and evaluation mechanisms for treatment adherence.

Abstract

Background: The LPG Bulk Filling and Transportation Station (SPPBE) at PT. Tambang Yokodelta Matungkas, North Minahasa, is a high-risk environment due to the flammable and pressurized nature of LPG. Identifying potential hazards is essential to ensure worker safety and prevent accidents. Objective: This study aims to analyze potential hazards and assess occupational risks using the Job Safety Analysis (JSA) method at the SPPBE of PT. Tambang Yokodelta. Methods: This qualitative research involved interviews and observations with eight informants, including the SPPBE manager, HSE head, technicians, and operators. Data were collected through interviews, field observations, and document review, and analyzed descriptively. Results: The study identified hazards classified into four risk levels: Low, Medium, High, and Extremely High. Low-risk activities, such as minor injuries, can be controlled with personal protective equipment (PPE). Medium risks, like falling during LPG handling, require additional PPE. High risks, including fire hazards during gas filling, need engineering controls. Extremely high-risk activities, such as gas explosions, require immediate management action and substitution measures. Conclusion: Workers at PT. Tambang Yokodelta face various risks. Effective control measures, including PPE, SOP compliance, engineering controls, and management commitment, are crucial for ensuring occupational safety.

Abstract

Digital transformation in health services faces significant challenges, particularly the gap between rapid technological advancements and the readiness of institutional work culture. This study investigates the cultural lag experienced during the implementation of digital systems at the East Nunukan Health Center. Using an observational qualitative approach, it examines health workers' responses to digital applications such as e-Puskesmas and various Health Information Systems. The findings reveal substantial resistance to change, attributed to factors like limited digital literacy, inadequate infrastructure, and the persistence of traditional work patterns. Despite the introduction of digital systems, healthcare practices remain largely dependent on manual methods, highlighting the disconnect between technological progress and the socio-cultural preparedness of healthcare organizations. The study concludes that this cultural lag within healthcare institutions can significantly impede the effectiveness of digital transformation efforts. To address these issues, the research advocates for ongoing training, improvement of supporting facilities, and the adoption of strategies that align with local work culture. This research provides valuable insights into the socio-cultural dynamics affecting the digitization of public services, especially in border regions with limited technological access.

Abstract

Introduction: Stunting begins to appear when the child is two years old and begins when the fetus is still in the womb. The increase in child morbidity and mortality is the impact caused by the incidence of stunting. Stunting children will be able to experience disturbances in their intelligence level, susceptibility to diseases, decreased productivity, and stunted economic growth, which impacts poverty. Method: This study is a literature review using the PRISMA method. Articles published in 2022-2024 were found by conducting database searches (Garuda and Google Scholar) and using the keywords "Stunting" and "Economic Income," and limited to using Chi-Square analysis so that articles meet the criteria for analysis, as many as 10 articles. Results: The search results for the ten articles were obtained, all stating the relationship between economic income and stunting. The author claims that Karl Marx's point of view highlights this problem as a social problem from a social system that subjugates a particular economic class. Dependence on the wage system, inadequate social security, and the exploitation of labor by capitalism trap many proletarian families in a cycle of poverty. This leaves children in these homes vulnerable to chronic malnutrition, which adversely affects their physical and cognitive development. Conclusion: The problem of low economic income in society as the cause of stunting is not only technical interventions, such as supplementary nutrition programs, but rather more fundamental structural changes, as seen from a Marxist perspective. Systematic reduction of economic inequality and stunting prevention can be achieved through wealth redistribution, wage system improvement, and labor exploitation elimination.

Abstract

Adolescence is a critical phase in individual development, especially for adolescent girls who begin to experience menstruation as a sign of reproductive maturity. However, not all adolescents experience menstruation normally; some experience disorders such as menstrual pain, irregular cycles, and prolonged bleeding. This study aims to explore in depth the subjective experiences of adolescent girls with menstrual disorders at SMP Negeri 2 Bunta, Banggai Regency, focusing on the nature of the disorders experienced, their interpretation of these experiences, coping strategies, and the social support received. This study employs a qualitative approach with a phenomenological design. The primary informants were female adolescents in grades VII–IX who had experienced menstrual disorders in the past six months, selected purposively until data saturation was achieved. Data collection techniques included in-depth interviews, non-participatory observation, and document analysis. Data were analyzed using the Miles and Huberman model through data reduction, data presentation, and drawing conclusions. The results of the study indicate that menstrual disorders are viewed as normal by most informants, despite causing physical discomfort and psychological anxiety. Adolescents' responses to these disorders vary, ranging from the use of warm compresses and medication to no intervention at all. The primary sources of information are mothers and social media, while the roles of teachers and schools remain limited. Social support is most commonly provided by family, although some adolescents also feel supported by peers and teachers. This study emphasizes the importance of systematic reproductive health education, school involvement in supporting adolescents, and the creation of safe and stigma-free discussion spaces regarding menstruation-related issues.

Abstract

Penelitian kualitatif ini bertujuan untuk mengungkap persepsi tenaga kesehatan terhadap dampak perubahan sosial budaya terhadap praktik pelayanan kesehatan di Puskesmas Kabupaten Minahasa Tenggara. Penelitian ini dilatarbelakangi oleh meningkatnya pengaruh transformasi sosial dan budaya digital terhadap perilaku masyarakat, kepercayaan, serta pola komunikasi dalam konteks pelayanan kesehatan. Tujuan utama penelitian adalah untuk menggambarkan pengalaman tenaga kesehatan dalam merespons dan beradaptasi terhadap perubahan sosial budaya tersebut sembari tetap menjaga mutu pelayanan. Data diperoleh melalui wawancara mendalam semi-terstruktur dengan tiga informan yaitu seorang perawat, dokter, dan bidan yang telah bekerja lebih dari sepuluh tahun. Hasil analisis tematik menunjukkan lima temuan utama: (1) Terdapat perubahan signifikan dalam sikap masyarakat seperti meningkatnya sikap kritis, ketergantungan pada informasi daring, dan menurunnya kepercayaan terhadap nasihat medis; (2) Perubahan ini memengaruhi cara tenaga kesehatan berkomunikasi, menyuluh, dan menjalin relasi dengan pasien; (3) Strategi adaptasi dilakukan melalui komunikasi empatik, pendekatan berbasis budaya lokal, dan pemanfaatan media digital; (4) Tantangan yang dihadapi mencakup miskomunikasi, resistensi budaya, dan rendahnya partisipasi masyarakat dalam program preventif; (5) Upaya menjaga kualitas pelayanan dilakukan melalui relasi interpersonal, kolaborasi internal, dan pendekatan berbasis komunitas. Penelitian ini menyimpulkan bahwa tenaga kesehatan di wilayah tidak hanya berperan sebagai pemberi layanan medis, tetapi juga sebagai mediator budaya yang harus mampu menghadapi dinamika sosial yang terus berkembang.
