Abstract

Today's modern teens generally have a sedentary lifestyle. This is very worrying because the sedentary lifestyle is one of the main factors in the emergence of various non-communicable diseases at an early age, one of which is obesity. The Covid-19 pandemic demands the government to apply Studying From Home to all students which has an impact on the lowering of physical activity carried out by students. This research is an online survey research using google form media which was conducted in the period April 2020 to July 2020, with the sample of this study were active students who took online lectures. The variables of this study were physical activity (MET), body mass index, physical activity classification, and nutritional status. The results of this study are in the form of physical activity. Students generally are sedentary in 168 (85.3%) respondents with an average MET of 432.19 (178.64), an average body mass index of 23.16 (4.96) kg / m2 with nutritional status dominated by normal nutritional status in 76 (38.6%) respondents. The results of statistical tests found that there was no significant relationship between physical activity and body mass index (BMI) (p-value> 0.05). On the other hand, although there is no significant relationship between the 2 variables, this must be a major highlight, because the sedentary lifestyle can still cause other non-communicable diseases such as hypertension and diabetes mellitus.

Abstract

Background: Chronic Kidney Disease (CKD) is a global health problem with increasing prevalence and incidence of kidney failure, bad prognosis, and high treatment cost. The prevalence of CKD increases with the increasing number of elderly population and the incidence of diabetes mellitus and hypertension. Comorbidities of patient with CKD, according to Indonesian Renal Registry in 2018, are hypertension or uncontrolled blood pressure (51%) and type 2 diabetes mellitus (21%). Methods: Cross-sectional studies of patients in “RT” Hospital Jakarta between 2018-2019. The independent variable in this research is hypertension comorbidities and medication adherence, whereas glomerulus filtration rate as the dependent variable. the correlation of cause and effect is tested with Independent T-Test and Mann Whitney Test as an alternative. Findings: The research includes 26 respondents, with prevalence of hypertension in diabetic nephropathy patient of 17 (65,4%). Population of patient with diabetic nephropathy with hypertension comorbidities has lower glomerular filtration rate (GFR) compared with group of diabetic nephropathy patients without comorbid hypertension (14.4% vs 28.6%). Hypertensive patients with no routine treatment have average GFR of 8.5%, and hypertensive patients with routine treatment have GFR of 16.15%. Statistical test results with Mann Whitney shows that there are no meaningful difference in average GFR between the two groups (p-value: 0.130).Conclusion: Blood pressure control and medication adherence can slow down the decline in renal function caused by complication of diabetic nephropathy, although there are no meaningful relation found in this research because of lacking in samples.

Abstract

Background: Public health is influenced by four factors namely behavioural factors, environmental factors, heredity and health service factors. A very dense settlement and poor sanitation can lead to the occurrence of environmental-based diseases such as pulmonary tuberculosis (TB). This study aims to look at aspects of economic conditions and the environment in the homes of TB, especially in RW 001 in the working area of ​​the Kedaung Kali Angke Village Health Center, Jakarta. Method: Survey research conducted at the homes of TB sufferers and their neighbors' homes. Data processing was carried out by looking at the results of the questionnaire, and the TB screening form, then intervening with sputum examination of respondents with TB symptoms. Results: There were 4.3% respondents with a history of previous TB disease, with a history of TB disease in the same household as 24.3% respondents. 64.3% of respondents roof top zinc/asbestos. There were 72.9% respondents who had ventilation less than 10% of the floor area and in terms of natural room lighting there were 64.3% respondents with less natural lighting. Conclusion: Some environmental factors that influence the incidence of pulmonary TB in RW 001 are the area of ​​the house that is lacking, solid occupancy, ventilation with less than 10% of the floor area and lack of natural lighting.

Abstract

Acne vulgaris is an inflammatory condition that affects the pilosebaceous glands on a localized basis. According to the Indonesian Cosmetics Dermatology Study, the prevalence of acne vulgaris increased between 2006 and 2009. Young women between the ages of 14-17 years have a prevalence of 83-85 percent, while men between the ages of 16-19 years have a prevalence of 95-100 percent. Acne vulgaris has a significant physical and psychological impact on adolescent sufferers. Accuracy in the treatment of acne vulgaris is critical because it has an impact on the patient's recovery and prognosis. A topical anti-acne cream containing retinoids, antibiotics, and corticosteroids is one of the best options because it contains all of the components necessary to treat acne. The purpose of this study was to ascertain the proportion of acne vulgaris sufferers in adolescents aged 14-19 years with mild, moderate, and severe acne levels prior to and following intervention, to ascertain the relationship between intervention and reduction in the severity of acne vulgaris, and to ascertain the proportion of acne vulgaris sufferers in adolescents who received intervention and experienced a reduction in the severity of acne vulgaris. This study is a retrospective cohort study conducted at the Indra Clinic. The sample of this study was the entire group of adolescents aged 14-19 years with acne vulgaris who came to the study site in the 2016-2017 period and met the inclusion criteria. The formulation observed was a combination of anti-acne cream with the active ingredient clindamycin 3%, Tretinoin 0.05%, and Dexamethasone 0.05%, with the comparative drug being a standard regimen containing 0.05% tretinoin. The dependent variables in this study were treatment success (improved or not) after 4 weeks intervention, side effects during treatment, and post-treatment symptoms. The results indicated the cure rate was 87,6 percent for the 97 respondents who received a combination formulation containing Clindamycin 3%, Tretinoin 0.05%, and Dexamethasone 0.05%, compared to a 71,0 percent cure rate for the 93 respondents who received standard therapy containing Tretinoin 0.05% (p-value : 0,008).

Abstract

Background: Psoriasis is a chronic inflammatory skin disease characterized by persistent itching and a high rate of recurrence. Until now, the primary issue with these two diseases has been their inability to respond to therapy; thus, it is critical to develop an appropriate combination therapy for these two diseases. Due to the fact that psoriasis patients experience alternating periods of remission and exacerbation, dermatologists should tailor psoriasis treatment to the severity of the disease at the time it manifests, with the goal of extending remission and improving quality of life. Thus, it is critical to achieve maximum treatment efficiency in order to prolong the period of remission and improve quality of life. Method: This study is a study comparing the effectiveness of a new treatment therapy using a combination therapy of 0.05% Clobetasol with 3% liquor carbonis detergent and 2% salicylic acid compared to standard therapy, namely 0.05% Clobetasol. This study is a restrospective cohort therapy at the Indra Clinic. The samples of this study were all cases of psoariasis of the skin at Indra's clinic in the 2016-2017 period that met the inclusion criteria. The independent variable of this study was the drug formulation with the dependent variable in the form of clinical improvement and the appearance of side effects. Results: The cure rate was 86,2 percent for the 29 respondents who received the combination formulation, compared to 54,5 percent for the 22 respondents who received standard therapy containing Clobetasol 0.05 percent (p : 0,028). There were no statistically significant differences in adverse events between the two intervention groups. Conclusion: Innovative drug formulations (clobetasol 0.05% with 3% liquor carbonis detergent and 2% salicylic acid) for psoariasis are proven to be more effective and superior than standard therapies

Abstract

Introduction: Coronavirus Disease 2019 (COVID-19) has been an epidemic since December 2019, and has had a huge impact on the world. Various protocols have been established to assess the parameters of post-covid-19 infection, starting from monitoring clinical symptoms and 14-day self-isolation, periodic qPCR examinations, blood laboratories, and radiological examinations. Case Report: A 41-year-old man was diagnosed with SARS-CoV-2 with mild symptoms, on periodic qPCR follow-up examinations, positive qPCR results were obtained even up to the 35th day post-covid-19 infection. Monitoring in terms of clinical symptoms found that all clinical symptoms were in remission on the 5th day after diagnosis. Radiological examination was carried out to assess the patient's improvement on day 36 and the results were bilateral pneumonia in the lungs Discussion: Clinical symptoms and periodic qPCR examinations for monitoring improvement are good to use, but it would be better if radiological examinations were carried out as parameters for improving patients after SARS-CoV-2 infection Conclusion: Radiological examination is an important parameter to assess improvement after SARS-CoV-2 . infection

Abstract

The Covid-19 case has become a pandemic and a major problem throughout the world, especially Indonesia due to its very easy and fast spread from person to person. These medical problems directly affect social life, especially legal issues that cause friction between the confidentiality of patient data as a form of private law and the disclosure of public information as a form of modern society's demands for open, actual, and transparent information. This research is a study that reviews normative juridical by means of analysis and synthesis of deductive conclusions from various statements in various secondary data sources which are all relevant and related to the main issues discussed in this journal with the aim of obtaining an overview of legal reviews and comparative studies related to this issue. . A literature search reveals the principle of legal certainty and legal protection for the community, especially in the era of the Covid-19 pandemic. The status of medical records is also a part of private legal aspects that are confidential and may not be disseminated to the general public without the permission of the owner or legislation. However, in the era of the Covid-19 pandemic, accompanied by the public's encouragement of public information disclosure, the principle of public interest must be guaranteed by the rule of law, by demonstrating a strong commitment to the principle of "nothing is above the law". Therefore, good governance is a must to create excellent service to the community. Further implementation of this policy by the Government as the regulator is important in order to facilitate public interests by not injuring individual rights.

Abstract

Background: Clinical Pathway (CPW) is a clinical documentation technique that focuses on clinical practice standards for nurses, physicians, and others. The clinical pathway is a two-way instruction for handling patients on clinical problems, diagnosis, and stages of care. Any intervention given and the development of the patient is documented systematically based on the time criteria set and expected to increase the service quality and lower hospital expenses. This paper aimed to measure the CP role in increasing hospital expenses efficiency. Methods: A systematic literature review was conducted to determine the effectiveness and efficiency of using clinical pathways for financing. The inclusion criteria were all types of research from descriptive to Randomized Controlled Trials containing at least 2 methods of comparison of interventions published in the last 30 years. Results: This review found 129 articles related to the relationship between clinical pathway use and the effectiveness of financing in hospitals. After going through the screening process based on the inclusion and exclusion criteria, it was found that only 15 articles met the requirements. After analyzing the data, the use of Clinical Pathway in managing patients can lower hospital expenses. Conclusion: Implementing a clinical pathway for patients’ management in hospitals can increase hospital expenses efficiency.

Abstract

A clinical pathway describes the best way to care for a specific diagnosis or procedure, including the sequencing and timing of interventions by doctors, nurses, and other healthcare professionals. It is a relatively new clinical process improvement tool that is gaining traction in hospitals across the United States, Australia, and the United Kingdom. Clinical pathways are created through the collaborative efforts of clinicians, nurses, pharmacists, physiotherapists, and other allied healthcare professionals with the goal of improving patient care quality. Clinical pathways have been shown to reduce unnecessary variation in patient care, reduce discharge delays through more efficient discharge planning, and improve clinical service cost-effectiveness. Clinical pathways' approach and objectives are consistent with those of total quality management (TQM) and continuous quality improvement (CQI), and are essentially the application of these principles to the patient's bedside. This article investigates the increasing use of clinical pathways, their benefits to healthcare organizations, their use as a tool for CQI activities directly related to patient care, and the medico-legal implications involved.

Abstract

Latar Belakang: Penyakit kardiovaskular adalah penyakit yang mengenai organ jantung dan pembuluh darah. Penyakit kardiovaskular diperkirakan menyebabkan kematian 17,7 juta orang per tahunnya. Prevalensi penyakit kardiovaskular di Indonesia diperkirakan sebesar 1,5%. Merokok adalah salah satu faktor risiko timbulnya penyakit kardiovaskular dan angka kejadian penyakit kardiovaskularnya ditemukan lebih tinggi dibandingkan yang tidak merokok. Prevalensi jumlah perokok laki-laki yang berusia diatas 15 tahun di Indonesia adalah sebesar 76,2%. Six Minute Walk Test adalah tes sederhana yang dapat digunakan untuk menilai kapasitas fungsional kardiovaskular seseorang. Subjek penelitian ini adalah petugas keamanan. Penelitian ini bertujuan untuk melihat perbandingan kapasitas fungsional kardiovaskular melalui Six Minute Walk Test antara perokok dan tidak merokok. Metode: Penelitian ini adalah penelitian analitik komparatif numerik antara 2 kelompok tidak berpasangan dengan desain cross-sectional. Hasil: Didapatkan 80,4% subjek merokok dan 19,6% subjek tidak merokok. Rata-rata subjek berusia 30,29 tahun. Kesimpulan: Tidak ditemukan adanya perbedaan kapasitas fungsional antara perokok dan non-perokok.
