Abstract

Background: The implantation of a permanent pacemaker (PPM) can reduce right ventricular function. Echocardiography using speckle tracking can detect a decreasing in right ventricular function earlier. The value of right ventricular global longitudinal strain (RVGLS) based on the location of the pacemaker lead between the apex and non-apex was currently unknown, although the placement of the correct pacemaker lead location was very important for evaluating right ventricular dysfunction to prevent right heart failure. This study aims to determined the comparison of RVGLS between pacemaker lead position in patients with permanent pacemaker. Methods: This study was a nested case-control study to assess the comparison of RVGLS between pacemaker lead position in patients with permanent pacemaker, who were divided into the right ventricular apex group (RVA) and the non-right ventricular apex group (NRVA). This study used data from the pacemaker registry and medical records of patients who had undergone pacemaker implantation since June 2021. The shapiro-wilk normality test was performed before analyzing all numerical data, followed by an independent t-test or Mann-Whitney test to determine the differences between groups. Results: In this study, there were 38 patients with permanent pacemakers, consisting of 18 samples with RVA group and 20 samples with NRVA group. In this study, no significant differences were found in age, sex, diagnosis, comorbidities, therapy, pacemaker mode, baseline QRS duration, pacing burden, puncture site, and initial echocardiography between of two groups. There was a significant difference in paced QRS duration between the RVA and RVNA groups (160 + 20 ms vs 140 + 28 ms, p=0.024). Based on statistical analysis, there was a significant difference in the value of RVGLS in the RVA group compared to the RVNA group (-14.87+4.48% vs -18.40+3.21%, p=0.015). Conclusion: The position of the apex right ventricular lead resulted in a lower value of RVGLS compared to the position of the non-apex right ventricular lead.

Abstract

It remains controversial whether antral follicle count is influenced by hormonal contraception.. This study quan-tified the effect of hormonal contraception on sonographic ovarian reserve markers in 32 users and 32 non-users of hormonal contraception. This was a prospective clinical trial study and has been performed in gynecology Department of M. Djamil Hospital Padang, primary health care March to July 2014. Samples then divided into two groups, group A was users of non hormonal contraception and group B was users of hormonal contraception. On day 2–4 of the menstrual cycle transvaginal sonography was performed and measured repeatedly in the same individual before and after the use of one cycle oral contraceptives (for group B), and on first month and second month (for group A). Statistical analysis to assess significance using the unpaired t test and chi square on SPSS 18.0 for windows. There was no significant association of the antral follicle count between users than non-users of hormonal contraception (p>0,05). There were no significant difference of antral follicle count between users and non- users of hormonal contraception. There were no significant difference of gain score of antral follicle count between users and non-users of hormonal contraception.Keywords: Antral Follicle Count, Users Of Hormonal Contraception, Non-Users Of Hormonal Contraception

Abstract

Preeclampsia is an important issue in obstetrics because it is still a major cause of maternal mortality compared to bleeding and infection. Similarly, the national prevalence, preeclampsia prevalence in West Sumatra is also constantly increasing. Many factors have been identified as risk factors. This study purposed to identify and analyze risk factors of preeclampsia consist of individual factors. This study was an analytical observational study with case-control design, done in Obstetrics and Gynaecology Department of RSUP DR. M. Djamil Padang on January-December 2013 until samples were completed. Samples consist of 70 preeclampsia patients as the treatment group and 70 healthy maternity patients as the control group in RSUP DR. M. Djamil Padang. The results showed there were significant differences between individual risk factors in the preeclampsia group than the control group.Keywords: Individual Risk Factors, Preeclampsia

Abstract

Management of preeclampsia is limited to blood pressure control and symptom relief in combination with close maternal/ fetal surveillance. Until recently, very little attention has been paid to preeclampsia's psychological consequences, especially maternal anxiety. Anxiety disorders can be identified using questionnaires instrument determining anxiety level, the Taylor Minnesota Anxiety Scale (TMAS) questionnaires is one among them. This study aimed to determine the TMAS maternal anxiety score difference between preeclampsia and normal pregnancy. This was an analytical study with a cross-sectional design conducted from March-August 2014 in 40 subjects, consisting of 20 women with preeclampsia and 20 women with normal pregnancy who met the inclusion and exclusion criteria. Anxiety was measured with the TMAS questionnaire. Statistical analysis to determine the TMAS maternal anxiety score difference between preeclampsia and normal pregnancy was t-test. In preeclampsia group, the mean of TMAS maternal anxiety score was 30,45 ± 9,65. While in the normal pregnancy group, the mean of TMAS maternal anxiety score was 18,50 ± 7,62. There was a significant TMAS maternal anxiety score difference between preeclampsia and normal pregnancy (p = 0,000).Keywords: TMAS maternal anxiety score, preeclampsia

Abstract

Pneumonia merupakan penyakit infeksi saluran pernapasan akut yang menjadi kausa utama kematian balita. Di Indonesia, pada akhir tahun 2000, angka kematian balita akibat pneumonia diperkirakan 4,9/1000 balita. Faktor sosio-ekonomi berkontribusi besar terhadap penyakit saluran pernapasan. Tujuan penelitian ini adalah mengetahui faktor sosio-ekonomi yang paling berpengaruh terhadap pneumonia pada balita. Penelitian dengan dengan disain krosseksional ini menggunakan sumber data sekunder Benefit Evaluation Study (BES) II oleh Pusat Penelitian Kesehatan Universitas Indonesia bekerja sama dengan Proyek Intensifikasi Pemberantasan Penyakit Menular, Departemen Kesehatan. Sampel diambil berdasarkan multilevel statistical frameworkdari 7.170 ba-lita pada 10.900 rumah tangga di 27 kabupaten di tujuh provinsi. Metode analisis yang digunakan adalah multilevel logistic regression. Penelitian ini memperlihatkan bahwa sosio-ekonomi rumah tangga berperan secara bermakna terhadap kejadian pneumonia balita. Rumah tangga miskin berisiko lebih besar untuk terkena pneumonia. Pada keluarga miskin, risiko pneumonia yang lebih besar disebabkan oleh faktor kontekstual lingkungan yang buruk berupa pencemaran di dalam rumah yang dikontrol faktor komposisi status gizi (95% CI OR 4.05- 4.78). Kebijakan intervensi program P2ISPA disarankan lebih mengutamakan intervensi pada faktor kontekstual lingkungan buruk pencemaran dalam rumah tangga miskin. Pneumonia is an acute respiratory tract infection disease that becomes a major cause of death among under five years old children. In Indonesia, in 2000, pneumonia specific cause of death rate among under five children is predicted to be 4.9/ 1000. The socio-economic factor has significant contribution to respiratory tract infection. The objective of this study is to know the socioeconomic factor that affect pneumonia among under five children. The study uses cross sectional study design using secondary data of Benefit Evaluation Study (BES) II conducted by Centre for Health Research, University of Indonesia in collaboration with Intensification of Infectious Diseases Eradication Project, MOH-RI. The study sample is selected based on multilevel statistical framework from 7170 under five children in 10900 households within 27 districts in seven provinces. Analysis method used in this study is multilevel logistic regression. This study shows that the low level of socioeconomic status affect significantly the pneumonia occurrence among under five children. The risk of pneumonia among lower socioeconomic household is higher than that of the high socioeconomic household. It was found that the association was found for poor environmental factor including in-house hygienic condition after controlled by nutritional status. The pneumonia occurrence among under five children is more influenced by environmental factors than individual factors (compositional effect). It is suggested to prioritize intervention on environmental factors to eradicate respiratory tract infection.

Abstract

Tuberculosis (TB) is a leading public health concern in Indonesia. It ranks second on the list of high-burden TB countries. In West Sumatra, 47% of TB cases are undetected, late diagnosed, and received incomplete treatment because of low-level awareness and knowledge and stigma, especially among the hardest to reach populations. The study aims to identify the best communication channel to reach those who live in vulnerable and remote areas. This study was a qualitative study applying in-depth interviews to the informal leaders, health officers, cultural artists, and religious leaders across districts in Mentawai and Solok Districts, which are remote and had the lowest case detection rates compared with other districts. The questionnaire was prepared with the perception of the channel to identify TB cases. The data were analyzed using the content analysis technique. Involving religious and informal leaders and using traditional music as a communication channel improved the population's awareness of TB symptoms and access to TB testing and treatment, as well as reduced TB-related stigma. This study found that the cultural and religious contexts play a major role in health communication on TB control for hard-to-reach populations in West Sumatera, Indonesia.

Abstract

The low utilization of antenatal care services has an impact on the condition of the pregnancy not being monitored which will cause danger signs of pregnancy to go undetected and can lead to maternal death. Padang Lawas is the area with the lowest coverage of antenatal care visits in north Sumatra province. The majority of pregnant women in Padang Lawas district do not receive standard antenatal care services. Efforts to increase coverage rates for Antenatal Care visits have been carried out by the Health Service but have not produced any real impact. Objective: This study aims to find out more about the perspective of antenatal care visits for pregnant women. Method: This study was qualitative with a grounded theory approach. The informants in this study were 5 mothers, 5 husbands, 3 midwives, and 1 community figure who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 4 themes: (1) Culture about pregnancy, (2) Infrastructure in antenatal care, (3) Economic issues, and (4) Mother's behavior in carrying out antenatal care. Conclusions: The findings show that there are several barriers of antenatal care visits in rural maternal communities, so the government needs to pay attention to them in following up on the barriers found in these areas.

Abstract

Weight gain that is not according to recommendations during pregnancy has become an epidemic throughout the world today and is a clinical problem with high prevalence and can affect the health of mothers and babies. Objective: This study aims to find out more about the perspective of increase in weight of pregnant women in deli serdang regency. Method: This study was qualitative with a descriptive approach. The informants in this study were 3 pregnant women with weight gain above recommendations, 3 pregnant women with weight gain below recommendations, 1 ob-gyn, 1 midwife, and 1 nutrition who were recruited using purposive sampling. Data were collected using semi-structured interviews. Data were analyzed using the Content Analysis. Results: The study emerged 5 themes: (1) Knowledge, (2) Perspective, (3) Behaviour, (4) Family support, and (5) Physical activity. Conclusions: The findings show that several factors influence the increase in weight of pregnant women in Deli Serdang Regency, so the health care provider needs to pay attention to improve their knowledge about weight gain during pregnancy according to the needs of pregnant women.

Abstract

The purpose of neonatal visits is to re-examine newborns, review counseling and anticipation guidelines with parents, identify symptoms of illness, and educate and support parents. The purpose of neonatal visits is to improve neonatal access to basic health services and to find out as early as possible if there are abnormalities in the baby or if there are problems. One important indicator to describe the level of public health is the infant mortality rate. Objective to find out more about the perspectives on neonatal visit coverage in Serdang Bedagai District, Indonesia. The study was a qualitative with phenomenology approach. Twenty-five informants were applied in this study using purposive sampling. Data were collected using in-depth interviews. Data were analyzed using content analysis by the collaizi method. The study emerged nine themes, namely: 1) knowledge about neonatal visits; 2) neonatal visit quantity standards; 3) neonatal visit quality standards; 4) health care workers providing neonatal visiting services; 5) purpose of neonatal visits; 6) relationship between education and neonatal visit compliance; 7) relationship between husband/family support and neonatal visit compliance; 8) reasons for non-compliance with neonatal visits; and 9) culture in support of public health. These results can be input for policymakers to increase the coverage of neonatal visits in Serdang Bedagai Regency, Indonesia.

Abstract

Ketidakpatuhan pasien tuberkulosis dalam pengobatan merupakan penyebab terpenting kegagalan pengobatan dan menjadi hambatan untuk mencapai kesembuhan Tujuan penelitian ini yaitu untuk mengetahui faktor-faktor yang berhubungan dengan kepatuhan berobat pasien tuberkulosis di Puskesmas Andalas. Penelitian ini adalah penelitian analitik observasional dengan rancangan cross sectional. Teknik pengambilan sampel menggunakan total sampling dengan jumlah sampel sebanyak 53 responden. Sampel penelitian adalah pasien tuberkulosis sensitif obat yang berusia diatas 18 tahun dan memiliki kartu TB-01. Pengumpulan data dilakukan dengan wawancara terstruktur menggunakan kuesioner. Analisis data menggunakan uji  Chi-square. Analisis 53 sampel didapatkan sebanyak 43 responden (86,8%) patuh dalam pengobatan, sedangkan sebanyak 7 responden (13,2%) tidak patuh dalam pengobatan TB paru. Hasil penelitian menunjukkan variabel yang berhubungan dengan kepatuhan berobat adalah efek samping obat (p=0,015) dan peran pengawas minum obat (p=0,001), dan variabel yang tidak berhubungan adalah umur (p=0,419), jenis kelamin (p=0,223), tingkat pendidikan (p=0,443), pekerjaan (p=1,000), lama pengobatan (p=0,661) dan jarak rumah (p=0,451). Kesimpulan penelitian ini didapatkan faktor-faktor yang berhubungan dengan kepatuhan berobat pasien TB yaitu, efek samping selama pengobatan dan peran pengawas minum obat
