Abstract

Prolanis adalah sistem pelayanan kesehatan menggunakan pendekatan proaktif dan dilaksanakan secara integratif dengan melibatkan peserta, fasilitas kesehatan dan BPJS kesehatan dalam rangka pemeliharaan bagi yang menderita penyakit kronis untuk pelayanan kesehatan secara efektif dan efisien. Data BPJS Kesehatan selama tiga tahun terakhir yaitu tahun 2019, 2020 dan 2021 tidak ada satu pun Puskesmas di Kota Kupang yang dapat memenuhi target Rasio Peserta Prolanis Terkendali (RRPT ? 5%) per tahun. Penelitian ini bertujuan untuk menganalisis faktor yang mempengaruhi pengendalian hipertensi pada peserta prolanis di Puskesmas Se - Kota Kupang tahun 2022. Metode dalam penelitian yaitu penelitian kuantitatif menggunakan pendekatan cross sectional dengan populasi dalam penelitian ini yaitu peserta prolanis di 11 Puskesmas Kota Kupang yang terdiagnosa hipertensi. Teknik pengambilan sampel dalam penelitian ini adalah teknik cluster random sampling. Data dalam penelitian ini diambil dengan menggunakan kuesioner yang disebar pada peserta prolanis di 11 Puskesmas Kota Kupang sebanyak 269 orang. Data dianalisis dengan menggunakan uji Regresi Linear Berganda dan hasil uji menunjukkan bahwa variabel berpengaruh signifikan terhadap pengendalian hipertensi peserta prolanis yaitu dukungan keluarga (p = 0,039), pola hidup (p = 0,015), keterjangkauan (p = 0,002) dan kepercayaan (p = 0,003). Sehingga menjadi masukkan yang baik terhadap pelaksanaan program pencegahan dan pengendalian penyakit kronis untuk dapat membantu pihak puskesmas mencapai target pelaksanaan program pencegahan yang efektif dan efisien.

Abstract

Diabetes represents a significant public health concern, affecting millions of individuals worldwide. Its prevalence is increasing, driven in part by lifestyle factors and the aging of the global population. This systematic review explores the potential of artificial intelligence (AI) in enhancing diabetes prevention, diagnosis, and management. The review highlights the promise of personalized and proactive healthcare enabled through AI. The research methodology employed an exhaustive review of the literature, the formulation of specific inclusion and exclusion criteria, a data extraction process from selected studies that focused on the role of AI in diabetes, and a comprehensive analysis to identify the specific domains and functions in which AI makes a significant contribution. The results of the conducted literature review indicate that artificial intelligence (AI) can be regarded as a transformative force in the following eight key areas within the field of diabetes care: 1) Management and Care of Diabetes, 2) Diagnostic and Imaging Technologies, 3) Health Monitoring Systems, 4) Development of Predictive Models, 5) Public Health Interventions, 6) Lifestyle and Dietary Management, 7) Enhancement of Clinical Decision Making, and 8) Engagement and Self-Management of Patients. Additionally, the utilization of AI may result in a reduction in the risk of mortality from diabetes.

Abstract

 Background : Chronic kidney disease is a condition of kidney damage which is characterized by a progressive decline in kidney function over several months or years so that the kidneys cannot maintain metabolic, fluid/electrolyte balance which results in uremia, and if not treated properly, This disease can cause death, therefore Chronic kidney disease patients need hemodialysis therapy. Chronic kidney disease patients increase every year, data patients at Ruteng Hospital is 35 patients and 8 people are participants in the informal sector of the National Health Insurance. The national health insurance policy is regulated in Presidential Decree No. 64 of 2020. This regulation is intended to re-regulate several aspects of program implementation, starting from increasing the amount of contributions, types of participation, the ecosystem of the national health insurance program, governance of subsidies by the Government for participants. Objective: Analyze and evaluate completely, in detail and systematically in connection with the implementation of the national health insurance policy on the amount of informal sector payments for hemodialysis services at Ruteng Hospital. Method: The research design used is a qualitative approach using descriptive analytical methods by collecting data from participants through in-depth interviews. This research was conducted at Ruteng Hospital. The population was 35 hemodialysis patients. The sampling technique was purposive sampling, taken from samples that met the inclusion criteria: the main participants were 7 patients and the supporting participants were 6 employees. Data collection techniques use primary and secondary data through interviews and documentation, data validity uses triangulation and data analysis uses thematic analysis. Results: dialysis patients only understand the usefulness of the national health insurance program to facilitate their health services in terms of costs and facilities without exploring all the elements contained in the program, the quality of service is very satisfying for dialysis patients both in terms of completeness of facilities and infrastructure as well as other supporting facilities, the safety of their care is guaranteed both in terms of ease of accessing program services and quality and other resources, the impact of the increase in rates shows that Hemodyalisis patients feel objections, resulting in negative impacts, moving treatment classes to lower classes and there are payment in arrears but there are no non-active participants and the positive impact is that there is an increase in the provision of higher quality health services and Health Social Security Agency can experience a surplus, there is an increase in the quality of health and productivity of life for Hemodyalisis patients in maintaining resilience and sustainability as well as social welfare. Conclusion: Health Social Security Agency always carries out monitoring and evaluation in connection with the determination of policies regarding determining the amount of national health insurance contributions based on the financial situation and conditions of the community and taking into account the ability of the State treasury to allocate a budget to subsidize various program operational costs while maintaining and improving system governance National health insurance, for Ruteng Hospital to always improve the quality of services in order to achieve patient satisfaction, for Hemodyalisis patients to be involved in providing contributions in the form of suggestions and criticism to the Government in formulating policies and to be involved in evaluating the national health insurance program.

Abstract

The implementation of the COVID-19 health protocol is one of the prevention efforts recommended by the government to reduce the transmission of COVID-19 in Indonesia. The market is one of the public places at risk of transmission of COVID-19, but there are still many traders who have not implemented health protocols properly. This study aims to determine the factors related to the behavior of the COVID-19 health protocol on traders at the Oesapa market, Kupang City. This type of research is quantitative research using a cross sectional design. The sample consisted of 100 people with accidental sampling technique. Data analysis used univariate and bivariate analysis with Chi-square test with a significance level of =0.05. The results showed that the factors related to the behavior of the COVID-19 health protocol on traders were age (p-value = 0.007), education level (p-value = 0.000), and level of knowledge (p-value = 0.003), while the unrelated are gender (p-value = 0.967), availability of information sources (p-value = 1,000), and availability of infrastructure (p-value = 0.249). Traders are expected to be able to improve the behavior of the COVID-19 health protocol. Market managers are expected to provide counseling by using posters about preventing COVID-19 in the market area.

Abstract

There are many causes of death for women n this world, one of which s breast cancer. Breast cancer s the second largest contributor to the death rate for women after cervical cancer. This disease s a condition where cells n the breast grow rapidly and attack all parts of the body. However, only a small proportion of women perform BSE, namely 25 to 30% of young women who do BSE. This study aims to analyze the determinants of breast self-examination (BSE) behavior n female adolescents at high schools n Kalabahi City, Alor Regency. This research s a quantitative study using a cross sectional study design method. The sample n this study were all young women attending high schools n Kalabahi City, Alor Regency, namely 312 young women. The data analysis used n this study was univariate analysis and bivariate analysis, using the chi-square test with a significant level of 95% (α = 0.05). The results showed that there was a significant relationship between knowledge (ρ-value = 0.000), attitude (ρ-value=0.000), nformation exposure (ρ-value=0.000), family history (ρ-value=0.000), and peer support (ρ-value=0.000) with BSE behavior. Young women are expected to be able to play an active role and have the willingness to seek nformation related to BSE and do so according to the right steps. Schools and health offices are expected to be a source of nformation for young women regarding BSE through various health promotion efforts, especially counseling or socialization related to BSE.

Abstract

Diabetes Melitus (DM) merupakan suatu penyakit menahun yang ditandai dengan kadar glukosa darah (gula darah) melebihi normal. Diabetes Melitus masih menjadi suatu masalah kesehatan yang serius di Nusa Tenggara Timur karena masih tingginya angka kejadian diabetes melitus, apabia dibiarkan maka akan menyebabkan terjadinya komplikasi. Penelitian ini bertujuan untuk mengetahui faktor yang berhubungan dengan pencegahan komplikasi penderita diabetes melitus di Puskesmas Sikumana. Jenis penelitian dalam penelitian ini adalah survei analitik, dengan rancangan penelitian, cross sectional. Jumlah sampel dalam penelitian ini sebanyak 85 orang penderita diabetes melitus yang belum mengalami komplikasi. Teknik analisis data menggunakan uji statistik Chi-square. Hasil penelitian menunjukkan bahwa terdapat hubungan antara pengetahuan (p-value= 0,023), sikap (p-value= 0,037), diet (p-value= 0,000), dan tidak terdapat hubungan antara dukungan keluarga (p-value= 1,000) dengan pencegahan komplikasi penderita diabetes melitus di wiayah kerja Puskesmas Sikumana. Kesimpulan, terdapat hubungan antara pengetahuan, sikap, diet, dan tidak terdapat hubungan antara dukungan keluarga dengan pencegahan komplikasi penderita diabetes melitus di wiayah kerja Puskesmas Sikumana

Abstract

ABSTRACTIntroduction: Oral health service quality has been severely impacted by the COVID-19 pandemic, including in Kupang, one of the cities in the east part of Indonesia. Like in other cities, community health centers (Puskesmas) are the frontline to provide dental healthcare, including avoiding the spread of COVID-19. The objective of this study therefore is to analyze the correlation between oral health service quality  (patient safety, effectiveness, patient centeredness, waiting time, efficiency, and equity) and patients’ satisfaction during COVID-19 pandemic. Methods: Cross sectional design was used to investigate the correlation of oral health services quality and patients’ satisfaction. Structured questionnaire was used to measure patients’ satisfaction. The number of samples involved in this study were fifty people. This study employed a non-probability sampling technique known as incidental sampling. The results of this research were obtained using multivariate multiple logistic regression analysis. Results:  Patients involved in this study consist of women (60%), teenager (50%), adult (40%), educational background is senior high school (56%), junior high school (20%), employment statuses are farmer (30%) and student (28%), monthly average income is < 2 million rupiahs (and government insurance user accounting for 70%). Based on statistical analysis, there is a significant relationship between oral health service quality and patients’ satisfaction (p = 0,002). Conclusion: Patients are satisfied with oral health service quality in Kupang city during COVID-19 pandemic particularly on an equity aspect.Keywords: oral health service quality, patient satisfaction, community health center, COVID-19 pandemic

Abstract

Diabetes melitus adalah gangguan metabolik yang ditandai dengan tingginya kadar gula dalam darah yang disebut hiperglikemia dengan gangguan metabolisme karbohidrat, lemak, dan protein. Menurut IDF tahun 2021, tercatat sebanyak 537 juta orang dewasa umur 20-79 tahun menderita diabetes di seluruh dunia. Penderita diabetes perlu melakukan self care dalam kehidupan sehari-hari untuk mengontrol glukosa darah dan mencegah terjadinya komplikasi. Penelitian ini bertujuan untuk mengetahui hubungan self efficacy dan dukungan keluarga dengan self care pada penderita diabetes melitus tipe II di Puskesmas Oesapa Kota Kupang. Jenis penelitian ini adalah analitik observasional dengan rancangan penelitian cross sectional. Populasi pada penelitian ini adalah seluruh penderita diabetes melitus tipe II di Puskesmas Oesapa dengan jumlah 317 orang. Teknik sampling menggunakan simple random sampling dengan sampel berjumlah 120 orang. Data dianalisis menggunakan uji Korelasi Spearman dengan signifikansi ?=0,05. Hasil penelitian menunjukan bahwa self efficacy dan dukungan keluarga berhubungan dengan self care yaitu dimensi level (p=0,001) dengan nilai r=0,310, dimensi strength (p=0,007) dengan nilai r=0,243, dimensi generality(p=0,000) dengan nilai r=0,586, dukungan emosional (p=0,013) dengan nilai r=0,225, dukungan penghargaan (p=0,001) dengan nilai r=0,302, dukungan instrumental (p=0,000) dengan nilai r=0,679, dan dukungan informasi (p=0,019) dengan nilai r=0,213. Diharapkan petugas puskesmas dapat merancang program edukasi yang efektif untuk menigkatkan pengetahuan penderita diabetes tentang pentingnya melakukan self care dalam mendukung perbaikan penyakitnya.

Abstract

Analisis spasial adalah salah satu cara pendataan dalam upaya manajemen lingkungan dan merupakan bagian dari manajemen penyakit berbasis wilayah. Salah satu penyakit berbasis lingkungan yaitu demam berdarah dengue. DBD adalah penyakit infeksi akut yang disebabkan oleh infeksi virus dengue yang ditularkan melalui gigitan nyamuk Aedes betina. DBD saat ini telah menyebabkan endemik di 100 negara di wilayah WHO, termasuk Indonesia. Kota Kupang merupakan ibu kota Provinsi NTT yang selama 2019-2022 menjadi salah satu penyumbang kasus DBD yang tinggi dimana pada tahun 2019 kasus DBD sebanyak 681, tahun 2020 meningkat sebanyak 821 kasus, tahun 2021 menurun menjadi 654 kasus dan tahun 2022 menurun menjadi 455 kasus. Penelitian ini bertujuan untuk menganalisis faktor risiko yang mempengaruhi kejadian DBD. Jenis penelitian observasional analitik dengan desain penelitian studi ekologi. Hasil penelitian menunjukkan tidak adanya pengaruh kepadatan penduduk dengan DBD dan terdapat satu wilayah kecamatan yang berada di wilayah High-High yaitu Kecamatan Kota Lama. Ketinggian wilayah tidak berpengaruh dengan DBD dan terdapat satu wilayah kecamatan yang berada di wilayah High-Low yaitu Kecamatan Kota Alak. Curah hujan berpengaruh dengan DBD tahun 2019-2020, sedangkan 2021-2022 tidak berpengaruh dengan DBD. Kelembaban udara berpengaruh dengan DBD. Kecepatan angin tidak berpengaruh dengan DBD. Saran yaitu melakukan pengendalian dan penanggulangan terhadap penyakit DBD terutama pada kecamatan yang memiliki kasus demam berdarah dengue tinggi dan peningkatan preventif terhadap DBD

Abstract

The concept of healthy-sick is no longer only related to the bio-psycho-social-spiritual aspects but also includes the physical and mental aspects. Chronic disease is a long-term and often incurable condition that requires continued management. Medical care, such as medication and surgery, is vital in managing chronic diseases. Non-medical treatments, such as positive thinking, a healthy diet, and fasting, can also significantly impact managing chronic diseases. Writing this article uses a library study or literature review. A holistic approach to health care can improve patient outcomes by improving physical and mental health, promoting better care coordination and communication, improving self-management skills, addressing psychosocial and spiritual needs, and providing patient-centric care.
