Abstract

In Indonesia, 50% of tuberculosis (TB) treatments are discontinued prematurely. The success of TB treatment depends on patient adherence to completing the treatment. Family support is crucial in enhancing TB patient adherence, particularly regarding emotional and appreciation support. This study aims to explore the complexity of family support to TB patient adherence in Deli Serdang Regency, North Sumatra Province, Indonesia. This qualitative study employed participatory observation and in-depth interviews with TB patients and their family members. The research was conducted from February to April 2024 in Deli Serdang Regency, North Sumatra, Indonesia. Informants included 10 TB patients and 16 family members, selected purposively based on data from the public health center and the TB Program Holder. The researcher was the primary instrument of the study, focusing on two dimensions of family support: emotional and appreciation. Data validity was checked through credibility testing, including triangulation. The study findings indicate that family emotional support is reflected in expressions of empathy and caring. Sadness is evident from family members’ tears upon learning that a relative tested positive for TB. Family support is shown by accompanying TB patients to the health center and reminding them to take treatment. Appreciation support is demonstrated by valuing the patient’s treatment process. Family members encourage and motivate the patient to adhere to the treatment. Emotional and appreciation support from family plays a significant role in TB patient adherence to treatment. Families can significantly contribute to the success of TB treatment by demonstrating empathy, caring, and appreciation for the patient.

Abstract

Early menarche, defined as the onset of menstruation before the age of twelve, has been linked to adverse physical and mental health outcomes and is a key predictor of rapid pubertal growth. Although data at the elementary school level in Indonesia remain limited, recent findings suggest a rising prevalence of early menarche in urban areas. This study provides local evidence from an urban school in Padang City, where early menarche appears to be increasing. This research examined associations between early menarche among elementary school girls in Public Elementary School (SDN) 03 Alai Timur, Padang City, and factors including nutritional status, maternal age at menarche, and exposure to mass media information technology. Statistical analyses were conducted using the Chi-square test and the Fisher Exact Test, with a significance level of p < 0.05. Results showed that three-quarters of the participants experienced early menarche. Overweight-obese nutritional status was significantly associated with higher rates of early menarche (95.2%; p=0.011), as was high exposure to mass media information technology (88.9%;  p=0.020). Although not statistically significant (p=0.071), girls whose mothers had experienced early menarche also showed higher rates (90.0%). Overall, early menarche was strongly associated with nutritional status and media exposure, while maternal age at menarche demonstrated a non-significant intergenerational effect. The findings highlight the need for schools to implement programs that educate young girls on proper nutrition, responsible media consumption, and reproductive health from an early age. Keywords:  Early Menarche, Elementary School Students, Nutritional Status, Maternal Menarche History, Mass Media Exposure.

Abstract

The rise in non-communicable diseases (NCDs) is driven by risk factors such as smoking, physical inactivity, low consumption of fruits and vegetables, and alcohol use. Community-based surveillance of these risk factors is essential for NCD prevention; however, the availability and use of accurate data remain limited in many regions. Therefore, research is needed to enhance both the quality and utilization of surveillance data in NCD prevention efforts. This study aims to examine community-based risk factor surveillance activities and their role in preventing NCDs in Padang. A qualitative approach using a phenomenological method was employed. Research informants included individuals responsible for NCD programs at community health centers, health cadres, and members of the productive-age population in Padang in 2024. Informants were selected through purposive sampling. Data were primarily collected via in-depth interviews guided by a structured interview protocol. Interview transcripts were analyzed using an interactive analysis model, which included data reduction, data presentation, and conclusion drawing/verification. Findings indicate that risk factor data collection at integrated health posts was conducted both manually and electronically, with data entered into the Ministry of Health's Asik application. Data processing and analysis were performed using the Asik application, with descriptive interpretations presented in tables and diagrams. Dissemination of risk factor information occurred through mini-workshops, although not all stakeholders were involved. Surveillance results were used to inform NCD prevention programs through counseling and community outreach. Overall, community-based risk factor surveillance data were collected manually and electronically via the Asik application, analyzed descriptively, and disseminated through workshops and follow-up activities supporting NCD prevention. The study highlights the need to improve the quality and use of community-based surveillance data, increase stakeholder engagement, evaluate intervention effectiveness, and assess the impact of surveillance applications on data processing and ongoing monitoring of NCD risk factors. Abstract

Background: My Healthy Indonesia Application is a cornerstone of Indonesia's health system digital transformation, designed to streamline vaccination recording and reporting. However, its implementation at community health centers in Padang City faces challenges, marked by data discrepancies and suboptimal vaccination coverage. Purpose: To identify healthcare worker performance in implementing the healthy Indonesia application. Method: A mixed-methods study was conducted in five Community Health Center in Padang City. The quantitative phase employed a cross-sectional design with total sampling of 127 health workers. Data were collected using validated questionnaires and analyzed using chi-square and multivariate logistic regression. The qualitative phase involved in-depth interviews with 13 key informants, analyzed using thematic analysis. This study investigates factors associated with health workers' performance in using My Healthy Indonesia Application and explores underlying reasons behind these challenges. Results: Quantitative analysis revealed that motivation (p=0.014), leadership (p=0.018), and workload (p=0.009) were significantly associated with performance, with motivation as the most dominant factor (OR 0.254; p=0.011). Knowledge (p=0.602) was not significantly associated. Qualitative findings generated seven themes: complex motivational dynamics dominated by professional responsibility; significant infrastructure limitations (reliance on personal devices/internet); persistent application bugs (data loss, duplication); increased administrative workload; a digital generation gap; adaptive strategies (time management, team collaboration); and a clear recommendation for a single, integrated application. Conclusion: Health worker performance in using My Healthy Indonesia Application presents a paradox: good performance persists despite significant systemic barriers, primarily driven by strong intrinsic motivation. However, this reliance on individual resilience is unsustainable. Improving My Healthy Indonesia Application effectiveness requires holistic interventions addressing technical issues, reducing workload, bridging the digital divide, and nurturing the professional dedication of health workers.
