Abstract

Liver cirrhosis is a major global health concern, often complicated by cardiac dysfunction such as diastolic heart failure. Tumor necrosis factor-alpha (TNF-α) and FIB-4 score are widely used biomarkers in cirrhotic patients, but their predictive value for cardiac complications remains unclear. To determine the effect of TNF-α and FIB-4 scores on the severity of diastolic dysfunction in patients with Child-Pugh C liver cirrhosis. This analytical cross-sectional study included 40 Child-Pugh C cirrhotic patients. Serum TNF-α levels were measured using ELISA. Diastolic function was evaluated using echocardiography. Ordinal logistic regression was used to analyze the association between TNF-α, FIB-4, and diastolic dysfunction grades. The majority of participants had moderate (65.0%) or severe (35.0%) chronic inflammation based on TNF-α levels, with a median TNF-α of 45.52 pg/mL. Significant liver fibrosis (FIB-4 >3.25) was observed in 82.5% of participants. Grade I diastolic dysfunction was the most prevalent (52.5%). TNF-α levels were significantly associated with the degree of diastolic dysfunction (p=0.042), whereas FIB-4 scores showed no significant correlation (p=0.533). Elevated TNF-α is strongly associated with worsening diastolic dysfunction in Child-Pugh C cirrhosis, suggesting its role in systemic inflammation and myocardial injury. In contrast, the FIB-4 index may not reflect cardiac involvement in advanced cirrhosis. TNF-α may serve as a potential biomarker for cardiovascular risk stratification in cirrhotic patients, supporting early intervention and integrated care approaches.

Abstract

Abstract

Infectious diseases are the leading cause of child mortality in Indonesia, yet many deaths can be prevented through immunization. The 2023 Indonesian Health Survey reported a national full basic immunization coverage of only 35.8%, a decrease from 57.9% in 2018 and far from the 100% target. Surakarta City achieved a coverage of 98.34% in 2023, showing strong regional commitment, although it still does not fully meet national goals. This study analyzes the influence of the Theory of Planned Behavior (TPB) on the use of immunization services for infants in Surakarta. Using a cross-sectional design, 200 mothers with infants aged 12–24 months were selected through multistage random sampling. Independent variables include intention, attitude, subjective norm, and perceived behavioral control; the dependent variable is the use of infant immunization services. Data were collected via structured questionnaires and analyzed using linear regression and path analysis. The study found that intention directly affects the use of immunization services (b=0.20; 95% CI=0.07 to 0.33; p<0.00). Attitude, subjective norm, and outcome evaluation indirectly influence immunization service use through intention. The path model demonstrated good fit (CFI = 0.979; TLI = 0.964; RMSEA = 0.079; SRMR = 0.037). Intention plays a crucial role in the use of immunization services, with attitude, subjective norm, and outcome evaluation affecting use through intention. The study model shows good fit. Increasing immunization uptake should focus on strengthening intention through positive attitudes, social norms, and education.

Abstract

The purpose of this study is to evaluate the viability and structural stability of L-PRF at 37°C over 14 days. It aims to determine its sustained growth factor release and potential effectiveness in regenerative clinical applications. This research employed a Systematic Journal Review (SJR) approach, surveying articles from databases such as PubMed, Scopus, and Google Scholar using specific keywords related to L-PRF. A total of 25 articles meeting the inclusion criteria were filtered for analysis. Furthermore, common experimental methods described in the literature include the preparation of L-PRF via blood centrifugation and viability testing over 14 days at 37°C, encompassing analysis of microscopic structure, mechanical properties, and biochemical and biological activities. Literature analysis indicates that L-PRF is an effective autologous biomaterial for regenerative applications, capable of continuously releasing growth factors such as PDGF, TGF-β, VEGF, and IGF for up to 14 days at body temperature. Cell viability within the fibrin matrix is maintained, positioning L-PRF as a promising active biological scaffold for accelerating tissue healing. Although challenges such as matrix degradation persist, modification strategies and optimized centrifugation protocols can enhance the effectiveness and durability of L-PRF in clinical applications.

Abstract

Caesalpinia bonduc (L) Roxb is a medicinal plant commonly utilized in traditional Indonesian medicine, particularly for the treatment of diabetes mellitus (DM). This study examined the effects of varying doses C. bonduc extract on fasting blood sugar (FBS), malondialdehyde (MDA), tumor necrosis factor-α (TNF-α), transforming growth factor-β (TGF-β), and pancreatic histology in male and female wistar rats with diabetes. The study design included six groups of 18 male and female rats (Wistar strain) each. To develop diabetes, nicotinamide (NA) (110 mg/kg) and streptozotocin (STZ) (45 mg/kg) were supplied, followed by oral administration of C. bonduc extract at dosages of 200, 400, and 600 mg/kg for 21 days. We utilized ELISA to analyze biochemical parameters and employed H&E staining to evaluate histological changes. A factorial design was employed for the statistical analysis to examine the effects of both dosage and sex. This study offers a unique comparative examination of the antidiabetic efficacy of C. bonduc across genders, a dimension seldom explored in experimental diabetes research. The findings indicated that C. bonduc markedly reduced levels of FBS, MDA, TNF-α, and TGF-β (p < 0.05) enhanced the structural integrity of the pancreas. There were no significant differences between males and female (p > 0.05). The dosage and sex influenced MDA and TGF-β. The extract of C. bonduc shown the ability to lower blood sugar, fight free radicals, and reduce inflammation; however, the intensity of these benefits depended on the dosage given. This means that it might assist persons with diabetes minimize oxidative stress and damage to the pancreas.

Abstract

Chronic kidney disease (CKD) is a major global health burden, with oxidative stress and hypertension contributing significantly to its progression. Although vitamin D3 is known for its pleiotropic effects, its influence on oxidative stress markers in obstructive nephropathy remains unclear. This study evaluated the effect of vitamin D3 supplementation on blood pressure, malondialdehyde (MDA) levels, and superoxide dismutase (SOD) activity in a rat model of CKD induced by Unilateral Ureteral Obstruction (UUO). In this experimental post-test-only control group study, 30 male white rats (Rattus norvegicus) were randomly assigned to five groups (n = 6 per group) following UUO induction: one control group (placebo) and four treatment groups receiving graded oral doses of vitamin D3 (36, 72, and 108 IU/rat) daily for 14 days. Blood pressure was measured using the tail-cuff method, and serum MDA levels and SOD activity were analyzed at the end of the treatment period. Vitamin D3 supplementation produced significant, dose-dependent improvements in all parameters (p-value < 0,01). Compared with controls, treated groups showed lower blood pressure, reduced MDA levels, and increased SOD activity. The highest dose (108 IU/rat) demonstrated the greatest reduction in oxidative stress and blood pressure. This study aimed to evaluate whether vitamin D3 supplementation ameliorates hypertension and oxidative stress in UUO-induced CKD in rats, supporting its potential as an adjunctive strategy to slow CKD progression. Further studies are required to confirm clinical applicability.

Abstract

Benign prostatic hyperplasia (BPH) progression is critically influenced by the androgen receptor (AR), Fibroblast growth factor-2 (FGF-2), and transforming growth factor-β (TGF-β) signaling axis. Although finasteride (FIN) is the standard therapeutic agent, its use is limited by adverse effects. This study evaluated the efficacy of Cosmos caudatus (CC) extract, administered alone or in combination with FIN, in modulating the AR–FGF-2–TGF-β axis in a testosterone-induced BPH rat model. Rats were divided into five groups (n = 6): Normal (N), BPH control (KN), FIN (KP; 0.44 mg/kg), CC extract (P1; 250 mg/kg), and combination (P2; CC 250 mg/kg + FIN 0.44 mg/kg). Prostatic AR, FGF-2, and TGF-β expression was analyzed using immunohistochemistry (IHC) with H-score quantification. Both KP and P1 significantly reduced AR (KP: 112 ± 15; P1: 105 ± 12 vs KN: 195 ± 18, p < 0.05) and FGF-2 (KP: 98 ± 14; P1: 92 ± 11 vs KN: 180 ± 20, p < 0.05), approaching normal values (AR N: 90 ± 10; FGF-2 N: 85 ± 9). In contrast, combination therapy (P2) failed to suppress AR (188 ± 17, p = 0.936) and FGF-2 (175 ± 19, p = 0.870) and induced a significant increase in TGF-β (P2: 145 ± 16 vs KN: 110 ± 12, p < 0.05). All treatment groups showed reduced epithelial thickness and prostate index (p < 0.05). These findings indicate that CC extract alone exerts therapeutic effects comparable to FIN by modulating the AR–FGF-2 axis, whereas combining it with FIN disrupts pathway regulation and promotes a pro-fibrotic response. The study supports the potential of CC extract as an effective alternative for BPH management without pharmacologic complications.
