Abstract

Mahkota Dewa (Phaleria macrocarpa) adalah tanaman obat populer di Indonesia yang berasal dari Papua dan tumbuh subur di iklim tropis. Buah tanaman ini kaya akan senyawa antibakteri, termasuk alkaloid, polifenol, flavonoid, saponin, dan tanin, yang tergolong dalam metabolit sekunder. Penelitian ini menyediakan kontribusi baru dalam bidang farmakologi dengan mengeksplorasi efektivitas ekstrak etanol dari daging buah Mahkota Dewa (Phaleria macrocarpa) terhadap Staphylococcus aureus dan Staphylococcus epidermidis pada berbagai konsentrasi. Tujuan penelitian untuk menilai aktivitas antibakteri ekstrak etanol dari daging buah Mahkota Dewa. Metode yang digunakan adalah desain Post Test Only Control Group dengan pengujian in vitro. Ekstrak etanol disiapkan pada konsentrasi 20%, 40%, dan 60% melalui proses maserasi. Uji fitokimia menunjukkan keberadaan flavonoid, alkaloid, saponin, terpenoid, tanin, dan fenol. Aktivitas antibakteri diuji dengan metode difusi cakram, dan zona hambat diukur setelah inkubasi selama 24 jam pada suhu 37°C. Hasil penelitian menunjukkan bahwa ekstrak etanol Mahkota Dewa memiliki aktivitas antibakteri yang signifikan terhadap Staphylococcus aureus dan Staphylococcus epidermidis. Konsentrasi 60% menghasilkan zona hambat yang paling besar, yaitu 1,48 mm untuk Staphylococcus aureus dan 4,08 mm untuk Staphylococcus epidermidis, sedangkan konsentrasi 20% dan 40% menunjukkan efek antibakteri yang lebih rendah. Analisis statistik menggunakan uji Kruskal-Wallis dan Mann-Whitney menunjukkan adanya perbedaan yang signifikan antara konsentrasi ekstrak dan kontrol. Kesimpulan bahwa ekstrak etanol dari daging buah Mahkota Dewa (Phaleria macrocarpa) menunjukkan sifat antibakteri yang signifikan terhadap S. aureus dan S. epidermidis, terutama pada konsentrasi 60%. Namun, efektivitasnya masih lebih rendah dibandingkan dengan kloramfenikol.

Abstract

ABSTRACT Stunting, a result of chronic malnutrition characterized by relatively short stature for the age of the child, remains one of the most common nutritional problems found in poor and developing countries and is closely related to the incidence of recurrent infections and inadequate psychosocial stimulation. As the prevalence of stunting reaches 31% in 2022, the Government of Indonesia targets to reduce the prevalence of stunting in Indonesia to 14% by 2024 through various programs involving various elements of government. This study aims to assess the implementation of stunting prevention and management programs in Tebing Tinggi. This qualitative study with a cross-sectional approach was conducted in Tebing Tinggi City in December 2023. The informants in this study consisted of three key informants, namely the Head of the Public Health Division of the Tebing Tinggi Health Office, the Primary Nutritionist of Puskesmas Satria, and the Nutrition Executive Officer of Puskesmas Teluk Karang, and six regular informants, namely parents with stunted children. The data was collected through in-depth interviews with informants with the help of in-depth interview guidelines. The implementation of the stunting prevention and management program in Tebing Tinggi has routine communication between implementation actors and program targets, has adequate human resources, funds, facilities, and infrastructure, positive disposition of implementation actors even without incentives, and has standard operational procedures and clear distribution of responsibilities. The implementation of the stunting prevention and handling program in Tebing Tinggi was good; however, it should be noted that there are still shortcomings in the aspect of communication with program targets. Keywords: Stunting, Implementation, Policy, Program  ABSTRAK Stunting, yaitu kondisi malnutrisi kronis yang ditandai dengan badan yang relatif pendek berdasarkan usia anak, masih menjadi salah satu permasalahan gizi yang paling sering ditemukan di negara-negara miskin dan negara berkembang dan berkaitan erat dengan kejadian infeksi berulang serta stimulasi psikososial yang tidak adekuat. Dengan prevalensi stunting yang mencapai 31% di tahun 2022, Pemerintah Indonesia menargetkan mampu menekan prevalensi stunting di Indonesia menjadi 14% pada tahun 2024 melalui berbagai program yang melibatkan berbagai elemen pemerintahan. Penelitian ini bertujuan untuk menilai bagaimana implementasi program pencegahan dan penanganan stunting di Kota Tebing Tinggi. Penelitian ini merupakan penelitian kualitatif dengan pendekatan potong-lintang yang dilakukan di Kota Tebing Tinggi pada Desember 2023. Informan pada penelitian ini terdiri dari tiga informan kunci yaitu Kepala Bidang Kesehatan Masyarakat Dinas Kesehatan Kota Tebing Tinggi, Nutrisionis Ahli Pratama Puskesmas Satria, dan Petugas Pelaksana Gizi Puskesmas Teluk Karang dan enam informan biasa, yaitu orang tua dengan anak stunting. Data penelitian ini dikumpulkan dengan wawancara mendalam dengan informan dengan bantuan pedoman wawancara mendalam. Penelitian ini menemukan bahwa implementasi program pencegahan dan penanganan stunting di Kota Tebing Tinggi memiliki komunikasi rutin antar aktor implementasi dan terhadap sasaran program, memiliki sumber daya manusia, dana, serta sarana dan prasarana yang adekuat, disposisi aktor implementasi yang positif walaupun tanpa insentif, dan memiliki standard operational procedure dan distribusi tanggung jawab yang jelas. Dapat disimpulkan bahwa implementasi program pencegahan dan penanganan stunting di Kota Tebing Tinggi berjalan dengan baik; namun perlu diingat bahwa masih terdapat kekurangan dalam aspek komunikasi dengan sasaran program. Kata Kunci: Stunting, Implementasi, Kebijakan, Program

Abstract

Carrots are root vegetables that are very common in Indonesia. Carrot plant (Daucus carita L.)  is a root vegetable plant that has a high content of beta carotene, rich in dietary fiber, natural antioxidants and high vitamin A which is 12,000 IU. The reddish-yellow color of carrots is due to the high content of carotene pigment. The sample used in this study was fresh carrot tubers. The tubers are separated from other impurities and then washed thoroughly and then drained and weighed. Next, the tubers are dried at a temperature of 30-40oC until the tubers are dry (marked when broken brittle). Simplisia that has dried (brittle) is powdered with a blender and stored in a tightly closed container and stored at room temperature. The preparation of the extract is carried out by maceration using 70% ethanol solvent. According to the Indonesian Herbal Pharmacopoeia (2008), as much as 1 part of simplisia dry powder is put into the maserator, added 10 parts of solvent. Soak for the first 6 hours while stirring occasionally, then let stand for 18 hours. Separate the maserat by filtering. Repeat the extraction process at least twice with the same type and amount of solvent. Furthermore, all the mafiber is collected, then evaporated with a  rotary evaporator at a temperature of ±50oC until a thick extract is obtained. The results showed that ethanol extract of carrot tubers with a simplisia weight of 500 grams produced a yield of 3.78% with an extract weight of 68.9 grams. The results below show that carrot tuber ethanol extract shows phytochemical screening results that contain flavonoids, alkaloids, glycosides, tannins and steroids. The results of the measurement of the diameter of the inhibitory zone above showed that ethanol extract of carrot tubers (Daucus carota L.) can inhibit the growth of  Pseudomonas aeruginosa and Staphylococcus aureus  bacteria at concentrations of 10-400 mg / mL. The measurement results obtained the diameter of the inhibitory zone with a strong inhibitory zone response category at all concentration variations. It was found that high concentrations had activity that did not differ significantly from low concentrations. Carrot tuber ethanol extract has a significant difference (p < 0.05) when compared to the positive and negative groups while the results of the measurement of bacterial killing power above show that carrot tuber ethanol extract (Daucus carota L.) can inhibit the growth of Pseudomonas aeruginosa and Staphylococcus aureus bacteria  at concentrations of 10 – 100 mg / mLl

Abstract

This study was conducted with the aim of proving carrot tuber extract (Daucus carota L.) formulated in cream form can prevent an increase in the amount of melanin in the skin tissue of male wistar rats (Rattus norvegicus) exposed to UVB light and see histopathological changes due to carrot tuber extract given as a prevention of increasing the amount of melanin. This study is experimental with a post-test only control group research design. The test method used was testing the amount of melanin modified using a 13 watt Exoterra UVB 200 lamp. The animals tested were rats (Rattus norvegicus) male wistar strain aged 2-3 months, weighing ± 200 grams. Male rats were randomly divided into 6 groups. Each group consists of 5 male rats. The test area on the back of the back is 2x2 cm2. Group I male rats without cream administration as a negative control group, Group II male rats for Parasol Face Sunscreen Cream as a positive group. Group III is the group with the provision of basic ingredient cream and group IV-VI is the group with the administration of 25% carrot tuber extract cream; 50 %; 75 %. topically for 4 weeks.. The evaluation was performed by observing the amount of melanin at week 4 of histopathology analyzing in the test area. The results will be processed with IBM SPSS 21.0 and analyzed with the steps of Descriptive Analysis, Normality Analysis with data using Shapiro-Wilk test and Homogeneity using Levene's test, Comparative Analysis using one way Anova test, followed by Post-hoc test with LSD test. If the data is abnormal, use non-parametric analysis of the Kruskal-Wallis test followed by the Mann-Whitney test. The results showed that extra ethanol of carrot tubers can prevent the increase in the amount of melanin. This was shown by its ability to reduce melanin and histopathological changes in a group of male rats applied carrot tuber ethanol extract cream within 4 weeks

Abstract

The kidneys are a pair of symmetrical organs that function to filter the blood and are located in the retroperitoneal region on the posterior wall of the abdomen. The kidneys are drained about 25% of cardiac output. Kidney damage due to the accumulation of kidney stones can be caused by ethylene glycol substances to form calcium oxalate crystals (CaC2O4). This crystal is common in urine specimens even in healthy animals. Crystal formation can be accelerated by the administration of ammonium chloride. This study aims to determine the nephroprotective effects of balakka ethanol extract to reduce creatinine and urea levels in rats induced by ethylene glycol and ammonium chloride. The results showed that the balakka ethanol extract with a dose variation of 50 mg/kg body weight, 100 mg/kg body weight, and 150 mg/kg body weight had nephroprotective activity in rats that had been induced by ethylene glycol and ammonium chloride. Where the induction group had the largest serum creatinine mean of 1.43 mg/dl. The EEB treatment group dose of 50 mg / kgBB had a serum creatinine value of 1.01 mg / dl. The EEB treatment group dose of 100 mg / kgBB had a serum value of 0.91 mg / dl. The balakka ethanol extract treatment group dose of 150 mg / kgBB had a serum value of 0.73 mg / dl. The induction group had the largest serum urea serum of 76.56 mg/dl. EEB treatment group dose of 50 mg / kgBB had serum urea value of 63.36 mg / dl. The EEB treatment group dose of 100 mg / kgBB had a serum urea value of 49.73 mg / dl. The balakka ethanol extract treatment group dose of 150 mg / kgBB had a serum urea value of 49.90 mg / dl. Abstract

This study aims to examine and analyze the characteristics and perceptions of the community regarding the National Health Insurance (Jaminan Kesehatan Nasional or JKN) in Toba district, as well as the participation of individuals in the JKN program. The research methodology employed in this study is quantitative in nature, utilizing a cross-sectional design. The population for this study consisted of 206,199 individuals, and the sampling technique employed was the Slovin method, resulting in a sample size of 399 participants. Based on the test results, there is a relationship between age and the community's perception of the national health insurance program (JKN) and their participation as JKN participants in Toba district. Gender also has a relationship with participation in the National Health Insurance program. The number of family members is also related to participation in the National Health Insurance program. Knowledge is associated with participation in the National Health Insurance program. The current needs of the community are related to participation in the National Health Insurance program. Expectations are associated with participation in the National Health Insurance program. Perceived product quality is also related to participation in the National Health Insurance program.

Abstract

Indonesia is in a state of drug emergency, characterized by a high level of vulnerability and related crimes. Moreover, victims of drug abuse required assistance to recover from their substance abuse issues. Objective: The aim of this study was to analyze the influence of a family approach and spiritual experiences on the recovery process of individuals addicted to drugs in recovery centers in Medan. This research adopted a quantitative approach with cross-sectional design. A sample of 120 drug abusers was recruited using purposive sampling technique. Data was collected through questionnaires consisting of 20 items on the family approach, 20 items on spiritual experiences, and 32 items on drug use issues. Multiple linear regression analysis was conducted to examine the data. The findings revealed a significant influence of the family approach (p=0.024) and spirituality (p=0.000) on the improvement of the recovery process of individuals addicted to drugs undergoing recovery in Medan recovery centers. Most of them (55%) are in the state of contemplation. It is expected that the government maintain synergy and support the management of drug recovery centers, as well as social workers who have concern about drug abuse, along with families and the broader community, in order to effectively address this issue.

Abstract

The most of Candida strain in oral candidiasis is nystatin resistance strain. Hence, it become important to investigate the natural products like red ginger (Zingiber officinale var. Rubrum) and garlic (Allium sativum) as antifungal against HIV patient-isolated Candida albicans. This experimental study used two antifungal assays (Disc diffusion and antibiofilm assay). Both red ginger and garlic extract were extracted by maceration method. Candida albicans isolate was isolated from a volunteer HIV Patient with oral candidiasis. The outcomes included the diameter of the inhibition zone and optical density (OD) that were expressed as mean and standard deviation and analysed by Kruskal-Walis. This study showed that the highest concentration of both red ginger (0.67 ± 0.55 mm) and garlic extract (0.67 ± 0.46 mm) had the widest diameter of inhibition zone. Meanwhile, the highest inhibition of biofilm formation was found in the lowest concentrations (25%) of red ginger (2.22 ± 1.24) and garlic extract (3.65 ± 0.24). Any concentration of red ginger or garlic extract did not significantly affect the antifungal activity (P-Value: 0.392). Overall, it can be concluded that both red ginger and garlic extract at any concentration showed a weak antifungal activity against HIV patient-isolated Candida albicans strain.

Abstract

Natural treatment is plant extract nanoemulsion. Forming nanoemulsions with a specific method may help skin wound healing. Fruit cardamom (Amomum compactum Sol. Ex Maton) was used as an antimicrobial, but its wound-healing activity was unstudied. This study compares nanoemulsion from cardamom seed ethanol extract to cardamom extract in suppressing bacterial growth, wound healing, and determining component makeup. Research methods with Lab experiments involve sampling, processing, extracting, and UV-VIS spectrophotometer testing secondary metabolite chemicals, creating nanoemulsions, and characterizing them with HPLC, FTIR, UV-VIS, and PSA. TPC and disc diffusion for antibacterial and wound healing characterization and SPSS analysis. The research results found high quantities of gallic acid, catechin, and epigallocatechin were found in 20% (v/v) cardamom extract nanoemulsion, with HPLC analysis showing levels of 4590.10 µg/mL, 2128.95µg/mL, and 755.42µg/mL, respectively. FTIR and UV-Vis tests indicate a single peak around the phenol compound's absorption area and describe its group by absorbing functional groups in the IR spectrum. Nanoemulsion suppressed E. coli and S. aureus better than cardamom extract. At 1% (v/v), nanoemulsion heals wounds faster than cardamom extract and gentamicin sulfate. The particle size of 53.13 ± 1.25 nm and particle size distribution of 0.56 ± 0.02 nm contribute to the stability and high activity of the nanoemulsion.

Abstract

It is hoped that the performance of all medical and non-medical personnel at the Hospital will be in top condition. Nurses are expected to provide good service. This research aims to develop a subjective workload assessment technique model as an effort to improve the performance of nurses in the Inpatient Room at Royal Prima Hospital. This research is a quantitative study with a cross sectional study design which was carried out in the inpatient ward of the Royal Prima Hospital from June to July 2023. The population was 120 people. Respondents who were selected (inclusion) or excluded through the selection process were those who had more than 3 months of experience working in a care unit or related work. Data were analyzed univariate, bivariate and multivariate. The results of the research show that there is an influence of time pressure, working hour schedules, multiple roles, revetitive action on improving the performance of inpatient nursing personnel which is subjectively reasoned by inpatient nurses at Royal Prima Hospital. The variable that has the most dominant influence is the dual role variable.The suggestion of this research is that the health service should provide input in the context of continuous guidance to hospitals, especially in managing hospital management to improve better performance.
