Abstract

Exposure to ultraviolet-B (UV-B) light contributes to a decrease in skin collagen levels, which can cause premature aging. This study aims to evaluate the effectiveness of telang flower (Clitoria ternatea L.) extract lotion in maintaining collagen levels and epidermal thickness of UV-B-exposed rat skin. A total of 30 Wistar male white rats (200-250 g) were divided into six treatment groups: negative control (distilled water), positive control (Vaseline), and four treatment groups of telang flower extract lotion with concentrations of 0.2, 0.5, 0.75, and 1%. Mice were exposed to UV-B light (311 nm, 70 mJ/cm² per session) for 14 days with a total dose of 420 mJ/cm². The lotion was applied twice daily, before and after UV-B exposure. Evaluation was performed by measuring collagen content and epidermal thickness using histology staining. Statistical analysis used One-Way ANOVA or Kruskal-Wallis if assumptions were not met. The results showed that the treatment group with telang flower extract significantly increased collagen content and epidermal thickness compared to the negative control (p < 0.05), with a dose of 0.5% showing the highest effectiveness. In conclusion, telang flower extract lotion has potential as a photoprotective agent against UV-B-induced skin damage.

Abstract

Penelitian ini bertujuan untuk mengetahui hubungan antara dukungan keluarga dan kepatuhan pasien gagal ginjal kronik dalam menjalani terapi hemodialisis di RSU Royal Prima Medan. Penelitian ini dapat berkontribusi pada praktik klinis atau kebijakan rumah sakit dalam meningkatkan kepatuhan terapi hemodialisis. Penelitian menggunakan desain analitik dengan pendekatan cross-sectional, melibatkan 57 responden yang ditentukan menggunakan rumus Slovin. Data dikumpulkan melalui kuesioner mengenai dukungan keluarga dan kepatuhan pasien, kemudian dianalisis menggunakan uji korelasi Spearman. Hasil penelitian menunjukkan bahwa mayoritas responden berusia di atas 40 tahun, berjenis kelamin laki-laki, berpendidikan SMA, serta telah menjalani hemodialisis selama satu tahun. Nilai rata-rata dukungan keluarga sebesar 42 menunjukkan kategori baik, sedangkan kepatuhan terapi memiliki nilai rata-rata 3,5 yang juga termasuk kategori baik. Hasil uji statistik menunjukkan nilai koefisien korelasi r = 0,309 dengan p < 0,001, menandakan adanya hubungan positif yang signifikan antara dukungan keluarga dan kepatuhan pasien hemodialisis, meskipun kekuatan hubungan tergolong lemah. Hal ini menunjukkan bahwa semakin baik dukungan keluarga, semakin tinggi pula tingkat kepatuhan pasien dalam menjalani terapi. Temuan ini menegaskan pentingnya peran keluarga dalam dukungan emosional dan motivasi agar pasien tetap konsisten menjalani pengobatan. Sehingga disarankan untuk meningkatkan dukungan keluarga di lingkungan rumah sakit atau klinis. The Influence of Family Support on Adherence to Hemodialysis Therapy in Kidney Failure Patients Abstract This study aims to determine the relationship between family support and adherence in patients with chronic kidney disease undergoing hemodialysis therapy at Royal Prima Medan General Hospital. This study can contribute to clinical practice or hospital policy in improving adherence to hemodialysis therapy. The study used an analytical design with a cross-sectional approach, involving 57 respondents determined using the Slovin formula. Data were collected through a questionnaire regarding family support and patient adherence, then analyzed using the Spearman correlation test. The results showed that the majority of respondents were over 40 years old, male, had a high school education, and had undergone hemodialysis for one year. The average value of family support was 42, indicating a good category, while adherence to therapy had an average value of 3.5, also in the good category. The results of the statistical test showed a correlation coefficient value of r = 0.309 with p < 0.001, indicating a significant positive relationship between family support and adherence in hemodialysis patients, although the strength of the relationship was relatively weak. This indicates that the better the family support, the higher the level of patient adherence in undergoing therapy. These findings emphasize the importance of the family's role in emotional support and motivation so that patients remain consistent in undergoing treatment. So it is recommended to increase family support in the hospital or clinical environment.

Abstract

Diabetes Mellitus (DM) adalah penyakit kronis dengan prevalensi tinggi secara global, di mana neuropati diabetik merupakan salah satu komplikasi yang sering dialami. Penelitian ini bertujuan untuk menganalisis hubungan antara dukungan keluarga, kepatuhan berobat, dan tingkat stres terhadap keberhasilan adaptasi pada pasien diabetes melitus dengan neuropati diabetik di RS Royal Prima Medan. Penelitian ini menggunakan desain kuantitatif dengan pendekatan cross-sectional. Data dikumpulkan melalui kuesioner dukungan keluarga berdasarkan model Nursalam yang terdiri dari 12 pertanyaan, kuesioner MMAS-8 untuk mengukur kepatuhan berobat, dan instrumen untuk mengukur tingkat stres. Data yang diperoleh dianalisis menggunakan uji Chi-Square dan Uji Fisher Exact Test apabila data tidak berdistribusi normal. Hasil penelitian menunjukkan bahwa sebagian besar pasien mendapatkan dukungan keluarga yang tinggi (73,3%) dan menunjukkan adaptasi yang baik (83,3%). Meskipun uji statistik menunjukkan tidak ada hubungan yang signifikan antara dukungan keluarga dan adaptasi pasien (p = 0,460), secara klinis, dukungan keluarga tetap memegang peranan penting dalam mendukung adaptasi pasien. Faktor-faktor lain seperti usia, tingkat pendidikan, dan durasi penyakit juga berperan dalam proses adaptasi. Temuan ini mengindikasikan pentingnya dukungan keluarga dalam pengelolaan diabetes melitus dan menyarankan perlunya program dukungan keluarga di rumah sakit. Penelitian lanjutan disarankan untuk mengeksplorasi faktor-faktor lain yang mempengaruhi adaptasi pasien, seperti tingkat stres, pendidikan, dan tingkat keparahan penyakit. The Relationship between Family Support, Treatment Compliance, and Stress Levels on Adaptation Success in Diabetes Mellitus Patients with Diabetic Neuropathy at Royal Prima Hospital Medan Abstract Diabetes Mellitus (DM) is a chronic disease with high global prevalence, and diabetic neuropathy is a common complication. This study aims to analyze the relationship between family support, medication adherence, and stress levels on the adaptation success of diabetes mellitus patients with diabetic neuropathy at Royal Prima Hospital Medan. This study uses a quantitative design with a cross-sectional approach. Data were collected using a family support questionnaire based on the Nursalam model consisting of 12 questions, the MMAS-8 questionnaire to measure medication adherence, and an instrument to measure stress levels. Data obtained were analyzed using Chi-Square test and Fisher Exact Test when data were not normally distributed. The results show that most patients received high family support (73.3%) and demonstrated good adaptation (83.3%). Although statistical tests showed no significant relationship between family support and patient adaptation (p = 0.460), clinically, family support plays an important role in supporting adaptation. Other factors such as age, education level, and disease duration also influence adaptation. These findings indicate the importance of family support in diabetes management and suggest the need for family support programs in hospitals. Further research is recommended to explore other factors affecting patient adaptation, such as stress levels, education, and disease severity.

Abstract

Highlights: 1. This study analyzed the potential of Meniran extract gel for wound healing therapy, a plant-based medicine that has not been thoroughly researched in the context of promoting a healthy lifestyle. 2. The analysis found that Meniran extract gel has antioxidant properties that can be used for wound treatment to improve human health. Abstract Research on the use of medicinal plants to treat numerous diseases has been widely conducted. However, for chronic wound healing, the availability of medicinal plants for treatment remains rare. Gels containing active plant-based compounds may provide a solution. Phyllanthus niruri, known as "meniran" in Indonesian, is a medicinal plant used traditionally to cure various diseases. This study aimed to examine the antioxidant activities of Meniran extract gel, potentially for wound healing. Meniran was extracted using 70% methanol and formulated into a gel. The Meniran gel was composed of three formulations: 3%, 4%, and 5% sodium carboxymethylcellulose (CMC-Na) supplemented with 5% Meniran extract. The quality of the gel was assessed based on homogeneity, spreadability, pH, and viscosity. This study utilized 2,2"²-azino-bis (3-ethylbenzthiazoline-6-sulfonic acid) (ABTS) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) assays to evaluate the antioxidant activity of Meniran gel at seven different concentrations through its scavenging capacities. The results were calculated to determine the median inhibitory concentration (IC50). Analysis of variance (ANOVA) and post-hoc Tukey's honestly significant difference (HSD) were used for statistical analysis (p < 0.05). The three Meniran gel formulations showed good quality with spreadability ranging from 5.37 to 5.59 cm, pH levels from 5.90 to 6.03, and viscosity from 9.080 to 9.230 cps. Higher gel concentrations led to an increase in the free radical DPPH- and ABTS-scavenging activities. The highest DPPH and ABTS assays were found at 1,000 μg/mL with IC50 values of 69.72 μg/mL and 20.15 μg/mL, respectively. In conclusion, Meniran gel formulas exhibit good standards and have antioxidant properties suitable for wound healing therapy.

Abstract

Background: Discharge planning is a continuous process carried out by nurses to help patients prepare for self-care after leaving the hospital. It involves collaboration among healthcare providers, patients, and families. Through health education, discharge planning provides essential information about the disease and its management to reduce the risk of relapse and rehospitalization. Objective: This systematic review aims to identify the effect of discharge planning implementation on patient satisfaction in the inpatient room. Design: This study employed a systematic review design to synthesize empirical evidence on the effect of discharge planning implementation on patient satisfaction in hospital inpatient settings. Data Sources: A systematic literature search was conducted using two electronic databases: Google Scholar and ScienceDirect. Articles published between January 2020 and April 2025 were identified using the keywords (“discharge planning” OR “discharge process” OR “transitional care”) AND (“patient satisfaction” OR “patient experience” OR “patient-reported experience”) AND (“hospital” OR “inpatient”). Review Methods: A total of 717 articles were initially identified. After removing duplicates and applying inclusion and exclusion criteria—full-text availability, English or Indonesian language, and quantitative study design—15 studies were included in the narrative synthesis. Article selection followed the PRISMA guidelines to ensure transparency and methodological clarity. The included studies were analyzed narratively to identify patterns, key themes, and overall effects of discharge planning on patient satisfaction. Results: Based on the results of several articles that fit the inclusion criteria, the implementation of discharge planning has a significant influence on the level of patient satisfaction. Conclusion: The implementation of discharge planning plays a crucial role in the hospital service system. Discharge planning can increase patient satisfaction with hospital services. Hospitals should develop standardized discharge planning protocols, strengthen nurse training programs, and ensure patient-centered implementation.

Abstract

Nurses play a vital role and bear primary responsibility in discharge planning due to their constant interaction with patients and families. Research on discharge planning in Indonesia, particularly for post-neurosurgery patients, is still limited and rarely explored in depth, especially using a qualitative approach. Through a qualitative approach, this study was able to reveal the perceptions, challenges, and strategies of nurses that are often not seen in quantitative data.  This study aims to analyse the role of nurses in discharge planning for families of patients who have undergone neurosurgery. This study employed a descriptive qualitative approach using phenomenological methods. The number of participants or informants based on saturation data or redundancy concepts can generally be achieved using purposive techniques. In-depth interviews were used in data collection. The data analysis process in this study used Colaizzi's approach. This study produced nine themes based on the Modified Concept of Meleis' Transitional Nursing Theory and Roy's Adaptive Nursing Model Theory. including medical education, clarity of information, involving the family, addressing concerns, support for access to follow-up services, follow-up support, patient/family readiness, post-discharge challenges, recommendations for services. The impact of discharge planning carried out by nurses prior to discharge home results in patients and their families being prepared to carry out nursing actions at home. Nurses should always be mindful of their role in the discharge planning process because quality discharge planning is not merely an “administrative procedure before discharge,” but rather a strategic investment. Cancer is the most often diagnosed disease in the world and the top cause of death. Doxorubicin is a widely used systemic therapy, although its efficacy is limited by dose-related toxicity, including haematological suppression, organotoxicity, and hepatotoxicity. Bay leaf is a commonly utilised plant in the community for treating and preventing many diseases because it includes secondary metabolites that are antioxidants such as phenolics, alkaloids, saponins, steroids, triterpenoids, and tannins. The purpose of this work is to characterise EEDS, identify its secondary metabolite content, and test its nephroprotective effect in doxorubicin-induced rats using SGPT and SGOT levels and liver histology. This is a laboratory experimental investigation using rats and comprises of treatment groups receiving 0.5% of CMC-Na, Nature E, or EEDS at doses of 100 mg/kgBW, 300 mg/kgBW, or 500 mg/kgBW, respectively, followed by doxorubicin. The results indicated that EEDS had 21.64 % water soluble material, 1.79 % total ash, 9.98 % water, 45.38 % ethanol soluble extract, and 0.39 % acid insoluble ash. EEDS included alkaloids, flavonoids, tannins, glycosides, saponins, and steroids/triterpenoids, as determined by phytochemical screening. EEDS doses of 100 mg/kgBW, 300 mg/kgBW, and 500 mg/kgBW can significantly lower SGPT by 34%; 52,46%; 59,31% and SGOT levels by 27,05%; 43,96%; 50,07% and can improve the liver histopathology of doxorubicin-induced rats by reducing pyknotic degeneration, inflammatory cell infiltration, bleeding, steatosis and sinusoidal dilatation with an effective dose of 500 mg/kgBW with a significant difference with the doxorubicin group followed by 0.5% CMC Na. The key to bay leaf ethanol extract has hepatoprotective activity.

Abstract

Job satisfaction among doctors is the main key to getting better health services. Changes in payment methods in the JKN era led to the dissatisfaction of specialists and general practitioners with the medical services received in BPJS patient care. This study analyzes the effect of perceptions about the distribution of medical services for BPJS patients on the job satisfaction of doctors. The research design used in this study was a descriptive analytic non-experimental study with a cross-sectional approach. The study was conducted at the Royal Prima General Hospital located in Medan City from May 2021 to August 2021. The sample in this study was selected using the purposive sampling method so that a sample of 77 people was obtained with the inclusion criteria of having worked for at least 2 years. Data were analyzed using the Chi-Square test and multiple logistic regression. The results showed that the total medical services received by general practitioners per month was 10 million rupiah. Meanwhile, the total number of medical visits and services received by specialist doctors per month is 25 million rupiah. Based on the results of the Chi-Square test, it can be seen that doctors' perceptions of medical services (number of medical services (p = 0.000), timeliness of delivery (p = 0.000), suitability of medical services with performance (p = 0.003), distribution system (p = 0.004), and transparency (p=0.011)) had a significant effect on doctor's job satisfaction. The results of multiple logistic regression test show that the variable with the greatest influence is the number of medical services. The variable number of medical services has a value of Exp(B) = 29,830 (95% CI 3,037-293,034) that means that respondents who state the number of good medical services are 29 times more likely to be satisfied than those who state that the number of medical services is less. Doctor's job satisfaction can be increased by the management's efforts to be more open to doctors regarding doctor's medical services, giving doctors the opportunity to improve their career paths, and conducting periodic evaluations of doctor's satisfaction through questionnaires.

Abstract

The role of specialist doctors in hospital services is large, making the bargaining position of specialist doctors high against hospitals. Various literature states that loyalty plays an important role in the formation of a person's decision in choosing a place of work and giving commitment. This study analyzes the effect of motivation and organizational climate on the loyalty of specialist doctors. This explanatory survey research uses a cross-sectional design. The study was conducted at the Royal Prima General Hospital Medan for 1 (one) month starting from July 2021 to August 2021. The sample was 50 people who were determined by the inclusion criteria were specialist doctors who had worked for at least 2 years. Data were analyzed using the Chi-Square test and multiple logistic regression. The results of statistical tests using Chi-Square show that motivation has a significant effect on the loyalty of specialist doctors, whereas the results of statistical tests show that physical and esteem needs have a p-value of less than 0.05. The organizational climate also affects the loyalty of specialist doctors, where the results of statistical tests show that flexibility, responsibility, standards, and commitment have p values less than 0.05. The results of the multiple logistic regression test show that the variable that has the greatest influence on the loyalty of specialist doctors is flexibility. With an Exp (B) value of 18,444 (95%CI 2,052- 165,779), Respondents who stated good flexibility had 18 times more loyal than those who stated poor flexibility. In conclusion, good flexibility will increase the loyalty of specialist doctors. This study concludes that motivation and organizational climate have a significant effect on the loyalty of specialist doctors. The aspect of flexibility in the organizational climate has the most influence on the loyalty of specialist doctors.

Abstract

Virgin Coconut Oil(VCO) is virgin coconut oil that is made enzymatically. The content of saturated fatty acids and flavonoid compounds in VCO can be used as an anti-inflammatory drug. This study aims to determine the activity of VCO as an anti-inflammatory. Anti-inflammatory activity was tested using the method of calculating the percentage of inflammation inhibition in white rat paws induced by intraplantar 1% carrageenan solution. The decrease in edema in the rat's feet was measured using a plethysmometer for 360 minutes. The results showed that VCO at a dose of 0.4 ml/200 g BW and 0.8 ml/200 g BW gave anti-inflammatory activity with the percentage of inflammation inhibition of 64.34 ± 9.90 and 52.66 ± 8, respectively. 94.
