Abstract

The potential of human resources must be managed as well as possible in order to provide optimal output. Therefore, a company must think about ways it can develop the potential of its employees. One of them is improving employee work performance, good work performance will provide benefits for employees and the company. Work performance is influenced by various factors, including self-actualization and appreciation. Employees who have the opportunity to actualize themselves and receive appropriate rewards tend to have high motivation to participate. This research aims to determine the effect of Self-Actualization and Reward on Employee Work Performance at the Tirta Muare Ulakan Regional Public Water Company, Sambas Regency, both partially and simultaneously. This research method uses quantitative research with causal associative research. The data used is primary data and secondary data. Primary data sources were obtained through distributing questionnaires to all employees in the company and then tested using the SPSS version 22 application with validity tests, reliability tests, normality tests, multicollinearity tests, heteroscedasticity tests, multiple regression tests, hypothesis tests and coefficient of determination tests. Abstract

Background: The relative contributions of hyperglycemia, hypertension, and metabolic adiposity to the progression of diabetic kidney disease (DKD) are debated, particularly in Southeast Asian populations and in the context of modern polypharmacy. We aimed to prospectively quantify the independent impact of glycemic burden, hemodynamic stress, and central adiposity on the 5-year incidence of DKD in an Indonesian primary care cohort. Methods: We conducted a 5-year, multi-center, prospective cohort study at 25 primary care clinics in Indonesia. We randomly sampled and enrolled 1,250 T2DM patients without pre-existing DKD (eGFR > 60 mL/min/1.73m² and normoalbuminuria). The primary composite outcome was incident DKD, defined as persistent albuminuria (ACR ≥ 30 mg/g on 2 of 3 occasions) or a sustained eGFR decline of ≥ 30%. Baseline predictors included HbA1c, Systolic Blood Pressure (SBP), and Waist-to-Height Ratio (WHtR). Multivariable Cox proportional-hazards models were used to estimate Hazard Ratios (HRs), adjusting for demographics, baseline eGFR, and baseline use of RAAS inhibitors (RAASi) and SGLT2 inhibitors. Results: Of 1,250 participants, 980 (78.4%) completed the 5-year follow-up. Over a median 4.9 years, 215 participants (21.9%) developed the composite DKD outcome. In the fully-adjusted multivariable Cox model, all three pathways were strong, independent predictors of incident DKD. The standardized HR for SBP (per 1-SD increase) was 1.68 (95% CI: 1.40–2.01; p<0.001), for HbA1c (per 1-SD increase) was 1.45 (95% CI: 1.22–1.73; p<0.001), and for WHtR (per 1-SD increase) was 1.39 (95% CI: 1.18–1.65; p<0.001). Conclusion: In this prospective primary care cohort, hemodynamic stress (SBP), glycemic burden (HbA1c), and metabolic adiposity (WHtR) were all independent, potent drivers of incident DKD, even after controlling for the use of protective cardio-renal medications. These findings confirm that a multi-pillar strategy, aggressively targeting blood pressure, glucose, and weight/metabolic health simultaneously, is essential for DKD prevention.

Abstract

Background: Autogenous arteriovenous fistulas (AVFs) are the preferred vascular access for hemodialysis, offering superior patency and lower complication rates compared to grafts or catheters. However, non-maturation occurs in 20–40% of cases, often due to juxta-anastomotic stenosis or accessory venous runoff, leading to prolonged catheter dependence and increased morbidity. Endovascular salvage strategies such as balloon-assisted maturation (BAM) and coil embolization achieve technical success rates exceeding 90% and are now central to access preservation. Case Presentation: We report a 66-year-old male with stage V chronic kidney disease on regular hemodialysis who developed an immature left radiocephalic AVF four months post-creation, complicated by ipsilateral hand edema. Duplex ultrasonography revealed draining vein diameters of 0.42–0.54 cm and flow volumes of 130–150 mL/min. Venography confirmed juxta-anastomotic stenosis with competitive runoff into the distal cephalic vein. The patient underwent endovascular salvage consisting of coil embolization of the cephalic runoff and balloon angioplasty of the draining vein using sequential 4.0 and 6.0 mm balloons. Post-procedural venography demonstrated elimination of distal runoff and restoration of antebrachial flow. Clinically, hand edema regressed, and AVF patency was preserved. Conclusion: Combined coil embolization and angioplasty can successfully salvage immature AVFs with venous stenosis and accessory runoff. This case underscores the importance of structured duplex surveillance, early identification of failing maturation, and timely endovascular intervention to preserve autogenous vascular access.

Abstract

Education is a fundamental pillar in creating quality human resources. One of the main keys to achieving superior quality education is the quality of learning that takes place in the classroom. However, South Sangatta 1 Middle School has problems with variations in the quality of teaching between teachers which are still quite significant and student participation in the learning process is still not optimal. This can be seen from the low level of attendance, active involvement in class discussions, and uneven learning outcomes. This research uses a qualitative research approach, with a case study research design. The case study was chosen because this research aims to explore and understand in depth the implementation of active supervision in improving the quality of learning at SMPN 1 Sangatta Selatan. Active supervision has proven to be effective in improving the quality of learning at SMPN 1 Sangatta Selatan. This can be seen from several indicators, namely teachers who take part in the active supervision program show an increase in their ability to develop and implement effective teaching methods and students who take part in the active supervision program show an increase in their participation in the learning process. Active supervision is an effective strategy to improve the quality of learning at SMPN 1 Sangatta Selatan. Active supervision can help teachers to improve the quality of their teaching, thereby increasing student participation and learning outcomes.

Abstract

Transportation is an alternative used by everyone to get to a destination. In research, several alternative means of transportation can be used by students who live far from their campus. Students can determine the closest route to get to their campus. The campus we take is UINSU Campus IV. This research aims to overcome the transportation problems of students who live in the Belawan area and its surroundings. The research also used the ant colony optimization (ACO) algorithm method, with this method to make it easier to get the shortest route with a short time and the lowest cost or still standard with the distance. The ant colony optimization method is an algorithm inspired by the natural life of ants regarding ant habits in finding food. This research aims to get the shortest route and the optimal means of transportation used. In this study, it was found that the optimal means of transportation as well as distance, time, and cost were motorcycle transportation with a distance of 64.5km and the time required was 1 hour 4 minutes and the cost to be incurred was Rp. 40,000.00. This research uses Google Maps data to determine the distance of a location. Although the results are not optimal, it can be used as a solution for students in calculating the distance, time and cost to be used.
