Abstract

Presupposition itself has been a cause of the diversity of interpretations since presupposition sticks to utterances, sometimes unsaid clearly, that will provide implied meanings based on the context that is being talked about. The purposes of the study are to investigate three major points: the kinds of presupposition, their functions, and the percentage of each presupposition in the mystery and thriller film script of “Escape from Pretoria” by Francis Annan. This research used a qualitative method to analyze the obtained data. The observation is also used to collect data in which data are obtained from film scripts. The writer applies Yule’s theory (1996) to analyze the presupposition kinds, Halliday’s theory (2003) to analyze presupposition functions, and Subana’s formula to gain the percentage of each presupposition. As a result, there were 177 presuppositions obtained in the film script, with 166 existential presuppositions (93.8%), six factive presuppositions (3.4%), four lexical presuppositions (2.2%,) and one counterfactual presupposition (0.6%). From 177 presuppositions, the data were classified into 23 regulatory functions (13.0%), 2 interactional functions (1.1%), 57 representational functions (32.2%), 79 personal functions (44.6%), 1 imaginative function (0.6%), 4 instrumental functions (2.3%) and 11 heuristic functions (6.2%). Therefore, the dominant presupposition is existential, and the dominant function of presupposition is personal. Keywords: film script of Escape from Pretoria, pragmatics, presupposition

Abstract

Introduction: Tuberculosis (TB) remains a global health problem that causes high morbidity and mortality. Based on its classification, TB is divided into drug-sensitive (DS) and drug-resistant (DR). Several risk factors susceptible to TB are malnutrition with low body mass index (BMI), vitamin D deficiency (VDD), and low total lymphocyte count (TLC) related to low immune status. This study aimed to examine the relationship between vitamin D (VD), BMI, and TLC in the TB population in Banjarmasin. Methods: This was an analytic observational study with a cross-sectional design. The total study sample was 42 patients, confirmed by rapid molecular testing, who had not been treated for TB in Banjarmasin from January to May 2023 and met the inclusion and exclusion criteria. Chi-Square and Fisher's exact statistical tests were used to see the relationship between VD, BMI, and TLC in DS TB and DR TB. Results: The median age of DS TB was 38 (24-52) years old, and DR TB was 51 (37-58) years old. Most of the gender was male (24 patients/57.1%). There was a statistically significant difference between VD and BMI in DS TB and DR TB (p = 0.048; p = 0.019). There was a significant relationship between VD and TLC in DS TB and DR TB (p = 0.048). Conclusion: VD and BMI significantly differed in DS TB and DR TB. There was a significant correlation between VD and TLC in TB patients.

Abstract

Modern technology has improved our understanding of cancer biology, especially anti-cancer medicines from cytotoxic chemotherapy, radiotherapy, targeted therapy, and immunotherapy. Nevertheless, these treatments can result in significant pulmonary toxicities, including interstitial lung disease (ILD) and radiation-induced lung injury (RILI), which can result in a high rate of morbidity and mortality despite being less severe than injuries to other organ systems. Lung injury mechanisms occur through various pathways, such as immune-mediated damage and oxidative stress. Through clinical history and examination, imaging techniques such as high-resolution computed tomography (HRCT), and the necessity of eliminating other possibilities of respiratory symptoms, lung injuries due to cancer therapies can be identified. The management strategies are based on the severity of the condition and may include discontinuing the responsible agent, corticosteroid treatment, and supportive care. The challenge is early identification and management of these lung injuries due to the variability in patient responses and the lack of comprehensive guidelines. Therefore, awareness is needed to monitor lung health in cancer patients undergoing therapy.

Abstract

Introduction: Targeted therapy, particularly epidermal growth factor receptor-tyrosine kinase inhibitor (EGFR-TKI), is the first-line treatment for non-small cell lung cancer (NSCLC). However, drug resistance has grown in the last few decades. This study compared the progression time of lung cancer patients treated with first- and second-generation EGFR-TKI. Methods: Based on cytology and histological results, this cross-sectional study included 1,008 participants diagnosed with lung adenocarcinoma (LUAD) from 11 Indonesian Respiratory Centers. Every three months, the response to treatment was assessed using the Response Evaluation Criteria in Solid Tumours (RECIST) criteria in 1.1. Significant differences in the clinical features of the three TKI treatment groups were identified using logistic regression analysis, the median time to disease progression was estimated using the Kaplan-Meier technique, and independent prognostic factors related to the time to progression (TTP) were assessed using Cox proportional hazards regression. Results: This study examined 505 patients, the majority of whom were females (50.9%), never smoked (59.8%), diagnosed at an advanced stage (99.2%), and had an Eastern Cooperative Oncology Group (ECOG) scale of 0-1 (83.2%). Approximately 98.1% of patients were treated with afatinib (14.8%), erlotinib (18.6%), and gefitinib (66.1%) due to common mutations. The groups did not differ significantly (p>0.05). The median overall survival (OS) rate was 9 months. The time to LUAD progression in lung cancer was significantly impacted by poor performance (p=0.001). Conclusion: Epidermal growth factor receptor-tyrosine kinase inhibitor treatment can only prolong the TTP of LUAD by up to 9 months, and the performance scale when receiving the EGFR-TKI significantly affects the prognosis.

Abstract

This study aims to determine the substitution rate of Artemia naupli to Phronima sp., which produces high-quality seahorses H. Barbouri juveniles This research was carried out using CRD (Completed Randomized Design) with five treatments and three replications. The treatments were natural feeding with different levels of substitution, (A) 100% Artemia nauplii, (B) 75% Artemia nauplii+25% Phronima sp. (C) 50% Artemia nauplii +50% Phronima, (D) 25% Artemia nauplii+75% Phronima sp. and (E) 100% Phronima sp. Data were analyzed with Analysis of Variance and W-Tuckey test to determine the significant difference between the treatments. The parameters of the study included the analysis of feed quality (proximate and amino acid analysis) and survival. Results of the ANOVA indicated that the substitution of Artemia nauplii to Phronima sp. had a significant effect (P <0.05) on survival rate. Based on the result of the proximate analysis of Phronima, the protein and fat content is insufficient for juvenile requirements, which only ranged from 37,12% and 3,82%. The results of the amino acid analysis of Phronima, the total of essential amino acids are deficient in juvenile requirements. The treatments of C and D produces the highest survival value of 96,67%. While in E treatment produce the lowest survival value of 63,33%. Based on the juvenile quality analysis, it can be concluded that the use of Phronima as an alternative feed for seahorse juveniles H. barbouri can be given at a substitution rate of up to 75%.

Abstract

Abstract

Background: Lung cancer is a leading cause of malignancy and mortality. Zinc and C-reactive proteins are known to affect lung cancer progression. However, inconsistent findings between zinc levels on plasma CRP levels were reported. This study aimed to determine the correlation between serum zinc and CRP levels in patients with advanced-stage non-small cell lung cancer (NSCLC) Methods: A cross-sectional study was conducted in the Regional Hospital of Doris Sylvanus Palangkaraya between January and April 2023. Thirty-five lung cancer patients were included. The blood specimen was analyzed for serum Zinc and CRP. Spearman's correlation test was used to analyze the data. Results: The zinc level was dominantly normal or low (71.4%), with a mean of 65.57 ug/dL. Most subjects have elevated CRP, with higher CRP in the deficiency zinc group (96.64 vs. 68.89 mg/L). However, no significant correlation was found between serum zinc and CRP levels (P=0.160; r=0.173). Conclusion: Serum zinc levels were not proven statistically correlated with inflammatory markers (CRP), particularly in non-small lung cancer patients.

Abstract

Lung cancer is a major health concern in Indonesia due to its increasing prevalence, late-stage diagnosis, younger population, and high mortality. Addressing this issue requires nationwide implementation of comprehensive lung cancer control, which includes risk reduction and prevention strategies, focusing on tobacco control and air pollution mitigation. Screening with low-dose computed tomography (LDCT) and early detection in symptomatic patients, along with TB screening programs and all non-communicable diseases, is strongly recommended to enhance early case findings, treatment effectiveness, and overall patient outcomes. A multidisciplinary team (MDT) approach is important to ensure accurate diagnosis and comprehensive care. Moreover, the integration of palliative care at the early stages of advanced lung cancer is vital, focusing on symptom management and enhancing the quality of life for patients. While national guidelines are available for the diagnosis and treatment of lung cancer, significant disparities in healthcare access remain across Indonesia. Thus, it is essential to improve universal health coverage and referral systems to guarantee equal access to lung cancer care for patients at all levels through advocacy and ease of access.
