Abstract

This study aims to determine the implementation of tuberculosis control policies at Puskesmas Glugur Darat. This research design uses quantitative and qualitative approaches, where the method is a qualitative approach. The results showed that the implementation of the tuberculosis control policy at the Glugur Darat Community Health Center had not run as well as possible. The cure rate for tuberculosis patients at Puskesmas Glugur Darat was 46.15% and less than the national target of> 85% and the treatment success rate of 65.38% was also less than the national target of 85%. In conclusion, the head of the puskesmas, tuberculosis officers, and cadres have used the national tuberculosis guidelines and the DOTS strategy well, but implementation has not gone well because there are still people who do not participate in health promotion activities. Abstract

Background:In an observational study of The Global Burden Disease stated skin diseases contributed to 1.79% of lost productive days worldwide due to itching, scratching, or secondary infections which accompanying. Skin diseases are among the three biggest diseases in Tanjung Balai City with 7.230 cases in 2017. Tanjung Balai is a coastal region on the East Coast of North Sumatra with Teluk Nibung as the largest Subdistrict which had characteristics of tropical climates, high density, the lack of basic sanitation and personal hygiene and it caused the population susceptible to skin diseases.The study is to find out the prevalence of skin diseases in Teluk Nibung2018.Methods:This was an observational study using data sourced from surveillance of infectious diseases in Teluk Nibung Health Center regarding the 10 biggest diseases in the one Primary Health Care (Puskesmas) in 2018. The study was conducted in April 2019. Data were analyzeddescriptivelyusing univariate analysis with table and graphs of skin diseases frequency and distributionResults: Skin disease wasthe third largest disease in Teluk Nibung Subdistrict with 2.208 casesin 2018. Disease trends increased throughout 2018 with significantly increased in May. The proportion of skin diseases wasbigger among the female group, which is 56%. The environmental conditions in Teluk Nibung reflected coastal areas with high-density population, unhealthy settlement conditions, and the lack of basic sanitation and personal hygiene.Conclusions:Skin Disease is still a major health problem in Teluk Nibung due to the climate, weather, and the lack of environmental health factor. It needsfor researchers conducted research on skin diseases related to basic sanitation, personal hygiene among society in the coastal area and stakeholders need to create policy improved health environment.

Abstract

Drug management is a series of activities related to the aspects of planning, procuring, receiving, storing, destroying, controlling, recording and reporting drugs. During the Covid-19 pandemic, many activities were hampered, especially in drug management. This study aims to describe the management of drug management in the health center often in the sub-district of Medan Tembung. This type of research is descriptive research using a qualitative approach. The number of informants in this study were 3 people consisting of the head of the puskesmas, the person in charge of the pharmacy and the pharmacy staff. Data collection was carried out through in-depth interviews and direct observation. The results showed that the management of medicines had not been implemented properly in accordance with Permenkes number 74 of 2016 concerning standards of pharmaceutical services at Puskesmas. During a pandemic, the number of patient visits at the health center has often decreased drastically so that the planning and procurement of medicines that are prepared and available are many that are not used and even have expired in 2021. It can be concluded that the distribution and recording and reporting are in accordance with the drug management standards at the Puskesmas. However, planning, procurement and storage are not in accordance with existing standards. It is expected that the puskesmas will often pay attention to and follow the established management guidelines.

Abstract

Exclusive breastfeeding is the best type of initial food for babies and up to 2 months of age. The low coverage of exclusive breastfeeding in the working area of the health center, often in Medan Tembung sub-district, is around 67 babies (35%) who are given exclusive breastfeeding out of 191 babies, influenced by lack of participation from the mother pregnant and breastfeeding mothers to the exclusive breastfeeding program at puskesmas frequently. Therefore, the implementation of the exclusive breastfeeding program has become weak. The aim of this study was to determine the implementation of the exclusive breastfeeding program which included goals and objectives, the communication process (socialization), resources and characteristics of the program implementing agency. This method of research is effective. The research informants were 1 head of the puskesmas, 2 people in charge of the exclusive breastfeeding program, and 3 midwives for health workers. Data collection was through in-depth interviews with key informants and informants on triangulation and observations. The results showed that the implementation of exclusive breastfeeding programs in puskesmas is generally less successful. The program was implemented mentoring during push-ups and testing in the posyandu project. Communication in the program does not achieve the goals and objectives. The implementation of an exclusive breastfeeding program can be effectively implemented by increasing the social activities associated with the program according to the plan and by providing a support program for the program.

Abstract

Artikel ini bertujuan untuk meningkatkan moderasi umat beragama pada masyarakat di desa Pematang Kuala. Moderasi umat beragama masa pandemi Covid-19 sangat penting bagi masyarakat desa Pematang Kuala. Mayoritas agama yang dimiliki masyarakat di desa tersebut Islam dan Kristen, di desa ini terdapat 12 suku yang berbeda-beda yang terdapat di desa tersebut. Adapun metode yang digunakan yaitu pendekatan kualitatif dengan metode deskriptif dengan observasi langsung dan hasil wawancara. Hasil dari penelitian ini akan menunjukkan upaya dari pemerintah desa, tokoh pemuka agama serta masyarakat setempat untuk dapat meningkatkan moderasi umat beragama di desa tersebut dengan mencari solusi untuk mencegah adanya konflik dan perpecahan di masyarakat di masa pandemi saat ini, sehingga dengan saling bertoleransi antar umat beragama akan tercapai hubungan yang rukun dan damai antara umat beragama di desa Pematang Kuala.

Abstract

Jaminan kesehatan sosial merupakan hak bagi penduduk melalui suatu Badan Penyelenggara Jaminan Sosial dan diatur dalam UU No. 40 Tahun 2004. Permenkes No. 71 Tahun 2013 mengenai Pelayanan Kesehatan Pada Jaminan Kesehatan Nasional menyatakan pelayanan kesehatan rujukan tingkat lanjutan adalah upaya pelayanan kesehatan perorangan yang bersifat spesialistik dan subspesialistik yang meliputi rawat jalan tingkat lanjutan, rawat inap tingkat lanjutan, dan rawat inap di ruang perawatan khusus. Tujuan penelitian ini untuk melihat bagaimana pelaksanaan terkait rujukan berjenjang pada peserta BPJS di Indonesia. Metode pada penelitian ini adalah literature review dengan menggunakan data sekunder yang diperoleh dari penelitian artikel jurnal dan skripsi yang diakses pada Google Scholar serta buku. Hasil penelitian ini menunjukkan pelaksanaan sistem rujukan berjenjang masih belum optimal pada beberapa FKTP di Indonesia, adapun kendala yang ditemukan pada pelaksanaan rujukan berjenjang adalah komunikasi dari SDM yang terlibat dalam pelayanan rujukan masih belum memberikan informasi, kurang memadai sarana dan prasarana untuk menunjang pelayanan rujukan berjenjang dan terdapat fasilitas kesehatan yang tidak menerapkan SOP yang mengatur tentang sistem pelayanan rujukan berjenjang.

Abstract

Permasalahan yang sering timbul di Puskesmas yaitu berupa ketersediaan tenaga kesehatan yang kurang, kemudian kelengkapan obat yang belum memadai, dan juga sikap dan perilaku petugas kesehatan terhadap pasien. Terkadang hubungan antara petugas kesehatan dengan pasien belum dapat tercipta secara baik dan menimbulkan rendahnya tingkat kepercayaan terhadap pelayanan yang diberikan. Hal tersebut dapat mempengaruhi minat masyarakat khususnya peserta BPJS kesehatan untuk memperoleh pelayanan kesehatan di Puskesmas. Tujuan penelitian ini adalah untuk mengetahui tingkat kepuasan pengguna BPJS terhadap pelayanan puskesmas Simalingkar. Jenis penelitian ini menggunakan penelitian kualitatif dengan pendekatan studi literatur. Data primer diperoleh melalui kuesioner dengan menggunakan pedoman pertanyaan untuk ditanyakan kepada masyarakat. Dari hasil penelitian tersebut maka diperoleh informasi berupa 10 responden puas dan 5 responden tidak puas terhadap dimensi Ressponsive, 11 responden puas dan 4 responden tidak puas terhadap dimensi Assurance, 12 responden puas dan 3 responden tidak puas terhadap dimensi Emphaty, 15 pasien puas dan tidak ada pasien yang tidak puas terhadap dimensi Tangibles, 13 responden puas dan 2 responden tidak puas terhadap dimensi Reability. Dapat disimpulkan bahwa Rata-rata pasien merasa puas dengan pelayanan pengguna BPJS di Puskesmas Simalingkar, Kota Medan.

Abstract

The Smart Community Movement Using Drugs (GeMa CerMat), a Ministry of Health program that involves cross-sectoral and community components. GeMa CerMat activities are aimed at increasing knowledge and skills, as well as changing people's behavior in choosing, obtaining, using, storing and disposing of medicines properly. Riskesdas 2013 data shows that 35.2% of households store medicines for self-medication. Referring to 35.2% of households who store drugs, 35.7% of them store hard drugs, and 27.8% of them store antibiotics and 86.1% of antibiotics are obtained without a prescription. This situation shows that self-medication has not been implemented properly. The research objective was to analyze the GeMa CerMat program in Medan City. The research method used in this research is descriptive research using a qualitative approach. The results of the study show that the Medan City Health Office data for the achievement of indicators in the Medan City area in 2019 is 60.6% with a target of 60%, in the previous year the indicator achievement was 5.13% with a target of 55% of the achievement of the indicators for the CerMat GeMa Program in the City area. Medan in 2020 the achievement target of 45% for Medan City was not achieved according to the target due to the Covid-19 pandemic so that the GeMa CerMat program was not implemented.

Abstract

Stunting or often called stunted or short is a condition of failure to thrive in children under five years of age (toddlers) due to chronic malnutrition and recurrent infections, especially during the first 1,000 days of life (HPK), from fetus to 23 month old child. Based on data from the Medan City Health Office, the stunting rate in Medan City was 491 with a percentage of 17.4% in 2019. Meanwhile, the stunting rate in Medan City in 2020 was 393 with a percentage of 0.71%. 491 cases of stunting under five, scattered in 25 districts and 104 villages. The highest case was in Medan Deli Subdistrict, with 101 cases and Titi Papan Village was a sub-district with the highest cases as many as 82 cases. The research design used in this study is descriptive research using a qualitative approach. The data collection technique in this study was carried out by means of observation. In the area of ​​Medan City, the STBM achievement is known to be 36.04% while the national target achievement is 68.06%. One of the factors causing the STBM achievement target not to be achieved is due to poor sanitation conditions that affect the health of children's growth and development which can lead to diarrhea, thus affecting children's nutrition.

Abstract

Vaccination is a process in the body, where a person becomes immune or protected from a disease so if the one time exposed to the disease then it will not hurt or only experience mild pain, usually with the administration of the vaccine. Vaccination not only aims to break the chain of transmission of the disease and stop the outbreak, but also in the long term to eliminate even mengeradikasi (destroy/ eliminate) the disease itself. This research method uses a qualitative approach to obtain data descriptive with data collection techniques to study literature (literature review). According to the Department of Health of the City of Medan in the implementation of the vaccination is carried out in two stages. For the first phase of Pemko Medan receive 20,000 vaccine covid-19, where for the first stage in priority to the health workers to gradually to the community. The second stage, Pemko Medan receive 96.000 vaccine covid-19, vaccination is the second stage is also intended for public service officer including the state civil apparatus (ASN) and others.
