Abstract

Patient satisfaction is a crucial indicator in assessing the quality of health services in Community Health Centers (Puskesmas). As the primary healthcare providers at the community level in Indonesia, Puskesmas play a vital role in ensuring accessible, affordable, and comprehensive health services. High patient satisfaction not only reflects the effectiveness of healthcare delivery but also influences patient adherence to medical recommendations, trust in the healthcare system, and overall public health outcomes. Therefore, understanding the factors that determine patient satisfaction is essential for improving service quality and formulating effective healthcare policies. Quality health services in Community Health Centers are one of the main indicators in increasing patient satisfaction. This study aims to analyze the determinants of patient satisfaction with Community Health Center services in Indonesia using secondary data from the 2017 Health Manpower Research (Risnakes). The research design uses a quantitative approach with Spearman Rank correlation analysis to see the relationship between independent variables, such as speed of service, friendly attitude of officers, communication, patient rights, and quality of health service facilities, to the dependent variable, namely patient satisfaction. The results showed that speed of service had the most dominant influence on patient satisfaction (r = 0.792; p < 0.05), followed by quality of service facilities (r = 0.473; p < 0.05) and the right to choose health services (r = 0.283; p < 0.05). Other factors, such as friendliness of officers, communication, and confidentiality of patient information, showed a weak but significant relationship. Overall, the independent variables contributed 99% to the dependent variable. This study emphasizes the importance of improving the efficiency of services and physical facilities in Community Health Centers to improve patient satisfaction. These findings are expected to be a reference in formulating better health service policies in Indonesia.

Abstract

The global HIV/AIDS health crisis continues to pose significant challenges, particularly for people of productive age. In Indonesia, the disease imposes a significant social and economic burden, with high HIV prevalence exacerbating the strain on public health and socio-economic systems. At the Bandar Khalipah Health Centre, the high number of HIV cases underscores the inadequacy of existing prevention efforts and health services. This study analyses the delivery system of the HIV health services programme at the Bandar Khalipah Health Centre using a qualitative case study approach. Data were collected through in-depth interviews, observations and document analysis with key informants including the health centre head, programme manager and clinical staff. Data analysiFs was conducted using ATLAS.ti version 9 software, which allowed for the systematic identification and categorisation of themes. The study identified key challenges in three primary programme components: input, process and output. In terms of input, critical human resource constraints and the lack of routine training for health workers were highlighted. In terms of output, while patient satisfaction with services was generally high, adherence to treatment was particularly low due to social stigma and inadequate community outreach. The findings highlight the importance of strengthening all components of the HIV healthcare programme at Bandar Khalipah Health Centre in order to improve service quality and patient outcomes. This includes building the capacity of health staff, promoting community engagement, and addressing social stigma. These findings have important implications for public health policies and strategies aimed at optimising local HIV services and reducing health inequalities.Keyword: System analysis, Program Implementation, Healthcare Services, HIV Infected Individuals, VCT

Abstract

The quality of health services is the effectiveness of high health services with service standards and professionals, using resources owned by hospitals or health centres safely, and following the rules, laws, norms, and ethics with government and consumer restrictions. This study aims to measure patient satisfaction with the quality of inpatient services at X hospital in Binjai. This study uses a type of quantitative survey research using a cross sectional approach, where researchers will collect primary data by asking questions to respondents to obtain the necessary data. The population in this study were inpatients who visited Hospital X Binjai, namely 409 inpatients in December 2023. The number of samples in this study were 33 people. This research was conducted from January to February 2024. Primary data comes from questionnaires based on interviews conducted directly. This study uses a sampling technique, namely purpsive sampling, with criteria with age> 19 years. The variables analysed include Tanglible (physical evidence), Reability (reliability), Assurance (guarantee), Responsiveness (responsiveness), Empathy (empathy). This study uses the T test using the SPSS version 20 application. This study shows that there is a difference between patient expectations and the reality obtained by patients (Sig < 0.05). The results showed that customers who had received inpatient services at hospital X in Binjai were satisfied with the services that had been provided, in the calculation of the customer satisfaction index, a customer satisfaction index value of 78% was obtained, with the interpretation that the patient was satisfied with the services provided, with this the quality of inpatient services at hospital X in Binjai was declared good. Abstract

The implementation of the National Health Insurance (Jaminan Kesehatan Nasional - JKN) in Indonesia has been a critical effort towards achieving universal health coverage, aiming to provide affordable and accessible healthcare for all citizens. This research evaluates the effectiveness of JKN in improving healthcare access and administrative efficiency. The study utilizes a qualitative approach, analyzing secondary data from various health reports and government publications to assess the progress and challenges faced by the JKN system. Key aspects evaluated include the coverage expansion, technological advancements like the Mobile JKN app, and the collaboration with healthcare providers. The results show that JKN has successfully increased healthcare accessibility, with over 98% of the population enrolled, thus contributing to the country's Universal Health Coverage status. However, challenges remain, particularly in the equitable distribution of healthcare facilities, especially in remote and underserved areas. Additionally, administrative inefficiencies, such as delays in claims processing and the growing financial burden, require continuous monitoring and reform. The study concludes that while JKN has made significant strides in expanding access to healthcare, further improvements in the distribution of healthcare facilities, operational efficiency, and sustainable financing mechanisms are essential to ensure that JKN can continue to meet the health needs of Indonesia's diverse population in the long term.

Abstract

Mental health is an important part of public health that requires special attention, especially in the context of primary services such as health centers. This study aims to evaluate the effectiveness of the role of BPJS Kesehatan in improving access to mental health services in Indonesia through a systematic literature review of selected articles from electronic databases (Google Scholar, PubMed, SpringerLink). The results of the study show that BPJS plays an important role in expanding access and reducing treatment costs by up to 50%, as well as supporting the successful rehabilitation of patients with mental disorders by 60%. However, challenges still arise from the lack of professionals, the length of waiting time, to the limited understanding of participants regarding the scope of services. Therefore, improvements are needed in the aspects of cost management, capacity building of health workers, strengthening coordination between agencies, and socialization to the community. These findings provide important input for efforts to optimize the national health social security system in facing the challenges of mental health services.

Abstract

Health is an important aspect for both individuals and the wider population. WHO (World Health Organization) is the largest health organization in the world which covers world health by distributing health aid funds to policies which will later become a reference for various countries to implement. The aim of this research is to determine the Management of Health Operational Assistance Funds (BOK) towards the Achievement of Minimum Service Standards (SPM) in the Productive Age Service Sector at the Sei Agul Community Health Center, Medan City. This research is qualitative research with an in-depth case study approach. This research involved four informants who understand, know and understand, namely the Head of the Community Health Center, the BOK Treasurer, the productive age program implementer, and the person in charge of the productive age program. The data analysis used in this research is case study analysis with data processing using a qualitative data processing application, namely Nvivo version 14. Management of BOK funds at the Sei Agul Community Health Center, Medan City has obstacles, namely adjusting the Community Health Center program with the Health Service program. In its implementation there are obstacles, namely the slow realization of the BOK budget and a less active community. Reporting has obstacles, namely too many reporting files and too short a time. In the evaluation, there were solutions to obstacles to program implementation that were not followed up.

Abstract

Jumlah pending klaim BPJS Kesehatan pada pelayanan rawat inap yang relatif tinggi di Rumah Sakit X Kabupaten Serdang Bedagai mencerminkan adanya permasalahan dalam penerapan manajemen mutu rumah sakit. Meskipun penerapan Total Quality Management (TQM) telah dilaksanakan di rumah sakit, namun kasus klaim yang tertunda masih sering terjadi. Kondisi ini berdampak negatif terhadap kelancaran arus kas rumah sakit. Penelitian ini bertujuan untuk menganalisis implementasi prinsip-prinsip TQM dalam upaya menurunkan pending klaim BPJS Kesehatan. Penelitian menggunakan pendekatan kuantitatif deskriptif dengan metode survei melalui penyebaran kuesioner kepada staf rumah sakit yang terlibat dalam proses klaim. Data dianalisis dengan metode Importance Performance Analysis (IPA) untuk mengidentifikasi kesenjangan antara tingkat kepentingan dan kinerja implementasi TQM. Hasil penelitian menunjukkan bahwa implementasi prinsip-prinsip Total Quality Management (TQM) di Rumah Sakit X secara keseluruhan memperoleh nilai rata-rata di atas 97, yang termasuk dalam kategori “memuaskan”. Berdasarkan analisis kartesius IPA, prinsip komunikasi dan peningkatan berkelanjutan termasuk dalam Kuadran B yaitu area yang menunjukkan kinerja tinggi namun tetap dianggap penting sehingga harus dipertahankan prestasinya. Gap terbesar ditemukan pada prinsip komunikasi (-0,13) dan peningkatan berkelanjutan (-0,12), yang menandakan perlunya perhatian untuk mempertahankan dan terus meningkatkan kinerja pada aspek tersebut. Implementasi TQM terbukti berkontribusi terhadap penurunan pending klaim, namun perlu perbaikan berkelanjutan dan penguatan koordinasi lintas unit untuk mencapai efisiensi maksimal. Implementasi prinsip-prinsip TQM secara konsisten dapat mengurangi pending klaim dan meningkatkan kualitas manajemen rumah sakit.

Abstract

Tembakau menjadi penyebab utama kematian lebih dari 8 juta orang di dunia setiap tahun, termasuk 1,2 juta kematian akibat paparan asap rokok pada perokok pasif. Di Indonesia, perilaku merokok masih sangat tinggi, dengan peningkatan signifikan pada kelompok anak-anak dan remaja. Kabupaten Deli Serdang menunjukkan prevalensi perokok yang tinggi di berbagai kelompok usia, dan angka tersebut masih jauh dari target RPJMN dan SDG’s Sebesar 8,7%. Penelitian ini bertujuan untuk mengetahui implementasi kebijakan Kawasan Tanpa Rokok pada fasilitas umum di Kecamatan X, Kabupaten Deli Serdang. Penelitian ini menggunakan metode kualitatif dengan pendekatan Fenomenologi. Teknik pengumpulan data dengan menggunakan wawancara, observasi, dan dokumentasi. Hasil penelitian menunjukkan bahwa implementasi kebijakan Kawasan Tanpa Rokok di Kecamatan X masih belum optimal. Kurangnya sosialisasi dan media informasi menyebabkan komunikasi kebijakan belum berjalan efektif. Sumber daya manusia dan sarana pendukung masih terbatas, disposisi pelaksana kebijakan menunjukkan kurangnya komitmen, dan struktur birokrasi belum terkoordinasi dengan baik. Akibatnya, pengawasan dan penegakan sanksi terhadap pelanggaran masih lemah. Penelitian ini memberikan saran kepada Pemerintah Daerah agar meningkatkan sosialisasi, penyediaan sarana pendukung, penguatan koordinasi antar lembaga, serta penegakan hukum yang konsisten untuk meningkatkan efektivitas kebijakan Kawasan Tanpa Rokok.

Abstract

Latar Belakang: Tingkat kesehatan anak di Indonesia masih tertinggal dibandingkan negara-negara Asia Tenggara. Berdasarkan laporan WHO 2020, indeks perkembangan dan kesehatan anak Indonesia berada di peringkat 117 dari 180 negara, jauh di bawah beberapa negara tetangga.Tujuan: Penelitian bertujuan mengidentifikasi faktor internal dan faktor eksternal yang mempengaruhi tingkat partisipasi ibu balita ke posyandu di wilayah kerja puskesmas belawan.Metode: Penelitian ini menggunakan pendekatan kualitatif dengan jenis penelitian deskriptif evaluatif. Penelitian dilaksanakan pada bulan Maret sampai April wilayah kerja Puskesmas Belawan Kota Medan Sumatera Utara. Pengambilan informan dilakukan dengan mengunakan menggunakan teknik purposive sampling. Pengumpulan data dilakukan melalui wawancara dan observasi, yang divalidasi menggunakan triangulasi sumber. Analisis data dilakukan dengan pendekatan SWOT.Hasil: Analisis ini menunjukkan bahwa strategi yang tepat untuk meningkatkan partisipasi ibu balita berada pada kuadran II SWOT (Weaknesses-Opportunities/WO). Skor IFAS menunjukkan kelemahan lebih dominan dibandingkan kekuatan (0,23), sementara skor EFAS menunjukkan peluang lebih besar dibandingkan ancaman (0,29).Kesimpulan: Menerapkan Strategi Turnaround (WO), yaitu memanfaatkan peluang eksternal untuk mengatasi kelemahan internal. Dengan starategi ini, diharapkan kehadiran ibu balita meningkat, sehingga Posyandu dapat berfungsi optimal dalam mendukung tumbuh kembang anak.

Abstract

Service quality is closely related to customer satisfaction because service quality can create a good relationship with the company and tell the company how to meet consumer needs. This study aims to determine the level of patient satisfaction with the services provided by the Primary Outpatient Clinic of UIN North Sumatra. The method in this study uses a quantitative approach, and the research design is carried out in a cross-sectional manner. The research sample of 50 respondents was selected through the purposive sampling method in accordance with the criteria of patients who received BPJS and had reading and writing skills. The results showed that all dimensions of SERVQUAL had a positive gap, with responsiveness as the highest dimension (0.49), followed by tangibles (0.36), assurance (0.35), reliability (0.32), and empathy (0.31). The CSI value of 82.61% indicates that patient satisfaction is in the very satisfied category. The conclusion of this study shows that the level of satisfaction of BPJS patients with the services at the Primary Outpatient Clinic of UIN North Sumatra is in the category of very satisfied. The responsiveness and tangibles dimensions received the highest ratings, while the empathy and a dimensions still needed improvement to be able to meet patient expectations more optimally and sustainably.
