Abstract

Sectio cesarea is a surgical procedure to deliver a fetus with an incision on the abdominal wall and uterine wall of the mother. Spinal anesthesia in SC can cause shivering. The risk of shivering can occur due to the length of surgery. Objective: To determine the relationship between the length of surgery and the incidence of shivering in SC patients. Methods: This research design uses a correlation with a cross-sectional approach. Sampling technique with Purpose sampling of 115 patients who performed spinal anesthesia on SC patients. The research instrument contains patient data, which includes age, length of surgery, and incidence of shivering. This study used an observation sheet to measure the incidence of shivering in patients undergoing cesarean section, and the results of the data were analyzed using the chi-square test. Results: Most respondents were 21-35 years old (81.7%), the majority of the duration of surgery was less than 60 minutes with 67 patients (58.3%), and the incidence of shivering was 62 patients (53.9%). Conclusion: There is a relationship between the length of surgery and the occurrence of shivering in cesarean section patients who use spinal anesthesia at Efarina Etaham Berastagi Hospital with a p-value of 0.008 (<0.05).

Abstract

Delivery via caesarean section can lead to complications, one of which is Post-Dural Puncture Headache (PDPH). PDPH occurs within 3 days, with 66% of PDPH cases emerging within the first 48 hours post-puncture. Although PDPH manifestations may spontaneously recover within one week or after the leak seals, PDPH disrupts the care provided by the mother for her baby. The purpose of this study to determine the incidence of PDPH in post-caesarean section patients with spinal anesthesia at RSUD Kardinah Tegal City. This quantitative research utilized a descriptive design with a cross-sectional approach. The sample size was 35, selected through total sampling, with the sample characteristics being patients aged 17-45 years, ASA 1-2, and 48 hours post-operation. The data was collected through observations using the International Headache Society questionnaire (IHS). Data analyzed with univariate analysis aimed at seeing a picture of the frequency distribution of each variable. The results indicate that PDPH occurred in 2 respondents (5.7%) at 48 hours post-caesarean section with spinal anesthesia, and in 3 respondents (8.6%) at 72 hours. The highest percentage of PDPH incidence at RSUD Kardinah Tegal City was observed within the first 72 hours post-caesarean section with spinal anesthesia. Based on these results, it is evident that post-caesarean section patients can experience PDPH on the third day, which can be used for evaluation and care planning for these patients.

Abstract

Hemodynamic variables such as blood pressure, mean arterial pressure (MAP), and pulse can change during cesarean section surgery performed under regional anesthesia. To stabilize the patient's hemodynamic status, colloid fluid treatment is administered because it has a longer half-life in the intravascular fluid. The research aims to understand the timing and volume of administering hydroxyethyl starch/HES 130/0.4 (6%) to restore hemodynamics to normal after spinal anesthesia in cesarean section surgery. The research method employs a quantitative study with an analytical observational design (cross-sectional). Data collection instruments include a hemodynamic observation sheet and a bedside monitor data measurement tool. This research was conducted over two months, from May to June. The sample for this research consists of 40 respondents using total sampling. The research location was conducted at Pusri Hospital in Palembang Indonesia. The study collected data by directly observing the respondents and recording the time and volume of hydroxyethyl starch 130/0.4 (6%) required for the hemodynamic status to return to normal. Data analysis was conducted using univariate analysis techniques. The results of the research analysis indicate that the time required for hemodynamic changes after spinal anesthesia has a standard deviation of 3.363, a mean value of 12.15, a maximum value of 20, and a minimum value of 5. HES 130/0.4 (6%), the amount of fluid given, and changes in blood flow after spinal anesthesia have a standard deviation of 30.783, a mean value of 99.65, a maximum value of 167, and a minimum value of 22.

Abstract

Shivering is a common postoperative issue that can exacerbate patient discomfort and hinder recovery. This study aims to evaluate the effectiveness of administering warm intravenous fluids in reducing shivering incidence in patients undergoing cesarean section (SC) with spinal anesthesia at RSUD Dr. Soekardjo, Tasikmalaya, Indonesia. Using a quasi-experimental design with a pretest-posttest control group model, 60 patients were divided into two groups: an experimental group receiving warm intravenous fluids and a control group receiving warm blankets. The study employed non-probability purposive sampling, dividing a total of 60 respondents into two groups: the experimental group (n = 30) and the control group (n = 30). An observation sheet for research instruments. The results of this study were tested using univariate analysis and bivariate analysis, specifically the Mann-Whitney U Test. Results indicate that warm intravenous fluids are significantly more effective in reducing shivering compared to warm blankets, with a p-value of 0.000 (p<0.05) in the Mann-Whitney test. This finding suggests that the use of warm intravenous fluids can improve patient comfort and accelerate postoperative recovery.

Abstract

Anxiety, characterized by feelings of unease, apprehension, and physiological reactions, is a common issue for patients facing surgery. Music therapy, which utilizes musical elements to promote well-being, has been proposed as a method to alleviate anxiety. This study aims to evaluate the effect of classical music therapy on anxiety levels in patients scheduled for cesarean section at Charitas Hospital Palembang. The study employed a pre-experimental design with a one-group pretest-posttest approach. The population included all patients undergoing preoperative care over the past three months, averaging 122 patients. A purposive sampling technique was used to select 40 participants, exceeding the required sample size of 30 to anticipate potential dropouts. The instrument used was the APAIS scale, with its validity and reliability confirmed through preliminary testing. Data were analyzed using univariate and bivariate methods, with statistical processing conducted using SPSS for Windows. The results indicated that the majority of participants were in early adulthood (26–35 years old), classified as ASA II (80%), had a high school education, and were predominantly housewives (50%). Before therapy, the mean anxiety score was 19.90, which decreased to 15.70 after the intervention. A paired t-test showed a significant reduction in anxiety (p < 0.05), suggesting that classical music therapy is effective in reducing preoperative anxiety in cesarean section patients.

Abstract

Enhanced Recovery After Surgery (ERAS) is a multimodal perioperative management aimed at faster recovery from major surgery, reducing the risk of complications, and shortening the length of stay compared to conventional perioperative methods. Hypotension is a side effect of a cesarean section with spinal anesthesia. This study aims to describe hypotension in patients undergoing cesarean sections with conventional spinal anesthesia methods and the Enhanced Recovery After Surgery (ERAS) method. This study used a quantitative research method with a descriptive design and a cross-sectional approach. The sampling technique was purposive sampling with a total of 40 cesarean section patients, consisting of 20 respondents in the conventional spinal anesthesia group and 20 respondents in the ERAS spinal anesthesia group. Blood pressure measurements were taken before and after spinal anesthesia using a bedside monitor. The data obtained included age and blood pressure, which were analyzed using statistics. The results showed that in the conventional spinal anesthesia group, 15 respondents experienced hypotension (75%), and 5 respondents did not experience hypotension (25%). In the Enhanced Recovery After Surgery (ERAS) spinal anesthesia group, 7 respondents experienced hypotension (35%), and 13 respondents did not experience hypotension (65%).

Abstract

Pregnancy, labor, and delivery lead to significant physiological changes, and having accurate knowledge of these changes is crucial for an effective spinal anesthetic technique during a caesarean section. Optimizing the mother's health before, during, and after a caesarean section is the goal of the ERACS care method, which is a specialized approach to surgical procedures. Issues with lower limb mobility, anxiety, and dependence on others are some mental health problems that may arise from prolonged recovery periods. Spinal anesthesia patients are evaluated using the Bromage scale to determine if they are ready to be released from the post-anesthesia room. Researchers at Panglima Sebaya Hospital aimed to compare the time taken by standard operating room procedures and ERACS to reach a Bromage Score (2) after spinal anesthesia in caesarean section patients. This study employed a cross-sectional design based on observational analytical research, which is a non-experimental quantitative technique. Out of 84 patients, 47 were treated with standard SC techniques, and 47 were treated with ERACS SC. The findings indicated that the conventional method took 181–240 minutes (3–4 hours) to achieve a Bromage score (2), while the ERACS method only took 60–120 minutes (1–2 hours). A significance value of 0.000 (2-tailed) was determined based on the independent t-test results. The results showed that the ERACS method required less time than the conventional method to achieve Bromage Score (2) in CS patients post-spinal anesthesia.
