Abstract

The enactment of Law No. 17 of 2023 on Health marks a significant reform in Indonesia's health legal system, revoking and replacing several previous laws, including Law No. 38 of 2014 on Nursing. This decision carries various implications for nursing practice, particularly concerning professional authority, competency standards, and legal protection for nurses in performing their duties. This study employs a qualitative approach through literature review methods involving legal documents, scientific journals, and expert opinions to analyze the regulatory impact of Law No. 17 of 2023 on the nursing profession. The findings indicate that the new law integrates the role of nurses into a more unified national health service system while formally recognizing the autonomy of nurses as professional healthcare providers. Nevertheless, the repeal of the Nursing Law is considered to create normative gaps, particularly in the technical regulation of nursing practices, ethical codes, and professional sanction mechanisms. This condition highlights the urgency for implementing regulations (government regulations or ministerial decrees) that can adequately accommodate the needs and legal protection of the nursing profession. Therefore, it is essential for stakeholders, including professional organizations and nursing education institutions, to actively engage in responding to these regulatory dynamics. This study is expected to contribute academically to the discourse on the development of health law that is more inclusive of all health professionals, particularly nurses.

Abstract

Despite ongoing efforts to reduce maternal mortality in Indonesia, antenatal care (ANC) coverage—particularly completion of ?4 visits (K4)—remains suboptimal in regions such as Bengkulu City. The involvement of community health cadres is recognized as a key strategy to improve maternal health outcomes; however, limited research has directly examined the effect of structured cadre-based empowerment on ANC knowledge, attitudes, and service utilization. This study aimed to examine the effect of ANC cadre empowerment on ANC knowledge, attitudes, and the frequency of ANC visits among pregnant women in Bengkulu City, Indonesia. This quasi-experimental study employed a pre-test and post-test control group design involving 40 pregnant women in Bengkulu City, Indonesia divided equally into intervention and control groups. The intervention group received structured cadre-based empowerment involving trained health cadres, while the control group received standard counseling. Data were analyzed using Wilcoxon and Mann-Whitney tests to assess differences in knowledge, attitudes, and ANC visit frequency. After the intervention, the knowledge scores in the intervention group significantly increased from 53.80 to 73.90 (p=0.020), while the control group showed no significant change. Attitude scores in the intervention group also increased significantly from 35.35 to 38.25 (p=0.014), unlike the control group (p=0.320). Regarding ANC visits, the intervention group had a higher mean number of visits (mean=4) compared to the control group (mean=3), with a statistically significant difference (p=0.001). Cadre-based empowerment through structured training significantly improved pregnant women’s knowledge, attitudes, and ANC service utilization. This suggests the importance of integrating cadre empowerment models into routine maternal health programs to support improved ANC compliance.

Abstract

Background: Stunting is a chronic nutritional problem that has long-term impacts on child growth and development and the future quality of human resources. Karanganyar Village in Pekalongan Regency is recorded as one of the areas with the highest prevalence of stunting, thus requiring the active role of Posyandu cadres in toddler screening activities to detect and address stunting early. Purpose: To identify the role of Posyandu cadres in toddler stunting screening activities as part of efforts to accelerate stunting reduction. Method: This study used a descriptive qualitative method with data collection techniques including in-depth interviews, participant observation, and documentation. Informants consisted of 5 active cadres as primary informants, 2 health workers as supporting informants, and 10 mothers of toddlers as triangulation informants. Sampling techniques used purposive and accidental sampling. Results: Integrated Health Service Post (Posyandu) cadres play a role in basic health services, early detection of nutritional disorders, and recording and reporting toddler growth data. Despite obstacles such as limited measuring instruments, minimal training, and a lack of incentives, cadres continue to demonstrate a high level of commitment to their duties. Conclusion: The role of cadres in toddler stunting screening activities significantly impacts the success of stunting reduction programs. However, this success requires ongoing training, logistical support, and an adequate incentive system. Suggestion:  The role of cadres is needed through policies that provide technical, structural, and social support so they can optimally and sustainably carry out their functions in stunting prevention efforts. Keywords: Posyandu cadres; Stunting; Toddler screening Pendahuluan: Stunting merupakan masalah gizi kronis yang berdampak jangka panjang terhadap tumbuh kembang anak dan masa depan kualitas sumber daya manusia.
