Abstract

This service activity aims to increase community empowerment in the field of digital economy and Islamic sharia through understanding the use of technology and halal certification towards strengthening agro-tourism areas. This is certainly in line with the government's vision which continues to strive to encourage Indonesia to become a world producer of halal products. Halal certification, which was originally voluntary, has now changed to mandatory. The implementation of halal certification is carried out in stages starting with food and beverage products on 17 October 2019 and non-food products starting on 17 October 2021. Another aim is to develop synergy between universities and village communities so that training and mentoring are needed. The material presented is how to create an online buying and selling and promotional account and how to prepare a manual document for a halal guarantee system. The results of this service can be used as input for lecturers and education staff to increase professionalism and also as input in preparing further service proposals so that they are worthy of being funded by the Higher Education

Abstract

Introduction : Oligohydramnios is a condition of reduced amniotic fluid. This is often detected accidentally during ultrasonography examination so that there is often a delay in diagnosis. Amnioinfusion is one of the therapies given to pregnant women with oligohydramnios Aim: This study aims to determine the characteristics of pregnant women with oligohydramnios who are treated with amnioinfusion Method: This was a descriptive study using medical record data of inpatients at the Fetomaternal Polyclinic, Dr. M. Djamil Hospital Padang from March 2022 to March 2023. The inclusion criteria were pregnant women diagnosed with oligohydramnios and the exclusion criteria were multiple pregnancies. Sampling was done by total sampling. Result: There were 12 pregnant womens who met the inclusion and exclusion criteria. Most of the subject aged 20-35 years (91.7%), high school graduates (58.3%), and multiparous (66.7%). There are as many as 50% of fetuses with congenital abnormalities. Most of the oligohydramnios were diagnosed in the third trimester (83.3%). Pregnant women with oligohydramnios have a relatively high proportion of preterm births (41.7%). Most of the subject still have good fetal outcome (alive) (66.7%). Conclusion: Pregnant women with oligohydramnios had age, education and parity who were not at high risk but more often had fetuses with congenital abnormalities and premature births. Amnioinfusion reduces fetal mortality

Abstract

Introduction: The pathophysiology of preeclampsy is not yet fully understood, but failure of tropoblastinvasion and placentation, which is influenced by factors such as pregnancy-associated plasma proteinA (PAPP-A) and insulin-like growth factor 1 (IGF-1), is thought to play a role.Aims: This study aimed to explore the difference in PAPP-A and IGF-1 levels between Early OnsetPreeclampsia (PEAD) and Late Onset Preeclampsia (PEAL) assuming that the role of PAPP-A andIGF-1 is more significant in the pathogenesis of PEAD than PEAL.Methods: This is an analytical observational study with a cross-partition comparative study design.Clinical data were obtained at Dr. M. Djamil Padang Hospital, while PAPP-A and IGF-1 levels weremeasured at the Biomedical Laboratory of the Faculty of Medicine, Andalas University. Samples aretested according to reagent procedures and analyzed by experts. Results: Average PAPP-A levels were 2.45+0.35 pg/mL in the early onset preeclampsy group and2.85+0.50 pg/mL in the late onset preeclampsy group. These two levels differed statisticallysignificantly (p=0.006). That means that low levels of PAPP-A are associated with and play a role inthe pathogenesis of early onset preeclampsy. Average IGF-1 levels were 4.66+0.91 pg/mL in the earlyonset preeclampsy group and 5.39+0.74 pg/mL in the late-onset preeclampsy group. These two levelsdiffered statistically significantly (p=0.010). That means that low levels of IGF-1 are associated withand play a role in the pathogenesis of early onset preeclampsy. PAPP-A levels were significantlypositively correlated with IGF-1 levels (p=0.000).Conclusion: PAPP-A levels are lower in PEAD than PEAL, as are IGF-1 levels. These findings confirmthe role of PAPP-A and IGF-1 in preeclampsia. Both of these hormones have potential as indicatorsand markers for the prediction and management of preeclampsy in early and late onset periods.

Abstract

Vitamin D deficiency (VDD) is identified as a public health problem in many countries, and pregnant women have been identified as a high-risk group, among whom the prevalence of VDD ranges between 20 and 40%. Vitamin D deficiency causes essential health problems, not only in the mother but also in the baby, because the mother's vitamin D stores are the primary source of vitamin D for the fetus. During pregnancy, severe vitamin D deficiency in mothers has been associated with biochemical evidence of impaired bone homeostasis, congenital rickets, and bone fractures in newborns. This study aims to determine the correlation between umbilical cord blood 25(OH)D levels in term pregnancy and Neonatal anthropometry. This research is analytical research using a cross-sectional approach. The sample in this study amounted to 40 people. Data analysts use people tests. The results of this study obtained an average umbilical cord serum 25(OH)D level of 14.70 ± 4.93. There was no correlation between umbilical cord blood 25(OH)D levels in term pregnancy and neonate anthropometry (p>0.05). The average level of 25(OH)D is included in the insufficiency category. Therefore, it is necessary to increase vitamin D supplementation during pregnancy.

Abstract

Background: Eclampsia is a convulsive condition associated with hypertensive disorders in pregnancy which threatens maternal and fetal life which generally requires intensive care. Worsening conditions can complicate multi-organ disorders, coagulation disorders, and infections. Case: A 20 year old, nulliparous woman, preterm gestational age 35-36 weeks of gestation, referred to M Djamil General Hospital with complaints of 3 seizures at home and loss of consciousness. At the time of examination, we found apathetic consciousness; blood pressure of 210/118 mmHg. Urine protein is +3. The patient was terminated by emergency cesarean section. The patient was anesthetized under general anesthesia, and was treated in the postoperative intensive care unit with a ventilator attached. During hospitalization, the patient developed recurrent seizure, laryngeal edema and cardiac arrest, but returned to circulation spontaneously after resuscitation. Hemodynamic and metabolic monitoring is strict on the patient and managed appropriately with good result. Discussion: Eclampsia preceded by preeclampsia is known as a theoretical disease with a two-stage pathogenesis. The principles of eclampsia management are control of seizure, regulation of blood pressure, and termination of pregnancy. Collaboration between multidisciplinary teams determines the success of managing eclampsia cases and improving outcomes for the better.

Abstract

Maternal mortality is one of the unresolved health problems in the world. Obstetric complications directly in maternal mortality (pregnancy complications, childbirth, and the puerperium). This study aims to analyze the relationship between the determinants of maternal mortality due to obstetric complications. This type of research is an observational analytic study with a case-control design. In this study, there were 48 case samples and 144 control samples with a ratio of 1:3. The data obtained in the form of patient medical record data. Data analysised by univariate, bivariate and multivariate. The results showed that the most common cause of maternal death was preeclampsia/eclampsia. Risk factors that have a significant relationship with maternal mortality due to obstetric complications are anemia status (OR: 2.111; CI: 1.053-4.233; p: 0.035), history of maternal disease (OR: 2.514; CI: 1.260-5.015; p: 0.009) and HR (OR: 2.121; CI: 1.062-4.237; p: 0,033). In the multivariate results, the dominant risk factor was a history of maternal disease followed by anemia status and HR. The tendency of mothers who have a history of disease will increase the risk of maternal death compared to mothers who do not have a history of the disease.
