Abstract

Sepsis neonatus menyebabkan morbiditas dan mortalitas yang bermakna. Berdasarkan penelitian pada populasi di  dua dekade terakhir, diperkirakan insiden sepsis neonatus secara global adalah 2202 per 100.000 kelahiran hidup, dengan mortalitas antara 11-19%. Kultur darah masih menjadi gold standar untuk konfirmasi diagnosis sepsis neonatus, tetapi masih terbatas untuk mendapatkan hasilnya, diperlukan waktu sekitar 24-72 jam. Sepsis berhubungan dengan disfungsi sel endotel yang berat. Disfungsi endotel dapat disebabkan oleh oksidan, hiperglikemia, ROS, endotoksin bakteri serta sepsis. Disfungsi endotel pada sepsis neonatus terjadi penumpahan dari bahan-bahan yang menyusun lapisan glikokaliks endotel. Lapisannya  disusun oleh  proteoglikan, glikoprotein dan glikosaminoglikan. Komponen glikokaliks yang berada di  sirkulasi seperti syndecan-1, hyaluronan, heparan sulfate dan chondroitin sulfate dapat ditemukan di plasma/serum dan urin, diharapkan dapat digunakan  sebagai biomarker diagnosis sepsis neonatus.

Abstract

The world has entered the digital era 4.0. Zakat and infaq are the drivers of economic improvement. This is because zakat and infaq funds are a source of funds for the community. Internal control is very necessary because good internal control is a reflection of good managerial practices. In order to maintain good trust, good managerial practices, one of which is through the existence of an internal control system. The company is expected to pay attention to human resources or employees by understanding the factors that can support employee performance. This study uses research design methods and data analysis methods. The aim is to empirically test the effect of internal control on accountability and management at BAZNAS Riau Province. Thus, it is hoped that the distribution of zakat, infaq, and appropriate funds can break the chain of poverty caused by income. Internal control is very necessary.

Abstract

Preeklampsia merupakan hipertensi yang timbul pada usia kehamilan lebih dari 20 minggu disertai proteinuria. Preeklampsia dapat menimbulkan komplikasi pada ibu dan janin. Penelitian ini bertujuan untuk mengetahui perbandingan faktor risiko maternal dan luaran bayi baru lahir pada kehamilan preeklampsia berat (PEB) dan kehamilan normal di RSUP Dr. M. Djamil Padang tahun 2018-2020. Penelitian ini merupakan penelitian analitik observasional dengan desain cross-sectional komparatif. Data yang diambil merupakan data sekunder, yaitu catatan rekam medis ibu hamil di RSUP Dr. M. Djamil Padang tahun 2018-2020. Sampel penelitian diambil secara consecutive sampling. Analisis bivariat menggunakan program komputer. Hasil penelitian menunjukkan terdapat perbedaan yang bermakna pada usia ibu, indeks massa tubuh (IMT) ibu, riwayat preeklampsia, berat badan lahir, usia gestasi, skor APGAR menit 1 dan 5, serta tidak terdapat perbedaan pada variabel paritas, ANC, tempat ANC, kematian neonatus dini dan penyebab kematian neonatus dini. Terdapat perbedaan faktor risiko maternal dan luaran bayi baru lahir antara kehamilan PEB dan kehamilan normal.

Abstract

This study aimed to explore healthcare team members’ perspectives towards the factors that can contribute to the quality improvement of comprehensive emergency obstetric and neonatal care (CEmONC) services at a tertiary hospital. This qualitative descriptive study used convenience sampling to collect the sample. The inclusion criteria were the health workers who have worked for at least one year, in which a total of 119 participants took part in this study. The data were obtained from an online questionnaire, and the collected data were then analyzed with qualitative thematic analysis. This study revealed that three key aspects affecting the success of CEmONC services: i) health human resources (high-quality healthcare workers, in adequate proportion, with excellent teamwork, and commitment to being on time); ii) facilities and infrastructure (the availability of the intensive care unit and 24-hour laboratory, and the access to the obstetric/neonatal ward), and iii) the hospital service system (well-integrated external referral system and internal consultation). As the highest referral healthcare center, hospital managers should monitor and evaluate these three factors periodically to ensure that high-risk mothers and babies can acquire appropriate subspecialty care in the hospital and further can reduce maternal and neonatal mortality rates.

Abstract

This study aims to examine the purchasing decision factors of Small and Medium Enterprise (SMEs) coffee products in Bireun Regency, Aceh, Indonesia. Lifestyle and Product Attractiveness were tested as key predictors. Meanwhile, consumer attitudes as mediation. We approached as many as 500 coffee-loving youths to be sampled. However, 200 of them finally responded. Furthermore, the data was analyzed using the Structural Equation Modeling (AMOS-SEM) method. Based on the test results, lifestyle markedly has a positive and significant impact on attitudes, thus leading to purchasing decisions. Product attractiveness is known to have a significant impact on attitudes but cannot significantly impact purchasing decisions. Consumer attitudes are a link in influencing the lifestyle and attractiveness of local coffee products. Finally, it can be stated that the decision to purchase local coffee products has become the lifestyle of Acehnese youth today young people and the attractiveness of innovative products will help coffee SMEs players increase sales.

Abstract

Penelitian ini bertujuan untuk menentukan biaya pokok produksi usaha ikan salai di Kecamatan Rantau Kopar, Kabupaten Rokan Hilir. Ikan salai Silais dan Baung merupakan produk unggulan dengan nilai ekonomi tinggi dan menjadi mata pencaharian utama masyarakat setempat. Namun, usaha ini menghadapi tantangan dalam penentuan biaya pokok produksi yang akurat, yang mempengaruhi penentuan harga jual dan keuntungan. Penentuan biaya pokok produksi didasarkan pada tiga elemen biaya: bahan baku, tenaga kerja, dan overhead. Untuk mengidentifikasi permasalahan yang dihadapi, dilakukan diskusi, wawancara, dan observasi langsung dengan nelayan dan pemilik usaha. Hasilnya menunjukkan kendala seperti keterbatasan bahan baku  akibat banjir dan pencemaran air sungai, serta penggunaan metode produksi tradisional yang memerlukan biaya perbaikan tinggi. Solusi yang diusulkan mencakup pembuatan tambak untuk budidaya ikan Baung, serta peningkatan peran pemerintah dalam memberikan bantuan modal dan pelatihan kepada pelaku usaha. Selain itu, diperlukan koordinasi dengan pabrik untuk mencegah pencemaran sungai. Pemerintah daerah juga diharapkan memberikan dukungan dalam bentuk bantuan permodalan dalam mengembangkan usaha ikan salai di Kecamatan Rantau Kopar, guna meningkatkan efisiensi dan keberlanjutan usaha tersebut.

Abstract

Penelitian ini bertujuan untuk menganalisis pengaruh Thin Capitalization, Capital Intensity, dan firm size terhadap tax avoidance pada perusahaan property dan real estate yang terdaftar di Bursa Efek Indonesia selama periode 2021-2023. Metode penelitian yang digunakan adalah kuantitatif dengan analisis regresi linier berganda dengan menggunakan SPSS 30. Sampel dipilih melalui purposive sampling, menghasilkan 183 sampel dari 61 perusahaan. Setelah membuang 86 data outlier, total sampel penelitian digunakan menjadi 97 sampel. Hasil penelitian dengan uji parsial menunjukkan bahwa thin capitalization, capital intensity berpengaruh terhadap tax avoidance, sedangkan firm size tidak berpengaruh terhadap tax avoidance. Hasil dengan uji simultan menunjukkan bahwa thin capitalization, capital intensity, dan firm size secara bersama-sama berpengaruh terhadap tax avoidance.

Abstract

Diabetes Mellitus is one of the chronic diseases and every year the number of sufferers is increasing in various parts of the world. This disease is a metabolic disorder in which the body is impaired in using glucose, protein, and fat due to insulin deficiency or insulin resistance. There are 422,000,000 people worldwide suffering from diabetes mellitus, mostly in low- and middle-income countries and about 1,600,000 deaths are associated with diabetes mellitus every year. This service aims to increase the knowledge, insight, and independence of patients in implementing Diabetes Self Management Education with the family to increase the self-efficacy of patients with diabetes mellitus. The method of this community service is training by providing Diabetes Self Management Education material to be applied in the patient's daily life. Participants are patients with diabetes mellitus and the closest family as patient companions, which was carried out at the Langsa Kota Health Center on May 29 - June 12, 2024. The results of this service were obtained before the intervention the majority of participants had moderate knowledge (78%) and after the intervention the majority had good knowledge (89.1%), there was also an increase in self-efficacy from before the intervention the majority was moderate (64%) after the intervention, the level of self-efficacy of the participants increased with a high majority (78.3%). It is recommended that health facilities apply diabetes self-management education to diabetics because if diabetics understand this self-management, they can manage their health wisely and increase self-efficacy.

Abstract

This study aims to emphasize the importance of staffing planning and facilities in improving work effectiveness at SMP IT Ar-Rahman Lareh Sago Halaban, in the field of education, work effectiveness is highly dependent on the availability of a competent workforce and adequate facilities. This research uses a qualitative approach with a case study method at SMP IT Ar-Rahman Lareh Sago Halaban. Data were obtained through interviews, observation, and document analysis. The results showed that good staffing planning, including recruitment, training and staff development, plays a significant role in improving the performance of teachers and school staff. In addition, adequate facilities, such as comfortable classrooms, well-equipped laboratories and other supporting facilities, are also very important for creating a conducive learning environment. The findings emphasize that integrated and sustainable planning in terms of staffing and facilities is key to improving work effectiveness at SMP IT Ar-Rahman Lareh Sago Halaban. Therefore, schools need to continuously improve efforts in planning and managing human resources and educational facilities to achieve optimal educational goals.

Abstract

BACKGROUND: Pre-eclampsia has varying onset patterns, which are challenging to determine due to their association with diverse clinical parameters, including blood pressure regulation, vascular remodeling, and placental hypoxia. These parameters influence the expression of key biomarkers such as microRNAs, metalloproteinases, and arterial pressure indices. Therefore, understanding the associations is crucial for improving early diagnosis and management. This study was conducted to compare miR-210 expression, mean arterial pressure (MAP), and matrix metalloproteinase (MMP)-2 levels between early-onset pre-eclampsia (EOPE) and late-onset pre-eclampsia (LOPE).METHODS: Pregnant women with EOPE (20–34 weeks) and LOPE (35 weeks to term) were included in this cross-sectional comparative study, and their blood samples were collected. miR-210 expression was quantified with reverse transcription polymerase chain reaction (RT-PCR), MAP was measured using sphygmomanometer, while MMP-2 levels were measured using enzyme-linked immunosorbent assay (ELISA).RESULTS: miR-210 expression was more frequently observed in the EOPE group (87.5%) compared to the LOPE group (77.5%), with a significant difference identified between the two groups. The odds ratio for miR-210 expression in EOPE compared to LOPE was 2.03 (95% CI: 1.06–6.72). The MAP was notably higher in the EOPE group (121.15 mmHg) than in the LOPE group (116.15 mmHg), with a significant difference observed. Conversely, MMP-2 levels were significantly higher in the LOPE group (390.99 ng/mL) compared to the EOPE group (271.35 ng/mL).CONCLUSION: There are significant differences in miR-210 expression, MAP and MMP-2 between EOPE and LOPE. These findings suggest that miR-210, MAP, and MMP-2 could be useful biomarkers for distinguishing between EOPE and LOPE, potentially guiding more effective management and intervention strategies.KEYWORDS: miR-210, MAP, MMP-2, EOPE, LOPE, pre-eclampsia
