Abstract

Acehnese women living in rural areas during the conflict immediately experienced economic problems and violations. Entering a normal phase, rural women returned to their roles outside the home, helping to build the family economy. Inequality in various sectors has exacerbated the powerlessness of women living in these areas, particularly in economic matters. This study analyzes the forms of post-conflict women's participation and the obstacles they face in economic empowerment. The documents in this study are supported by various field data, informal interviews, and various sources of information from research literature on the burdens of rural women qualitatively. The results of the study indicate that women in North Aceh, particularly in the rural communities of Nisam, Guha Ulue, Alue Rambe, and Pulau Iboh in Kutamakmur District, are mostly farmers who also work as housewives. Their participation is still lacking in the planning, implementation, and evaluation of village development. Women's empowerment in Aceh faces various structural problems, including patriarchy, discriminatory interpretations of Islamic Sharia law, poverty, and low levels of political participation at the village level. The research findings indicate that rural women in Aceh have significant potential and a strategic role in village development but still face various cultural and structural barriers that limit their participation in empowerment.

Abstract

Background: Gastroschisis is a congenital abdominal wall defect characterized by herniation of abdominal organs through a paraumbilical defect without a protective membrane. Prenatal detection plays a crucial role in planning delivery and optimizing neonatal management. Case Presentation: We report a case of a 39-year-old pregnant woman, G4P2A1L2, who presented at 36–37 weeks of gestation with latent phase of labor and a history of two previous cesarean sections. Antenatal ultrasonography revealed a singleton live intrauterine fetus with suspected gastroschisis, demonstrated by herniation of fetal bowel outside the abdominal cavity without a covering membrane. Fetal well-being assessment using cardiotocography showed a category I tracing. Delivery was performed by cesarean section due to maternal obstetric indications. A male neonate was delivered with a birth weight of 2415 grams, and postnatal Ballard scoring indicated a gestational age of approximately 36 weeks. The neonate was planned for further management according to clinical condition. Conclusion: Prenatal diagnosis of gastroschisis allows for appropriate delivery planning and early neonatal management. A multidisciplinary approach involving obstetricians, neonatologists, and pediatric surgeons is essential to improve outcomes in neonates with congenital  abdominal wall defects.

Abstract

Pendahuluan: Anemia hemolitik autoimun yang terjadi pada masa kehamilan merupakan kondisi yang jarang dijumpai, namun berpotensi menimbulkan keadaan yang mengancam keselamatan ibu dan janin. Manifestasi klinis berupa anemia berat yang disertai ikterus, serta tidak adanya perbaikan kadar hemoglobin meskipun telah dilakukan transfusi darah, dapat menjadi petunjuk penting dalam menegakkan kecurigaan terhadap anemia hemolitik autoimun. Laporan Kasus: Wanita berusia 21 tahun dengan kehamilan pertama yang menunjukkan seluruh karakteristik klinis tersebut. Proses diagnostik menghadapi kendala karena pemeriksaan Coombs test memberikan hasil negatif, sehingga diagnosis tidak dapat ditegakkan secara langsung berdasarkan pemeriksaan penunjang standar. Penegakan diagnosis akhirnya didukung oleh respons klinis yang cepat dan signifikan setelah pemberian terapi kortikosteroid oral, yang memperkuat dugaan adanya mekanisme autoimun sebagai penyebab hemolisis. Setelah diagnosis ditegakkan dan terapi yang sesuai diberikan, terjadi perbaikan kondisi pasien dan peningkatan kadar hemoglobin, sehingga memungkin untuk dilakukan terminasi kehamilan. Kesimpulan: Keberhasilan tatalaksana maternal dan perinatal yang optimal dapat dicapai melalui pengenalan dini, kecurigaan klinis yang adekuat, serta penatalaksanaan yang cepat dan tepat pada kasus anemia hemolitik autoimun dalam kehamilan.

Abstract

Pendahuluan: Angka Kematian Ibu (AKI) di Indonesia hingga kini masih cukup tinggi dan belum mencapai target global maupun nasional. Strategi utama untuk menurunkan AKI adalah Antenatal Care (ANC). Perlu adanya kolaborasi antar tenaga kesehatan yang terlibat pada layanan ANC untuk meningkatkan pelayanan kepada ibu hamil. Objektif: Mengetahui tingkat persepsi tenaga kesehatan terhadap implementasi Interprofessional Collaboration (IPC) pada layanan ANC di Puskesmas Air Tawar dan Puskesmas Lapai. Metode: Penelitian ini menggunakan metode analitik observasional dengan rancangan cross sectional dengan kuesioner Collaborative Practice Assessment Tool (CPAT). Sampel penelitian adalah 42 tenaga kesehatan yang ditentukan dengan metode total sampling. Hasil: 41 tenaga kesehatan memiliki persepsi yang tinggi terhadap IPC dengan komponen hubungan antar anggota, hubungan tim dengan masyarakat, koordinasi dan pembagian peran, kepemimpinan, dan komponen misi, tujuan, dan sasaran memiliki persentase tertinggi (97,6%) sedangkan komponen pembuatan keputusan dan manajemen konflik memiliki persepsi yang sedang (71,4%). Tidak terdapat perbedaan yang signifikan antara seluruh komponen CPAT dengan usia, profesi, dan lama kerja responden (p > 0,05). Pembahasan: Persepsi tinggi di sebagian besar domain menandakan budaya kolaboratif; skor sedang pada pengambilan keputusan–konflik menunjukkan perlunya pelatihan, SOP bersama, dan mekanisme resolusi terstandar. Simpulan: Tenaga kesehatan memiliki tingkat persepsi yang tinggi terhadap IPC pada layanan ANC, tetapi masih terdapat persepsi sedang pada satu komponen persepsi. Tidak terdapat pengaruh antara usia, profesi, dan lama kerja dengan persepsi tenaga kesehatan dalam implementasi IPC pada layanan ANC.

Abstract

Introduction: Legal protection for healthcare professionals is a critical issue, particularly concerning pregnancy termination procedures involving medical emergencies. This study focuses on how Indonesian law regulates pregnancy termination in cases involving anencephalic fetuses, especially when the gestational age exceeds 14 weeks, and examines the protection afforded to doctors and the practical challenges they face. The research aims to analyze the legal framework provided under Law Number 17 of 2023 on Health and assess its implementation in the field. Method: Using a juridical-normative and empirical-juridical approach with a descriptive-analytical method, this study utilizes primary data from interviews with doctors, legal scholars, and religious figures, and secondary data from relevant laws and academic literature. Result: The findings reveal that while the law allows pregnancy termination under 14 weeks for medical emergencies or pregnancies from sexual violence, it lacks clear provisions for cases exceeding 14 weeks, such as anencephaly. Although anencephaly poses a serious medical risk, legal uncertainty persists for healthcare workers who perform terminations after this threshold. Conclussion: Terminations conducted following medical standards and with consent should not be criminalized. Therefore, legal reforms and clearer regulatory guidelines are essential to ensure adequate protection for medical personnel handling such exceptional cases.

Abstract

Kepulauan Meranti termasuk salah satu Kawasan Pengembangan Ketahanan Pangan Nasional karena menjadi penghasil sagu terbesar di Indonesia. Tanaman sagu di Kabupaten Kepulauan Meranti telah berkembang dan dimanfaatkan masyarakat sejak lama, dan mempunyai nilai ekonomi yang cukup membantu masyarakat di sekitar kawasan sagu. Tujuan pengabdian kepada masyarakat adalah untuk memberikan penyuluhan dan pemahaman kepada pelaku usaha pabrik dalam menentukan harga pokok produksi dengan lebih efisien dan akurat. Tim pengabdian memberikan solusi pada permasalahan pabrik sagu Ahai Desa Maini Darul Aman dengan melakukan analisis dan mengidentifikasi semua komponen biaya produksi yang terkait pada pabrik seperti biaya bahan baku sagu, biaya tenaga kerja, biaya energi, biaya pemeliharaan mesin, dan biaya overhead pabrik. Selanjutnya data biaya tersebut digunakan untuk menghitung harga pokok produksi dengan lebih akurat. Metode pelaksanaan kegiatan pengabdian masyarakat ini yaitu dalam bentuk sharing discussion dengan para pelaku usaha Pabrik Sagu Ahai Desa Maini Darul Aman. Pelaku usaha pabrik diharapkan memahami dan menerapkan metode Penentuan Harga Pokok Produksi sesuai dengan kaidah akuntansi biaya yang mencakup langkah langkah yang jelas untuk mengidentifikasi, menghitung dan mengelola biaya produksi sagu secara efesien.

Abstract

Background: Maternal obesity is associated with disrupted metabolic regulation, including elevated leptin levels that may affect fetal growth. Previous studies have reported mixed findings on maternal and cord blood leptin concentrations, with some showing higher leptin levels in infants born to obese mothers, while others found no significant differences. Associations between maternal obesity and neonatal outcomes have also been inconsistent. This study aims to generate new evidence from an Indonesian cohort by comparing maternal and cord blood leptin levels, as well as neonatal outcomes, between obese and normal-weight mothers.Materials and Methods: Analytical cross-sectional study enrolled 30 obese and 30 normal-weight mothers from two hospitals in Padang, Indonesia. Maternal and cord blood samples were collected at delivery, and leptin levels were measured using ELISA. Ethical approval and informed consent were obtained. Neonatal outcomes included birth weight, length, head circumference, and ponderal index. Data were analyzed using Independent Samples t-test or Mann–Whitney U test, with p<0.05 considered statistically significant. Results: Maternal leptin levels were significantly higher in obese mothers than in normal-weight mothers (70.8±39.0 vs. 44.5±42.5 ng/mL; p=0.002). Cord blood leptin levels did not differ between groups (19.7±14.9 vs. 16.1±17.5 ng/mL; p=0.394). Neonates of obese mothers had higher birth weight (p=0.003), while other anthropometric measures were similar.Conclusion: Maternal obesity was associated with elevated maternal leptin levels and increased neonatal birth weight, while cord blood leptin did not differ significantly between groups. These findings suggest that maternal leptin reflects maternal metabolic status rather than fetal growth.Keywords: maternal leptin, cord blood leptin, obesity, pregnancy, neonatal outcome

Abstract

Background: More than half of adolescents in low- and middle-income countries feel unprepared for menarche due to a lack of menstrual knowledge, which leads to poor menstrual hygiene practices that increase reproductive health risks. This problem worsens in boarding schools, where restrictions like bans on mobile phones, limited contact with parents, and the absence of formal health programs severely limit access to accurate menstrual health information. Peer education leverages adolescents' natural dependence on peers for sensitive issues and is especially vital in environments with limited information, such as boarding schools. This study aimed to evaluate the effectiveness of peer education in enhancing Menstrual Hygiene Management (MHM) knowledge and attitudes among female students in a boarding school and to explore their experiences with this intervention.Method: An explanatory sequential mixed-methods design was employed. The quantitative phase involved a One Group Pre-test Post-test design with 31 purposively selected students to assess changes in knowledge and attitude following the peer education intervention. Five trained peer educators conducted 120-minute sessions using standardized modules. Quantitative data were analyzed using paired t-tests and N-gain scores. The qualitative phase adopted a phenomenological approach with in-depth interviews involving six primary informants (students) and five supporting informants (peer educators) to explore the intervention experiences and the mechanisms behind observed changes. Qualitative data were analyzed thematically with source triangulation to enhance credibility.Result: Mean knowledge scores increased significantly from 30.10 to 72.04 (p<0.001, N-gain=0.612, moderate effectiveness), with the largest improvement in pad disposal knowledge (90.3 percentage points). Mean attitude scores rose from 54.41 to 80.72 (p<0.001, N-gain=0.586, moderate effectiveness). Qualitative findings showed that students gained evidence-based knowledge, corrected menstrual myths, and expressed readiness to adopt proper hygiene practices. However, deeply rooted cultural beliefs remained resistant after a single session. The findings confirmed that knowledge improvements do not automatically lead to uniform behavioral change. Boarding schools should implement multiple-session peer education programs with spaced repetition, invest in peer educator training, establish student-led health clubs, and incorporate a formal MHM curriculum before menarche.

Abstract

Background: Hypertension in pregnancy needs to beware out because it can progress to more severe conditions, namely preeclampsia and eclampsia, thereby endangering the mother and the unborn baby. Risk factors in Hypertension in pregnancy are as age, education, occupation, parity, gestational age, economic factors, personal and family history of hypertension, obesity and activity/exercise.Objective: This study aims to determine the risk factors for Hypertension in pregnancy in Deli Serdang Regency, North Sumatra.Method: This study method used analytic observational with a cross sectional design. The study subjects were pregnant women at Public Health Center of Deli Serdang Regency who were considered the inclusion criteria and were selected as study subjects. The sampling technique by means of random sampling amounted to 164. Univariate analysis used to get an overview of the frequency distribution and percentage of the variables to be studied. Bivariate analysis used to determine the relationship between the independent variable and the dependent variable. The statistical test was Chi-Square with CI 95%.  If p < 0.05. Multivariate analysis with logistic regression statistical tests to determine the influence of the independent variables on the dependent variable. Independent variables with p value <0.25 were included in the multiple logistic regression test model and selected using the enter method. The strength of the influence of the independent variable on the dependent variable is the Exp value (ß).Result: The results of the study proved the results of Multivariate analysis, can be synthesized that the risk factors for pregnancy hypertension were influenced by factors: education (p=0.045), parity (p=0.047), gestational age (0p=0.040), personal history of hypertension (p=0.013), history family (p=0.049), and obesity (p=0.016). While the factors of age, occupation, economy and activity/exercise have no effect on risk factors for hypertension in pregnancy. Based on Exp value (B), the most influential factor was personal history of hypertension (p=0.013). This is indicated by the Exp value (B) = 2.710 which is in the interval 1.234 – 5.953, followed by obesity (p=0.016) with the Exp value (= 2.612) which is in the interval 1.192 – 5.725.

Abstract

Background: The stunting rate in Indonesia in 2022 was 21.6%. I was reduced when compared to the SSGI data in 2021 of 24.4%. The stunting rate appears to be decreasing from year to year, but a joint commitment is still needed to achieve the target of reducing stunting to 14% by 2024. Stunting prevention is a much more effective step than stunting treatment itself. Prevention of stunting can be done by optimizing the role of mothers in the golden phase of child development. stunting prevention in Indonesia still focuses on pregnancy and toddlers, while prevention during the preconception period is still limited.Purpose:To synthesize and critically effective steps that can be taken to prevent stunting in the preconception periodMethods:The research method employed was a literature review, conducted by using primary studies from PubMed, google scholar, and ScienceDirect with the keywords “stunting; prevention; praconception” published between 2017-2023, in English, open access, and relevant to the topicResults:A total of 397 research articles were screened for eligibility, and 15 articles met the inclusion criteria. Based on the 15reviewed articles, devide into three themes: Nutritional status during preconception, Supplementation during preconception and Lifestyle during preconceptionConclusion: This review found that there are several efforts made in preventing stunting in the preconception period. it is recommended that health workers can optimize the prevention of stunting in the preconception period.
