Abstract

Introduction: Several studies on tuberculosis (TB) using spatial and time clustering analyses have been conducted in Indonesia, however none have specifically focused on regions characterized by diverse topography. This study aimed to identify geospatial clusters of TB cases in West Sulawesi Province, Indonesia, an area known for its topographic variability. Methods: An ecological study design was employed. TB case data, including bacteriologically confirmed and clinically diagnosed cases, were obtained from the Tuberculosis Information System (Sistem Informasi Tuberkulosis, SITB) of the West Sulawesi Provincial Health Office, covering the period from January 1, 2020, to December 31, 2023. Spatial visualization was performed using QGIS version 3.40.0. Cluster detection and spatial pattern analysis were conducted using SaTScan version 10.2.5. Results: TB cases in West Sulawesi formed clusters. A total of 17 clusters were identified—4 primary (most likely) clusters and 13 secondary clusters. In 2020, the primary cluster was located in Bambang Village, Bambang Subdistrict. In 2021, the primary cluster was in Lambanan Village, Mamasa Subdistrict. In 2022, the primary cluster was in Ulumambi Barat Village, Bambang Subdistrict, and in 2023, the primary cluster was again in Lambanan Village, Mamasa Subdistrict. Conclusion: This study found that the most likely TB clusters from 2020 to 2023 were consistently located in the eastern part of West Sulawesi Province, specifically in Mamasa Regency, an area characterized by mountainous terrain. This suggests that various environmental, social, and economic factors unique to mountain communities may influence TB transmission dynamics. The findings highlight the need for geographically tailored intervention strategies, including mobile TB services, community-based education, enhanced surveillance systems, the establishment of local TB support networks, and improved healthcare infrastructure adapted to mountainous areas. Future research should consider integrating genotypic, molecular, and geospatial approaches to advance global TB control efforts.

Abstract

Smoking behavior among adolescents is a concerning public health issue, especially in remote indigenous communities such as the Suku Anak Dalam. In their daily lives, smoking has become an ingrained habit and is considered normal. This study aims to understand the smoking behavior of Suku Anak Dalam adolescents in Tanjung Village, Bathin VIII Sub-District, Sarolangun Regency, and to identify the social and cultural factors that influence this behavior. This study employed a qualitative approach. Data were collected through in-depth interviews and participant observation involving seven active-smoking adolescents and nine supporting informants (parents, traditional leaders, and community facilitators). The study found that adolescent smoking behavior is influenced by peer pressure, weak parental supervision, and a social environment that normalizes smoking. Cigarettes are perceived as something ordinary and even a necessity. Parental control is weak and the advice given tends to be inconsistent. Smoking behavior among Suku Anak Dalam adolescents results from the interaction of peer influence, permissive parenting, and the normalization of smoking within their environment. This behavior develops from a young age and continues without adequate supervision from parents or community figures

Abstract

Hypertension is a global health problem as it causes high morbidity and mortality, with approximately 9.4 million deaths every year. This condition shows that hypertension remains a significant health burden. The aim of this study was to identify the factors associated with the incidence of hypertension in Jambi City. This research is a secondary data analysis with a cross-sectional design, using data from the Sehat Indonesiaku Application (ASIK) in 2023, conducted from August 2024 to May 2025. The sample included all individuals aged ≥ 15 years recorded in the ASIK application, selected using a total sampling technique. Data were analyzed using Chi-Square tests and Logistic Regression to identify risk factors. The proportion of hypertension among individuals aged ≥ 15 years was 35.2%. Variables significantly associated with hypertension among those aged ≥ 15 years were sex [Adj PR = 1.09; 95% CI: 1.01–1.17], age ≥ 40 years [Adj PR = 3.82; 95% CI: 3.53–4.13], physical activity [Adj PR = 0.82; 95% CI: 0.75–0.88], fruit and vegetable consumption [Adj PR = 0.89; 95% CI: 0.83–0.96], salt consumption [Adj PR = 1.56; 95% CI: 1.41–1.73], and obesity [Adj PR = 1.94; 95% CI: 1.80–2.10]. Age is the most influential factor for hypertension incidence; therefore, the community is advised to limit salt intake, maintain a healthy nutritional status to prevent obesity, and increase physical activity, especially among men aged ≥ 40 years.

Abstract

Accelerating the reduction of maternal and child mortality rates can be done by accessing quality maternal health services by antenatal care. This study aimed to identify the spatial distribution pattern of antenatal service coverage in Jambi Municipality in 2019-2021. Global and local Moran Index using Local Indicator for Spatial Autocorrelation (LISA) analysis was used in this study to map hotspots and lowspots of antenatal service coverage. This research used an ecological study with the unit of analysis of all sub-district in Jambi Municipality, totalling 62 sub-districts. Data on antenatal care coverage (K1 and K4) from 2019 to 2021 at the sub-district level were obtained from 20 community health centers. The results of this study found a positive autocorrelation of K1 and K4 from 2019 to 2021. The results of the analysis using LISA found that K1 and K4 coverage hotspots have the same pattern in the northwest direction of Jambi Municipality covering Danau Teluk, Pelayangan, Pasar Jambi districts and K1 and K4 lowspots with the same pattern in the Southwest, Southeast, and South directions covering Alam Barajo and Kota Baru districts. Knowing the pattern of distribution of antenatal service coverage (K1 and K4) can determine the priority areas that need to be improved antenatal service coverage in Jambi Municipality.

Abstract

Indonesia ranks as the second largest country in terms of the number of tuberculosis (TB) cases. It is estimated that approximately 33% of TB cases remain undiagnosed and unreported. Identifying a health-seeking behavior including diagnostic delay in Jambi Municipality is necessary to increase tuberculosis (TB) case finding. This study aims to identify health-seeking behavior, calculate diagnostic delay, and identify factors associated with diagnostic delay, namely individual, social, and structural aspects among TB cases. This study used a cross-sectional design among bacteriologically confirmed pulmonary TB cases with a new treatment history in Jambi Municipality. The number of participating samples was 135 people with multi-stage random sampling. This study revealed that the most common health-seeking behaviors among TB cases were visiting drug stores/pharmacies (20.7%) and community health centers (CHCs) (20.0%). Diagnostic delay was experienced by 75.7% of TB cases with a median of 48 days and an interquartile range of 22-107 days. Variables associated with diagnostic delay were being unmarried (aOR 0.3, 95%CI 0.1-0.9, p = 0.031), moderate social support (aOR 0.2, 95%CI 0.1-0.6, p = 0.004), visit drugstore/pharmacy after symptom onset (aOR 8, 2, 95%CI 1.6-42.3, p = 0.012), the number of health facility visits made before diagnosis was 2-6 (aOR 3.1, 95%CI 1.1-9.3, p = 0.039), and TB stigma from the community perspective was high (aOR 0.3, 95%CI 0.1-0.8, p = 0.019). Evaluation of public-private mix (PPM) activities, especially those involving pharmacies/drug stores and private health facilities, and evaluation of TB screening at CHCs need to be considered. Massive education, community involvement, and psychosocial support are also needed to prevent delays in TB diagnosis.
