Abstract

Background: The main target of human immunodeficiency virus (HIV) is cluster of differentiation 4 (CD4) T lymphocytes and several other immune cells that have CD4 receptors. They are also present in skin and mucosa, such as Langerhans cells (LC). Mucocutaneous lesions are one of the first clinical presentations of immunosuppression in HIV seropositive patients that manifest at different stages of the infection and require early diagnosis and prompt treatment. Purpose: To determine the clinical characteristics and the pattern of various mucocutaneous manifestations in Human immunodeficiency virus/Acquired immune deficiency syndrome (HIV/AIDS) patients at Intermediate Care and Infectious Diseases Dr. Soetomo General Academic Hospital Surabaya. Methods: This is a descriptive retrospective study with a cross-sectional design. The study subject was classified as all HIV-AIDS patients with mucocutaneous manifestations treated in Intermediate Care and Infectious Diseases Dr. Soetomo General Academic Hospital Surabaya in 2019. Result: Out of the 614 patients who participated in the study, 72.1% were males. The majority of patients were in the age group 25–49 years (75.4%). The most common risk factor was heterosexuality (41.7%). Based on the distribution of mucocutaneous manifestations, the most common mucocutaneous manifestation was candidiasis mucocutan 387 patients (49.4%) followed by the pruritic papular eruption (PPE) 118 patients (15.1%) and human papillomavirus infection 57 patients (7.3%). Conclusion: Mucocutaneous manifestations occur throughout the course of HIV infection, and they can be considered as good clinical indicators for the progression of the disease and underlying immune status in resource-poor settings.

Abstract

Background: Skin prick test (SPT) is indicated in type I hypersensitivity reactions (rapid type), among which is Atopic Dermatitis (AD). However, the results of SPT for the tested allergens are not always accompanied by history of known allergens in AD patients. Purpose: To identify the characteristics of AD patients who underwent SPT. Methods: This was a descriptive retrospective study to determine the demographics, allergy history, SPT results, and positive and negative clinical relevance in AD patients treated at Allergy Immunology Division of Dermatology and Venerology Outpatient Clinic Dr. Soetomo General Academic Hospital Surabaya-Indonesia from January 2017 to December 2019. Results: A total of 284 AD patients were included. The adult age group (20-59 years) predominated the AD patient's visit every year with 58 patients (60.42%), 47 patients (46.08%), and 65 patients (75.58%) in 2017, 2018, and 2019, respectively. Only 60 AD patients underwent SPT, where most patients were allergic to dust with 30 patients (50.0%). House dust mite (HDM) allergen had the highest percentage of positive SPT result with 41 patients (63.3%). The greatest clinical relevance was found in cat fur allergen, where all patients (100.0%) had clinical relevance to their SPT result. Conclusion: This study shows that most AD patients were in adult age group and had a history of dust allergy. The highest percentage of positive SPT result was found in HDM allergen and the greatest clinical relevance was found in cat fur allergen.

Abstract

Background: Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae that affects peripheral nerves and skin. The interaction between M. leprae and Schwann cells causes irreversible damage to peripheral nervous tissue afterward disability occurs. Involvement of the major nervous trunks of the extremities can produce sensory-motor deficits. The risk factors are delay in diagnosis, gender, age, type of leprosy, duration of disease, number of affected nerves, leprosy reaction, type of treatment, socioeconomic factors, education, ethnicity, and occupation. Purpose: The aim was to study the profile of leprosy patients with disability who seek treatment in Leprosy Division Dermatovenerology Outpatient Clinic RSUD Dr. Soetomo Surabaya. Methods: This was a descriptive retrospective study of leprosy patients with disability from January 2017 to December 2019. Result: The results of this study obtained a total of 275 leprosy patients with disabilities, which consisted of 76 patients (27.6%) with grade-1 disability and 199 patients (72.4%) with grade-2 disability. The majority were male (73.4%) and aged 25-44 years old (42.2%). Other dominant risk factors were MB type leprosy (92%), duration of disease more than 12 months (52.3%), no leprosy reaction (68%), and had received multidrug therapy (45.5%). Conclusion: Knowledge of disability risk factors can assist in improving management and education to prevent disability in leprosy patients.

Abstract

Background: Verruca (non genital warts) is a quite common public health problem caused by viral infection of low risk Human Papilloma Virus (HPV) types, risking patients of all backgrounds. There is still no ideal treatment for verruca with little knowledge regarding the effectiveness of the available therapies, which results in the lack of therapeutic standard for patients. Purpose: To evaluate the incidence, management, and effectiveness of verruca treatment. Methods: This was a descriptive, retrospective study with a cross-sectional design.  All patients diagnosed with verruca in Tumor and Skin Surgery Division of the Dermatology and Venereology Outpatient Unit at Dr. Soetomo General Academic Hospital Surabaya between January 2016 and December 2020 were included in the study. Result: The results of this study obtained a total of 113 verruca patients, consisting of 61 (53.98%) male patients and 52 (46.01%) female patients, mostly aged 20-60 years old (45 patients [39.82%]). The upper extremity was the most common location where the lesion was found (39 patients [34.5%]). Electrosurgery was the most performed method (38 patients [33.6%]). Thirty-one patients who were treated and returned to the clinic at least once during the month were evaluated for the resolution of the lesion after undergoing therapy. Among 31 patients, resolution was reported in 35.5% of cases. Conclusion: Verruca is a common condition in general practice with many treatment options available yet often neglected. Knowing the available evidence for efficacy of those treatments is critical for dermatologist when considering treatment and educate patients.

Abstract

Background: Vitiligo is a depigmentation disorder characterized by the progressive loss of melanocytes from the epidermis and hair follicles. Vitiligo affects all age groups and is defined by acquired, idiopathic, progressive, different-sized, circumscribed, milky white depigmentation. Characteristics and clinical patterns of vitiligo will also help to understand the pathogenesis of the disease. Purpose: The aim of this study is to evaluate the characteristics profile and clinical features of vitiligo patients. Methods: The research material was taken from the medical record of vitiligo patients in the Division of Cosmetics Outpatient Clinic Dermatovenerelogy Dr. Soetomo General Hospital Surabaya from 2018-2020. Result: This study was obtained by a number of new vitiligo patients 115 (0.3%) of the total number of new outpatient clinic visits of Dermatovenereology Dr. Soetomo General Hospital Surabaya. The majority of the gender is female (53.9%), late adolescence (26.96%), students (25.22%), and the onset of vitiligo 1-5 years (30.43%). The most common risk factors were idiopathic (51.3%) and clinical types are non-segmental vitiligo (53.04%). Conclusion: Vitiligo predominantly affects in females with active age groups of 17-25 years old, and students.

Abstract

Background: Acne vulgaris (AV) often occurs at the age of puberty, most people with AV feel ashamed of their appearance. It has been believed that green tea contains high level antioxidant, such as Epigallocatechin-3-gallate (EGCG). Purpose: This study is to compare quality of life from the patient of acne vulgaris before and after using 3% Epigallocatechin-3-Gallate (EGCG) cream as adjuvant therapy of 0,025% tretinoin cream with 0,025% tretinoin cream alone in the treatment of AV. Methods: This study is a quasi-experimental study with a pre-test and post-test design, and a control group design aiming to know the difference in quality of life of 46 samples after administration of EGCG cream combined with tretinoin cream compared to the group that was given tretinoin cream alone in patients with AV for 12 weeks, and then evaluate patients satisfaction and change in quality of life after treatment using the Dermatology Life Quality Index (DLQI). Result: Statistical analysis found that there was a significant difference (p-value <0.05) in the quality-of-life value of research subjects in the treatment group when compared between the initial arrival to week 12 with a p-value of 0.000. Distribution of DLQI values in research subjects who were given 3% EGCG cream therapy as adjuvant therapy with 0.025% tretinoin cream. Conclusion: Both groups showed an improvement in the DLQI value with the interpretation of the DLQI value at week 12 being both 100%, but the treatment groups DLQI decline faster than the control groups.

Abstract

Background: Vitiligo is a long-term autoimmune disorder marked by the loss of melanocytes and the formation of white macules. Vitiligo can have an impact on the patient's look and quality of life. The Dermatology Life Quality Index (DLQI) is a questionnaire that analyzes the quality of life of skin patients in general, but it is not disease-specific. To assess the quality of life of vitiligo patients in Indonesia, a disease-specific instrument is required. Purpose: This study aims to analyze the validity and reliability of the Vitiligo Quality of Life (VitiQol) questionnaire in Indonesia and translate the VitiQol questionnaire into Indonesian. Methods: Using cross-sectional analytic observation, this study was performed on 39 participants with vitiligo at Dr. Soetomo Surabaya between December 2021 and January 2022. The investigation consisted of three phases: the Indonesian translation of VitiQol, testing for validity, and assessing reliability. Result: The VitiQol questionnaire is a valid and reliable questionnaire. Valid with a correlation coefficient between 0.40 and 0.80 and reliability close to perfection. There was a significant correlation between VitiQol and DLQI (r=0.511, p=0.001). Conclusion: According to the findings of this study, the Indonesian VitiQol is a valid and reliable instrument for measuring the life quality of vitiligo patients in Indonesia.

Abstract

Background: Superficial fungal foot infection is an infectious disease caused by dermatophyte, Candida spp., and mold. Risk factors are thought due to poor personal hygiene, frequent used of closed shoes, and high physical activity. Purpose: Investigate the profile and the risk factors of superficial fungal foot infections in military students. Methods: This is a cross-sectional study. Inclusion criteria for this study were navy military students with lesions on their feet. Result: This study involved 194 students as the populations with 97 students as the sample of the study. The average age were 17-25 years old, and male students being the major. Erythema and scale were found in the plantar and interdigital pedis. Students with good personal hygiene were 81 students among 97 students (83,5%). Wearing closed shoes for > 8 hours were found in 72 students among 97 students (74.2%). Most of the students exhibit a high physical activity with an average score of 26,880 MET minutes/week. Positive KOH 20% and cultures were found in 30 students (30.9%). Negative KOH 20% and positive culture were found in 27 students (27.8%). Positive KOH 20% and negative cultures were found in 9 students (9.3%). Negative KOH 20% and negative fungal culture results were found in 31 students (31,9%). Mold was the predominant fungal type. Conclusion: The students had good personal hygiene. The average wears closed shoes for >8 hours. Mold was the predominant fungal type in this study.

Abstract

Stunting adalah masalah gizi kronis yang ditandai dengan pertumbuhan fisik anak yang tidak sesuai dengan standar pertumbuhan anak seusianya. Terdapat berbagai faktor yang berkontribusi terhadap meningkatnya prevalensi stunting, antara lain: kurangnya pengetahuan bagi calon pengantin dan ibu hamil tentang pentingnya asupan gizi yang baik dan ketersediaan makanan serta minuman berkualitas dan jumlah yang memadai bagi masyarakat. Salah satu kendala utama dalam mengatasi stunting adalah keterbatasan dukungan dari pemerintah, baik dari segi pengetahuan maupun dana. Oleh karena itu, pengabdian masyarakat ini dirancang untuk mendukung upaya pemerintah desa dalam mencegah stunting. Berdasarkan hasil diskusi dengan aparatur desa, terungkap adanya kebutuhan signifikan atas bantuan dari pemerintah pusat. Dalam program ini, metode yang digunakan meliputi sosialisasi mengenai stunting melalui pertemuan tatap muka dan diskusi yang berfokus pada topik-topik krusial terkait stunting, yang kemudian disajikan kepada perwakilan pemerintah desa yang hadir

Abstract

Kemunculan pandemi COVID-19 berdampak pada kesehatan emosional mental dan krisis nutrisi pada 80 juta anak di Indonesia. Pemantauan tumbuh kembang anak harus tetap dilakukan dalam masa pandemi. Teknologi informasi dapat dimanfaatkan untuk membuat proses pencatatan dan pelaporan tumbuh kembang anak dengan efektif dan efisien, yang sebelumnya dilakukan secara manual menggunakan kertas pada praktiknya. Penelitian ini bertujuan mengembangkan model konsep perancangan sistem informasi tumbuh kembang anak berbasis less paper untuk membuat pencatatan, pelaporan, serta pemantauan kesehatan anak balita efektif dan efisien. Metode yang digunakan adalah literature review dan system development yang berfokus pada desain konseptual dari mobile parenting. Hasil penelitian adalah bentuk model sistem informasi yang menjelaskan gambaran konsep perencanaan sistem pemantauan tumbuh kembang anak balita. Aplikasi mobile parenting dirancang akan memiliki tiga fitur utama yaitu pengkajian pertumbuhan, pengkajian perkembangan, dan catatan harian anak (daily report). Sistem ini dapat digunakan oleh orang tua dalam memantau tumbuh kembang anak secara mobile, dan juga untuk pengasuh daycare dalam pencatatan hasil pengkajian secara langsung ke dalam aplikasi. Konsep ini direkomendasikan untuk selanjutnya dapat dikembangkan ke dalam prototype.
