Abstract

As a sector playing a crucial role in ship construction and repair, the shipbuilding industry faces significant challenges in material procurement, where materials constitute 50-70% of the total project cost. One of the materials requiring paramount attention is the ship's main engine. Effective supply chain management is critical to enhancing efficiency and effectiveness in the procurement process of ship engines. However, supply chain performance measurement research, especially in ship engine procurement, still needs to be improved. Therefore, this study aims to identify relevant performance metrics, analyze critical performance indicators, and provide recommendations based on the SCOR (Supply et al.) framework to improve ship engine procurement. The research results present 24 metrics distributed into four performance attributes based on SCOR 12.0, with 22 selected metrics aligned with the company's context. Analysis using the AHP method indicates that the reliability attribute has the highest weight (0.296), with the metric "Percentage of procurement received on time as per request" being the top priority (weight 0.143). This study provides a holistic view of crucial aspects of the ship engine procurement supply chain, supporting companies in enhancing their performance in the procurement process.

Abstract

The rapid epidemic growth of HIV is continuing in Indonesia. There are some factors which have influenced the spreading of this epidemic in Indonesia, such as the poor awareness to avoid unsafe free sex attitude and the sharing of needles and syringes among intravenous drug users (IDUs). The sexual transmission of HIV has also apparently increased in Tulungagung. Commercial sex workers play a significant role in the spread of HIV in Tulungagung. People in Tulungagung have worked at other countries as Indonesian migrants. This condition can cause the increase number of HIV-1 case and the possibility of genetic variation (subtype) HIV-1 in Tulungagung. This research is aimed to analyze the subtype and to determine estimation of transmission mode on infected patient of HIV-1 and AIDS who came to Seruni clinic Dr. Iskak hospital in Tulungagung. 40 HIV?AIDSpatients were interviewed to determine the subtype and the transmission mode. The results showed that 14 of 40 plasma samples (35%) were successfully to amplified and sequenced. OverallCRF01-AE wereidentified as predominant subtype among HIV/AIDS patients in Tulungagung. Based on individual information, 31 of 40 subjects (77%) were heterosexual transmission.

Abstract

Malaria remains a health problem in Indonesia. West Kalimantan is a malaria endemic area with high and lowincidence. Landak District is one of the malaria endemic area. Malaria cases were found in the areas around illegalgold mining and oil palm plantations. The aims of this study were to describe the characteristics of the breeding sitesand species of Anopheles larvae found in high malaria cases area, namely Amboyo Utara Village and low clinicalmalaria cases, area namely Mandor Village. This research is a descriptive research with cross sectional design. Thesamples were Anopheles larvae collected with Accidental sampling technique in the breeding sites. Environmentalcharacterization of breeding sites were physical characteristic including water temperature and sun exposure,chemical characteristic including water pH and salinity, and biological characteristics including water biota. Theresults of this study ware environmental characteristics that have the potential to breed Anopheles mosquitoes inAmboyo Utara Village, including water temperature 26-30ï‚°C, shandy, water pH 5.0-7.6, salinity 0.2-1.0 ppt, biotaswater hyacinth, grass and tadpole. The Mandor village, water temperature 29-30 ºC, shandy, pH of 6.9-8.0, salinity of0.5 ppt, water biota grass. Anopheles species found in Amboyo Utara village were larvae of An. vagus (94.30%), An.tessellatus (3.42%), An. subpictus (1.62%), An. indefinitus (0.81%) and An. maculatus (0.81%). Characteristics ofbreeding sites in Mandor village were larvae of An. maculatus (11.11%), An. subpictus (3.70%), and An. vagus(85.18%). The conclusion of this study was that di erent species found at breeding sites with di erent environmentalcharacteristics in both high and low malaria areas in Landak District, West Kalimantan Province.

Abstract

COVID-19 is an infectious disease as a result of a kind of corona virus. COVID-19 is now a pandemic affecting many countries. This study aims to know the effectiveness of booster vaccines to reduce the severity of illness, confirm infection, hospitalization, death in humans infected with COVID-19. For Specific purpose, to analyze the severity of COVID-19 disease in humans by booster and without booster. The design of this study was a systematic review and meta-analysis based on observational studies, published in databases such as Pubmed, Embase, MedRxiv, Nature and Scopus. In the search for articles, the limitations of 2021 to 2022 are used. This research was analyzed quantitatively through the Review Manager 5.4.1 program. Study was taken from 13 journals that met the criteria for a meta-analysis. With the population aged over 18 years, and using the type of vaccine BNT162b2 or mRNA. The population of this study came from Israel, Italy, England, Qatar, Brazil, Turkey, Puerto-Rico, Northern Bangkok, Vicinities and Thailand. Significant results were obtained for each outcome. The OR values of BNT162b2 booster vaccine against confirmed infection OR 0.16 (95% CI 0.06 – 0.45), against symptomatic disease 0.22 (95% CI 0.11 – 0.44), against asymptomatic disease OR 0.72 (95% CI 0.69 – 0.74), against hospitalization OR 0.12 (95% CI 0.06 – 0.22), against severe disease OR 0.15 (95% CI 0.07 – 0.33), and against death OR 0.10 (95% CI 0.04 – 0.31). Administration booster vaccines is effective in reducing infection rates, disease severity, and deaths from COVID-19.

Abstract

Background: Psoriasis is a skin disease that occurs due to proliferation and differentiation disturbances of the epidermis. This chronic condition often reduces the human quality of life. There have been studies investigating psoriatic patient profiles and treatments, but few are conducted in Surabaya. Hence, those studies cannot fully represent the population demographic in Surabaya. Purpose: To investigate the prevalence of psoriasis vulgaris and the sociodemographic profile of the patients in Dr Soetomo General Academic Hospital. Methods: This was a Retrospective study psoriasis vulgaris patients in January 2016–December 2018. The data were collected by retrieving Electronic Medical Records (EMR) as secondary data. Result: The psoriasis vulgaris prevalence of outpatients was 0.46% and 4.59% in inpatients. Most study subjects were adults aged 18 and above (98.1%), most of the patients were obese (46.3%), the most common provoking factor was focal tooth infection (33.8%) and stress (28.8%), and none reported family history of psoriasis. Conclusion: Some results are similar to other studies conducted in Surabaya, which were the obesity and Type 2 Diabetes Mellitus (DM) being strong a factor to psoriasis development. However, the result on psoriasis family history was not in line with other studies.

Abstract

Background: Leprosy is a neglected tropical disease caused by chronic granulomatous infection of Mycobacterium leprae. Indonesia ranks third in new case findings, with 84% of the case being multibacillary (MB) leprosy. MB leprosy cases have a higher risk of leprosy reactions and physical disabilities that decrease quality of life. Purpose: To determine the demographic, clinical characteristics, and white blood analysis of newly diagnosed MB leprosy patients, especially concerning leprosy reactions. Methods: This is a descriptive retrospective study with a cross-sectional design that describe the following data: domicile, gender, age, treatment status, disabilities, body mass index (BMI); bacterial index (BI), morphological index (MI), white blood cell (WBC) and differential counts, and thrombocyte count. Result: This study included 176 adult MB cases, predominantly male aged 20–39 years old with average BMI, lived in Surabaya with negative history of multi-drug therapy, disability, BI, nor MI. The grade 2 disability (G2D) percentage in this study setting than in Indonesia (10.7% vs. 6.43%). The WBCs, especially neutrophil count, was higher in T2R group. Monocyte and lymphocyte counts were relatively similar. There was an increase in thrombocyte count in leprosy reaction groups. Conclusion: MB leprosy in the endemic area, which is more commonly found in productive-aged male, displayed higher G2D than global Indonesia population. Thus denotes the importance of active case findings. The difference in blood analysis characteristics between MB leprosy with and without reactions may serve as the foundation for future study.

Abstract

Background: The main target of human immunodeficiency virus (HIV) is cluster of differentiation 4 (CD4) T lymphocytes and several other immune cells that have CD4 receptors. They are also present in skin and mucosa, such as Langerhans cells (LC). Mucocutaneous lesions are one of the first clinical presentations of immunosuppression in HIV seropositive patients that manifest at different stages of the infection and require early diagnosis and prompt treatment. Purpose: To determine the clinical characteristics and the pattern of various mucocutaneous manifestations in Human immunodeficiency virus/Acquired immune deficiency syndrome (HIV/AIDS) patients at Intermediate Care and Infectious Diseases Dr. Soetomo General Academic Hospital Surabaya. Methods: This is a descriptive retrospective study with a cross-sectional design. The study subject was classified as all HIV-AIDS patients with mucocutaneous manifestations treated in Intermediate Care and Infectious Diseases Dr. Soetomo General Academic Hospital Surabaya in 2019. Result: Out of the 614 patients who participated in the study, 72.1% were males. The majority of patients were in the age group 25–49 years (75.4%). The most common risk factor was heterosexuality (41.7%). Based on the distribution of mucocutaneous manifestations, the most common mucocutaneous manifestation was candidiasis mucocutan 387 patients (49.4%) followed by the pruritic papular eruption (PPE) 118 patients (15.1%) and human papillomavirus infection 57 patients (7.3%). Conclusion: Mucocutaneous manifestations occur throughout the course of HIV infection, and they can be considered as good clinical indicators for the progression of the disease and underlying immune status in resource-poor settings.

Abstract

Background: Skin prick test (SPT) is indicated in type I hypersensitivity reactions (rapid type), among which is Atopic Dermatitis (AD). However, the results of SPT for the tested allergens are not always accompanied by history of known allergens in AD patients. Purpose: To identify the characteristics of AD patients who underwent SPT. Methods: This was a descriptive retrospective study to determine the demographics, allergy history, SPT results, and positive and negative clinical relevance in AD patients treated at Allergy Immunology Division of Dermatology and Venerology Outpatient Clinic Dr. Soetomo General Academic Hospital Surabaya-Indonesia from January 2017 to December 2019. Results: A total of 284 AD patients were included. The adult age group (20-59 years) predominated the AD patient's visit every year with 58 patients (60.42%), 47 patients (46.08%), and 65 patients (75.58%) in 2017, 2018, and 2019, respectively. Only 60 AD patients underwent SPT, where most patients were allergic to dust with 30 patients (50.0%). House dust mite (HDM) allergen had the highest percentage of positive SPT result with 41 patients (63.3%). The greatest clinical relevance was found in cat fur allergen, where all patients (100.0%) had clinical relevance to their SPT result. Conclusion: This study shows that most AD patients were in adult age group and had a history of dust allergy. The highest percentage of positive SPT result was found in HDM allergen and the greatest clinical relevance was found in cat fur allergen.

Abstract

Background: Leprosy is a chronic granulomatous infectious disease caused by Mycobacterium leprae that affects peripheral nerves and skin. The interaction between M. leprae and Schwann cells causes irreversible damage to peripheral nervous tissue afterward disability occurs. Involvement of the major nervous trunks of the extremities can produce sensory-motor deficits. The risk factors are delay in diagnosis, gender, age, type of leprosy, duration of disease, number of affected nerves, leprosy reaction, type of treatment, socioeconomic factors, education, ethnicity, and occupation. Purpose: The aim was to study the profile of leprosy patients with disability who seek treatment in Leprosy Division Dermatovenerology Outpatient Clinic RSUD Dr. Soetomo Surabaya. Methods: This was a descriptive retrospective study of leprosy patients with disability from January 2017 to December 2019. Result: The results of this study obtained a total of 275 leprosy patients with disabilities, which consisted of 76 patients (27.6%) with grade-1 disability and 199 patients (72.4%) with grade-2 disability. The majority were male (73.4%) and aged 25-44 years old (42.2%). Other dominant risk factors were MB type leprosy (92%), duration of disease more than 12 months (52.3%), no leprosy reaction (68%), and had received multidrug therapy (45.5%). Conclusion: Knowledge of disability risk factors can assist in improving management and education to prevent disability in leprosy patients.

Abstract

Background: Verruca (non genital warts) is a quite common public health problem caused by viral infection of low risk Human Papilloma Virus (HPV) types, risking patients of all backgrounds. There is still no ideal treatment for verruca with little knowledge regarding the effectiveness of the available therapies, which results in the lack of therapeutic standard for patients. Purpose: To evaluate the incidence, management, and effectiveness of verruca treatment. Methods: This was a descriptive, retrospective study with a cross-sectional design.  All patients diagnosed with verruca in Tumor and Skin Surgery Division of the Dermatology and Venereology Outpatient Unit at Dr. Soetomo General Academic Hospital Surabaya between January 2016 and December 2020 were included in the study. Result: The results of this study obtained a total of 113 verruca patients, consisting of 61 (53.98%) male patients and 52 (46.01%) female patients, mostly aged 20-60 years old (45 patients [39.82%]). The upper extremity was the most common location where the lesion was found (39 patients [34.5%]). Electrosurgery was the most performed method (38 patients [33.6%]). Thirty-one patients who were treated and returned to the clinic at least once during the month were evaluated for the resolution of the lesion after undergoing therapy. Among 31 patients, resolution was reported in 35.5% of cases. Conclusion: Verruca is a common condition in general practice with many treatment options available yet often neglected. Knowing the available evidence for efficacy of those treatments is critical for dermatologist when considering treatment and educate patients.
