Abstract

Acute ischemic stroke is frequently accompanied by stress-related (reactive) hyperglycemia and may involve coagulation activation and endothelial dysfunction, reflected by D-dimer and asymmetric dimethylarginine (ADMA) levels, respectively. These factors may influence clinical outcomes. The aim of this study was to evaluate the associations of reactive hyperglycemia, D-dimer, and plasma ADMA with stroke outcomes in patients with acute ischemic stroke. A cross-sectional study was conducted among patients with acute ischemic stroke admitted to the neurology ward and stroke unit of Dr. Zainoel Abidin Hospital, Banda Aceh, Indonesia, between May and September 2024. Functional stroke outcome (FSO) was assessed using the Barthel Index, and disability stroke outcome (DSO) was assessed using the modified Rankin Scale (mRS). A total of 123 patients were included. The findings indicate that mean admission blood glucose was significantly higher in the improved FSO group than in the unchanged-worsened group (p=0.004), whereas mean blood glucose did not differ significantly across DSO categories (p=0.194). Mean D-dimer was significantly higher in the unchanged-worsened FSO group than in the improved group (538.6±249.4 vs 398.4±128.5 ng/mL; p<0.001). Across DSO categories, D-dimer showed a significant difference only between the no-disability and moderate-disability groups (p=0.044), without a consistent graded pattern. Mean ADMA levels were not significantly different between FSO groups (p=0.136), but it was statistically significant between DSO categories (slight vs moderate disability), p=0.045. The present analysis indicated that elevated D-dimer, dyslipidemia, heart disease, and GCS were significantly associated with FSO. Systolic blood pressure, diastolic blood pressure, reactive hyperglycemia, and hypertension were significantly associated with DSO severity. Overall, elevated D-dimer was more strongly associated with Barthel Index-based functional outcome, whereas reactive hyperglycemia was associated with mRS-based disability outcome.

Abstract

Acute ischemic stroke (AIS) is a leading cause of morbidity and mortality, with cerebral artery stenosis serving as an important prognostic factor. While revascularization therapies benefit selected patients, most rely on pharmacological strategies. However, evidence regarding the effect of sequential anticoagulant–antiplatelet therapy on vascular stenosis and neurological outcomes remains limited. The aim of this study was to evaluate changes in cerebral artery stenosis, assessed using digital subtraction angiography (DSA), and neurological deficits, assessed by the National Institutes of Health Stroke Scale (NIHSS), in patients with first-onset AIS treated with anticoagulant and antiplatelet therapy. A prospective cohort study was conducted involving 35 patients who received low-molecular-weight heparin or warfarin for seven days, followed by 90 days of oral antiplatelet therapy (aspirin or clopidogrel). Sixteen patients consented to repeat DSA at 90 days. Among these, the median stenosis decreased from 44.5% (30–90%) to 44.0% (20–90%) (p=0.003). In the full cohort (n=35), the median NIHSS improved from 10 (5–17) at baseline to 9 (2–14) at 90 days (p<0.001). Correlation analysis demonstrated a positive but non-significant association between stenosis reduction and NIHSS improvement (r=0.474, p=0.064). These findings suggest that sequential anticoagulant–antiplatelet therapy in first-onset AIS was associated with a modest but statistically significant reduction in arterial stenosis and meaningful improvement in neurological function. Although vascular and clinical outcomes were not significantly correlated, the observed trend highlights the importance of structured pharmacological therapy and the potential role of serial vascular imaging in follow-up care.

Abstract

Neutrophil-to-lymphocyte ratio (NLR), an accessible biomarker derived from routine blood counts, has been associated with stroke severity and outcomes. However, its association with angiographically confirmed stenosis has not been fully established. The aim of this study was to investigate the correlation and association between the NLR and the severity of intracranial arterial stenosis assessed by digital subtraction angiography (DSA). An observational analytic study with a cross-sectional design was conducted. Patients with acute ischemic stroke who underwent DSA were included using a total sampling method. Eligible patients were aged >18 years, had ischemic stroke onset <14 days, and had no prior thrombolysis, endovascular treatment, or surgery for cerebral stenosis. Those with infection, hematological disorders, malignancy, or immunosuppression were excluded. NLR was calculated from complete blood count results at admission, while stenosis severity was quantified using the Warfarin–Aspirin Symptomatic Intracranial Disease (WASID) method. A total of 44 ischemic stroke patients who underwent DSA were included. Pearson correlation test revealed a strong positive correlation between NLR and the severity of intracranial arterial stenosis (r=0.671; p<0.001). In subgroup analysis, NLR showed a strong positive correlation with stenosis severity in the cerebral arteries (r=0.707; p<0.001), but not in the carotid arteries (r=0.434; p=0.182). One-way ANOVA revealed significant differences in NLR across stenosis severity groups (p<0.0001), with higher NLR in moderate and severe stenosis compared with mild stenosis (p=0.017 and p=0.0003, respectively). These findings suggest that NLR reflects the inflammatory burden contributing to vascular narrowing and may serve as a simple and widely available biomarker for identifying ischemic stroke patients with a higher burden of intracranial arterial stenosis, particularly in settings where access to advanced imaging is limited.

Abstract

Stroke is a leading cause of death and disability worldwide, and mortality in acute intracerebral hemorrhagic (ICH) stroke is influenced by many factors, and early identification of high-risk patients is crucial for guiding clinical management. This study aimed to evaluate the role of blood pressure, blood glucose level, and Glasgow coma scale (GCS) on admission as predictors of 10-day in-hospital mortality and to develop a predictive scoring system in patients with acute ICH stroke. A cross-sectional study was conducted at Dr. Zainoel Abidin Hospital, a provincial referral hospital in Banda Aceh, Indonesia, in 2025. Patients with acute ICH were consecutively recruited. Clinical parameters on admission, including systolic and diastolic blood pressure, random blood glucose level, and GCS, were recorded. Associations with 10-day mortality were assessed with a Chi-squared test, and a predictive scoring system was developed based on independent predictors. A total of 62 patients were included in this study. Higher systolic blood pressure (≥140 mmHg), diastolic blood pressure (≥90 mmHg) and GCS <9 on admission were significantly associated with 10-day mortality (p=0.031, p=0.023 and p<0.001, respectively). Multivariate analysis identified that GCS <9 was the only independent predictor. A predictive scoring system assigned 8 points for GCS <9, 5 points for systolic ≥140 mmHg, 4 points for diastolic ≥90 mmHg, and 1 point for random blood glucose ≥200 mg/dL, estimating patient-specific mortality risk, highest when all risk factors were present. This study indicates that GCS <9 and elevated blood pressure on hospital admission are key predictors of 10-day mortality in acute ICH. The developed scoring system may assist in early risk stratification and management, and further exploration of predictive models is warranted to optimize clinical outcomes.

Abstract

Indonesia, as a unitary state, adheres to a government system based on the principle of decentralization as mandated in the 1945 Constitution of the Republic of Indonesia. Through decentralization, the state grants autonomy to regions to regulate and manage certain governmental affairs, including the authority to create regional legal products as instruments for regulating regional autonomy. This authority is constitutionally legitimized in Article 18, paragraph (6) of the 1945 Constitution; however, it is not absolute, as it must remain within the framework of the Unitary State of the Republic of Indonesia and the national legal system, which is hierarchical. This research is normative legal research using a statutory, conceptual, and historical approach. The study analyzes the position and limits of the authority of autonomous regions in the formation of regional legal products, as well as the juridical problems that arise in practice. The results of the study indicate that regional legislative authority is a legitimate attribution of authority constitutionally, but it is limited by the principles of the unitary state, the hierarchy of legal regulations, the mechanisms of supervision and guidance from the central government. In practice, the formation of regional legal products still faces juridical problems, such as discrepancies between the material content and regional authority, the weak application of the principles of good legislative process, and the duality of supervision authority and harmonization between the Governor as the representative of the central government in the region and the Regional Office of the Ministry of Law and Human Rights.

Abstract

The COVID-19 pandemic has caused significant disruption to the global education system, particularly in STEM education, which requires direct interaction and experimental practice. This study aims to examine the phenomena of learning loss and the digital divide in post-pandemic STEM education and to identify recovery strategies. The method employed is a systematic literature review (SLR) using the PRISMA approach, focusing on peer-reviewed articles from 2020–2022 retrieved from Scopus, Web of Science, and Google Scholar. The findings indicate that learning loss has occurred significantly, particularly in mathematics, and has been exacerbated by technological access disparities, leading to a widening gap in learning outcomes. The interaction of these two factors gives rise to learning inequality, which has the potential for long-term impacts on the quality of human resources in the STEM field. Therefore, comprehensive recovery strategies are needed through strengthening technology access, adapting competency-based curricula, utilizing educational technology, and implementing targeted interventions for vulnerable groups.

Abstract

This article discusses the development of literary appreciation learning models in higher education, emphasizing systematic, creative, and collaborative approaches. The aim is to identify strategies that enhance students’ interest, comprehension, and engagement in appreciating literary works. The ADDIE model serves as the primary framework, combined with creative-innovative and collaborative methods. Findings suggest that integrating theory, practice, and digital technology into literary appreciation learning significantly improves students’ cognitive, affective, and evaluative abilities.

Abstract

Compensation is one of the key factors in human resource management, as it can influence the level of employee motivation and performance. This study aims to analyze the effect of financial and non-financial compensation on the performance of managers and employees of BUMDESMA in Tanah Datar Regency, with work motivation as a mediating variable. The object of this study includes all managers and employees of BUMDESMA (Joint Village-Owned Enterprises) in Tanah Datar Regency, totaling 60 individuals, who were also used as the research sample. This study employs the Structural Equation Modeling method based on Partial Least Squares (SEM-PLS). The results indicate that financial compensation has a negative and significant effect on the performance of managers and employees of BUMDESMA. Meanwhile, non-financial compensation has a positive and significant effect on performance. Work motivation is also found to have a positive and significant effect on the performance of managers and employees. In addition, both financial and non-financial compensation have a positive and significant effect on work motivation. The findings further reveal that work motivation is able to mediate the effect of both financial and non-financial compensation on the performance of managers and employees of BUMDESMA in Tanah Datar Regency.

Abstract

Gamification has emerged as a transformative pedagogical approach in science and technology education. This narrative review explores current implementation trends and educator perspectives by analyzing 28 selected articles published between 2016 and 2024. The study identifies key technological trends, including the integration of Virtual Reality (VR), game-based physics simulations, and Education 4.0 tools. The findings indicate that gamification significantly enhances student motivation, academic achievement, and engagement in complex scientific subjects. Furthermore, the review highlights educator perspectives, revealing that while teachers generally perceive gamification as a potent methodology, its adoption is often hindered by barriers such as limited digital self-efficacy, technical constraints, and the need for specialized training. The synthesis suggests that a successful gamified ecosystem requires a balance between immersive technology and robust pedagogical support. This review provides valuable insights for educators and policymakers seeking to integrate gamified strategies into STEM curricula to foster 21st-century skills.
