Abstract

Highlights: • Vitamin D supplementations in different doses yield different outcomes. • Multi-day vitamin D administration of 1000-6000 IU in patients with COVID-19 has more positive impacts than a single high dose of vitamin D. • Patient morbidity, length of hospitalization, and patient mortality improved with multi-day vitamin D administration. Abstract: This meta-analysis aimed to determine whether there is any optimal dose of vitamin D for morbidity, length of hospitalization, and mortality in patients with COVID-19. We conducted a comprehensive search in three online databases for eligible studies until February 28, 2022. Odds ratio (OR) and standardized mean difference (SMD) were applied as summary statistics of primary outcomes. The study quality of the literatures collected was assesed using the Cochrane risk of bias tool version 2 (RoB 2). Eight randomized clinical trials (RCT) were included in the study. In our analysis, we found that there was no significant difference in morbidity when vitamin D was administered to COVID-19 patients [OR=0.50 (95% CI=0.13-1.96); SMD=-0.14 (95% CI=-0.55-0.28)]. Duration of hospitalization [SMD=-0.12 (95% CI=-0.39-0.15)] and mortality [OR 0.47 (95% CI=0.19-1.17)] of COVID-19 patients in five studies also showed no significant difference compared to patients who do not take vitamin D. However, when we analyzed two other studies, we found that in patients who did not take vitamin D, mortality was lower [SMD=0.43 (95% CI=0.29, 0.58)]. Compared to a single high dose of vitamin D, the multi-day vitamin D administration of 1000-6000 IU in patients with COVID-19 resulted in improved patient morbidity, length of hospitalization, and patient mortality.

Abstract

Highlights: 1. This study was the first in Gresik, Indonesia, to conduct mass screening of tuberculosis among waste workers, comprising various demographic profiles. 2. Although the tuberculosis screening of the waste workers showed negative results, mass examination using an acid-fast bacilli smear remains crucial to detect latent infections and prevent transmissions. Abstract Tuberculosis is a major source of illness and mortality worldwide. Therefore, tuberculosis screening is important to increase the number of active case-finding in a community. Bacteriological examinations can be used to initiate community-based active case-finding. Waste workers face a high risk of contracting Mycobacterium tuberculosis due to their constant exposure to waste. This study aimed to conduct bacteriological examinations to identify tuberculosis infections among waste workers in Gresik, Indonesia, who were considered a high-risk group. An analytical observational study was carried out throughout September 2023 in the Ngipik subdistrict, Gresik, East Java, Indonesia. The primary study was conducted using a cross-sectional design using interviews, measurements, and sputum examinations for all samples. The screening involved a total of 72 waste workers who had daily contact with various types of sewage and waste, including medical waste. The data were analyzed descriptively because of the constant variable. Following the analysis, the data were presented using a table and a figure. According to the acid-fast bacilli examinations, all of the samples yielded negative results. Nevertheless, this study concludes that it is imperative to carry out mass tuberculosis screening in various settings, specifically among those with a high risk. Mass screening can uncover the potential of latent tuberculosis and, thus, reduce its transmission.

Abstract

Highlights: 1. Comics centered around tuberculosis offer a novel method tailored for specific audiences, specifically children or those of school age, to learn about the disease. 2. The tuberculosis comics feature the etiology, prevention, and treatment of tuberculosis conveyed through their unique design. 3. Comics can be an innovative promotional method to support the preventive campaign against tuberculosis. Abstract As a tropical country, Indonesia continues to grapple with the prevalence of tuberculosis. This study conducted by the Department of Public Health and Preventive Medicine at Universitas Airlangga, Surabaya, Indonesia, presented a novel approach to prevent tuberculosis through measures tailored to the socio-cultural context of the population. Specifically, this study assessed how effective the use of tuberculosis comics is as an educational tool to inform junior high school students about tuberculosis. This research was quasi-experimental, with a one-group pre-test-post-test design. Seventy junior high school students in Dukun District, Gresik, Indonesia participated in this study. Each participant received a questionnaire consisting of ten questions about tuberculosis. Afterward, tuberculosis education was shared through comics. The post-test was carried out using the same questions as the pre-test. The data were analyzed using the R Program for Windows, version 4.1.3 (Auckland University, New Zealand). The analysis revealed a significant difference between the pre-test and post-test results (p < 0.0001). As indicated by the data, comics proved to be an effective method of educating people about infectious diseases, specifically tuberculosis. Here, we introduce an educational tool designed to revitalize the educational method for disseminating knowledge about infectious diseases. Ultimately, comics can increase students' interest in learning about tuberculosis, including its etiology, prevention, and treatment strategies.

Abstract

Milk is a nutrient-rich product produced by dairy cattle and plays an important role in preventing stunting in children under five years of age. In the production of milk-based products for toddlers, the quality of raw materials is a critical factor, particularly in terms of standardization and consumer acceptance. East Java Province is one of the largest milk-producing regions in Indonesia, with potential fresh milk sources originating from Jember, Probolinggo, and Banyuwangi Regencies. This study aimed to determine the best fresh milk raw material from these local sources by integrating chemical quality evaluation and hedonic analysis. A quantitative descriptive method was applied using Lactoscan analysis to assess the chemical composition of fresh milk samples. The results showed that all samples met the Indonesian National Standard (SNI) requirements for fresh milk, indicating comparable chemical quality across regions. Although minor variations in nutritional components were observed, these differences were not decisive in determining overall quality. Based on hedonic analysis, fresh milk from Banyuwangi Regency was identified as the most preferred sample. This finding highlights the novelty of the study, demonstrating that when chemical quality among milk samples is relatively similar, sensory preference becomes the determining factor in selecting the best raw material for toddler-oriented milk products.

Abstract

BACKGROUND: Alzheimer’s disease (AD) and vascular dementia (VaD) impose a substantial public health burden in Indonesia; however, accessible blood-based biomarkers for early screening remain limited. Although cerebrospinal fluid β-amyloid 1–42 is an established biomarker, its invasive nature restricts its use for population-level screening. Therefore, it is necessary to have locally-produced serum β-amyloid 1–42 ELISA kit that is specifically designed for Indonesian elderly population. In this study, a locally-produced β-amyloid 1–42 ELISA kit was validated and used for the screening of AD, VaD and mild cognitive impairment (MCI) Indonesian population.METHODS: A cross-sectional study including 166 subjects: 31 AD, 34 VaD, 34 MCI patients, and 67 cognitively normal controls was conducted. All participants underwent cognitive assessments including Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment-Indonesian version (MoCA-Ina), as well as brain magnetic resonance imaging (MRI) 3-Tesla for the assessment of medial temporal atrophy/white matter changes. Fasting venous blood sampling was taken from each subjects for the measurement of serum β-amyloid 1-42 measurement using locally-produced ELISA kit.RESULTS: Median serum β-amyloid 1–42 levels were 11.03, 10.99, and 10.99 pg/mL for the AD, VaD, and MCI subjects, respectively. The β-amyloid 1–42 levels were correlated with MMSE scores in all group (AD: r=−0.455, p=0.010; VaD: r=−0.419, p=0.014; MCI: r=−0.412, p=0.015). The validity analysis of the locally-produced serum β-amyloid 1–42 ELISA kit, showed sensitivity of 94.12% (95% CI: 87.3–97.9), specificity of 80.36% (95% CI: 72.4–86.8), and diagnostic accuracy of 83.56% (95% CI: 77.2–88.5).CONCLUSION: Serum β-amyloid 1–42 levels are lower in AD and VaD subjects compared to MCI and control subjects. Serum β-amyloid 1-42 is inversely correlated with cognitive function across all groups based on MMSE score. Additionally, the locally-produced β-amyloid 1-42 ELISA kit demonstrated sensitivity of 94.12% and specificity of 80.36%, meeting Global CEO Initiative Consensus for pre-screening tools, supporting its potential as a scalable, non-invasive screening biomarker in Indonesian primary care settings.KEYWORDS: G-banding karyotyping, next generation sequencing, non-invasive prenatal testing

Abstract

Forest and land fires are a critical issue in realizing a disaster-resilient city, particularly in the Integrated Independent City (KTM) Rasau Jaya, which functions as a transmigration center and an emerging agropolitan area. This study analyzes the spatio-temporal patterns of forest and land fires from 2015 to 2024 and their relationship with land use, land tenure status, and disaster vulnerability levels. Hotspot data were obtained from NASA-MODIS satellite imagery via the SiPongi platform and analyzed using overlay (superimpose) methods and a descriptive quantitative approach. The application of the Nearest Neighbor Ratio (NNR) and Kernel Density Estimation (KDE) successfully identified clustered fire patterns in greater depth. A total of 256 hotspots were detected with an NNR value < 1, indicating a clustered spatial pattern and affirming that their distribution is not random but concentrated in vulnerable landscapes such as smallholder plantations, dry fields, and peat-dome areas. The analysis also reveals that areas with unclear land tenure—particularly state land or unregistered parcels—experience the highest fire intensity compared with land under Hak Milik (HM) or Hak Guna Bangunan (HGB), which tend to be safer due to stronger management accountability. This study recommends mitigation strategies through spatial monitoring technologies, community capacity building, and sustainable land-use management to support the transformation of KTM Rasau Jaya into a climate-resilient agropolitan city.
