Abstract

Stunting is a health issue that requires serious attention due to its impact on the growth and development of children. This study investigates the determinants of stunting among children in 514 districts in Indonesia, with a focus on the impact of various socio-economic and environmental factors using secondary data from Badan Pangan Nasional in 2018-2019 and 2022-2023 which was published online. Two separate models were employed for periods before and after the COVID-19 pandemic using multivariate linear regression. For the period before the COVID-19 pandemic (2018-2019), the model reveals significant associations between stunting and several variables including life expectancy, food expenditure, nutritional consumption per capita, and healthcare workforce ratio. However, the impact of some factors such as the duration of female schooling, and lack of clean water, and electricity was not statistically significant. In contrast, the model for the period after the COVID-19 pandemic (2022-2023) shows a different pattern of associations. Here, stunting is significantly correlated with life expectancy, duration of female schooling, nutritional consumption per capita, healthcare workforce ratio, and access to clean water, while factors such as poverty, food expenditure, and access to electricity show weaker or statistically insignificant associations. These findings highlight the evolving nature of factors contributing to childhood stunting, suggesting the need for tailored interventions addressing socio-economic and environmental determinants to combat this pressing public health issue effectively.

Abstract

Highlights: ABI was not influenced by diabetic status due to potential confounders. A more severe form of diabetic ulcer was associated with a lower ABI value.   Abstract Introduction: Diabetic foot ulcer (DFU) is one of the most significant complications of uncontrolled type 2 diabetes mellitus (T2DM) that may affect a patient's prognosis and quality of life. This study aimed to identify the association between diabetic status, DFU severity, and other clinical factors with ankle-brachial index (ABI) score and category. Methods: This was a cross-sectional study under a consecutive sampling frame conducted from 1 November 2022 to 31 January 2023 at Universitas Airlangga Hospital, Surabaya. Primary data for ABl were measured from T2DM patients with DFU in the Thoracic and Cardiovascular Polyclinic. Meanwhile, secondary clinical data were collected from the Department of Internal Medicine based on the inclusion and exclusion criteria specified in the patient's medical records. The International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 26 was utilized for the statistical analysis. Results: Out of 30 included patients, 63.33% had uncontrolled diabetic status, and the average ABI score was 0.999 ± 0.19. DFU severity was significantly associated with the ABI score and ABI category. Patients indicated with angioplasty exhibited a markedly lower ABI score than those without (mean 0.32 vs 1.01; p < 0.001). Indication of angioplasty was the only clinical factor significantly associated with a lower ABI score (p < 0.001). Conclusion: The status of T2DM was unrelated to ABI. Future research is recommended to advance the understanding of peripheral artery disease in diabetic foot ulcer patients.

Abstract

Highlights: The highest distribution of breast cancer patients’ age based on the 5-year age interval was 45- 49 years old. There was no significant difference between tumor margin and age of breast cancer patients. There was no significant difference between tumor margin and histopathological grade.   Abstract Introduction: Mammography is an X-ray technique used to take images of the breast. It is the primary diagnostic method for breast tumors. Breast imaging reporting and data system (BI-RADS) classification is needed to determine benign or malignant masses by accessing the mass's shape, margin, density, and other features. However, the tumor margin is the most helpful one. This study aimed to know the difference in the distribution of tumor margin types in each histopathological grading in breast cancer patients. Methods: This was an observational analytic study with a comparative approach using secondary data from medical records of patients with breast cancer at the radio diagnostic and anatomical pathology installation of Dr. Soetomo General Academic Hospital, Surabaya, from January 2017 to December 2021. All statistical data were performed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 27, with a p<0.05 considered statistically significant. Results: Out of 235 cases, the highest distribution of breast cancer patients’ age interval was 45-49 years old (20.9%), the primary tumor margin type was spiculated (64.3%), and the highest distribution of histopathological grading was grade 3 (53.2%). There was no significant difference between tumor margin and age of breast cancer patients (p=0.815), with spiculated tumor margin as the most common type in all age intervals. There was no significant difference (p=0.163) in the distribution of tumor margin types in each histopathological grading, with spiculated tumor margin as the most common type in every grade. Conclusion: There was no significant difference between tumor margin and age of breast cancer patients, and there was no significant difference between tumor margin and histopathological grading.

Abstract

Evaluation is carried out to determine the effectiveness of the training. Evaluations that are still carried out manually/conventionally by collecting data through filling in instruments, have constraints including; Speed of submission of evaluation results report; delay in submitting the issuance of a certificate because the results of the report are used as a requirement for filing; limited storage space for files/report archives; risk of damage, loss of data either caused by viruses, dysfunction of device operations or accidental factors in deleting files. For this reason, a system is needed that can help and facilitate the evaluation process starting from recording, storing and reporting. System development was carried out using qualitative research methods and input-process-output system development methods using the steps of the System Development Life Cycle (SDLC) method of the prototype model. The PIECES Framework is used to analyze system requirements to produce user satisfaction levels. The results of the research are the design of a system consisting of Context Diagrams, Entity Relationship Diagrams (ERD), Table Relational Diagrams (TRD), Standard Operational Procedure (SOP) Flow Diagrams and Data Flow Diagrams (DFD). Users of this system are Admin (program person in charge), Assessors, Organizers and Leaders. This information system is able to facilitate the process of implementing and reporting the results of evaluation implementation, ease of accessing and presenting reports, providing data storage space and better data management compared to conventional methods.

Abstract

ABSTRACT One woman dies from cervical cancer every two minutes. Nearly 90% of deaths from cervical cancer each year occur in women in low and middle income countries where screening efforts are not optimal. Health workers are expected to be role models for the community who carry out early detection. This study aims to analyze factors related to early detection behavior of cervical cancer in female workers. This research is an observational study with a cross sectional design. The sample in this study were female workers aged 30-50 years, married and sexually active who were willing to be respondents. Women who had cervical cancer and underwent total hysterectomy were not included in this study. Data collection was carried out by conducting interviews with respondents using a questionnaire. The independent variables in this research are education, profession, knowledge, attitudes, affordability of access to services, and family support. Meanwhile, early detection behavior for cervical cancer is the dependent variable. A total of 167 respondents were involved in this research. There is no significant relationship between knowledge and accessibility and early detection of cervical cancer. Occupation of health workers (p=0.13, OR=0.146), non-health workers (p= 0.014, OR=0.105) and family support variables (p= 0.001, OR= 5.997) are factors related to early detection behavior of cervical cancer. The factor found to most influence the behavior of early detection of cervical cancer was family support with OR 5.997 (95% CI OR: 2.190-16.419). Lack of family support will have a 5 times greater influence on female workers not carrying out early detection of cervical cancer. Good family support, supported by sufficient information and high motivation does not rule out the possibility of respondents carrying out early detection examinations for cervical cancer. Keywords: Cervical Cancer, Early Detection, Pap Smear  ABSTRAK Satu wanita meninggal karena kanker serviks setiap dua menit.
