Abstract

Diabetes Mellitus (DM) is a major global health challenge with a steadily increasing prevalence over recent decades. Early detection through glycemic parameters such as HbA1c and Fasting Blood Glucose (FBG) is essential for effective promotive-preventive interventions. This community service activity aimed to optimize DM screening using practical and efficient Point of Care Testing (POCT) among 99 adult participants in South Jakarta. The average HbA1c level was 5.15% (SD 1.09), while the mean FBG was 109.15 mg/dL (SD 42.55). A total of 11.1% of participants were diagnosed with diabetes based on HbA1c criteria, and 18.2% based on FBG. Glycemic abnormalities were more prevalent among women and the elderly, highlighting vulnerabilities in glucose metabolism within these groups. Integrating POCT screening with comprehensive health education effectively facilitated early identification of at-risk individuals and improved community awareness of DM prevention. These findings support routine community-based screening as a sustainable promotive-preventive strategy to reduce the burden of DM and prevent long-term complications.

Abstract

Early detection of cardiomegaly is essential to prevent progression toward heart failure, especially in aging populations where cardiovascular remodeling is common. This community-based program evaluated the use of chest radiography for identifying cardiomegaly while increasing public awareness of cardiovascular risk factors. A total of 99 elderly participants (median age 73 years; 71.7% female) underwent radiographic screening. Cardiothoracic ratio (CTR) assessment showed that 36.4% had cardiomegaly (CTR ≥50%), while 63.6% were within normal limits. Cardiomegaly prevalence was higher in participants over 70 years and more frequent in females. These findings align with known age- and sex-related cardiac remodeling patterns, including postmenopausal estrogen decline, which contributes to hypertension, endothelial dysfunction, and myocardial fibrosis. The program demonstrated that community-based radiological screening is a practical tool for early detection of structural cardiac abnormalities and can support timely referral and preventive care. Despite limitations in design and sample size, the activity highlights the value of integrating simple imaging screening with health education to reduce cardiovascular disease burden.

Abstract

Chronic kidney disease (CKD) is a growing global health issue, particularly among the elderly, as it progresses silently and often remains undetected until advanced stages. Early identification is essential to slow the deterioration of kidney function and reduce cardiovascular complications. This community engagement program aimed to optimize serum creatinine testing and estimated glomerular filtration rate (eGFR) calculation as simple yet reliable biomarkers for CKD screening. A total of 99 participants (mean age 71.85 ± 8.08 years) underwent laboratory assessments and health education. The mean creatinine level was 1.41 mg/dL, with an average eGFR of 53.14 mL/min/1.73 m²; most participants (55.6%) fell within the moderate impairment category. Based on KDIGO criteria, only 1% had normal kidney function, whereas 5.1% had reached end-stage renal disease. Age, cardiometabolic factors, and sex differences were associated with varying degrees of dysfunction. Combining biomarker screening with education effectively increased awareness and supported community health workers. Serum creatinine and eGFR assessment are recommended as promotive-preventive strategies for early CKD detection in high-risk populations.

Abstract

Liver disease is a major contributor to global morbidity and mortality, with its burden increasing due to epidemiological transition and lifestyle-related risk factors. A key challenge is the lack of early detection, as many cases are identified only at advanced stages when treatment becomes less effective. This community engagement program aimed to perform liver function screening through SGOT and SGPT measurements as a promotive–preventive strategy to enhance community health. Conducted at Asisi Church, South Jakarta, the program involved 99 elderly participants (mean age 71.85 years) who received education on liver disease risk factors and underwent biochemical assessment. The mean SGOT level was 26.17 U/L and SGPT 22.59 U/L, with no participants exhibiting elevated transaminases. These findings indicate a low prevalence of abnormal liver enzyme levels, though interpretation should consider possible healthy volunteer effects and age-related changes. The program highlights the value of integrating simple biochemical screening with health education to improve awareness and support early prevention of liver disease in community settings.

Abstract

Cardiovascular disease remains the leading cause of global mortality, with a growing burden particularly in low- and middle-income countries, including Indonesia. Atherogenic dyslipidemia—marked by an imbalance between apolipoproteins, specifically elevated Apolipoprotein B and reduced Apolipoprotein A—has emerged as a more accurate cardiometabolic risk indicator than conventional lipid measurements. This community service program aimed to enhance early detection and health education regarding apolipoprotein profiles among the elderly population in South Jakarta. A total of 99 participants (mean age 71.85 years; 71.7% female) underwent ApoA and ApoB testing and received targeted health education. Results showed that 16.2% of participants had elevated ApoB levels, while all participants (100%) exhibited ApoA levels within the high-risk category. Based on the ApoB/ApoA ratio, 25.3% of participants were classified as having high cardiovascular risk. These findings highlight the need to integrate apolipoprotein-based screening into primary healthcare services, supported by educational interventions focusing on lifestyle modification, to help reduce the community burden of cardiovascular disease.

Abstract

Chronic obstructive pulmonary disease (COPD) is a health problem with increasing prevalence in aging populations, contributing to a decline in the quality of life among older adults. This community service program aimed to optimize health education through spirometry assessments in an urban population. The activity was conducted at Asisi Church, South Jakarta, involving 99 participants (mean age 71.85±8.08 years; majority female 71.7%). Pulmonary function was assessed by spirometry, measuring FVC, FEV1, and FEV1/FVC ratio. Results showed that most participants exhibited a restrictive pattern, with reduced FVC (mean 58.8%; 89.9% decreased) and FEV1 (67%; 77.8% decreased), although the FEV1/FVC ratio remained within the normal range (91.9%). These findings highlight that the decline in lung capacity is more prominent in urban older adults. Integrating simple screening with health education sessions proved effective as a promotive–preventive strategy to raise community awareness regarding regular monitoring of lung function and adopting sustainable healthy lifestyles.

Abstract

Introduction. Chronic kidney disease (CKD) is a major public health issue, particularly among the elderly, where metabolic disturbances accelerate renal decline. Diabetes mellitus contributes to kidney dysfunction through insulin resistance and altered glycemic control. This study investigates the relationship between diabetes-related parameters—fasting blood glucose, HbA1c, fasting insulin, HOMA-IR, and kidney function, as measured by estimated glomerular filtration rate (eGFR) in an elderly population. Methods. A cross-sectional analytic study was conducted among 82 elderly residents of Bina Bhakti Nursing Home. Venous blood samples were collected to assess fasting blood glucose, HbA1c, fasting insulin, and HOMA-IR. Kidney function was estimated using the CKD-EPI equation. Results. Age showed a significant negative correlation with eGFR (r = –0.266, p = 0.016), indicating age-related renal decline. HOMA-IR (r = 0.247, p = 0.025) and fasting insulin (r = 0.224, p = 0.043) demonstrated weak but significant positive correlations with eGFR. Conversely, fasting glucose (r = 0.055, p = 0.623) and HbA1c (r = 0.029, p = 0.795) had negligible, non-significant correlations with eGFR, suggesting limited predictive value for renal impairment. Insulin growth factor exhibited a weak, non-significant negative correlation (r = –0.104, p = 0.355). Conclusion. Age remains a key determinant of declining kidney function in the elderly. Insulin resistance markers showed mild but unexpected associations, warranting further longitudinal studies to confirm these findings and explore underlying mechanisms.

Abstract

Latar Belakang: Komposisi tubuh dan distribusi adipositas memiliki peran penting dalam regulasi metabolik pada perempuan dewasa, namun asosiasi diferensial antara parameter antropometri dengan kendali tekanan darah, glukosa, dan lipid belum sepenuhnya tereksplorasi di level komunitas.  Tujuan: Menganalisis korelasi antara komposisi tubuh dan antropometri dengan tekanan darah, homeostasis gula darah, dan profil lipid pada perempuan dewasa di Salatiga. Metode: Penelitian potong lintang melibatkan 51 perempuan dewasa dengan rata-rata usia 41,65 tahun. Pengukuran meliputi antropometri (indeks massa tubuh, lingkar perut, lingkar panggul), analisis komposisi tubuh (lemak subkutan regional, massa otot rangka segmental), dan parameter biokimia (glukosa darah puasa, profil lipid, tekanan darah). Analisis korelasi Spearman digunakan untuk evaluasi hubungan antar variabel. Hasil: Adipositas sentral menunjukkan korelasi positif dengan glukosa darah puasa dan trigliserida, serta korelasi negatif dengan HDL cholesterol. Massa otot rangka tubuh utama menunjukkan korelasi negatif signifikan dengan dysglycemia. Namun, asosiasi antara parameter komposisi tubuh dengan tekanan darah tetap lemah dan tidak signifikan, mengindikasikan regulasi tekanan darah dipengaruhi mekanisme independen. Kesimpulan: Adipositas sentral dan massa otot rangka memiliki peran regulatif diferensial dalam kendali metabolik glukosa dan lipid, sementara tekanan darah diregulasi melalui mekanisme multi-sistem. Penelitian longitudinal dengan detailed mechanistic assessment sangat direkomendasikan untuk elucidasi lebih lanjut.

Abstract

Latar Belakang: Diabetes mellitus tipe 2 (DMT2) terus meningkat secara global dan menjadi tantangan kesehatan masyarakat, terutama pada populasi urban dengan prevalensi obesitas abdominal yang tinggi. Indeks Triglyceride-Glucose (TyG) dan variannya yang dimodifikasi telah berkembang sebagai biomarker sederhana namun sensitif untuk mendeteksi resistensi insulin dan risiko gangguan metabolik. Penelitian ini bertujuan menganalisis korelasi antara TyG dan TyG yang dimodifikasi dengan kadar hemoglobin terglikasi (HbA1c) pada masyarakat dewasa di Kelurahan Kota Bambu, Jakarta Barat. Metode: Penelitian menggunakan desain potong lintang dengan total 135 responden berusia 18–65 tahun. Pemeriksaan glukosa puasa, profil lipid, dan HbA1c dilakukan sesuai prosedur standar, sementara indeks TyG dan variannya dihitung menggunakan formula baku yang mengombinasikan parameter laboratorium dan antropometri.  Hasil: Menunjukkan adanya korelasi positif bermakna antara TyG dan HbA1c (r = 0,170; p = 0,048), serta korelasi positif pada TyG×IMT (r = 0,169; p = 0,050) dan TyG×WC (r = 0,199; p = 0,021), sedangkan TyG×WHtR menunjukkan hubungan searah namun tidak signifikan. Kesimpulan: Konsistensi arah hubungan ini menggambarkan bahwa peningkatan indeks berbasis glukosa dan trigliserida selaras dengan peningkatan risiko glikemik, meskipun perubahan HbA1c masih berada pada rentang normal. Temuan ini menegaskan potensi TyG sebagai marker skrining dini untuk mendeteksi disfungsi metabolik subklinis pada populasi perkotaan. Diperlukan penelitian longitudinal untuk mengevaluasi nilai prediktif TyG terhadap kejadian diabetes serta memastikan perannya dalam strategi skrining komunitas yang cost-effective.

Abstract

ABSTRACT Urinary tract infection (UTI) is a common disease among the elderly, especially in nursing homes. Knowing the individual risk factors that impact the incidence of UTIs is essential for the development of prevention strategies. This study aims to identify the relationship between fasting blood sugar (FBG), HbA1c, fasting insulin levels, upper arm circumference (MUAC), and calf circumference (CC) on the incidence of UTIs in the elderly group conducted at Bina Bhakti Nursing Home in 2024. A cross-sectional study was conducted using a total sampling method. UTI diagnosis was based on urinalysis findings of positive leukocytes, nitrites, or leukocyte esterase. HbA1c, fasting blood glucose, and insulin levels were measured from venous blood samples and analyzed in a laboratory. MUAC and calf circumference were measured using a measuring tape. Statistical analysis included descriptive statistics, and regression analysis was performed to evaluate the relationship between numerical variables. The study involved 93 respondents with an average age distribution of 74.19 years (61-97 years) and the majority were female (82.8%). Diagnosis of UTI was based on urine examination with findings of positive leukocytes, positive nitrite, or positive leukocyte esterase. The average HbA1c value of the respondents was 7.59% which was predominantly moderate as many as 52 respondents (55.9%), the average MUAC value was 23.61 cm, and the calf circumference was 28.72 cm. Most respondents had optimal insulin levels (mean 4.83 ng/mL) and normal FBG values (mean 86.54 mg/dL). Regression analysis showed a significant correlation (p0.05) between age and HbA1c values with the incidence of UTI. However, no significant correlation was found between gender, FBG, fasting insulin levels, MUAC, and calf circumference with the incidence of UTI. The results of this study conclude that increasing age and HbA1c values correlate with an increase in the incidence of UTIs in the elderly. Further research is needed to better understand the causal relationship between these factors and the incidence of UTI in nursing homes. Keywords: Calf Circumference, Fasting Blood Sugar, HbA1c, Nursing Home, Upper Arm Circumference, Urinary Tract Infection.  ABSTRAK Infeksi saluran kemih (ISK) merupakan penyakit yang umum terjadi pada kelompok lanjut usia, terutama di lingkungan panti werdha. Mengetahui faktor risiko individu yang berdampak terhadap kejadian ISK sangat penting untuk pengembangan strategi pencegahan. Penelitian ini bertujuan untuk mengidentifikasi hubungan antara kadar gula darah puasa (GDP), HbA1c, kadar insulin puasa, lingkar lengan atas (LiLA), serta lingkar betis terhadap kejadian ISK pada kelompok lanjut usia yang dilakukan di Panti Werdha Bina Bhakti pada tahun 2024. Penelitian ini menggunakan desain potong lintang dengan metode pengambilan sampel total. Diagnosa ISK ditegakkan berdasarkan pemeriksaan urin dengan temuan leukosit positif, nitrit positif, atau leukosit esterase positif. Kadar HbA1c, gula darah puasa, dan insulin diukur melalui pengambilan darah vena dan diperiksa di laboratorium. Lingkar lengan atas dan lingkar betis diukur menggunakan pita pengukur. Analisis statistic mencakup penyajian deskriptif dan analisis regresi diterapkan untuk mengevaluasi hubungan antara variabel numerik. Penelitian ini melibatkan 93 responden dengan distribusi usia rata-rata 74,19 tahun (61-97 tahun) dan mayoritas berjenis kelamin perempuan (82,8%). Rata-rata nilai HbA1c responden adalah 7,59% yang didominasi derajat sedang sebanyak 52 responden (55,9%), Rata-rata nilai LiLA sebesar 23,61 cm dan lingkar betis sebesar 28,72 cm. Sebagian besar responden memiliki kadar insulin yang optimal (rata-rata 4,83 ng/mL) dan nilai GDP yang normal (rata-rata 86,54 mg/dL). Analisis regresi menunjukkan adanya korelasi signifikan (P0,05) antara usia dan nilai HbA1c dengan kejadian ISK. Namun, tidak ditemukan korelasi signifikan antara jenis kelamin, GDP, kadar insulin puasa, LiLA, dan lingkar betis dengan kejadian ISK. Hasil penelitian ini menyimpulkan bahwa peningkatan usia dan nilai HbA1c berkorelasi dengan peningkatan kejadian ISK pada lansia. Penelitian lanjutan diperlukan untuk memahami lebih dalam hubungan kausal antara faktor-faktor tersebut dengan kejadian ISK di panti werdha. Kata Kunci: Gula Darah Puasa, HbA1c, Infeksi Saluran Kemih, Lingkar Betis, Lingkar Lengan Atas, Panti Werdha.
