World Health Organization dalam Laporan Tahunan Kesehatan Dunia 2008 âPrimary Health Care â Now More Than Everâ, mengemukakan lima masalah serius pelayanan kesehatan di dunia: (1) inverse care; (2) impoverishing care; (3) fragmented care; (4) unsafe care; (5) misdirected care (WHO, 2008). Sistem pelayanan kesehatan yang âfragmentedâ membawa akibat yang tidak diinginkan: inefisiensi, ketidakefektifan, ketidakadilan, komoditisasi, komersialisasi, deprofesionalisasi, depersonalisasi, ketidakpuasan terhadap pelayanan kesehatan (Stange, 2009).

Metode yang diperkenalkan oleh penulis, baik untuk menulis, merevisi, dan mengirimkan naskah artikel, maupun merespons keputusan redaksi, jelas. Dengan tebal 376 halaman, cakupan buku ini cukup komprehensif, ditulis dengan seksama, dan ditata dengan apik. Dibandingkan dengan buku âhow to writeâ lainnya, pembaca membutuhkan waktu yang lebih lama (12 minggu) untuk selesai membaca. Tetapi buku ini lebih efektif untuk merealisasi keinginan para penulis naskah artikel, yaitu mengirimkan dan menerbitkan artikel akademis pada jurnal.

Background: Emotional Quotient (EQ) is an ability to manage emotional life with intel-ligence. Self-motivation is one of EQ factor. Students who were accepted through PMDK admission had been likely to be able to manage their emotional life since adolescence. The purpose of this research is to analyze the difference between medical students who were accepted through PMDK admission and SNMPTN entrance examination. Methods: This was an observasional analytic study with cross sectional approach, condeucted in April 2012 at Faculty of Medicine, Sebelas Maret University. Samples were selected using purposive random sampling method. EQ scores were measured using emotional quotients inventory questionnaire and statistically tested by Kolmogorov- Smirnov test for normality distribution and Mann-Whitney test using SPSS 17.0 program for Windows. Results: Result from Mann-Whitney test showed a significant mean difference of EQ scores (p < 0.001) between medical students who were accepted through PMDK admission (116.67 ± 5.24) and SNMPTN entrance examination  (106.67 ± 7.12). Conclusions: This study found statistically significant difference of EQ scores between medical students who were accepted through PMDK admission and through SNMPTN entrance examination at Faculty of Medicine, Sebelas Maret University. The EQ scores of student accepted through PMDK admission was higher than those of student accepted through SNMPTN entrance examination. Background: High emotional intelligence can prevent adolencents from deviant behaviour, particullary premarital sexual behavior. This study aims to prove that high emotional intelligence can reduce premarital sexual behavior. Methods: This study is an analytical observational research using cross sectional approach. This study had been done in April 2012 in SMA Negeri 3 Surakarta. 83 samples had been choosed by simple random sampling. This study use premarital sexual behavior as dependent variable, emotional quotient as independent variable, attitudes about premarital sexual behavior, bad influence of friends and religion as controlled variables. Questionnare to measure all variables had been tested validity and reliability. Samples were analyzed using binary logistic regression with SPSS 17.0. Results: This study shows that by controlling confounding factors, adolescents who have high emotional intelligence maybe lead to good sexual behavior by 6.84 times bigger than adolescents with low emotional intelligence (OR = 6.84; CI 95% = 1.75-26.72; p = 0.006). Conclusion: There is a relationship between emotional intelligence and premarital sexual behavior by controlling confounding factors. High emotional intelligence can reduce bad premarital sexual behavior. Keywords: premarital sexual behavior, emotional intelligence

Background: Chronic Obstructive Pulmonary Disease (COPD) is a non-infectious chronic and progressive disease, that has become one of the major causes of morbidity and mortality in the world. The treatment management of COPD is complex. Factors that can optimize treatment include patient acceptance to disease process, knowledge, trust in the treatment, effective patient-doctor communication, and adherence to treatment. This study aimed to analyze the association between knowledge of COPD and adherence to treatment among patients with COPD. Methods: This analytic study was observational with cross-sectional approach. A sample of 40 study subjects was selected by exhaustive sampling from outpatients with COPD visiting the Pulmonary Clinics, RSUD Dr. Moewardi Surakarta. The data were collected by interview using a set of questionnaire. The data was analyszed using multiple logistic regression model on SPSS version 17 for Windows. Results: Patients with high level of knowledge had 20 times as many level of adherence to COPD treatment than those with low level of knowledge (OR=21.01; 95%CI 2.08 to 211.98; p = 0.001). This estimate has controlled for the effects of confounding variables such as payment method, age, and gender. Conclusion: There is a statistically significant association between knowledge of COPD and adherence to treatment among patients with COPD. This conclusion is drawn after controlling for the effects of confounding factors such as payment method, age, and gender. Keywords: COPD, knowledge, adherence to treatment

Background: Stroke is a cerebrovascular disease which until now is rated third causing death in the world. Stroke can cause physical disability to psychosocial disfunction, such as cognitive decline. One of cognitive decline that caused by stroke is dementia. This study aimed to analyze the relationship between gender and vascular dementia incident among post stroke patients. Methods: This analytic study was observational with cross-sectional approach. A sample of 40 study subjects was selected by fixed-exposure sampling from outpatients with post-stroke visiting the Neurology Clinics, RSUD Dr. Moewardi Surakarta. They were grouped into two, male and female. The data was analyszed using multiple logistic regression model on SPSS version 17 for Windows. Results: Female patients had 1/100 times as many level of adherence to post-stroke vascular dementia than male patients (OR = 0.01; 95%CI 0.001 to 0.25; p = 0.004). This estimate has controlled for the effects of confounding variables such as age and level of education. Conclusions: There is a statistically significant relationship between gender to vascular dementia on post-stroke patients. Objective: The aim of the study is to determain the relation between blood glucose level and cognitive function score in stroke ischemic patient at RSUD Dr. Moewardi. Background: . An ischemic stroke is death of an area of brain tissue (cerebral infarction) resulting from an inadequate supply of blood and oxygen to the brain due to blockage of an artery. Cognitive impairments that may be experienced after the occurrence of strokeinclude difficulty concentrating for the amount of time, organization, forgetfulnessand being easily confused. The state of hyperglycemia is common in the acute phase of stroke caused by stress and an increase in serum catecholamines. This study aimed to proof that their blood glucose levels rise will increase infarct size, decreased cognitive function and clinical output effects are worse, especially in non-hemorrhagic stroke Methods: This analytic study was analytic observational with cross-sectional approach. A sample of 50 study subjects was selected by random sampling from outpatients with stroke ischemic visiting the Neurology Clinics, RSUD Dr. Moewardi Surakarta. The data were collected by interview using a set of questionnaire. The data was analyszed using multiple linear regression model on SPSS version 17 for Windows. Results: There is a negative significant association between blood glucose level and Patients stroke ischemic with blood glucose level â¥145mg/dL has MMSE score 5,39 point lower than blood glucose level <145mg/dL ( b= -5,39 ; CI 95% -6,98 sampai -3,81; p<0,001). This estimate has controlled for the effects of confounding variables such as level of education and age. Conclusion: There is a statistically significant association between blood glucose level and cognitive function score in stroke ischemic patient. Background:Isoflurane and halothane have been used broadly. Airway irritation is one of the side effect from both isoflurane and halothane and have fairly important clinical implication in the provision of both anestethic agent. Methods:The Research is observational analitic research. The population of the research was the patient completed surgery with general anesteshia in IBS(InstalasiBedahSentral) RSUD dr. Moewardi, Surakarta and was transferred to the ward. The data of odynophagia and the incidence of chough were analyzed using Chi Square method. Results:This quote sampling research uses 30 samples divided into two groups, 15 peoples get Isoflurane as their anestethic agent, the other 15 get Halothane. The results of all samples obtained from both groups experienced a sense of hoarseness in the throat. Data incidence of odynophagia obtained that 15 patients of isoflurane experienced odynophagia and 7 patients found in the halothane. Data incidence of chough obtained that 7 patients of isoflurane experienced chough and none of the halothane patient experienced chough. Analitical result on the incidence of odynophagia obtained that patients given isoflurane are at risk for odynophagia 35.13 times greater than halothane, and the increased risk was statistically significant (OR: 35.13, 95% CI 1.78 to 693.30, p = 0.002) and the results cough incidence analysis, found patients given isoflurane are at risk for experiencing cough 27.35 times greater than halothane, and the increased risk was statistically significant (OR: 27.35, 95% CI 1:36 to 539.7, p = 0.004). Conclusions:The conclusions of this study isisoflurane has the effect of irritating the respiratory tract is greater than halothane as inhaled anesthetic. Isoflurane itself can still be used in elective surgery such as halothane when accompanied by adequate premedication Keywords:Isoflurane-Halothane-Iritation-Airway.Â

Background: Incidence of cervical cancer each year has increased and the risk of death from cervical cancer is also increased. Cigarette smoke is one of the risk factors associated with cervical cancer which contains chemicals such as nicotine and benzopiren. The content of these chemicals may increase the risk of cervical cancer by various mechanisms. The purpose of this study was to determine the incidence rate of cervical cancer associated with exposure to passive smokers is connected. Methods: This type of research is an case-control. Sample of this study were gynecologic cancer patients with a total sample of 60 people. Measuring instrument used was a validated questionnaire, and performed data analysis with multiple logistic regression analysis. Result: Found a statistically significant association between passive smoking with the incidence of cervical cancer. Passive smokers will have 11,5 ââtimes the cervical cancer than non-passive smokers (p=0,001, OR = 11,5). Conclusion: Based on studies of passive smoking women would run the risk of cervical cancer will have 11,5 times higher than non-passive smokers. Key words: passive smoker, cervical cancerÂ

Background: The anxiety at the students approaching teenagers age is the manifestation of the transition of emotional, morality, and relationship in family. A study was performed to the division of acceleration and RSBI classes in SMA Negeri 1 Surakarta creates the Difference of Anxiety Level between them because there is a psychological pressure constituting the high achievement demand. Methods: This was an analytic observational research with cross sectional approach. The subjects were the students of SMA Negeri 1 Surakarta. The sampling technique was using purposive sampling. The research data obtained by two different questionaire, the L-MMPI questionnaire, and Taylor Manifest of Anxiety Scale questionnaire (T-MAS). This study analysed statistically with Chi Square test. Results: 58 samples were consisted of 26 accelerationâs students and 32 RSBIâs students. Based on analyzed data, there were differences of anxiety level at the students of acceleration and RSBI (Rintisan Sekolah Bertaraf Internasional) programs in SMA Negeri 1 Surakarta (p=0.036;OR 0.30). Conclusions: The differences of anxiety level were statistically significant between accelerationâs students and RSBIâs students (p=0.036). Anxiety level for students in acceleration was 0.3 times higher than the students in RSBI program. Keywords: anxiety, acceleration, RSBI.Â
