Abstract

Background: COVID-19 is a new disease caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). COVID-19 became a pandemic disease that killed 4 million people for almost 2 years. COVID-19 patients with comorbidities or risk factors are more prone to have a worse prognosis, one of which is heart disease which can be checked through troponin I values. This study aimed to determine the relationship between troponin I levels and mortality in patients with COVID-19.Subjects and Method: This was a systematic review and meta-analysis by searching for articles from databases such as Science Direct, Springer, Google Scholar, and PubMed using the keywords: troponin I, cohort, fatality, cardiac biomarker, death, coronavirus, COVID-19 mortality, and COVID-19. Ten accredited articles published between 2019-2021 were included for meta-analysis. The subjects of this study were 10,499 COVID-19 patients. The intervention in this study was high troponin I levels, while the comparison was normal troponin I levels. The outcome of this study is mortality in COVID-19 patients. Data processing using Review Manager software version 5.3 for the relationship between troponin I levels and mortality in COVID-19 patients.Results: From a meta-analysis of 10 primary research articles in China, Spain, New York (United States), France, Turkey, and Philadelphia (United States) showed an association between high troponin I levels and an increased risk of mortality in COVID-19 patients (aOR= 3.07; 95% CI= 1.68 to 5.61; p<0.003).Conclusion: High troponin I levels increase the risk of mortality in COVID-19 patients.Keywords: troponin I, heart disease, mortality, COVID-19.Correspondence: Hesthi Krisnawati. Abstract

Background: Obesity is a problem that is starting to be found, not only in urban areas with high socioeconomic status but also in children living in rural areas, even from middle to lower socio­economic groups. This study aimed to examine the effectiveness of electro-acupuncture in obesity cases.Subjects and Method: Meta-analysis was carried out with PICO: The population in this study were obese patients aged 16-65. The intervention was in the form of electro-acupuncture therapy. Comparison in the form of sham/ placebo acupuncture. The outcome is weight loss. A meta-analysis study was applied with electronic data sources: Clinical Key, Google Scholar, MED­LINE/PubMed, Science Direct, Scopus. The articles included were full-text with a Ran­domized Control Trial (RCT) study design. There were 10 articles used in this study with a sam­ple size of 818 people who were divided into two groups (455 people in the acupuncture group and 363 people in the sham/placebo acupuncture group). Articles were analyzed using the Review Manager 5.4 application. The data were analyzed using Standardized Mean difference (SMD) and the heterogeneity of the research sample.Results: There was high heterogeneity between one experiment and another (I2= 86%; p<0.001), so the Random Effect Model (REM) was used. Electroacupuncture therapy was effective in reducing body weight with a Standardized Mean Different (SMD) of 0.50 compared to false acupuncture (SMD= -0.50; 95% CI= -0.92 to -0.08; p= 0.020).Conclusion: Acupuncture therapy can lose weight in obesity cases with a Standardized Mean Different (SMD) of 0.50 compared to fake acupuncture.Keywords: acupuncture, obesityCorrespondence: Nurmila Mutiah. Abstract

Background: Low Back Pain is pain, aches and pains that occur in the lower back area and can be local pain or radicular pain. Acupuncture is a non-pharmacological therapy that can be used as an option to treat low back pain. This study aims to examine the effectiveness of acupuncture and sham acupuncture on reducing pain in cases of low back pain.Subjects and Method: Meta-analysis was carried out with PICO as follows: The population in this study were patients with low back pain with an age range of 20-95 years. Intervention in the form of acupuncture therapy. Comparison in the form of sham acupuncture. Outcome in the form of pain scale. A meta-analysis study was applied to this study with electronic data sources Clinical Key, Google Scholar, MEDLINE/PubMed, Science Direct, Scopus, and Springer. The article used is a full-text article with a Randomized Control Trial (RCT) study design. There are 9 articles used in this study. Articles were analyzed using the Review Manager 5.3 application. The results of this study aim to determine the Standardized Mean difference (SMD) and the heterogeneity of the research sample.Results: The heterogeneity in the results of this study was I^2=79%; p < 0.001, so it was analyzed by Random Effects Model (REM). Acupuncture was more effective in reducing pain than sham/placebo acupuncture, with a statistical significance of (SMD -0.59; 95% CI = -0.88 to -0.30; p<0.001.Conclusion: Acupuncture is more effective than sham/placebo acupuncture in reducing pain scale in cases of low back pain.Keywords: Acupuncture, Sham Acupuncture, Low Back PainCorrespondence: Nurtama Aditya Nugraha. Abstract

Background: Smoking is the leading cause of premature death in both developed and developing countries. Smoking cessation efforts can be classified into pharmacological and therapeutic strategies. Varencline is the latest discovery for pharmacological therapy for smoking cessation programs. This study aims to determine the role of varenicline on smoking cessation efforts.Subjects and Method: This was a systematic review and meta-analysis conducted using PRISMA flow diagrams. Search articles through journal databases including: Google Scholar, MEDLINE/ PubMed, Science Direct, Scopus, and Spinger Link by selecting articles published in 2011-2021. The keywords used are “varenicline” OR “pharmacotherapy” AND tobacco OR smoke OR smoked OR smoker AND “smoking cessation”. The research instrument used to assess smoking cessation efforts was the Fagerstrom Test for Nicotine Dependence (FTND). The inclusion criteria were full paper articles with Randomized Controlled Trial (RCT) research methods, the relationship measure used was Mean SD, the intervention given was varenicline, the research subjects were adult smokers who wanted to quit smoking. Eligible articles were analyzed using the Revman 5.3 application.Results: Meta-analysis of 9 articles showed results (SMD -0.11; 95% CI = -0.17 to -0.05; p<0.001).Conclusion: Varenicline has a statistically significant effect in reducing the frequency of smoking per day.Keywords: Varenicline, smoking cessation, quitting smokingCorrespondence: Muhamad Zainul Lafif. Abstract

Background: Osteoarthritis (OA) of the knee joint has the highest prevalence among all types of rheumatic diseases. The most common symptom of knee OA is pain around the joint. Patients with OA require long treatment, especially using analgesic and anti-inflammatory drugs. Turmeric is efficacious as a pain reliever and anti-inflammatory. This study aims to estimate the magnitude of the effect of turmeric extract (Curcuma domestica) on pain in patients with knee OA compared to placebo and non-steroidal anti-inflammatory drugs (NSAIDs), based on the results of a number of previous similar studies.Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, population: patients with knee OA. Intervention: turmeric extract. Comparison: placebo and NSAIDs. Outcome: pain reduction. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles: “Knee Osteoarthritis” OR “Knee OA” AND “Curcuma longa” OR “Curcuma domestica” OR “Turmeric extract” AND “Placebo” OR “NSAID” AND “pain” OR “visual analogue scale” OR “ VASE". The articles included are full-text with RCT study designs from 2011 to 2021. The selection of articles is carried out using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 15 RCT studies were selected for systematic review and meta-analysis. Data collected from 10 studies showed that patients with knee OA who received turmeric extract experienced 1.60 units lower joint pain than placebo (SMD= -1.60; 95% CI= -2.23 to -0.97; p<0.001). Data from 5 studies showed that patients with knee OA who received turmeric extract experienced 0.06 units higher joint pain than NSAIDs. The comparison between the turmeric extract group and the NSAID group did not show any significant difference (SMD= 0.06; 95% CI= -0.10 to -0.22; p= 0.490).Conclusion: Administration of turmeric extract was more effective than placebo and not inferior to non-steroidal anti-inflammatory drugs in reducing pain in patients with knee OA.Keywords: osteoarthritis, turmeric, painCorrespondence: Danang Ardiyanto. Abstract

Background: Low Back Pain (LBP) is a generalized pain from the second lumbar vertebra to the sacroiliac joint, and is generally a lifelong health disorder. LBP is often the cause of people feeling a decrease in ability in daily functional activities. Patients often complain of symptoms of pain, muscle spasms and impaired function. One of the interventions that can be used to reduce LBP is therapy using transcutaneous electrical nerve stimulation (TENS). TENS is a device used to relieve pain by stimulating nerves through electrodes transcutaneously. TENS is one of the modalities that can improve neuromuscular function or reduce pain and is very useful for patients with complaints of low back pain who experience pain and muscle weakness around the pelvis, trunk, and lower legs. LBP patient.Subject and Method: This study was a meta-analysis with the following PICO, population: low back pain patients. Intervention: administration of TENS. Comparison: not given TENS. Result: decreased pain. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “transcutaneous electrical nerve stimulation” OR “TENS” AND “reduction pain” OR “pain relief” AND “low back pain”. The articles included are full-text English with a randomized control trial study design from 2007 to 2022. Article selection was carried out using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 . application.Results: A total of 12 case studies from continental Europe, America and Asia were selected for systematic review and meta-analysis. Abstract

Background: Dysmenorrhea is the most common cause of lower abdominal pain and has a negative impact on women's quality of life resulting in activity limitations. Non-pharmacological management can be used as an intervention in the management of dysmenorrhea. Acupressure is a therapy that is given by massaging or pressing certain points. The purpose of this study was to analyze the effect of acupressure therapy on reducing dysmenorrhea pain.Subject and Method: This study was a meta-analysis with the following PICO, population: Women aged 16-30 years old with primary dysmenorrhea. Intervention: administration of Acupressure Therapy. Comparison: Acupressure therapy was not performed. Results: Decreased pain in primary dysmenorrhea. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Abstract

Background: Heavy metals, such as cadmium, are heavy metals that are hazardous to health. Chronic exposure to cadmium is associated with decreased glomerular filtration rate and increased risk of chronic renal failure. This study was conducted to determine how much influence cadmium exposure has on the incidence of chronic kidney failure in adults. The purpose of this study was to analyze the effect of cadmium exposure on the incidence of chronic kidney failure in adults.Subject and Method: This study is a systematic review and meta-analysis using the PRISMA flowchart with PICO as follows, Population: Adults. Intervention: Exposure to Cadmium. Compa­rison: No Cadmium Exposure. Outcome: Chronic Renal Failure. Search articles using several data­bases including PubMed, Google Scholar, and Science Direct with the search keywords “adults” AND “cadmium exposure” AND “chronic kidney failure”. The articles submitted ranged from 2013 to 2021. Analysis was performed using RevMan 5.3 as a statistical program.Results: There are 11 articles, originating from the continents of Asia and Europe. The results of the meta-analysis showed that exposure to cadmium increased the risk of developing chronic renal failure in adults by 2.81 times compared with adults who were not exposed to cadmium and was statistically significant (aOR = 2.81; 95% CI = 1.74 to 4.51; p < 0.001).Conclusion: Cadmium exposure may increase the risk of chronic kidney failure in adults.Keywords: cadmium, chronic renal failure, adultsCorrespondence: Fathia Mutiara Zahra. Abstract

Background: Stroke is the third leading cause of death in the world after heart and cancer and the disease that causes serious and permanent disability is the first in the world. Hydrotherapy in stroke plays a role in developing, maintaining, and restoring movement and function with motor training based on an understanding of the pathophysiology, neurophysiology, kinematics and kinetics of normal motion. The purpose of this study was to analyze the effect of hydrotherapy on improving motor functional ability in post-stroke patients.Subjects and Method: This study was a meta-analysis with the following PICO, population: stroke patients. Intervention: administration of hydrotherapy. Comparison: Standard therapy. Result: motor functional improvement. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “hydrotherapy” OR “Aquatic therapy” AND “functional enhancement” AND “post stroke”. The articles included are full-text English with a randomized control trial study design from 2002 to 2022. Article selection was carried out using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A meta-analysis of 9 studies from China, Korea, Italy, Spain and Brazil concluded that hydrotherapy has an effect on post-stroke functional improvement. Post-stroke patients who received hydrotherapy intervention had motor functional ability 0.68 units higher than those who did not receive hydrotherapy. Abstract

Background: Hypercholesterolemia is a risk factor for cardiovascular disease that can cause death and disability, so it must be managed properly. Garlic flour (GP) is one of the dosage forms of garlic that has the potential to reduce cholesterol. This study is a systematic review and meta-analysis that aims to determine the effectiveness of GP in reducing total cholesterol (TC) and low density lipoprotein (LDL).Subjects and Method: Search articles through the Pubmed database, Google scholar, Proquest, Springer link, and Science direct with the appropriate keywords. Population= patients with hyper­cholesterolemia. Intervention= GP. Comparison= placebo. Outcome= TC and LDL. Inclusion crite­ria included complete articles in English or Indonesian published from 2010-2022, Randomized Con­trolled Trial (RCT) design, subjects >18 years old with hypercholesterolemia, the study had a complete fat profile outcome, duration of intervention 2 weeks-12 months. The flow of article selection is based on the PRISMA flow diagram. Meta-analysis using RevMan 5.3 with effect size standardized mean difference (SMD) and Random Effect Model (REM) analysis model.Results: A total of 10 articles were eligible for a meta-analysis. The GP group had TC levels 1.49 units lower than the placebo group (SMD = -1.49; 95% CI = -1.95 to -0.55; p = 0.005) and LDL levels 0.68 units lower than the placebo group (SMD= -0.68; CI95). %= -1.08 to -0.27; p=0.001). Subgroup analysis based on the length of the intervention showed that administration of GP <12 weeks only significantly reduced TC while administration > 12 weeks significantly reduced TC and LDL.Conclusion: GP is effective in lowering TC and LDL especially at 12 weeks or more.Keywords: garlic powder, hypercholesterolemia, RCTCorrespondence: Zuraida Zulkarnain.
