Abstract

Background:Patients with diabetes have an increased risk of morbidity and mortality from several conditions, such as cardiovascular, cerebrovascular, or kidney diseases and heart failure. In addition, economic analysis indicates that mean total costs associated with microvascular complications have almost doubled compared with those for patients without these complications. This study aimed to analyze factors associated with self preventive care among patients with type 2 diabetes mellitus in Surakarta, using a path analysis model.Subjects and Method:A cross sectional study was conducted at Dr. Moewardi Hospital, Surakarta, Central Java, from March to April 2018. A sample of 200 type DM patients was selected by total sampling. The dependent variable was self preventive care. The independent variables were perceived severity, susceptibility, threat, benefit, cues to action, self efficacy, attitude, and family support. The data were collected by questionnaire and analyzed by path analysis.Results:Self preventive care directly increased with perceived threat (b= 3.21; 95% CI= 1.98 to 4.44; p<0.001), perceived benefit (OR= 0.68; 95% CI= -0.11 to 1.47; p=0.092), and self efficacy (OR= 0.14; 95% CI= 0.01 to 0.26; p=0.029). Self preventive care was indirectly associated with perceived severity, attitude, family support, and cues to action.Conclusion:Self preventive care directly increases with perceived threat, perceived benefit, and self efficacy, and is indirectly associated with perceived severity, attitude, family support, and cues to action.Keywords:type 2 diabetes mellitus, self preventive care, Health Belief ModelCorrespondence:Ummy Prasetyowati. Abstract

Background: Gender dysphoria refers to discomfort or distress that is caused by a discrepancy between an individual’s gender identity and the gender assigned at birth (and the associated gender role and/or primary and secondary sex characteristics). Gender reassignment surgery is performed to change primary and/or secondary sex characteristics. For a male to female gender reassignment, surgical procedures may include genital reconstruction (vaginoplasty, penectomy, orchidectomy, clitoroplasty) and cosmetic surgery (breast implants, facial reshaping, rhinoplasty, abdominoplasty, thyroid chondroplasty (laryngeal shaving), voice modification surgery (vocal cord shortening), hair transplants). This study aimed to examine the determinants of cosmetic hormones among transgender in Yogyakarta, Indonesia, using Theory of Planned Behavior (TPB).Subjects and Method: This was a cross-sectional study carried out in Yogyakarta, Indonesia, in November 2018. A sample of 201 transgender was selected by snowball sampling and fixed disease sampling, consisting of 67 transgender using cosmetic hormone and 134 transgender not using the cosmetic hormone. The dependent variable was the use of cosmetic hormones. The independent variables were age, knowledge, education, intention, attitude, perceived behavioral control, and subjective norm. The data were collected by questionnaire and analyzed by path analysis.Results: Cosmetic hormone use was directly increased by strong intention (b= 1.65; 95% CI= 0.98 to 2.31; p<0.001), positive attitude (b= 1.15; 95% CI= 0.48 to 1.81; p= 0.001), but decreased by strong perceived behavioral control not to use cosmetic hormone (b= -1.02; 95% CI= -1.70 to -0.34; p=0.003). Cosmetic hormone use was indirectly affected by age, education, locus of control, attitude, perceived behavioral control, subjective norm, and social norm.Conclusion: Cosmetic hormone use is directly increased by strong intention, positive attitude, but decreased by strong perceived behavioral control not to use the cosmetic hormone. Abstract

Background: HIV / AIDS (Acquired Immune Deficiency Syndrome) is still a health problem in the world including in Indonesia. Approximately 36.7 people are infected with HIV in the world and in Indonesia until March 2017 recorded 242,699 cases. The recorded amount does not reflect the real number in the community because HIV infection often does not show specific symptoms before entering the stage of AIDS. This study aimed to examine the application of the health belief model on adherence to antiretroviral therapy among HIV/ AIDS patients in Surakarta, Central Java.Subject and  Method: This was a cross sectional study conducted at Dr. Moewardi Hospital, Surakarta, Central Java, on February 2018. A total sample of 284 HIV/AIDS patients was selected for this study by exhaustive sampling. The dependent variable was ARV Adherence. The independent variables were perceived susceptibility, perceived severity, perceived benefits, perceived barriers, cues to action, and self-efficacy. The data were collected using a questionnaire and analyzed by a path analysis.Results: Adherence directly increased with self-efficacy (b= 2.10; 95% CI = 0.85 to 3.34; p= 0.001), perceived benefit (b= 2.08; 95% CI= 1.15 to 3.00; p <0.001), and cue to action (b= 3.61; 95% CI= 0.86  to 6.36; p= 0.010) but decreased with higher perceived barrier (b= 2.08; 95% CI= 1.15 to 3.00; p<0.001). Adherence indirectly decreased with higher perceived severity and perceived susceptibility.Conclusion: Self-efficacy, perceived benefit, perceived barrier,  and cue to action directly affect the adherence to antiretroviral therapy. Abstract

Background: HIV infection remains a major global health problem, especially among bisexuals, men who have sex with men (MSM), and homosexuals. The number of AIDS patients in Indonesia from January to March 2017 was 673 people, the number of HIV infections was 10,376 people, and number of AIDS death was 61 people. The purpose of this study was to analyze the determinants of the use of mobile VCT of HIV in MSM community in Surakarta, Central Java.Subjects and Method: A cross-sectional study was conducted in Surakarta, Central Java, from October to November 2018. A sample of 200 MSM was selected by fixed disease sampling, including 50 MSM who used mobile VCT service and 150 MSM who did not use mobile VCT service. The dependent variable was mobile VCT of HIV use. The independent variables were intention, attitude, cues to action, perceived benefit, perceived barrier, and perceived susceptibility. The data were collected by questionnaire and analyzed by path analysis.Results: Mobile VCT of HIV use was directly and positively affected by intention (b= 1.67; 95% CI= 0.73 to 2.56; p<0.001), attitude (b= 1.47; 95% CI= 0.43 to 2.52; p= 0.006), cues to action (b= 1.22; 95% CI= 0.29 to 2.14; p= 0.009), and perceived benefit (b= 1.99; 95% CI= 1.04 to 2.95; p<0.001). It was negatively affected by perceived barrier (b= -1.58; 95% CI= -2.49 to -0.67; p= 0.001). Mobile VCT use was indirectly affected by cues to action, perceived seriousness, and perceived susceptibility.Conclusion: Mobile VCT of HIV use is directly and positively affected by intention, attitude, cues to action, and perceived benefit, but is negatively affected by perceived barrier.Keywords: mobile VCT use, HIV infection, determinants, men who have sex with men, Health Belief ModelCorrespondence: Nadia Ayu Irma Nindiyastuti. Abstract

Background: In spite of international agreements and national laws, the marriage of girls <18 years of age is common worldwide and affects millions. Child marriage is a human rights violation as it prevents girls from obtaining an education, enjoying optimal health, bonding with others their own age, maturing, and ultimately choosing their own life partners. Child marriage is driven by poverty. Therefore, village level of prosperity may have a contextual effect on child marriage. The purpose of this study was to examine the contextual effect of the village on early marriage in Wonogiri, Central Java.Subjects and Method: A case-control study was conducted at 25 villages in Wonogiri, East Java, in December 2018. A sample of 225 women was selected by fixed disease sampling. The dependent variable was an early marriage. The independent variables were the intention, attitude, perceived behavior control, paternal education, maternal education, and information exposure. The data were collected by questionnaire and analyzed by multilevel analysis.Results: The risk of early marriage increased with exposure to negative information (b= 0.97; 95% CI= 0.20 to 1.72; p<0.012), negative attitude (b= 1.67; 95% CI= 1.00 to 2.67; p<0.001), and strong intention (b= 1.86; 95% CI= 1.03 to 2.70; p<0.001). The risk of early marriage decreased with high paternal education (b= -0.81; 95% CI= -1.57 to -2.14; p= 0.009), high maternal education (b= -0.65; 95% CI= -1.37 to 0.85; p= 0.083), strong perceived behavior control (b= -1.77; 95% CI= -1.77 to -0.20; p= 0.013). Village had contextual effect on early marriage with ICC= 15%.Conclusion: The risk of early marriage increases with high exposure to information, negative attitude, and strong intention. The risk of early marriage increases with high paternal education high maternal education, strong perceived behavior control. Abstract

Background: Worldwide, in 2014, 23.8% of the children under-five years of age were stunted following the WHO definition, 7.5% were wasted but 6.1% had overweight or were obese. Developing countries host the bulk of the global stunting and child mortality rate. This study aimed to examine life course factors associated with stunting in children aged 2-5 years, using path analysis model.Subjek dan Metode: A case control study was conducted in Tulungagung, East Java, from October to November 2018. A sample of 200 children under five was selected by fixed disease sampling. The dependent variable was stunting. The independent variables were maternal mid-upper arm circumference (MUAC), maternal height, exclusive breastfeeding, complementary feeding, birth weight, birth length, infection disease, and clean water supply and sanitation. Data on maternal height was measured by microtoise. The other data were collected by maternal and child health book and questionnaire. The data were analyzed by path analysis.Results: Stunting directly decreased with maternal MUAC (b= 2.47; 95% CI= 0.33 to 2.86; p= 0.013), maternal height (b=3.10; 95% CI= 0.79 to 3.54; p= 0.002), exclusive breastfeeding (b= 4.78; 95% CI= 2.05 to 4.91; p<0.001), complementary feeding (b= 2.35; 95% CI= 0.25 to 2.83; p= 0.019), normal birth weight (b= 3.64; 95% CI= 1.26 to 4.21; p<0.001), normal birth length (b= 4.10; 95% CI= 1.63 to 4.62; p<0.001), no infection disease (b= 3.28; 95% CI= 0.87 to 3.47; p= 0.001), and clean water supply (b= 2.99; 95% CI= 0.72 to 3.49; p<0.001). Stunting indirectly affected with infection disease and birth weight.Conclusion: Stunting directly decreases with maternal MUAC, maternal height, exclusive breastfeeding, complementary feeding, normal birth weight, normal birth length, no infection disease, and clean water supply. Abstract

Background: Speech-language ability is one of several developmental abilities that must be masters by children under-five. Data from the Central Biro of Statistics showed that in 2014 there were 131 children under-five with speech-language ability problem. This study aims to determine the associations between sibling role, parenting pattern, maternal education and knowledge, and speech-language ability of children 3-5 years old.Subjects and Method: This was an analytic observational study using cross sectional design. The study was carried out in Gondangrejo Sub-District, Karanganyar District, Central Java. A sample of 80 children aged 3-5 years old were selected for this study by random sampling. The dependent variable was speech–language ability. The independent variables were sibling role, parenting pattern, maternal education, and knowledge. The data were collected by a set of pre-tested questionnaire. Logistic regression was employed for data analysis.Results: Sibling role (OR= 13.23; 95% CI= 2.42 to 72.45; p=0.003), parenting pattern (OR= 7.58; 95% CI= 1.47 to 38.96; p= 0.015), maternal knowledge (OR= 9.64; 95% CI= 2.10 to 44.17; p= 0.004), and maternal education (OR=7.84; 95% CI =1.47 to 41.96; p=0.016) were associated with speech-language ability.Conclusion: Sibling role, parenting pattern, maternal knowledge, and maternal education are associated with speech-language abilityKeywords: sibling, parenting pattern, education, speech-languageCorrespondence: Arif Siswanto. Abstract

Background: Iron deficiency can cause anemia which adversely affects children's growth, especially in terms of achievement, quality of life, and performance as human resources in the future. Birth weight, exclusive breastfeeding complementary feeding (EBF-CF) have a direct relationship with the incidence of anemia in infants. The purpose of this study is to examine the factors that influence anemia in children under five at the Banguntapan II Bantul Health Center, Yogyakarta.Subjects and Method: The cross sectional study was conducted at 25 posyandu in the working area of Banguntapan II Bantul Health Center, Yogyakarta, Indonesia. A sample of 200 children aged 24–60 months was selected by simple random sampling. The dependent variable is anemia in infants. Independent variables are exclusive breastfeeding, breastfeeding, birth weight, nutritional status (BB / TB), mother's education, and mother's work. Anemia data was measured by Hb meter, other variable data were collected using questionnaires. The data were analyzed using a multiple logistic regression with a multilevel approach.Results: The risk of anemia in infants is reduced by exclusive breastfeeding (b = –5.77; 95% CI = -11.30 to -0.24; p = 0.041), EBF-CF administration is appropriate (b = –12.24; 95% CI = -22.47 to -2.00 , p = 0.019), birth weight 2500–4000 g (b = –5.04; CI 95% = -9.87 to 0.21; p = 0.040), good nutritional status (-2SD - + 2SD) (b = –9.11; CI 95% = -17.99 to 0.23; p = 0.044), high maternal education (high school) (b = -10.96; 95% CI = -20.61 to 1.31; p = 0.026), and mothers who work at home (b = -5.98; 95% CI = -11.27 to -0.68; p = 0.026). Posyandu strata showed that there was a contextual influence of posyandu on variations in anemia incidence (ICC = 62.62%).Conclusions: Exclusive breastfeeding, proper EBF-CF, normal birth weight, good nutritional status, high maternal education, and mothers working at home reduce the risk of anemia in infants.Keywords: anemia, children under five, exclusive breastfeeding, multilevelCorrespondence: Rokhayati. Abstract

Background: Preschool development is an influential basic stage for further development. Parents and siblings have an important role for a child's development. Parenting may be the best tool for the development of complete and integrated childhood personality. This study aimed to examine the influence of psycho-socio-economic factors, parenting style, and sibling rivalry, on mental and emotional development of preschool children.Subjects and Method: This was an analytic observational study using cross sectional design. The study was conducted at 4 Community Health Centers, Sidoarjo District, East Java, from March to May 2017. A sample of 120 preschool children were selected for this study by multistage random sampling. The dependent variable was mental and emotional development. The independent variables were maternal education, family income, number of children, belief of child value, parenting style, and sibling rivalry. The data were collected by a set of questionnaire. Path analysis was employed for data analysis.Results: Mental and emotional development was directly affected by authoritative parenting style (b=4.81; 95% CI= 3.05 to 6.56; p=<0.001) and sibling rivalry (b=2.45; 95% CI= 0.92 to 3.99; p=0.002). Authoritative parenting style was positively affected by maternal education ≥senior high school (b=2.14; 95% CI= 0.03 to 4.24; p=0.046), family income ≥ minimum regional wage (b=1.41; 95% CI= 0.07 to 2.75; p=0.038), and positive belief of child value (b=1.34; 95% CI= 0.01 to 2.68; p=0.049). Family income was affected by maternal education ≥senior high school (b=2.84; 95% CI= 1.85 to 3.83; p<0.001). Sibling rivalry was affected by number of children ≥2 (b=1.85; 95% CI= 1.06 to 2.65; p<0.001). Number of children ≥2 was affected by positive belief of child value (b= 3.77; 95% CI= 2.27 to 5.27; p<0.001).Conclusion: Mental and emotional development is directly affected by parenting style and sibling rivalry. Abstract

Background: The prevalence of obesity is increasing in the world, especially in Indonesia. Obe­sity can cause health problems, such as hyperinsulinemia, diabetes, cardiovascular disease, hyper­ten­sion, immunological disorders, certain types of cancer, etc. This study aims to analyze the influence of consumption of water, fruit and vegetables and sociocultural views on the risk of obe­si­ty in senior high school adolescents.Subjects and method: This was a cross-sectional study conducted at 25 senior high schools in Banyuwangi, East Java, from March to April 2018. Samples of 225 students were selected using ran­­dom sampling technique. The dependent variable was obesity. The independent variables were con­sumption of water, fruit and vegetables and socio-cultural views. The data were collected using ques­tionnaires and analyzed using multilevel logistic regression.Results: Water consumption (b = -2.67; CI 95% = -3.77 to -1.58; p = <0.001), fruit and vegetables (b = -1.16; CI 95% = -1.88 to -0.44; p = 0.002), activity physical (b = -1.12; 95% CI = -2.08 to -0.15; p = 0.023) and socio-cultural views (b = -1.89; CI 95% = -2.72 to -1.05; p = <0.001) reduce risk of obesity . Genetic factors (b = 3.12; 95% CI = 1.98 to 4.26; p = <0.001) increase the risk of obesity. Senior high school has a contextual effect on the risk of obesity with ICC = 9.89%.Conclusion: The risk of obesity decreases with adequate consumption of water, consumption of enough fruits and vegetables, enough physical activity and living in a society with a socio-cultural outlook with a good diet. The risk of obesity increases with the presence of factors.
