Abstract

Background: Patient adherence can be influ­enced by several factors that can affect com­pli­ance with hemodialysis treatment. Adherence is highly treated while undergoing hemodialysis in order to achieve success in hemodialysis (HD) treatment. Adherence is one of the beha­viors of a person in achieving successful HD treatment. The success of a treatment can be influenced by several fac­tors, one of which is the use of HD. This study aimed to determine the effect of compliance with hemodialysis.Subjects and Method: The was an obser­vational analytic study with a cross-sectional design. The study was conducted in January 2020. The sample used was 120 chronic kid­ney failure patients undergoing hemodialysis at Dr. Soehadi Prijonegoro Sragen Hospital. The de­pendent variable was compliance with hemo­dialysis. The independent variables are: know­ledge, length of care, family income. Data were collected by means of a question­naire. Data analysis was performed using path analysis with Stata 13.Results: The use of HD in patients with chro­nic renal failure had a positive associa­tion with high HD knowledge (b= 1.22; 95% CI= 2.21 to -3.98; p= 0.027), length of stay (b= 2.09; 95% CI= 4.29 to 3.05; p<0.001), and has no positive relationship with family income (b = 0.40; 95% CI = 0.80 to 1.41; p = 0.426). The use of HD in patients with chro­nic renal failure is influenced indirectly by family income and length of treatment.Conclusion: The use of HD in chronic renal failure patients has a direct relationship with knowledge of HD and length of treatment, and has no direct relationship with family income. Abstract

Background: Community Health Center is a first-level health service facility that organizes public health efforts and individual health efforts. Good health service is supported by work performance of the health personnel. This study aimed to analyze the contextual effect of community health center and socio determi­nants on the work performance of health personnel.Subjects and Method: This was a cross sectional study conducted at Karanganyar com­munity health centers, in Karanganyar, Central Java, from October to November 2019. A sam­ple of 210 health workers was selected random­ly. The dependent variable was work perform­ance. The independent variables were age, edu­cation, tenure, distribution of services, work motivation, job skills, job satisfaction, health center accreditation status, and work environ­ment. Data were collected by question­naire and analyzed by a multilevel multiple logistic regression run on Stata 13.Results: Good work performance increased with age ≥38 years (b= 1.09; 95% CI= 0.19 to 1.99; p= 0.018), adequate service (b= 0.96; 95% CI= -0.28 to 2.19; p= 0.128), high motivation (b= 0.93; 95% CI= 0.09 to 1.77; p= 0.030), good job skills (b= 0.97; 95% CI= 0.06 to 1.88; p= 0.037), good job satisfaction (b= 0.92; 95% CI= 0.05 to 1.78; p= 0.037), and good working environment (b= 0.95; 95% CI= 0.11 to 1.80; p= 0.026). Good work performance decreased with education ≥Diploma III (b= -0.40; 95% CI= -1.67 to 0.87; p= 0.535) and tenure ≥3 years (b= -0.71; 95% CI= -1.79 to 0.37; p= 0.199). Community health center had strong contextual effect on work performance with ICC= 18.00%.Conclusion: Good work performance incre­ases with age ≥38 years, adequate service, high motivation, good job skills, good job satisfac­tion, and good working environment. Good work performance decreases with education ≥Diploma III and tenure ≥3 years. Abstract

Background: Midwives play an important role in reducing maternal and neonatal mor­bidity and mortality. The performance of mid­wives is an important indicator to improve the quality of maternal and child health services. The purpose of this study was to analyze the factors that affect the performance of midwives.Subjects and Method: This cross-sectional study was conducted at a community health cen­ter in Mojokerto, East Java, from February to April 2020. A sample of 200 midwives was selec­ted by random sampling. The dependent va­riable was the performance of the midwife. The independent variables were age, income, workload, work motivation, perceived financial compensation, education, employment status, social support, and marital status. The perfor­man­ce of midwives was measured by the 6-DSNP. Other data were collected using a ques­tion­naire. Data were analyzed by path analysis using the Stata 13 application.Results:The performance of midwives increa­s­ed with age ≥ 42 years (b= 3.19; 95% CI= 1.62 to 4.75; p <0.001), family income ≥ 7 million (b= 3.03; 95% CI= 1.50 to 4.56; p <0.001 ), light workload (b= 2.66; 95% CI= 1.19 to 4.13; p <0.001), strong work motivation (b = 3.32; 95% CI= 1.60 to 5.04; p <0.001), sufficient per­ceived financial compensation (b = 3.96; 95% CI= 1.68 to 4.99; p <0.001). The performance of mid­wi­ves was indirectly affected by edu­ca­tion, em­ploy­­ment status, social support and marital sta­tus.Conclusion: Age, family income, workload, work motivation, and perceived financial com­pen­sation affect the performance of midwives.Keywords: performance, midwives, path analy­sisCorrespondence: Veterina Rizki Amalia. Abstract

Background: The government's efforts to maintain and improve the highest health status with all resources in the health sector, including personnel, funds, facilities, phar­macy, medical devices, and technology, pro­vide quality health care. Community satis­faction as the service receiver is a comparison between the ser­vices received and the services expected. This study aimed to examine associations of perception towards doctors, nurses, and facilities with perceived quality and patient satisfaction on inpatient care at Nabire Hospital Papua.Subjects and Method: A cross-sectional study was con­ducted at Nabire Hospital, Papua, Indonesia, from March to May 2020. A sample of 207 inpatients was selected by stra­ti­fied random sampling based on the ty­pe of inpatient class I, II, and III.Results: Inpatient satisfaction was affected by age <50 years (b= -0.36; 95% CI= -1.63 to 0.91; p=0.579), type of non-BPJS insurance (b= 0.19; 95% CI= -0.80 to 1.19 p= 0.697), length of stay <7 days (b= -2.39; 95%CI= -3.86 to -0.92; p <0.001), patients’ perception towards adequate inpa­tient facilities (b=3.11; 95%CI=2.13 to 4.07; p <0.001), good quality of inpatient care (b= 2.74; 95% CI= 1.74 to 3.72; p <0.001). The good qua­lity of inpatient care was affected by male sex (b= 0.91; 95% CI= 0.10 to 1.72; p= 0.028), age <50 years (b= -0.24; 95% CI= -1.26 to 0.78; p= 0.644), type of BPJS insurance (b= 0.41; 95% CI= -1.22 to 0.39; p= 0.318), percep­tion towards adequate facilities (b= 1.85; 95% CI= 1.04 to 2.65; p<0.001), good perception to­wards nurses (b= 2.18; 95%CI= 1.04 to 3.33; p <0.001), and good perception towards doc­tors (b= 1.68; 95% CI= 0.89 to 2.47; p <0.001). The good perception towards doc­tors was affected by the education of patients ≥Senior High School (b= 1.16; 95% CI= 0.31 to 1.83; p= 0.005).Conclusion: Inpatient satisfaction is affected by age <50 years, type of non-BPJS insurance, length of stay <7 days, perception towards ade­quate facilities, and good quality of inpatient care.Keywords: Quality, satisfaction, inpatient, path analysisCorrespondence: Agustina Utii. Abstract

Background: Fulfillment of premium collec­ti­vity is related to willingness and compliance to pay BPJS Kesehatan premiums. Increasing parti­cipation in the informal sector is not in line with compliance in paying BPJS Health premiums. This study aims to analyze the influ­ence of factors of willingness and com­pliance to pay premiums for BPJS Kesehatan independent participants in Jember Regency.Subjects and Method: This was a cross sectional study conducted in 31 sub-districts in Jember Regency, East Java, in July-September 2020. Sampling of 200 BPJS Kesehatan Man­diri participants was conducted using random sampling. The dependent variable is willingness to pay and compliance to pay the BPJS Health premium. The independent variables include education, income, willingness to pay, imitation of the referral group and number of family members. Data were collected using a question­naire. Data were analyzed using multiple linear regression, multiple logistic regression and path analysis with Stata 16.Results: Factors that affect the willingness to pay are education (b= 45531.18; 95% CI= 35335.1 to 55727.26; p <0.001) and (b= 2.13 units; 95% CI= 1.41 to 2.86; p <0.001), income (b= 17469; 95% CI= 7171.90 to 27766.09; p <0.001), and (b= 1.19; 95% CI= 0.46 to 1.92; p <0.001). Compliance to pay is influenced by the willingness to pay factor (b= 4.19; 95% CI = 2.19 to 8.04; p <0.001) and (b= 0.64; 95% CI= 0.02 to 1.27; p= 0.044) with an average willing­ness to pay Rp 44,792, imitation of the refe­rence group (b= 2.59; 95% CI = 1.39 to 4.85; p= 0.003) and (b = 0.95; 95% CI = 0.33 to 1.58; p= 0.003) and the number of family members (b= 1.90; 95% CI= 1.02 to 3.56; p= 0.044).Conclusion: Willingness to pay premiums is directly affected by education and income. Abstract

Background: The management of health assets in Hospitals is a complex matter. State-onwed Hospitals use two applications from the Ministry of Internal Affairs developed by the Development Audit Agencynamely Regional Management Information System of Regional Assets (Simda BMD) and the Ministry of Health namely Application of Infrastructure and Medi­cal Devices (ASPAK). This study aims to deter­mine the management of health assets based on the applications of the Ministry of Internal Affairs and the Ministry of Health in three Regional Public Hospitals in Boyolali Regency.Subjects and Method: This was a descrip­tive-qualitative study using a study case ap­pro­ach. This study was carried out in three Regio­nal Public Hospitals in Boyolali Regency. The main informants were goods treasurer, ware­house section, staff in charge of planning and reporting section, staff in charge of ASPAK,  head of medical support, and head of adminis­trative subdivision. The data were collected using in-depth interviews, non-participative observation, and document observation. The data were analyzed using three  main steps namely data reduction, data display, and conclusion drawing.Results: The staff in charge of asset manage­ment in RSUD Boyolali had a sufficient profi­ciency in the asset concept and management by using the two applications. However, not all steps were conducted on time andfollowing the guidance.Conclusion: The training is required to improve the capability of the staff in charge of asset management. Abstract

Background: Patient satisfaction is one indi­cator in the success of a treatment in the health sector. There are several ways to make this happen, including by simplifying the process of consultation, treatment and medication. Effec­tiveness in health services is very import­ant. Telemedicine is a way offered to increase effec­tiveness in health services.Subjects and Method: Meta analysis was performed with PICO as follows: The popula­tion in this study were patients. Health service intervention is in the form of telemedicine. Comparison in the form of Non Telemedicine/ visit services. Outcome in the form of patient satisfaction. The meta-analysis study was applied to this study with electronic data sources: Clinical Key, Google Scholar, MED­LINE/PubMed, Science Direct, Scopus. The article used is a full-text article with a rando­mized control trial (RCT) study design. There were 8 articles used in this study with a sample size of 2123 people who were divided into two groups (1113 people in the telemedicine group and 1010 people in the control/ non teleme­dicine group). Articles were analyzed using the Review Manager 5.3 application. The results of this study aim to determine the Standardized Mean Difference (SMD) and the heterogeneity of the research sample.Results: There is high heterogeneity between one experiment and another (I2= 81%; P <0.0001) so that the Random Effect Model (REM) is used. The application of Telemedicine was able to increase patient satisfaction with Standardized Mean Different (SMD) of 0.41 compared to the control/ non-telemedicine group (SMD 0.41; 95% CI= 0.19 to -0.62; p= 0.0002).Conclusion: Providing telemedicine services was able to increase patient satisfaction with Standardized Mean Different (SMD) of 0.41 compared to non-telemedicine.Keywords: Telemedicine, patient satisfactionCorrespondence: Sri Umiati. Abstract

Background: The quality of a health institution is determined by human resources (HR) and the quality of staff services. The quality of health human resources is measured by employee performance and productivity especially services that are directly related to patients, such as the Department of Medical Rehabilitation. This study aimed to analyze the psychosocial economic determinants of the performance of medical rehabilitation health personnel.Subjects and Method: This study used a cross-sectional method. This study was conducted from May 2020 to June 2020. The sample was selected by exhaustive sampling by 200 medical rehabilitation health personnel at Dr. Moewardi, Surakarta. The dependent variable was performance. The independent variables were motivation, skill, work atmosphere, facilities and infrastructure, work experience, training, education, leadership style, incentive, reinforcement, vicarious learning, and observational learning. The data were collected using questionnaires and analyzed using path analysis with STATA version 13.Results: Medical rehabilitation health personnel had a likelihood of performing well with participative leaders (b=2.24; 95%CI=0.09 to 1.44; p=0.025), sufficient incentives (b=3.19; 95%CI=0.42 to 1.76; p<0.001), high motivation (b=3.38; 95%CI=0.51 to 1.93; p<0.001), by attenting training and education for medical rehabilitation (b=1.86; 95%CI=-0.03 to 1.27; p=0.064), good reinforcement (b=1.72; 95%CI=- 0.09 to 1.42; p=0.086), training and education through incentives (b=1.86; 95%CI=- 0.02 to 1.11; p=0.062), conducive work atmosphere through motivation (b=2.49; 95%CI=0.16 to 1.42; p=0.013).Conclusion: The performance of medical rehabilitation health personnel is affected by leadership style, incentives, motivation, training and education, work atmosphere, and reinforcement.Keywords: social cognitive, performance, path analysisCorrespondence: Sisybania, Masters Program in Public Health, Sebelas Maret University, Jl. Abstract

Background: Tertiary prevention of hyper­ten­sion is a preventive effort to make patients with hypertension do not have disabilities and further complications. It also improves the quality of life and makes life longer. The bio­psy­chosocial aspect affects individuals in lear­n­ing, doing, and maintaining certain beha­viors based on social cognitive theory. This stu­dy aimed to analyze the biopsychosocial deter­mi­nants of tertiary preventive behavior among patients with hypertension.Subjects and Method: This study was an analytical observational study with a cross-sectional design. The study was conducted at 25 Integrated Health Posts (Pos­bin­du) in Sragen, Central Java. A sample of 200 hypertensive patients was selected by stratified random sam­pling. The dependent variable was the ter­tiary preventive behavior of hypertension. The in­­de­pendent variables were observational lear­ning, role model, imitation, vicarious learning, rein­forcement, self-efficacy, self-regulation, out­­­­­come expectation, and attitude. The data were collected by ques­tion­naire and analyzed by a multilevel multiple logis­tic regression.Results: Hypertension preventive behavior in­crea­sed with observational learning (b= 2.85; 95% CI= 0.17 to 5.53; p= 0.037), role model (b= 2.73; 95% CI =0.73 to 4.73; p=0.007), imi­tation (b= 3.46 ; 95% CI= 0.73 to 6.19; p= 0.013), vica­­rious learning (b= 2.54; 95% CI =0.16 to 4.93; p= 0.036), reinforcement (b= 3.89; 95% CI= 0.96 to 6.82; p= 0.009), self-efficacy (b= 3.04; 95% CI= 0.41 to 5.67; p= 0.024), self-re­gu­­­­lation (b= 2.28; 95% CI= 0.02 to 4.54; p= 0.048), outcome expectation (b= 3.56; 95% CI= 0.75 to 6.38; p= 0.013), and a positive attitude (b= 2.86; 95% CI= 0.26 to 5.47; p= 0.031). Posbindu had a contextual effect on ter­tiary preventive behavior of hypertension with an Intra Class Correlation (ICC) value of 63.05%.Conclusion: Tertiary preventive behavior of hypertension increases with high observational learning, strong role model, strong imitation, high vicarious learning, strong reinforcement, high self-eff­icacy, high self-regulation, positive outcome expec­­­­tation, and positive attitude. Abstract

Background: Hypertension is the single largest con­tributor to the global burden of disease (GBD). The Chronic Disease Management Prog­ram (Prolanis) is a health service system and a pro­active approach implemented in an integrated manner. This study aimed to determine the ef­fec­tive­ness of Prolanis on the "CERDIK" healthy be­ha­vior for hypertensive patients.Subjects and Method: This cross-sectional stu­dy was conducted in hypertensive patients in 25 working areas of health centers in Gunung­ki­dul, Special Region of Yogyakarta, from January to February 2020. A sample of 200 hypertensive pa­tients was selected by simple random sampl­ing. The dependent variable was the "CERDIK" healt­hy behavior for hypertensive patients. The in­dependent variables were chronic disease ma­na­gement program (Prolanis), education level, peer support, family support, and sex. Other data we­re collected by using a questionnaire. Data we­re analyzed by path analysis using the Stata 13 application.Results: “CERDIK" healthy behavior for hyper­ten­­sive patients increased with active prolanis acti­vity (b= 5.00; 95% CI= 2.80 to 7.20; p <0.001), higher education (b= 1.52; 95% CI= 0.48 to 2.54; p= 0.004), strong family support (b= 1.­38; 95% CI= 0.20 to 2.57; p= 0.023), strong pe­er support (b= 0.61; 95% CI= -0.67 to 2.51; p= 0.­352), and female (b= 2.8; 95% CI= -5.01 to -0.64; p= 0.011). Education, family support, and peer sup­port were associated with “CERDIK” healthy behavior through Prolanis.Conclusion: “CERDIK" healthy behavior for hyper­ten­­sive patients increases with active pro­lanis acti­vity, higher education, strong family support, strong pe­er support, and female.
