Abstract

Background: Neonatal mortality in the world is mostly caused by premature birth (43%), and in Indonesia the biggest cause of neonatal death is premature birth (45%). One of the causes of pregnant women experiencing premature labor, namely urinary tract infections due to changes in the urinary tract system during pregnancy. This study aims to analyze the primary study of the effect of urinary tract infections in pregnant women on preterm delivery with a meta-analysis. Subjects and Method: This study is a meta-analysis with the following PICO, population: pregnant women. Intervention: Having a urinary tract infection. Comparison: No urinary tract infection. Outcome: Premature delivery. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles (“urinary tract infection” AND “preterm labor” OR “premature birth” OR “premature labor”). The included article is full text in English with a case-control study design from 2006 to 2022 and reports the adjusted odds ratio in a multivariate analysis. The selection of articles is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 aplikasi application. Results: A total of 10 case-control studies from Iran, Iraq, India, Tanzania, Ethiopia, and Peru were selected for the meta-analysis. The data collected showed that pregnant women with urinary tract infections increased the occurrence of preterm labor by 2.19 times compared to pregnant women who did not experience urinary tract infections (aOR= 2.19; 95% CI= 1.80 to 2.66; p<0.001). Conclusion: Urinary tract infection during pregnancy increases the incidence of preterm labor. Keywords: urinary tract infection, premature delivery, meta-analysis. Abstract

Background: During pregnancy there are physiological changes associated with the pregnancy process that can cause urinary tract infections caused by the enlargement of the uterus, decreased urine flow through the ureters and decreased bladder tone during pregnancy. So that healthy pregnant women become vulnerable to serious complications in the urinary tract and will have an impact on the mother and baby who will be at great risk of causing premature birth and low birth weight. This study aims to estimate the magnitude of the effect of urinary tract infections in pregnant women on premature birth and low birth weight. Subjects and Method: This study is a systematic review and meta-analysis conducted with PRISMA flow diagrams. Search articles through journal databases including: Google Scholar, MEDLINE/PubMed, Science Direct, Spinger Link and Directory of Open Access Journal (DOAJ) by selecting articles published in 2005-2021. The keywords used were “Urinary Tract Infection Pregnancy” AND “Preterm Labor”, “Urinary Tract Infection Pregnancy” AND “Low Birth Weight”, “Urinary Tract Infection Pregnancy” AND “Preterm Labor” AND “Low Birth Weight” AND “adjusted Odds Ratio". Inclusion criteria included full-text articles, case-control study designs. The analysis used logistic regression with adjusted odds ratio. Eligible articles were analyzed using the Revman 5.3 application. Results: A total of 17 articles were reviewed in this study with a case control study design. A meta-analysis of 11 articles showed that urinary tract infections in pregnant women increased the risk of preterm delivery by 2.49 times compared with pregnant women without urinary tract infections (aOR = 2.49; 95% CI = 2.04 to 3.05; p<0.001), whereas 7 The article shows that urinary tract infections in pregnant women can increase the risk of increasing the risk of LBW by 1.54 times compared to pregnant women without urinary tract infections (aOR = 1.54; 95% CI = 1.24 to 1.91; p<0.001). Conclusion: Urinary tract infections increase the risk of preterm birth and low birth weight.

Abstract

Background: The prevalence of overweight and obesity among women continues to increase every year. Overweight and obesity can lead to complications in pregnancy such as Excessive gestational weight gain (EGWG) and Gestational diabetes mellitus (GDM). Physical activity intervention is an option to prevent EGWG and GDM. The study aims to analyze the effects of physical activity interventions in reducing EGWG and GDM. Subjects and Method: The study used a meta-analysis study design by following PRISMA guidelines. The research question was formulated in PICO.  The population was pregnant women with a Body Mass Index (BMI) of > 25 kg/m2. The intervention used was measured physical activity. The comparison was without measured physical activity. The outcomes were EGWG and GDM. Article search was carried out through the databases such as PubMed, Cochrane Library, Scopus Database, and PEDro which was published from 2011 to 2020. The article search used the following keywords: "pregnancy" OR "pregnant woman" AND "obese" OR "overweight" AND "excessive gestational weight gain" OR "gestational weight gain" OR "weight gain" AND "gestational diabetes mellitus" OR "gestational diabetes" AND "exercise " OR "antenatal exercise" OR "physical activity". Articles that met the criteria were full-text articles with a Randomized Control Trial (RCT) study design. The assessment of the quality of the study was conducted with Cochrane Collaboration's Tool for Assessing Risk of Bias and the articles were subsequently analyzed using the Review Manager 5.4 application. Results: A total of 10 articles analyzed in this study came from Asian, European, Australian, and American countries with a total sample of 2,086. The results of the meta-analysis showed that overweight and obese pregnant women who performed measured physical activity did not experience weight changes compared to those who did not engage in physical activity (SMD= 0.03; 95% CI= 0.17 to 0.73; p=0.770).  However, overweight and obese pregnant women who do measured physical activity experience changes in fasting sugar levels compared to those who did not do measured physical activity (SMD= 0. 20; CI95%= 0.30 to 0.10; p=0.001). Conclusion: The results of this study state that physical activity interventions are effective in reducing GDM but not for EGWG in overweight and obese pregnant women. Keywords: physical activity, excessive gestational weight gain, gestational diabetes mellitus, overweight and obese pregnant women, meta-analysis. Correspondence: Hapsari Cahyaningrum. Abstract

Background: A woman has an increased risk of breast cancer due to increased exposure to the hormone estrogen during her life due to early menarche, late menopause, and/or the absence of childbearing. Epidemiological data show that menopausal status has a risk factor for breast cancer. The aim of this study was to investigate overweight and obesity as risk factors for premenopausal and postmenopausal breast cancer. Subjects and Method: A systematic review and meta-analysis study was conducted by systematically reviewing articles published from 2010-2021 in the PubMed, Google Scholar, SpringerLink and Science Direct databases with the keywords overweight AND obesity AND "breast cancer" OR "breast carcinoma" AND "premenopausal women", overweight AND obesity AND "breast cancer" OR "breast carcinoma" AND "postmenopausal women". Inclusion criteria included articles in English, full text, using an observational study design, and displaying adjusted odds ratios. Articles were reviewed using PRISMA diagrams and analyzed using Revman 5.3. Results: A meta-analysis of 10 articles concluded that overweight women were 1.35 times more likely to develop premenopausal breast cancer than women with a normal BMI (aOR= 1.35; 95% CI= 0.98 to 1.86; p= 0.070). A meta-analysis of 10 articles concluded that obese women were 1.27 times more likely to develop premenopausal breast cancer than women with normal BMI (aOR= 1.27; 95% CI= 0.95 to 1.69; p= 0.110). A meta-analysis of 9 articles concluded that overweight women had a 1.28 times risk of developing postmenopausal breast cancer compared with women with a normal BMI (aOR= 1.28; 95% CI= 1.00 to 1.63; p= 0.050). A meta-analysis of 11 articles concluded that obese women were 1.52 times more likely to develop postmenopausal breast cancer than women with normal BMI (aOR= 1.52; 95% CI= 1.17 to 1.98; p= 0.002). Conclusion: Overweight and obesity increase the risk of pre-menopausal and postmenopausal breast cancer.
