Abstract

Background: Social capital has been shown to play an important role in influencing an individual's health, including the incidence of hypertension in adulthood. Adults with hypertension who usually receive services at elderly posyandu with high strata are less likely to develop hypertension than posyandu with low strata. This study aims to determine the relationship between social capital and hypertension in adults in elderly posyandu.Subjects and Method: This study is an observational analytical study with a cross-sectional approach conducted in 25 Posyandu Elderly from May 2024 to July 2024 A total of 200 adult research subjects were selected using stratified random sampling and simple random sampling. The independent variable in this study is social capital and the dependent variable is hypertension. Data collection was carried out using a questionnaire. The analysis test in this study is a multilevel model double logistic regression analysis test conducted using the STATA 13 model.Results: Each increase in one unit of the social capital score will be followed by a decrease in systolic blood pressure of 0.94 mmHg (b= -0.94; 95% CI = -1.48 to -0.40; p= 0.001) and a decrease in systolic blood pressure of 0.32 mmHg (b= -0.32; 95% CI = -0.56 to -0.08; p= 0.008). Each one-year increase in age will be followed by an increase in systolic blood pressure of 0.43 mmHg (b = 0.43; 95% CI = 0.16 to 0.70; p = 0.002) and an increase in diastolic blood pressure of 0.14 mmHg (b = 0.02; 95% CI = 0.26 to 0.70; p = 0.018). There was no difference in systolic blood pressure between men and women (b= -2.84; 95% CI = -8.04 to 2.36; p= 0.284).Conclusion: Individuals who have good social capital lower the risk of hypertension. Increasing age and gender of women increases the risk of hypertension.

Abstract

Background: TB is the leading cause of death among people living with HIV. In 2020, an estimated 214,000 people living with HIV died from TB. People with AIDS have a severely damaged immune system. They are increasingly suffering from severe diseases, called opportunistic infections (OI). This study aims to analyze and estimate the influence of gender, TB Coinfection, Opportunistic Infection on mortality in ODHIV with Antiretroviral Therapy.Subjects and Method: Systematic review and meta-analysis using PRISMA flowchart and the PICO model. Population: ODHIV with ART. Intervention: Women, Co-infected TB, Opportunistic Infections. Comparison: Male, no co-infected TB, no Opportunistic Infection. Outcome: Mortality Article search using Google Scholar, PubMed and Science Direct databases. The keywods used include "HAART" or "HIV" or "HIV/AIDS" and "Gender" and "TB Coinfections" or "TB" and "Opportunistic Infections" and "Mortality" and "Antiretroviral Treatment". The 17 included articles are fulltext in English with a cohort design study from 2008 to 2023 and report on the Hazard Ratio in a multivariate analysis. Data analysis using the RevMan 5.3 application.Results: A total of 17 cohort studies involving 23,651 research subjects from Vietnam, China, Japan, Ethiopia, and South Africa. The data collected showed that female ODHIV had a mortality risk of 0.7 times compared to male ODHIV (aHR= 0.70; CI 95%=0.60 to 0.79; p<0.001). In ODHIV with coinfected TB has a mortality risk of 1.86 times compared to ODHIV without coinfected TB (aHR= 1.89; CI 95%=1.36 to 2.61; p<0.001). ODHIV with Opportunistic Infection has a 1.90-fold risk of mortality compared to ODHIV without Opportunistic Infection (aHR= 1.90; CI 95%=1.50 to 2.42; p<0.001).Conclusion: Female gender decreases the risk of mortality, while TB coinfection and Opportunistic infection increase the risk of mortality in ODHIV with Antiretroviral Therapy.

Abstract

Background: Sexual transmission of HIV among men who have sex with men (MSM) continues to increase. There are several factors that affect the occurrence of HIV infection in Male Sex Addiction. In this study, we will discuss three factors, namely condom use, anal bleeding and sex group ownership. This study aims to analyze and estimate the strength of the relationship between the incidence of HIV infection and condom use, anal bleeding, and sex group ownership.Subjects and Method: A systematic review and meta-analysis study was conducted using the PICO model. Population: male sex male. Intervention: not using condoms, anal bleeding after sex, and having sex groups. Comparison: using a condom, no anal bleeding after sex, and no sex group. Outcome: HIV infection. The basic data used includes Google Scholar, Proquest, Scopus, PubMed, Zendy, ScienceDirect, BMC, Elsivier, and Springer Link. The inclusion criteria are full-text articles with a cross-sectional study design using multivariate and attaching aOR values and published 1996-2023. The Data analysis using the Review Manager 5.3 application.Results: This meta-analysis was carried out on seven cross-sectional studies originating from China, Tanzania, and Brazil. Total sample size=13,188 people. The risk of HIV infection in men who have sex with men increased with the use of no condoms (aOR= 1.35; CI 95%= 1.10 to 1.65; p= 0.004), there was anal bleeding after sex (aOR= 1.89; CI 95%= 1.53 to 2.34; p<0.001), and group sex (aOR= 2.30; CI 95%= 1.62 to 3.25); p<0.001).Conclusion: The risk of HIV infection in men who have sex with men increases by not using condoms, having anal bleeding after sex, and having group sex.

Abstract

Background: Chronic kidney failure (CKD) is a non-communicable disease in which kidney function begins to decline progressively in a matter of months or even years and can affect the patient's quality of life. The purpose of this study is to determine the effect of family support, gender and age on the quality of life of patients with chronic kidney failure at Dr. Moewardi Hospital Surakarta.Subjects and Method: This study used a cross-sectional design conducted at Dr. Moewardi Surakarta Hospital in June 2024. The target population of the study was 920 patients with chronic kidney failure at Dr. Moewardi Hospital Surakarta. A total of 200 study subjects were selected using simple random sampling. The dependent variables are family support, age, and gender. The independent variable is quality of life. The study instruments used were questionnaires and checklists and the data was analyzed using multiple linear regression analysis and processed using Stata 13.Results:There was a statistically significant relationship between family support and quality of life in patients with chronic kidney failure. Each increase in family support score by one unit will be followed by an improvement in the quality of life of chronic kidney failure patients by 0.31 units (b = 0.31; CI 95% = 0.42 to 0.57; p = 0.023). Statistically, there is a significant relationship between age and quality of life of patients with chronic kidney failure. Each one-year increase in age will be followed by an improvement in the quality of life of chronic kidney failure patients by 0.02 units (b = 0.02; CI 95% = 0.05 to 0.38; p = 0.012). There is a very small difference in quality of life between female and male chronic kidney failure patients. Female patients with chronic kidney failure were 0.01 years lower than men, but the difference was statistically very insignificant (b = -0.80; CI 95% = -0.51 to 0.50; p = 0.975).Conclusion: There was a positive relationship between family support and age and quality of life in patients with chronic kidney failure and there was no significant difference by gender between women and men.

Abstract

Background: Following a stroke, cognitive ability, and cardiovascular fitness decline, exerting a substantial influence on physical rehabilitation and overall quality of life. Stroke management encompasses a multidisciplinary approach, with emerging studies highlighting the role of exercise in enhancing cognitive ability and cardiovascular fitness post-stroke. This study aims to analyze the effects of exercise on cognitive ability, cardiovascular fitness, and quality of life in stroke survivors, shedding light on potential therapeutic benefits. Subject and Methods: The PICO Model eligibility criteria were employed to execute the article search including P= post-stroke patients; I= exercises C= standard care, O= cognitive ability OR cardiovascular fitness OR quality of life. The study extracted articles from PubMed and Science­Direct databases, with keywords "exercises" AND "cognitive ability" OR "cardiovascular fitness" OR "quality of life" AND "post-stroke patients". Inclusion criteria required full-text articles with rando­mized controlled trial (RCT) designs, focusing on post-stroke patients and evaluating improve­ments in cognitive ability, cardiovascular fitness, or quality of life. PRISMA diagram and Review Manager 5.4.1 tool were used to analyse the selected articles. Results: 25 studies included in the review had 2,110 participants, across Asia; Australia, Europe, North America, and South America eligible for meta-analysis. The result indicated a positive effect of exercise on cognitive ability (SMD=0.68, CI 95% (0.21 to 1.16); p=0.005); cardiovascular fitness (SMD=0.63, CI 95% (0.17 to 1.08); p=0.007) and quality of life (SMD=0.60, CI 95% (-0.08 to 1.27); p=0.080). Conclusion: Engagement in diverse forms and durations of exercise emerges as a crucial element in post-stroke rehabilitation. This intervention signifies a promising avenue toward enhancing cognitive abilities, and cardiovascular fitness, and potentially elevating quality of life among stroke survivors.

Abstract

Background: Reduced sleep quality will have an impact such as decreased concentration, emotional disturbances, health and daily activities. There are several factors outside of puberty (hormonal) that cause the percentage of insomnia sufferers to be higher among adolescents, including lifestyle and environment which are the causes of insomnia in adolescents. Positive findings have been found in a study reporting that acupuncture and herbal butterfly pea flower tea can improve patient health and sleep quality. This study aims to determine the comparison of acupuncture therapy and herbal butterfly pea flower tea. Subjects and Method: This study is a quantitative study with a Randomized Controlled Trial (RCT) design. This study was conducted at SMKN 5 Merangin in March-April 2024. A sample of 120 students was selected using simple random sampling. The dependent variable in this study is sleep quality in cases of insomnia. The independent variables in the study are acupuncture therapy and butterfly pea herbal tea. Result: The best intervention for improving sleep quality was acupuncture (Mean= 3.65; SD= 1.12), which was better than butterfly pea tea (Mean= 2.13; SD= 1.09) or nothing (Mean= 0.20; SD= 1.20), and the difference between the groups was statistically significant (p<0.001). Conclusion: The best intervention to improve sleep quality was acupuncture compared to butterfly pea herbal tea.

Abstract

Background: Low back pain is common in healthcare workers including doctors, nurses, midwives and physiotherapists, due to the nature of their work. These healthcare workers are often involved in tasks that involve heavy lifting. Manual patient handling, and awkward postures that are an integral part of care may have an effect on musculoskeletal injuries. This study aims to analyze the effect of heavy lifting on low back pain among healthcare workers. Subjects and Method: This study is a systematic review and meta-analysis study. The search for articles was carried out in accordance with the PICO Model eligibility criteria which include: P = Health workers, I = Heavy lifting, C = No heavy lifting, O = Low back pain. The articles used came from 2 databases, namely: PubMed and Science Direct. The keywords used were "heavy lifting" AND "low back pain" OR "musculoskeletal of low back pain” AND "healthcare professional" OR "healthcare workers". The inclusion criteria in this study included full-text articles with a cross-sectional study design, the relationship measure used was the adjusted Odds Ratio (aOR), articles published in the range 2014-2022, and the outcome was Low back pain. Articles were analyzed using the PRISMA diagram and the Review Manager 5.4.1 application. Results: 9 articles from Ethiopia, Japan, Malaysia, Nigeria, Pakistan, Saudi Arabia and Uganda. Healthcare workers who performed heavy lifting increased lower back pain by 4.36 times compared to those who did not, and the results were statistically significant (aOR=4.36; 95% CI=3.07 to 6.20; p<0.001). Conclusion: A meta-analysis of 9 cross-sectional studies concluded that heavy lifting increases low back pain in healthcare workers.

Abstract

Background: Diabetes mellitus is a chronic condition that occurs when there is an increase in glucose levels in a person's blood because the body cannot produce enough of the insulin hormone, or cannot use the insulin it produces effectively. This study aims to estimate the effect of acupuncture therapy on changes in HbA1c in people with type II diabetes mellitus. Subjects and Method: This research is a systematic review and meta-analysis carried out using the PRISMA flow diagram and PICO model including Population = Type II diabetes patients; Intervention = Acupuncture Therapy; Comparison= No acupuncture therapy or sham acupuncture; Outcome= Change in HbA1c and quality of life. Article searches through journal databases include: PubMed, Science Direct, Google Scholar, Springer Link, BMC and Hindawi. Search keywords: acupuncture, AND "type 2 diabetes mellitus", "acupuncture for diabetes mellitus type 2" AND HbA1c OR "quality of life" AND "randomized controlled trial". Inclusion criteria are full paper articles, Randomized Controlled Trial (RCT) research. The measure of relationship used is Mean SD. Eligible articles were analyzed using the Revman 5.3 application. Results: Randomized Controlled Trial (RCT) from China, England, Iran, Egypt and Taiwan. Acupuncture therapy can reduce HbA1c in type II diabetes mellitus patients by 0.57 units higher than in controls (SMD -0.57; 95% CI= -1.15 to 0.02; p=0.060). Acupuncture therapy improved the quality of life in type II diabetes mellitus patients by 0.87 units compared to controls (SMD 0.87; 95% CI= 0.00 to 1.74; p=0.050). Conclusion: Acupuncture therapy is effective in reducing HbA1c levels and improving quality of life in patients with type II diabetes mellitus.

Abstract

Background: The management of diabetes mellitus is often faced with the challenge of adherence to antidiabetic herbs. Poor adherence to diabetes medication increases the risk of complications and death. This study aims to analyze the determinants related to treatment adherence in Type 2 DM patients. Subject and Method: The cross-sectional study was carried out at UPF Yankestrad Dr. Sardjito Tawangmangu Hospital. A total of 201 adult patients with Type 2 DM were selected in this study. The dependent variable is treatment adherence in Type 2 DM patients. The independent variables were education, knowledge, access to health services, family support, income, and side effects of treatment. Data was collected using questionnaires. Data were analyzed by double linearregression. Result: There was a statistically significant positive association between knowledge and medication adherence (b = 0.19; 95% CI = 0.12 to 0.26; p&lt;0.001); access to healthcare (b = 0.37; 95% CI = 0.26 to 0.47 p&lt;0.001); family support (b = 0.29; 95% CI = 0.20 to 0.39; p&lt;0.001); income (b = 0.15; 95% CI = 0.01 to 0.29; p= 0.030); and there was a negative and statistically significant association between adverse events and medication adherence (b = -0.33; 95% CI = -0.49 up to -0.18; p&lt;0.001). The model fit between the independent variables included in the linear multiple regression analysis model together was able to explain the variation in treatment adherence by 67.12% (Adj R-squared= 67.12%). Conclusion: Treatment adherence in Type 2 DM patients is positively related to knowledge, access to services, family support, and income. Abstract

Background: The COVID-19 pandemic has spread around the world and is impacting the mental health of everyone, including healthcare workers. Health workers are at the forefront of treating patients infected with COVID-19. are at high risk of experiencing mental health problems during the COVID-19 pandemic. The purpose of this study was to estimate the magnitude of the effect of COVID-19 patient care on the level of anxiety and depression in nurses in hospitals, with a meta-analysis of primary studies conducted by previous authors.Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, population: nurses. Intervention: serving COVID-19 patients. Comparison: does not serve COVID-19 patients. Outcome: anxiety and depression. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “Anxiety” AND “Depression” AND “Nurses” OR “Healthcare worker” AND “COVID-19”. The included article is a full-text English language with a cross-sectional study design from 2020 to 2021 and reports the adjusted Odds Ratio (aOR) in a multivariate analysis. The selection of articles is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application.Results:  A total of 12 cross-sectional studies from America, Asia, and Europe were selected for systematic review and meta-analysis. The data collected showed nurses who served COVID-19 patients had a risk of experiencing anxiety as much as 1.75 times compared to nurses who did not serve COVID-19 patients and this result was statistically significant (aOR= 1.75; 95% CI= 1.29 to 2.37; p<0.001 ). A meta-analysis of 12 studies from America, Asia and Europe showed that nurses caring for COVID-19 patients had a 1.1 times risk of developing depression compared to nurses who did not serve COVID-19 patients, but this result was not statistically significant (aOR= 1.10; 95% CI= 0.53 to 2.30; p= 0.80).Conclusion: The effect of caring for COVID-19 patients increases the risk of anxiety and depression in nurses in hospitals.Keywords: Anxiety, Depression, Nurses, COVID-19Correspondence: Andang Sudarmono.
