Abstract

ABSTRACT Background: Job satisfaction generally describes how satisfied a person is in carrying out their work. Job satisfaction can also be influenced by various factors such as competitive salaries, adequate staff numbers, a pleasant work environment, opportunities for growth and professionalism, reasonable workload, recognition by superiors, positive relationships with colleagues, autonomy at work, security. work, career advancement and fair rewards. This study aims to estimate the effect of age, a safe work environment and fair payment on the job satisfaction of health workers. Subjects and Method: A systematic review and meta-analysis was conducted using PRISMA guidelines and the PICO model. The articles used range from 2014 to 2023. Population = Health workers. Intervention=age range >30 years, safe work environment, fair pay. Comparison= age range <30 years, unsafe work environment, fair pay. Outcome = job satisfaction. Articles were collected from databases such as Google Scholar, PubMed, and Science Direct. The literature search used the keywords "Safety Environment or Conducive Environment" AND "Fair Salary" AND "Health Provider" AND "Job Satisfaction" AND "Cross Sectional". A total of 9 articles met the inclusion criteria for meta-analysis, and were further assessed using RevMan 5.3. Results: Meta-analysis with a cross-sectional study design of 9 primary study articles from Ethiopia and Myanmar showed that age over 30 years did not affect job satisfaction among health workers (aOR=1.00; 95% CI= 0.94 to 1.07; p= 0.940). A safe work environment (aOR= 1.23; 95% CI= 0.92 to 1.65; p= 0.160) can increase health workers' job satisfaction, but this increase is not statistically significant. Fair pay (aOR= 2.38; 95% CI= 1.78 to 3.19; p<0.001); can increase job satisfaction of health workers and these results are statistically significant. Conclusion: Age over 30 years does not affect job satisfaction among health workers. A safe work environment can increase health workers' job satisfaction, but this increase is not statistically significant. Fair pay can increase health workers' job satisfaction and this result is statistically significant. Keywords: safety environment, fair salary, health provider, job satisfaction

Abstract

Background: Quality of health services is an effort to provide health services to individuals and populations in increasing the likelihood of desired health outcomes. Patient satisfaction is an indicator in assessing the quality of health services. This study aims to analyze the factors that influence the quality of health services and satisfaction of inpatients in hospitals. Subjects and Method: The research, with a cross-sectional study design, was carried out at public and private hospitals in Mataram City from November to December 2023. A total of 207 inpatients were selected using simple random sampling in the study. The dependent variables are health service quality and patient satisfaction. The independent variables are gender, education level, income, BPJS status (payment type), inpatient class, and length of stay. Collecting data on perceptions of health service quality and patient satisfaction using a structured questionnaire prepared based on Parasuraman's Servqual concept (1985). Results: Patient satisfaction is significantly influenced by perceived quality of health services (b = 0.81; 95% CI = 0.76 to 0.86; p < 0.001) and patient education level (b = -0.08; 95% CI = -0.17 to -0.003; p = 0.04 ). Perceived quality of health services is significantly influenced by income factors (b= 0.22; 95% CI= 0.09 to 0.35; p= 0.001) and patient inpatient room class (b= 0.40; 95% CI= 0.28 to 0.52; p <0.001) . Perception of service quality is not significantly influenced by the factors gender, education, BPJS status, and length of stay. Conclusion: Patient satisfaction is significantly influenced by the quality of health services and the patient's level of education. The quality of health services is significantly influenced by income factors and the patient's inpatient room class. Abstract

Background: Health insurance implemented in Indonesia is managed by the Social Security Administering Body (BPJS). In its administration, BPJS Health works based on the principle of non-profit social protection. Willingness to Pay (WTP) has been carried out in Nigeria which uses a community-based health insurance system. This study aims to analyze the willingness to pay (Willingness to Pay) contributions in independent national health insurance membership among informal sector business actors in the Mandalika tourist area of Lombok. Subjects dan Method: This type of study is observational analytic using a cross-sectional study design. The target population in this study is all informal sector business actors in the Mandalika Lombok tourist area. The sampling technique in this study is random sampling, namely a random sampling method that involves dividing the population into groups called strata, based on certain relevant characteristics.n. Results: This study explains that informal sector workers with income ≥ IDR 2,000,000/month are on average willing to pay insurance premiums of IDR 64,573/month (Mean= 64,573; SD= 41,292). Workers with income < IDR 2,000,000/month are on average willing to pay insurance premiums of IDR 24,880/month (Mean= 24,880; SD= 14,206). Overall, workers in the informal sector are on average willing to pay insurance premiums of IDR 48,100/month (Mean= 48,100; SD= 38,229). Willingness to pay insurance premiums is significantly influenced by income. Every increase in income of IDR 1,000,000 will be followed by a higher willingness to pay for health insurance of IDR 21,745 (b= 0.02; 95% CI= 0.01 to 0.03; p <0.001). Willingness to pay premiums was not significantly influenced by gender (b= 346.13; 95% CI= -15241.31 to 15933.56; p = 0.965) or age (b= -7110.17; 95% CI= -22958.38 to 8738.04; p = 0.375). Conclusion: The results of this study conclude that the willingness to pay insurance premiums is significantly influenced by income. The higher the income, the higher the willingness to pay premiums, and this effect is statistically significant. Keywords: Willingness to pay, Income, Informal workers

Abstract

Background: Tuberculosis is a public health problem both in Indonesia and internationally. Tuberculosis is an infectious disease that can affect anyone and is one of the 10 leading causes of death worldwide. TB ranks second with the highest cases in Indonesia after India. Tuberculosis Infection Control (TBIC) is a form of TB prevention which is the core of safe and high-quality health services. TBIC is often neglected in patient management practice. This study aims to analyze and estimate the effect of tuberculosis infection control training on improving the performance of staff in health facilities. Subjects and Method: The meta-analysis was carried out using the PICO format including: (1) Population: Health personil; (2) Intervention: TBIC Training; (3) Comparison: No TBIC training; and (4) Outcome: Performance. Article searches were carried out using several databases such as Google Scholar, PubMed, and Elsevier. Search for articles from 20 October 2022 to 21 January 2023. Keywords used: “Tuberculosis infection control training” OR “Performance” AND “Health personel” AND “Health facility” AND “Multivariate” AND “Cross-sectional”. The inclusion criteria for the included articles were full text articles, relationship measures using multivariate analysis (aOR), English articles with a cross-sectional design, and article publications in 2012-2022. Articles are collected using PRISMA flow diagram guidelines. Data were analyzed using the Review Manager 5.3 application. Results: A total of 13 cross-sectional studies, from the continents of Asia and Africa. Nine articles from East Africa (Ethiopia), 2 articles from West Africa (Nigeria), 1 article from Central Africa (Cameroon), and 1 article from South Asia (Nepal). The results showed that health personil who received TBIC training had a 2.35 times better chance of performance compared to health personil without training, and this result was statistically significant (aOR= 2.35; 95% CI= 1.96 to 2.81; p <0.001). Conclusion: Tuberculosis infection control training can improve the performance of workers in health facilities. Abstract

Background: Early initiation of breastfeeding has the potential to reduce the risk of neonatal death. Babies who do not start breastfeeding within the first hour after birth have a higher risk of death. This study aimed to determine the relationship between the Health Belief Model construct and early breastfeeding initiation behavior. Subjects and Method: The study was conducted in February-March 2024 with a sample of 200 mothers who had children aged 0-3 months at Posyandu in the Karanganyar Regency working area. This study used an observational analytic design with a cross-sectional study approach. Sampling used was fixed disease sampling. The dependent variable was early initiation of breastfeeding and the independent variables were perceived vulnerability, perceived severity, perceived benefits, perceived barriers, cues to action and self-efficacy. The data were collected using questionnaire and analyzed using multiple logistic regression analysis. Results: This study provided empirical evidence that a number of constructs in the Health Belief Model can be used as predictors of early breastfeeding initiation. Mothers who have a high perceived vulnerability (OR= 4.48; CI 95%= 1.12 to 17.89; p= 0.034), high perceived severity (OR= 4.14; CI 95%= 1.16 to 14.87; p=0.029), high perceived benefit (OR= 4.37; CI 95%= 1.15 to 16.56; p=0.030), and high self-efficacy (OR= 16.76; CI 95%= 3.91 to 71.79; p <0.001) showed a positive relationship with increasing early breastfeeding initiation behavior. Conclusion: There is a positive relationship between perceived vulnerability, perceived severity, perceived benefits, and self-efficacy with early breastfeeding initiation behavior.

Abstract

Background: The World Health Organization (WHO) reports that 2 out of 3 babies do not get exclusive breastfeeding and only 41% of babies aged 0-6 months get exclusive breastfeeding. It is believed that ANC and PNC examinations can increase the possibility of mothers to start and maintain exclusive breastfeeding, because mothers will receive information and support about the benefits of exclusive breastfeeding, the importance of giving colostrum, and the right technique for positioning and attachment in breastfeeding. This study aims to analyze the effect of ANC and PNC on the practice of exclusive breastfeeding. Subjects and Method: This study is a systematic and meta-analysis study, with the following PICO Population = Breastfeeding mothers. Intervention = ANC <4 times and no PNC. Comparison = ANC ≥4 times and PNC. Outcome = Exclusive breastfeeding. The articles used in this study were obtained from several databases including PubMed, Google Scholar, EBSCO, Springer Link, Science Direct, and Pro Quest. The keywords used are “ANC and exclusive breastfeeding”, “PNC and exclusive breastfeeding”. The inclusion criteria were full-text article observational study designs. Articles are collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application. Results: Based on the results of a meta-analysis of 9 primary studies originating from Ethiopia, Tanzania, The Gambia, Saudi Arabia, and Indonesia, it was found that regular ANC, which was 4 times or more, has the possibility of giving exclusive breastfeeding by 1.55 times ANC (aOR=1.55; 95% CI=1.29 to 1.88; p=0.001) and mothers who conduct PNC have the possibility of giving exclusive breastfeeding by 2.43 times (aOR=2.43; 9 5%CI=1.78 to 3.31; p=0.001). Abstract

Background: Postpartum depression is a form of depression that occurs during the puerperium with specific symptoms that appear 4-6 weeks after delivery and can last for months, affecting the happiness and emotional relationship between mother and child. This study aims to analyze the effect and estimate the size of the effect of family support, family income levels and domestic violence on the incidence of depression among postpartum women with a meta-analysis. Subject and Method: The meta-analysis was carried out using the PRISMA flowchart and the PICO model. Population: postpartum mothers. Intervention: strong family support, high family income levels and domestic violence. Comparison: weak family support, low family income level and no domestic violence. Outcome: postpartum depression. The online databases used are Google Scholar, Hindawi, PubMed, Science Direct, Scopus, and ResearchGate. There were 18 cross-sectional studies published in 2013-2023 that met the inclusion criteria. Analysis was performed with RevMan 5.3. Results: Meta-analysis of 18 articles with cross-sectional research designs from different countries, with a sample size of 29,638 research subjects. The results of the meta-analysis show that there is an effect of family support on the risk of postpartum depression. Mothers who received high family support had a risk of experiencing postpartum depression by 0.69 times lower compared to those who received low family support (aOR= 0.69; 95% CI= 0.38 to 1.27; p=0.240). Mothers with high family income levels have a risk of experiencing postpartum depression by 0.50 times lower compared to low family income levels (aOR= 0.50; 95% CI= 0.28 to 0.90; p=0.020). Mothers who experience domestic violence have a risk of postpartum depression by 4.20 times higher compared to those who do not experience domestic violence and the relationship is statistically significant (aOR=4.20; CI 95% =2.56 hingga 6.88; p<0.001). Conclusion: Strong family support and high levels of family income reduce the risk of postpartum depression. Meanwhile, domestic violence increases the risk of postpartum depression. Abstract

Background: Maternal age <19 years and anemia in pregnancy are often associated with adverse neonatal outcomes. This study aims to analyze the relationship between maternal age <19 years and anemia in pregnancy (Hb <11 g/dL) on the incidence of low birth weight babies (LBW) and premature birth. Subjects and Method: The research meta-analysis took cohort studies published in 2018-2022. The PICO formulation used: the population is pregnant women, the intervention consists of groups of pregnant women aged <19 years and pregnant women with anemia (Hb<11 gr/dL), comparison is the group of pregnant women aged 20-35 years and pregnant women are not anemic while Outcomes were LBW and premature birth. Search articles through databases: PubMed, springer, science direct and google scholar with the keywords “Adolescence pregnancy OR young OR Teenage AND Pregnancy AND neonatal outcome OR Low birth weight OR preterm birth OR Low weight”, “Anemia OR Maternal anemia AND Neonatal outcome OR Preterm birth". Data analysis using the Review Manager (RevMan) 5.4 application. Results: A total of 7 cohort articles on pregnancy aged <19 years and 5 cohort articles on anemia were analyzed. Articles come from Canada, Turkey, England, Russia, Finland, China, Chinese Taipei, Ethiopia, Zambia and Bangladesh. The sample sizes were n=43,399 (maternal age and LBW), n=55,049 (maternal age with premature birth), n=16,544,075 (anemia and premature birth). Pregnant women aged <19 years have a 1.63 risk of giving birth to LBW babies (aOR= 1.63; 95% CI= 0.96 to 2.78; p= 0.070) and 1.41 times giving birth to premature babies (aOR= 1.41; 95% CI= 0.78 to 2.56; p= 0.250). Pregnant women with anemia have a 1.60 times risk of giving birth to premature babies (aOR= 1.60; 95% CI= 1.10 to 2.32; p= 0.010). Conclusion: The age of pregnant women <19 years does not have a significant effect on the occurrence of LBW and premature birth. Pregnant women with anemia increase the risk of premature birth. Abstract

Background: Cervical cancer is a sexually transmitted disease (STD) with the fourth largest number of new cases and deaths worldwide. This study aimed to analyze the effects of the use of hormonal contraception and a history of STD on the risk of cervical cancer in women. Subjects and Method: The meta-analysis was carried out using the PRISMA flowchart and the PICO model. Population: women of reproductive age. Intervention: use of hormonal contraception and history of sexually transmitted infections Comparison: not using hormonal contraception and history of sexually transmitted infections Outcome: Cervical Cancer. The process of searching for articles is by searching through online journal databases which include Google Scholar, Elsevier and PubMed. With keywords “oral pill” OR “1 month injection” OR “3 month injection” OR “implant” OR “hormonal contraception” AND “cervical cancer” OR “syphilis” OR “chlamydia” OR “herpes” OR “gonorrhea” OR ”HIV” AND “Sexually transmitted infections” AND “Cross sectional” AND aOR. Articles were selected using the PRISMA flow and data analysis using the Review Manager 5.3 application. Results: This meta-analysis consisted of 12 articles from Africa and Asia . The total sample was 8,240. The use of hormonal contraception (aOR = 2.34; 95% CI= 1.83 to 4.66; p<0.001) and had history of STD (aOR = 1.97; 95% CI = 1.49 to 2.61; p<0.001) increased the risk of cervical cancer. Conclusion: The use of hormonal contraception and has history of std increase the risk of cervical cancer. Abstract

Background: Childbirth education and assistance with hypnobirthing is an autohypnosis technique, namely an effort to instill positive intentions through suggestion into the soul or subconscious mind during pregnancy and preparation for childbirth. This study aims to analyze the effectiveness of childbirth education and assistance with hypnobirthing in improving childbirth outcomes. Subjects dan Methode: The Randomized Controlled Trial (RCT) study was conducted at the Pratama D'Maryam Clinic, Yogyakarta from August to December 2023. A total of 154 pregnant women were selected for this study. Dependent variables are anxiety, depression, complications, and delivery time. The independent variables are childbirth education and assistance with hypnobirthing. Therapy is given 2 times a week with a duration of 3 hours. The study instrument is a questionnaire. Data analysis used Independent T-Test. Results: Providing childbirth education accompanied by assistance with hypnobirthing is effective in reducing anxiety levels in pregnant women (Effect Size= 1.22; p<0.001), reducing depression levels (Effect Size= 1.19; p<0.001), and shortening labor time (Effect Size= 0.08; p = 0.016). Conclusion: Providing childbirth education accompanied by assistance with hypnobirthing is effective in reducing anxiety levels in pregnant women, reducing depression levels, and shortening labor time.
